Emerging evidence suggests that microbial dysbiosis plays vital roles in many human ***,knowledge of whether the microbial community in thyroid tumor is related to tumorigenesis remains *** this study,we aimed to explore the microbial community in thyroid tissues and its contribution to papillary thyroid cancer(PTC).In parallel,we performed microbial profiling and transcriptome sequencing in the tumor and adjacent normal tissues of a large cohort of 340 PTC and benign thyroid nodule(BTN)*** microbial signatures were identified in PTC,BTN,and their adjacent nontumor ***-thyroid tissue bacteria were verified by means of bacteria staining,fluorescence in situ hybridization,and immunoelectron *** found that 17 bacterial taxa were differentially abundant in PTC compared with BTN,which included enrichment in PTC of the pathobionts Rhodococcus,Neisseria,Streptococcus,Halomonas,and Devosia,and depletion of the beneficial bacteria *** differentially abundant bacteria could differentiate PTC tumor tissues(PTC-T)from BTN tissues(BTN-T)with an area under the curve(AUC)of 81.66%.Microbial network analysis showed increased correlation strengths among the bacterial taxa in PTC-T in comparison with ***-corresponding bacteria(i.e.,Erwinia,Bacillus,and Acinetobacter)were found to be enriched in PTC with Hashimoto’s ***,our integrative analysis revealed that the PTC-enriched bacteria had a positive association with key PTC-oncogenic pathway-related genes,including BRAF,KRAS,IRAK4,CTNNB1,PIK3CA,MAP3K7,and *** conclusion,our results suggest that intratumor bacteria dysbiosis is associated with the thyroid tumorigenesis and oncogenic signaling pathways of PTC.

Objective： To investigate the best surgical mode for the patients of nasopharyngeal carcinoma with recurrent and persistent lymph nodes after radiotherapy. Methods： The clinical data of 88 patients of nasopharyngeal carcinoma with recurrent and persistent lymph nodes after radiotherapy were analyzed retrospectively. The levels of involved lymph nodes and the relationship among the levels were analyzed; the survival rate and recurrent rate of the surgical modes including radical neck dissection （RND）, modified radical neck dissection （MRND）, selective neck dissection （SND）, and lymph node resection （LNR） were analyzed; the role of postoperative radiotherapy was evaluated. Results：（1） The recurrent and persistent lymph nodes mainly located in level Ⅱ（55.6% and 58.6%, respectively）, next was level Ⅲ and rarely in level Ⅳ, Ⅴ, and Ⅰ, but the number of levels Ⅳ Ⅴ, and Ⅰ with cancer-bearing lymph nodes was relatively more than that of clinical measurement. （2） Patients with lymph nodes involved in level Ⅲ and Ⅳ, usually, have other levels involved simultaneously; the percentages were 63.6% and 88.9%, respectively. However, the lymph nodes in level Ⅱ and Ⅴ were mainly isolated. （3） The 5-year survival rate and recurrent rate of the whole group were 42.77% and 22.7%, respectively. （4） The 5-year survival rates of RND, MRND, SND, and LMR groups were 39.75%, 60.00%, 37.87%, and 44.10%, respectively; the differences were insignificant （Log-rank = 1.0, P = 0.8011）; the recurrent rate between the extensive and local surgery groups were insignificant （X^2 = 0.470, P = 0.493）. （5） The 5-year survival rates of the patients with and without postoperative radiotherapy were 39.06% and 45.26%, respectively; the difference was insignificant （Log-rank = 0.06, P = 0.8138）. Conclusion： The extensive surgery was recommended when the recurrent and persistent lymph nodes were more than one level involved or very large or immovable, otherwise, the SND should be performed and postoperative radi

