A ciliated hepatic foregut cyst (CHFC) is a rare foregut developmental malformation usually diagnosed in adulthood. Five percent of reported cases of CHFC transform into squamous cell carcinoma. We report the presentation, evaluation, and surgical management of a symptomatic 45-year-old male found to have a 6.2 cm CHFC. Contrast tomography-guided fine-needle aspiration demonstrated columnar, ciliated epithelium consistent with the histologic diagnosis of CHFC. The intracystic levels of carbohydrate antigen (CA) 19-9 and carcinoembryonic antigen (CEA) were extremely high (978118 U/mL and 973 μg/L, respectively). Histologically, the wall of the cyst showed characteristic pseudopapillae lined with a ciliated stratified columnar epithelium, underlying smooth muscle, an outer fibrous layer and no atypia. Immunohistochemistry for CA19-9 and CEA was positive. This is the first case report of a CHFC in which levels of CA 19-9 and CEA were measured. Our findings suggest that a large sized multilocular cyst and elevated cyst CA19-9 and CEA levels do not exclude a CHFC from consideration in the diagnosis. CHFCs should be included in the differential diagnosis of hepatic lesions. Accurate diagnosis of a CHFC is necessary given its potential for malignant transformation, and surgical excision is recommended.

When forced to drop from the plant,flightless arboreal insects can avoid reaching the risky ground by maneuvering their body through the *** wingless pea aphids(Acyrthosiphon pisum)are threatened by natural enemies,they often drop off their host plant while assuming a stereotypic posture that rotates them in mid-air,aligning them with their feet pointing *** position may increase their chances of re-clinging onto lower plant parts and avoid facing the dangers on the ground,although its effectiveness in realistic field conditions has not been *** performed both laboratory and outdoor experiments,in which we dropped aphids upon host plants to quantify clinging success in plants with different characteristics such as height and leaf *** aphids had twofold higher clinging rates than dead ones,indicating that clinging success is indeed affected by the active aerial-righting of dropping *** ability to cling was positively dependent on the plants’foliage cover as viewed in vertical direction from ***,we released aphids in commercial alfalfa(Medicago sativa)fields with varying plant heights and foliage cover and induced them to ***(up to 75%)of the aphids avoided reaching the ground in taller plants(65 cm),and 17%in shorter plants(21 cm),demonstrating the efficiency of the aphids’response in averting risks:both those of an approaching enemy on the plant and the plethora of new risks on the ***,even in complex field environment,the aerial-righting mechanism can substantially reduce the possible risks following escape from a predator.

AIM To identify the effect of hydrogen-rich water(HRW) and electrolyzed-alkaline water(EAW) on high-fat-induced non-alcoholic fatty acid disease in *** Mice were divided into four groups:(1) Regular diet(RD)/regular water(RW);(2) high-fat diet(HFD)/RW;(3) RD/EAW; and(4) HFD/EAW. Weight and body composition were measured. After twelve weeks, animals were sacrificed, and livers were processed for histology and reverse-transcriptase polymerase chain reaction. A similar experiment was performed using HRW to determine the influence and importance of molecular hydrogen(H2) in EAW. Finally, we compared the response of hepatocytes isolated from mice drinking HRW or RW to palmitate *** EAW had several properties important to the study:(1) pH = 11;(2) oxidation-reduction potential of-495 mV; and(3) H2 = 0.2 mg/L. However, in contrast to other studies, there were no differences between the groups drinking EAW or RW in either the RD or HFD groups. We hypothesized that the null result was due to low H2 concentrations. Therefore, we evaluated the effects of RW and low and high HRW concentrations(L-HRW = 0.3 mg H2/L and H-HRW = 0.8 mg H2/L, respectively) in mice fed an HFD. Compared to RW and L-HRW, H-HRW resulted in a lower increase in fat mass(46% vs 61%), an increase in lean body mass(42% vs 28%), and a decrease in hepatic lipid accumulation(P arison to mice drinking regular *** H2 is the therapeutic agent in electrolyzed-alkaline water and attenuates HFD-induced nonalcoholic fatty liver disease in mice.

Wormlions are small fly larvae that dig pits in loose soil to trap their prey. Similar to other trapbuilding predators, like spiders and antlions, they depend on the habitat structure for successful trap construction and prey catch. We examined whether sites at which wormlions are present differ in sand depth and particle size from nearby sites, at which wormlions are absent. Next, in the laboratory we manipulated both sand depth and type (fine vs. coarse) to determine their joint effect on microhabitat pref ere nee, the size of the con structed pit, wormlion movement, and their latency to respond to prey. We expected better performance by wormlions in fine and deep sand, and the sand in wormlions' natural sites to be finer and deeper. However, in only partial agreement with our expectations, wormlion sites featured finer sand but not deeper sand. In the laboratory, wormlions preferred both fine and deep sand, and moved more in shallow and coarse sand, which we interpret as an attempt to relocate away from unfavorable conditions. However, only deep sand led to larger pits being constructed and to a faster response to prey. The preference for fine sand could, therefore, be related to other ben efits that sand provides. Fin ally, body mass was a domi nant factor, interacting with the preference for both deep and fine sand: deep over shallow sand was more favored by large wormlions and fine over coarse sand by smaller ones. Our results suggest that several factors should be incorporated when studying microhabitat selection.

