Chronic pancreatitis is a long-standing inflammation of the pancreas,characterized by progressive inflammatory and fibrotic changes,resulting in permanent structural damage of pancreatic parenchyma[1].Pancreatitis(chronic or acute)is the primary risk factor for pseudo-splenic artery aneurysms,along with pancreatic pseudocysts and *** the disease progresses,patients with chronic pancreatitis may develop complications due to exocrine and endocrine pancreatic functional loss,such as fat malabsorption with steatorrhea,glucose intolerance,and ultimately diabetes mellitus[1,2].Furthermore,severe pancreatic inflammation can cause weakening of vessel walls,with subsequent formation of arterial *** artery is the most commonly affected visceral artery[2,3].Liver cirrhosis,portal hypertension,liver transplantation,atherosclerosis,hypertension,pregnancy and multiparity are main risk factors for true splenic artery aneurysms[2].Rupture and intraperitoneal bleeding is the cause of death in 30%−50%of ***,prompt diagnosis and appropriate treatment are of great clinical importance[4,5].Herein,we present a case of a giant splenic artery pseudoaneurysm within a walled of necrosis involving pancreatic parenchyma,as a complication of long standing chronic pancreatitis.

Nitroglycerine effects on portal vein mechanics and oxidative stress in portal hypertension

The Global Navigation Satellite System(GNSS) plays a crucial role in providing Positioning, Navigation, and Timing(PNT) services, but its signal is easily blocked indoors, which cannot meet the demand for high-precision positioning and timing in indoor scenarios. To address this issue, we proposed the Multi-Scale Non-Orthogonal Multiple Access(MS-NOMA) integrated signal, which can achieve sub-meter positioning accuracy and nan-meter level time synchronization accuracy while ensuring the Quality of Service(QoS) requirement and power budget of communication system. But the existing study did not consider the mutual interference between Device-to-Device(D2D) and MSNOMA signals. Our major contributions are: Firstly, we derive the communication performance of cellular communication users(C-Users), the communication performance of D2D users(D-Users) and the ranging performance of positioning users(P-Users) based on the D2D signals multiplexing MS-NOMA signals uplink scenario. Secondly, we model a power allocation problem that considers QoS requirement and power constraints. Then, we propose a Particle Swarm Optimization-Based(PSO-Based) Power Allocation(PSOBPA) strategy which allocates the signal power of the C-Users in the MS-NOMA signal system and the signal power of D-Users. The numerical results show that our proposed PSOBPA strategy can improve the total system throughput by approximately 15% and the average ranging accuracy by around 4% compared to the average power allocation algorithm with the same bandwidth.

AIM:Тo examine the effects of nitroglycerine on portal vein haemodynamics and oxidative stress in patients with portal ***:Thirty healthy controls and 39 patients with clinically verified portal hypertension and increasedvascular resistance participated in the *** di-ameters,portal diameters and portal flow velocities were recorded using color flow imaging/pulsed Doppler ***-section area,portal flow and index of vascular resistance were *** collected blood samples,superoxide anion radical (O 2-),hydrogen per-oxide (H 2 O 2),index of lipid peroxidation (measured as TBARS) and nitric oxide (NO) as a marker of endothelial response (measured as nitrite-NO 2-) were ***-dependent analysis was performed at basal state and in 10th and 15th min after nitroglycerine (sublingual 0.5 mg) ***:Oxidative stress parameters changed sig-nificantly during the study.H 2 O 2 decreased at the end of study,probably via O 2-mediated disassembling in Haber Weiss and Fenton reaction;O 2-increased signifi-cantly probably due to increased diameter and tension and decreased shear rate *** O 2-and H 2 O 2 degradation products,like hydroxyl radical,initi-ated lipid *** blood flow was to some extent lower in patients than in controls due to double paradoxes,flow velocity decreased,shear rate decreased significantly indicating non Newtonian char-acteristics of portal blood ***:This pilot study could be a starting point for further investigation and possible implemen-tation of some antioxidants in the treatment of portal hypertension.

AIM： To demonstrate the necessity of intraoperative endoscopy in the diagnosis of secondary primary tumors of the upper digestive tract in patients with obstructive hypopharyngeal carcinoma. METHODS： Thirty-one patients with hypopharyngeal squamous cell carcinoma had been operated, with radical intent, at our Institution in the period between 1978 and 2004. Due to obstructive tumor mass, in 7 （22.6%） patients, preoperative endoscopic evaluation of the esophagus and stomach could not be performed. In those patients, intraoperative endoscopy, made through an incision in the cervical esophagus, was standard diagnostic method for examination of the esophagus and stomach. RESULTS： We found synchronous foregut carcinomas in 3 patients （9.7%）. In two patients, synchronous carcinomas had been detected during preoperative endoscopic evaluation, and in one （with obstructive carcinoma） using intraoperative endoscopy. In this case, preoperative barium swallow and CT scan did not reveal the existence of second primary tumor within esophagus, despite the fact that small, but T2 carcinoma, was present. CONCLUSION： It is reasonable to use intraoperative endoscopy as a selective screening test in patients with obstructive hypopharyngeal carcinoma.
