Comparison of outcomes between immediate implantbased and autologous reconstruction: 15-year, single-center experience in a propensity score-matched Chinese cohort

Objective:The number of immediate breast reconstruction(IBR)procedures has been increasing in China.This study aimed to investigate the oncological safety of IBR,and to compare the survival and surgical outcomes between implant-based and autologous reconstruction.Methods:Data from patients diagnosed with invasive breast cancer who underwent immediate total breast reconstruction between 2001 and 2016 were retrospectively reviewed.Long-term breast cancer-specific survival(BCSS),disease-free survival(DFS),and locoregional recurrence-free survival(LRFS)were evaluated.Patient satisfaction with the breast was compared between the implantbased and autologous groups.BCSS,DFS,and LRFS were compared between groups after propensity score matching(PSM).Results:A total of 784 IBR procedures were identified,of which 584 were performed on patients with invasive breast cancer(implantbased,n=288;autologous,n=296).With a median follow-up of 71.3 months,the 10-year estimates of BCSS,DFS,and LRFS were 88.9%[95%confidence interval(CI)(85.1%–93.0%)],79.6%[95%CI(74.7%–84.8%)],and 94.0%[95%CI(90.3%–97.8%)],respectively.A total of 124 patients completed the Breast-Q questionnaire,and no statistically significant differences were noted between groups(P=0.823).After PSM with 27 variables,no statistically significant differences in BCSS,DFS,and LRFS were found between the implant-based(n=177)and autologous(n=177)groups.Further stratification according to staging,histological grade,lymph node status,and lymph-venous invasion status revealed no significant survival differences between groups.Conclusions:Both immediate implant-based and autologous reconstruction were reasonable choices with similar long-term oncological outcomes and patient-reported satisfaction among patients with invasive breast cancer in China.

乳腺癌是我国女性发病率最高的恶性肿瘤。随着乳腺癌综合治疗技术的进步和患者生存期的延长,为提高乳腺癌患者的生存质量,乳腺肿瘤整形外科受到越来越多的重视。乳腺肿瘤整形外科主要包括保乳整形和乳房重建两个技术体系。相较于西方发达国家,我国乳腺肿瘤整形外科虽起步较晚,但发展迅速,在很多领域都取得长足的进步。本文将就植入物乳房重建、自体组织乳房重建、脂肪移植乳房成形、内窥镜辅助乳房重建、乳头乳晕重建、患者报告结局评价等亚专科领域,对我国乳腺肿瘤整形外科的研究进展、当前学科发展的不足之处,及未来发展方向进行综述。

Strategic interaction between institutional investors and supervision department:a theoretical analysis of low‑price collusion in SBIC

We introduce evolutionary game method to analyze low-price collusion in inquiry market of Sci-Tech Innovation Board of China(SIBC)from the perspective of strategic interaction between large institutional investors(LIIs),small and medium-sized institutional investors(SMIIs),and supervision department(SD).The results show that supervision behaviors of SD,and quotation behaviors of institutional investors,are subject to supervision conditions.Under the condition that benefits of tough supervision are lower a lot than minimum benefits of light supervision(light supervision condition),SD will choose light supervision and institutional investors will turn to illegal quotation in response.Finally,a steady-state equilibrium with low-price collusion will form in SIBC’s inquiry market even with a large supervision penalty for illegal quotation.On the contrary,under the condition that benefits of tough supervision are higher a lot than maximum benefits of light supervision(tough supervision condition)and with a large penalty for illegal quotation,SD and institutional investors will choose tough supervision and legal quotation.Further numerical simulations under light supervision condition show that:(1)High-price culling rule will become a booster for low-price collusion and accelerate SMIIs’evolutionary process to imitative quotation.(2)Blindly increasing penalties for illegal quotation or reducing the culling rate is not an appropriate approach to solve the problem of low-price collusion since it cannot shift supervision condition from light into tough and make SD supervise toughly.(3)Institutional investors’choices of quotation strategies are more volatile and highly susceptible to supervision behaviors of SD when facing exogenous uncertainty.Therefore,the keys to solving the problem of low-price collusion are shifting supervision condition from light into tough through increasing incremental benefits of tough supervision,and providing institutional investors with a stable and predictable supervision policy.In conclusion,