The domesticated olives originated based on decades of selection from the natural forest around the Mediterranean basin being one of the oldest cultivated tree crops. The requirement for new olive varieties increased markedly during the last 100 years as a result of a worldwide transformation changing from a traditional extensive to a modern intensive mechanized industry. The traditional olive cultivars still widely used are selections from the local autochthonous cultivars which are well adapted to their natural environment. However, the level of production of those is usually rather low and many of them failed to respond enough to modern intensification methods. The wild olive population is gradually diminishing in all regions thus, natural selection is not efficient enough for developing the cultivars required for the new intensive olive industry. Cross breeding between cultivars with specific characters became the basis for the required new cultivars. The genetic diversity in progenies of most olive cultivars is extremely wide and thus becomes the basic source for breeding of the needed new cultivars. However, the genetic information on olive heredity and identification of specific gene characteristics is still limited. Genomic information is required for rational choice of genitors in accordance with each breeding aim, particularly as most criteria required for the new varieties are of multi-gene nature. The availability of advanced genomic information will also serve as basis for molecular engineering when accepted for use in the olive commodity. Geographically isolated olive populations developed during generations of inbreeding in remote regions are another important genetic resource of characters required for breeding the cultivars for the new modern mechanized olive orchards. The use of molecular markers based on AFLP, SSRs, DArT and SNPs is important for the choice of genitors leading to a reduction of the progeny size required in olive breeding. Presentl

Designing electrocatalysts for HER in alkaline conditions to overcome the sluggish kinetics associated with the additional water dissociation step is a recognized challenge in promoting the hydrogen *** this end,delicately tuning the atomic-scale structure and surface composition of nanoparticles is a common strategy and,specifically,making use of hybrid structures,can produce synergistic effects that lead to highly active ***,we present a core-shell catalyst of Ag@MoS_(2)that shows promising results towards the hydrogen evolution reaction(HER)in both 0.5 M H_(2)SO_(4)and 0.5 M *** this hybrid structure,the MoS_(2)shell is strained and defective,and charge transfer occurs between the conductive core and the shell,contributing to the electrocatalytic *** shelling process results in a large fraction of Ag_(2)S in the cores,and adjusting the relative fractions of Ag,Ag_(2)S,and MoS_(2)leads to improved catalytic activity and fast charge-transfer *** suggest that the enhancement of alkaline HER is associated with a cooperative effect of the interfaces,where the Ag(Ⅰ)sites in Ag_(2)S drive the water dissociation step,and the formed hydrogen subsequently recombines on the defective MoS_(2)*** study demonstrates the benefits of hybrid structures as functional nanomaterials and provides a scheme to activate MoS_(2)for HER in alkaline conditions.

髋、膝关节置换术后有较高的发生血栓性疾病的危险。我们研究了一种多重预防方法,于1995年在纽约特种外科医院(Hospital for Special Surgery,HSS)开始运用。它包括术前对患者患血栓性疾病(VTE)的风险进行评估,然后根据评估结果在术前、术中、术后应用一系列安全有效的预防措施,以降低术后血栓和出血的风险。这些措施包括术前停用促凝血药物及患者的自体血采集;使用硬膜外低压麻醉,在全髋关节置换手术(THA)中股骨侧处理之前,静脉给予肝素;术后使用充气加压装置,患肢穿戴弹力袜并强有力地主动背伸和跖屈踝关节,同时尽早下床活动,以降低血液淤滞。如果这些措施能够很好地实施,对于术前没有VTE诱发因素且术后能早期活动的患者,不需要术后使用强效的抗凝药物,从而减少常规使用抗凝药物导致的并发症发病率和死亡率以及医疗费用。此方法已在近万例髋、膝关节置换术患者中应用,结果显示血栓、出血发生率以及总死亡率均很低。

医学模拟技术是利用各种模拟手段,再现临床医学的工作场景,为学习者提供一个无风险的学习临床知识和技能的条件与环境。医学专家和教育学家均认为,以模拟技术为基础的医学教育(Simulation Based Medical Education,SBME)能通过提高医生的临床表现和诊疗水平来改善患者疗效。SBME最主要的优势是允许医生在没有任何风险的模拟的医疗环境中犯错误,并在自身错误中学习,从而减少医生在真实的临床环境中发生错误的机会,以及培养医生在真实的临床环境中不能避免地发生错误时,能拥有正确的心态和采取恰当的补救措施。
