Objective:To elucidate how spinal manipulative therapy(SMT)exerts its analgesic effects through regulating brain function in lumbar disc herniation(LDH)patients by utilizing resting-state functional magnetic resonance imaging(rs-f MRI).Methods:From September 2021 to September 2023,we enrolled LDH patients(LDH group,n=31)and age-and sex-matched healthy controls(HCs,n=28).LDH group underwent rs-f MRI at 2 distinct time points(TPs):prior to the initiation of SMT(TP1)and subsequent to the completion of the SMT sessions(TP2).SMT was administered once every other day for 30 min per session,totally 14 treatment sessions over a span of 4 *** did not receive SMT treatment and underwent only one f MRI ***,participants in LDH group completed clinical questionnaires on pain using the Visual Analog Scale(VAS)and the Japanese Orthopedic Association(JOA)score,whereas HCs did not undergo clinical scale *** effects on the brain were jointly characterized using the amplitude of low-frequency fluctuations(ALFF)and regional homogeneity(Re Ho).Correlation analyses were conducted between specific brain regions and clinical ***:Following SMT treatment,pain symptoms in LDH patients were notably alleviated and accompanied by evident activation of effects in the *** comparison to TP1,TP2 exhibited the most significant increase in ALFF values for Temporal_Sup_R and the most notable decrease in ALFF values for Paracentral_Lobule_L(voxelwise P30;FDR correction).Additionally,the most substantial enhancement in Re Ho values was observed for the Cuneus_R,while the most prominent reduction was noted for the Olfactory_R(voxelwise P30;FDR correction).Moreover,a comparative analysis revealed that,in contrast to HCs,LDH patients at TP1 exhibited the most significant increase in ALFF values for Temporal_Pole_Sup_L and the most notable decrease in ALFF values for Frontal_Mid_L(voxelwise P30;FDR correction).Fur

Is adjuvant chemotherapy necessary for patients with microinvasive breast cancer after surgery？

Objective: Survival and treatment of patients with microinvasive breast cancer(MIBC) remain controversial. In this paper, we evaluated whether adjuvant chemotherapy is necessary for patients with MIBC to identify risk factors influencing its prognosis and decide the indication for adjuvant ***: In this retrospective study, 108 patients with MIBC were recruited according to seventh edition of the staging manual of the American Joint Committee on Cancer(AJCC). The subjects were divided into chemotherapy and non-chemotherapy *** compared the 5-year disease-free survival(DFS) and overall survival(OS) rates between groups. Furthermore, we analyzed the factors related to prognosis for patients with MIBC using univariate and multivariate analyses. We also evaluated the impact of adjuvant chemotherapy on the prognostic factors by subgroup analysis after median follow-up time of 33 months(13-104months).Results: The 5-year DFS and OS rates for the chemotherapy group were 93.7% and 97.5%, whereas those for the nonchemotherapy group were 89.7% and 100%. Results indicate that 5-year DFS was superior, but OS was inferior, in the former group compared with the latter group. However, no statistical significance was observed in the 5-year DFS(P=0.223) or OS(P=0.530) rate of the two groups. Most relevant poor-prognostic factors were Ki-67 overexpression and negative hormonal receptors. Cumulative survival was 98.2% vs. 86.5% between low Ki-67(≤20%) and high Ki-67(>20%). The hazard ratio of patients with high Ki-67 was 16.585 [95% confidence interval(CI), 1.969-139.724; P=0.010]. Meanwhile, ER(-)/PR(-) patients with MIBC had cumulative survival of 79.3% compared with 97.5% for ER(+) or PR(+) patients with MIBC. The hazard ratio for ER(-)/PR(-) patients with MIBC was 19.149(95% CI, 3.702-99.057; P<0.001). Subgroup analysis showed that chemotherapy could improve the outcomes of ER(-)/PR(-) patients(P=0.014), but not those who overexpress Ki-67(P=0.105).Conclusions

Surgical exposures of the distal humeral fractures: An anatomical study of the anterior, posterior, medial and lateral approaches

Purpose:Exposure of the articular surface is the key to the successful treatment of intra-articular fractures of distal humerus. Anterior, posterior olecranon osteotomy as well as medial and lateral approaches are the four main approaches to the elbow. The aim of this study was to compare the exposure of distal articular surfaces of these surgical ***:Twelve cadavers were used in this study. Each approach was performed on six elbows according to previously published procedures. After completion of each approach, the exposed articular surfaces were marked by inserting 0.5 mm K-wires along the margins. The elbow was then disarticulated and the exposed articular surfaces were painted. The distal humeral articular surfaces were then closely wrapped using a piece of fibre-glass screen net with meshes. The exposed articular surfaces and the total articular surfaces were calculated by counting the number of meshes, ***:The average percentages of the exposed articular surfaces for the anterior, posterior olecranon osteotomy, medial and lateral approaches were 45.7% ± 2.0%, 53.9% ± 7.1%, 20.6% ± 4.9% and 28.5% ± 6.3%, ***:The anterior and posterior approaches provide greater exposures of distal humeral articular surface than the medial and lateral ones in the treatment of distal humeral fractures.