In this study,flame responses to acoustic disturbances with different frequencies and amplitudes were experimentally investigated in a lean premixed swirl-stabilized combustor operating at different bulk velocities.The total heat release rate fluctuations and spatial CH*chemiluminescence distributions were captured using a photomultiplier tube and high-speed camera,respectively.The results indicate that the heat release rate exhibits a relatively drastic oscillation and high-order harmonics for low-frequency disturbances.When the bulk velocity and forcing frequency were doubled simultaneously,similar flame structures were observed in the CH*chemiluminescence distributions.As the bulk velocity increases,the gain of the Flame Describing Function(FDF)extends toward the higher frequencies,and the delay time of the flame response decreases.The similarity among FDFs at different bulk velocities was effectively captured by introducing a non-dimensional parameter,defined as the ratio of the flame response delay to the forcing time scale,to replace the dimensional forcing frequency.Furthermore,the availability of the newly defined non-dimensional parameter was verified for flames with different swirl numbers,as this played an important role in determining the flame structures and associated unsteady heat release rate.

Gasification of iron coke and cogasification behavior of iron coke and coke under simulated hydrogen-rich blast furnace condition

To explore the iron coke application in hydrogen-rich blast furnace,which is an effective method to achieve the purpose of low carbon emissions,the initial gasification temperature of iron coke in CO_(2) and H_(2)O atmosphere and its cogasification reaction mechanism with coke were systematically studied.Iron coke was prepared under laboratory conditions,with a 0-7wt%iron ore powder addition.The properties of iron cokes were tested by coke reactivity index(CRI)and coke strength after reaction(CSR),and their phases and morphology were evolution discussed by scanning electron microscopy and X-ray diffraction analysis.The results indicated that the initial gasification temperature of iron coke decreased with the increase in the iron ore powder content under the CO_(2) and H_(2)O_((g))atmosphere.In the 40vol%H_(2)O+60vol%CO_(2) atmosphere,CRI of iron coke with the addition of 3wt%iron ore powder reached 58.7%,and its CSR reached 56.5%.Because of the catalytic action of iron,the reaction capacity of iron coke was greater than that of coke.As iron coke was preferentially gasified,the CRI and CSR of coke were reduced and increased,respectively,when iron coke and coke were cogasified.The results showed that the skeleton function of the coke can be protected by iron coke.

目的研究缺血再灌注(ischemia-reperfusion,IR)皮瓣不同组织层次中硫氧还蛋白-1(thioredoxin-1,Trx-1)的含量和损伤程度的差异,探讨Trx-1对特定层次的保护作用和机制。方法(1)2013年6月至2014年12月,对中国医学科学院整形外科医院收治的10例行腹壁下动脉穿支皮瓣乳房再造的女性患者进行组织标本采集,包括游离皮瓣组织(IR组,n=10)和正常皮肤组织(对照组,n=10),患者年龄(47.4±3.8)岁。(2)将20只雄性ICR小鼠采用随机数字表法分为IR皮瓣模型组和对照组,每组10只,采集2组皮瓣组织。对临床采集的组织和小鼠皮瓣组织,采用免疫组化染色和TUNEL染色检测和分析不同组织层次Trx-1表达水平以及细胞凋亡指数。(3)构建人永生化角质形成细胞(HaCaT)缺氧-复氧模型,采用蛋白质印迹法验证Trx-1对表皮细胞缺氧-复氧损伤的保护作用和机制。应用SPSS 20.0对实验结果进行分析,对定量资料2组间的比较采用配对t检验或t′检验;对蛋白表达量的组间比较进行方差分析和两两比较q检验,P0.05)。在HaCaT细胞缺氧-复氧损伤模型中,Trx-1含量较对照组明显下降(P<0.01),ASK-1、p-p38和c-PARP的表达量显著上调(P<0.01)。外源性Trx-1预处理能够显著提升缺氧-复氧细胞中的Trx-1含量(P<0.01),并显著降低p-p38和c-PARP的表达水平(P<0.01)。结论表皮基底层和真皮乳头层是皮瓣缺血再灌注早期损伤的主要组织层次,补充该层次Trx-1的含量能够通过抑制ASK-MAPK信号通路减轻皮瓣缺血再灌注损伤,有望改善游离皮瓣移植的存活率。

