Evaluation of teplizumab's efficacy and safety in treatment of type 1 diabetes mellitus:A systematic review and meta-analysis

BACKGROUND Islets of Langerhans beta cells diminish in autoimmune type 1 diabetes mellitus(T1DM).Teplizumab,a humanized anti-CD3 monoclonal antibody,may help *** long-term implications on clinical T1DM development,safety,and efficacy are *** To assess the effectiveness and safety of teplizumab as a therapeutic intervention for individuals with *** A systematic search was conducted using four electronic databases(PubMed,Embase,Scopus,and Cochrane Library)to select publications published in peerreviewed journals written in *** odds ratio(OR)and risk ratio(RR)were calculated,along with their 95%*** assessed heterogeneity using Cochrane Q and I2 statistics and the appropriate P *** There were 8 randomized controlled trials(RCTs)in the current meta-analysis with a total of 1908 T1DM patients from diverse age cohorts,with 1361 patients receiving Teplizumab and 547 patients receiving a *** was found to have a substantial link with a decrease in insulin consumption,with an OR of 4.13(95%CI:1.72 to 9.90).Teplizumab is associated with an improved Cpeptide response(OR 2.49;95%CI:1.62 to 3.81)and a significant change in Glycated haemoglobin A1c(HbA1c)levels in people with type 1 diabetes[OR 1.75(95%CI:1.03 to 2.98)],and it has a RR of 0.71(95%CI:0.53 to 0.95).CONCLUSION In type 1 diabetics,teplizumab decreased insulin consumption,improved C-peptide response,and significantly changed HbA1c levels with negligible side *** appears to improve glycaemic control and diabetes management with good safety and efficacy.

Peak load and wind energy emission pressure rise more as wind energy penetration keeps growing,which affects the stabilization of the PS(power system).This paper suggests integrated optimal dispatching of thermal power generators and BESS(battery energy storage system)taking wind energy emission grading punishment and deep peak clipping into ***,in order to minimize wind abandonment,a hierarchical wind abandonment penalty strategy based on fuzzy control is designed and introduced,and the optimal grid-connected power of wind energy is determined as a result of minimizing the peak cutting cost of the ***,considering BESS and thermal power,the management approach of BESS-assisted virtual peak clipping of thermal power generators is aimed at reducing the degree of deep peak clipping of thermal power generators and optimizing the output of thermal power generators and the charging and discharging power of ***,Give an example of how this strategy has been effective in reducing abandonment rates by 0.66% and 7.46% individually for different wind penetration programs,and the daily average can reduce the peak clipping power output of thermal power generators by 42.97 and 72.31 MWh and enhances the effect and economy of system peak clipping.

Based on thermal-entransy theory, the multi-objective constructal design of quadrilateral heat generation body(HGB) with similar shapes of leaves is studied further. The relationship between the aspect ratio of quadrilateral HGB and average temperature difference based on thermal-entransy dissipation is compared with that between the aspect ratio of quadrilateral HGB and the maximum temperature difference(MTD). The relationship between a composite function, consisting of linear weighting sum of the average temperature difference and MTD, and aspect ratio is obtained, and the optimal aspect ratios under minimum composite function with different weighting coefficients are obtained. Using the NSGA-Ⅱ algorithm, the Pareto frontier containing a series of compromise results of average temperature difference and MTD is obtained, and optimization results are compared using the deviation index. There is no aspect ratio to make both MTD and average temperature difference reach the minimum, and the optimal aspect ratio under the minimum MTD is smaller than that under the minimum average temperature difference. The optimal aspect ratio is obtained by making the composite function reach the minimum, and the optimal aspect ratios obtained by minimizing the composite function with different weighting coefficients are different. Compared with the construct of the initial design, the value of the composite function with optimal construct decreases by 1.9%, and the aspect ratio of the quadrilateral HGB decreases by 9.1%. The average temperature difference with the optimal construct increases by 2.1%, and the MTD with the optimal construct decreases by 5.6%. The deviation index under multi-objective optimization is smaller than that under single-objective optimization, and the obtained construct has better comprehensive thermal conductivity. Compared with TOPSIS and LINMAP decision-making methods, the average temperature difference with composite function optimization increases by 0.5

In air traffic control communications (ATCC), misunderstandings between pilots and controllers could result in fatal aviation accidents. Fortunately, advanced automatic speech recognition technology has emerged as a promising means of preventing miscommunications and enhancing aviation safety. However, most existing speech recognition methods merely incorporate external language models on the decoder side, leading to insufficient semantic alignment between speech and text modalities during the encoding phase. Furthermore, it is challenging to model acoustic context dependencies over long distances due to the longer speech sequences than text, especially for the extended ATCC data. To address these issues, we propose a speech-text multimodal dual-tower architecture for speech recognition. It employs cross-modal interactions to achieve close semantic alignment during the encoding stage and strengthen its capabilities in modeling auditory long-distance context dependencies. In addition, a two-stage training strategy is elaborately devised to derive semantics-aware acoustic representations effectively. The first stage focuses on pre-training the speech-text multimodal encoding module to enhance inter-modal semantic alignment and aural long-distance context dependencies. The second stage fine-tunes the entire network to bridge the input modality variation gap between the training and inference phases and boost generalization performance. Extensive experiments demonstrate the effectiveness of the proposed speech-text multimodal speech recognition method on the ATCC and AISHELL-1 datasets. It reduces the character error rate to 6.54% and 8.73%, respectively, and exhibits substantial performance gains of 28.76% and 23.82% compared with the best baseline model. The case studies indicate that the obtained semantics-aware acoustic representations aid in accurately recognizing terms with similar pronunciations but distinctive semantics.