Insights into microscopic oil occurrence characteristics in shales from the Paleogene Funing Formation in Subei Basin, China

The microscopic occurrence characteristics primarily constrain the enrichment and mobility of shale *** study collected the lacustrine shales from the Palaeogene Funing Formation in the Gaoyou Sag, Subei Basin. Conventional and multistage Rock-Eval, scanning electron microscopy, and nuclear magnetic resonance(NMR) T1-T2were performed to analyze the contents and occurrence characteristics of shale oil. Low-temperature nitrogen adsorption-desorption(LTNA/D) experiments were conducted on the shales before and after extraction. The relationships between shale oil occurrence with organic matter and pore structures were then discussed. Predominantly, the shale oil in the Funing Formation is found within fractures, with secondary occurrences in interparticle pores linked to brittle minerals and sizeable intraparticle pores associated with clay minerals. The selected shales can be categorized into two types based on the nitrogen isotherms. Type A shales are characterized by high contents of felsic and calcareous minerals but low clay minerals, with larger TOC and shale oil values. Conversely, Type B shales are marked by abundant clay minerals but diminished TOC and shale oil contents. The lower BET specific surface area(SSA), larger average pore diameter, and simpler pore surfaces and pore structures lead to the Type A shales being more conducive to shale oil enrichment and mobility. Shale oil content is predominantly governed by the abundance of organic matter, while an overabundance of organic matter typically equates to a reduced ratio of free oil and diminished fluidity. The BET SSA, volumes of pores less than 25 and 100 nm at extracted state all correlate negatively with total and adsorbed oil contents but display no correlation with free oil, while they have positive relationships with capillary-bound ***, pore water is mainly saturated in micropores(<25 nm) and minipores(25-100 nm), as well as adsorbed oil, while free oil, i.e., bound and movable

A Verifiable Trust-Based CP-ABE Access Control Scheme for Cloud-Assisted Renewable Energy Systems

Renewable Energy Systems(RES)provide a sustainable solution to climate warming and environmental pollution by enhancing stability and reliability through status acquisition and analysis on cloud platforms and intelligent processing on edge servers(ES).However,securely distributing encrypted data stored in the cloud to terminals that meet decryption requirements has become a prominent research ***,managing attributes,including addition,deletion,and modification,is a crucial issue in the access control scheme for *** address these security concerns,a trust-based ciphertext-policy attribute-based encryption(CP-ABE)device access control scheme is proposed for RES(TB-CP-ABE).This scheme effectivelymanages the distribution and control of encrypted data on the cloud through robust attribute key *** introducing trust management mechanisms and outsourced decryption technology,the ES system can effectively assess and manage the trust worthiness of terminal devices,ensuring that only trusted devices can participate in data exchange and access sensitive ***,the ES system dynamically evaluates trust scores to set decryption trust thresholds,thereby regulating device data access permissions and enhancing the system’s *** validate the security of the proposed TB-CP-ABE against chosen plaintext attacks,a comprehensive formal security analysis is conducted using the widely accepted random oraclemodel under the decisional q-Bilinear Diffie-Hellman Exponent(q-BDHE)***,comparative analysis with other schemes demonstrates that the TB-CP-ABE scheme cuts energy/communication costs by 43%,and scaleswell with rising terminals,maintaining average latency below 50ms,ensuring real-time service *** proposed scheme not only provides newinsights for the secure management of RES but also lays a foundation for future secure energy solutions.

The development of effective adsorbents with high amine efficiency and CO_(2)adsorption almost unaffected by humidity is extremely *** this study,we introduce an innovative solid amine adsorbent,TETA/DEA@FS,composed of triethylenetetramine(TETA)and diethanolamine(DEA)functionalized fumed silica(FS),which exhibits exceptional capability in selectively capturing trace CO_(2)from N_(2).TETA/DEA@FS shows an exceptionally high capacity of CO_(2)adsorption of 1.13 mmol/g at the temperature of 298 K and the pressure of 0.0004 bar(1 bar=100 kPa),and achieves an unprecedented CO_(2)/N_(2)IAST selectivity of 1.70×10^(12).TETA/DEA@FS exhibits high amine efficiency,with breakthrough experiments demonstrating that CO_(2)adsorption remains nearly unaffected by ***,TETA/DEA@FS demonstrates rapid CO_(2)adsorption kinetics and outstanding cyclic stability.

Towards few-shot mixed-type dialogue generation

Building an agent capable of conducting both open-domain and task-oriented dialogues, known as mixed-type dialogues, has been an enduring challenge for the AI community. Previous approaches have focused on constructing largescale human-annotated datasets for training models. However, annotating these datasets is expensive and hinders the practical application of these models. This paper identifies a novel challenge, few-shot mixed-type dialogue generation. To address this challenge, we first present a pre-trained dialogue generation framework with modular-based architecture and prompttuning component. Additionally, we collect a mixed-type dialogue dataset that combines persona-chat with conversational recommendation or task-oriented dialogues within a single dialogue session. Specifically, the modular-based architecture allows us to easily incorporate more supervised signals and human-annotated information, thereby facilitating the learning of sessionlevel dialogue logic. We pre-train this dialogue generation framework using multiple external datasets and then fine-tune it on the mixed-type dialogue dataset we collected. Experimental results demonstrate that the three key designs — modular-based architecture, prompt-tuning component, and model pre-training — significantly enhance the performance of this framework compared to state-of-the-art baselines.