PRICING FOR CORPORATE PROPERTY RIGHTS TRANSFER WITH STOCHASTIC MARKETS

This paper employs the real option theory to develop a pricing model for the transfer of property rights.We list the conditions for the good,intermediate and bad firms respectively,and work out the closed-form solution to the equilibrium transfer price,the optimal transfer timing.Using the comparative static analysis,we find that for good firms the transfer price of the target is increasing in its capital.The higher the capital of the target owns,the faster it will be transferred.For intermediate and bad firms,similar conclusions can be derived.The larger gap between the acquirer＇s size and market power and those of the target,the lower the transfer triggered price.The transfer price goes up as the capital ratio of the acquirer over the target diminishes,while it is decreasing in the amount of the capital the target owns.

With the development of digital technology,the way that banks serve small-and medium-sized enterprises(SMEs)has undergone a disruptive change.Besides labor-intensive ways,banks can now serve SMEs through technologyintensive ways such as big data and artificial intelligence(AI).This has given rise to internet banks and made traditional banks face the choice of digitalization,thus igniting the technological competition in the banking sector.This paper builds a competition model among three types of banks,including small and medium internet banks with unique data advantages,large banks with economies of scale,and regional small and medium banks with comparative advantages in acquiring internal information.Then,the paper studies the digitalization strategies of different types of banks and their influence on the financing options of SMEs.The model shows that there may be a digital matching relationship between banks and SMEs in the era of AI.Large banks and small and medium internet banks have the advantage in the research and development and application of digital loan technology and serve SMEs with a more digital footprint.Regional small and medium banks have an advantage in traditional loan means and serve SMEs with a less digital footprint.This paper suggests that banks choose the transformation direction and development orientation according to their own conditions,such as bank scale,data accumulation,and internal information screening capability because this affects not only the development of banks and but also the extent to which AI can improve the financing environment for SMEs.These conclusions not only provide a theoretical basis for the decision-making mechanism of bank digitalization strategy and the applicability of AI to different types of banks but also have important policy implications for the government to better promote the digital transformation of banks and alleviate the financing constraints for SMEs.

目的探讨应用吲哚菁绿(ICG)血管造影指导带蒂横行腹直肌(TRAM)皮瓣乳房重建术中皮瓣修剪的可行性。方法采用前瞻性研究方法,分析天津医科大学肿瘤医院和郑州大学附属郑州中心医院2015年1月至2017年6月收治的64例带蒂TRAM皮瓣乳房重建患者的临床资料。使用随机数字表法将患者分为造影组(34例)和对照组(30例)。造影组采用ICG血管造影指导TRAM皮瓣修剪,对照组采用传统经验修剪皮瓣。2组患者术后12 h皮瓣血供异常发生率、皮瓣相关并发症发生率及重建乳房术后外观评价等级的比较,均采用χ~2检验,患者术后总体满意度评分比较采用t检验。结果造影组患者未出现ICG血管造影相关并发症,并且,依据造影结果切除血流灌注不良的部分后,乳房塑形满意。造影组患者术后12 h皮瓣血供异常发生率为2.9%(1/34),对照组为13.3%(4/30),2组比较,差异无统计学意义(χ~2=1.165,P=0.280);并且,造影组患者皮瓣相关并发症发生率为2.9%(1/34),对照组为10.0%(3/30),2组比较,差异也无统计学意义(χ~2=0.418,P=0.518)。术后随访1年,造影组患者乳房外观评价为非常好者占97.1%(33/34),明显高于对照组的76.7%(23/30)(χ~2=4.338,P=0.037);造影组与对照组患者总体满意度评分分别为(8.91±0.38)分和(8.47±0.57)分,2组比较,差异有统计学意义(t=3.622,P=0.001)。结论吲哚菁绿血管造影能在术中动态显示带蒂TRAM皮瓣的血供范围,对指导皮瓣修剪有一定的帮助,有助于获得较好的乳房外观。