Carbon monoxide(CO), a colorless and odorless gas, is toxic at high concentration and should cause serious hypoxia or even death of organisms. On the other hand, CO at appropriate level has various significant physiological functions and medicinal values such as anti-oxidation, anti-apoptosis, cell protection and vasodilation[1-3]. Therefore, how to control its level in vivo is a pivotal issue for its bio-applications. Iron-based carbon monoxide-releasing molecules(FeCORMs) are received arising attentions in recent years. In the present work, several iron(II) tricarbonyl triiodo complexes with a common fac-[Fe(CO) I]anion and a variety of simple cations(cation = Na, 1;K, 2;Mg, 3;Ca, 4;NH, 5) were facilely synthesized by the reaction of the precursor [Fe(CO) I] and iodo salts, Fig. 1. The structures of these complexes were clarified by series of techniques. Degradations with CO-release of them in DMSO and D2 O were determined by infrared spectroscopies. Dynamic analysis revealed that the rate of CO-release in D2 O were almost untuned by the cation, and were affected by the concentration of iodide ions. Therefore, these iron(II) tricarbonyl triiodo complexes could be explored as a promising candidates as CORMs for CO-delivery.

In recent years, the clinical incidence of thyroid cancer has been increasing year by year, and its risk assessment and clinical management methods have also been accordingly modified and constantly improved. There are great differences between the clinical diagnostic and therapeutic modes and disease management of thyroid cancer employed by various medical institutions in China, particularly with regard to the clinical application of serum marker of thyroid cancer. To this end, the China Anti-Cancer Association Thyroid Cancer Specialized Committee Chinese Association of Thyroid Oncology organized this compilation of ExpertConsensus on Clinical Application of Serum Marker of Thyroid Cancer to help and impel relevant clinical institutions and professionals to standardize clinical diagnosis, treatment, and long-term management of thyroid cancer, and to properly utilize the serum marker for scientific auxiliary clinical diagnosis and assessment of thyroid cancer before and after operation.

The Prognostic Value of Pathological and Molecular Margins Marked by p53 and eIF4E in Laryngeal Carcinoma

Objective: To study the prognostic value of the pathological margin and molecular margin marked by eIF4E and P53 protein in laryngeal carcinoma. Methods: The prognostic value of pathological and molecular margin was studied in 253 cases and 67 cases respectively, the latter were pathological negative margin chosen from the former. Immunohistochemisty was used to detect the expression of eIF4E and p53 proteins. Results: The rate of pathological, p53 and eIF4E positive margins was 20.2%, 19.4% and 32.8% respectively. The recurrent rate of those with positive margins was higher than that of negative margins, which including pathological margin (70.6% vs 35.1%, P =0.0000), p53 margin (69.2% vs 33.3%, P =0.018) and eIF4E margin (63.6% vs 28.9%, P =0.018); The survival rate of those with negative margins was higher than those with positive margins, including pathological margin (the 5-year cumulative survival rate was 37.52% and 64.37% respectively, P =0.0023), p53 margin (the 5-year cumulative survival rate was 24.62% and 75.69% respectively, P =0.0012) and eIF4E margin (the 5-year cumulative survival rate was 43.31% and 77.52% respectively, P =0.0006). Conclusion: The prognosis of those with both pathological and molecular positive margins was worse than that of the negative margins; Both the eIF4E and p53 were useful markers to pick out the poor prognostic patients from those with pathological negative margin, and the former seemed to be more potential.

目的分析应用游离股前外侧皮瓣（anterolateral thigh，ALT）修复下咽环形缺损的近期效果。方法回顾性分析2012年3月至2014年9月，设计带观察窗皮岛的游离股前外侧皮瓣修复收治于中山大学肿瘤防治中心头颈外科和汕头大学医学院附属肿瘤医院头颈外科的22例下咽环形缺损患者的病历资料。通过观察近期修复效果总结该技术的要点、优势和并发症。结果环形下咽缺损的长度7-9cm。切取的ALT皮瓣大小（8～9）cm×（11～18）cm。设计观察窗皮岛的面积（2．0～3．0）cm×（2．5-4．0）cm。移植皮瓣存活率为91％（20／22）。2例发生皮瓣坏死的患者采用带蒂胸大肌皮瓣进行二期重建。所有患者术后吻合口均通畅。59％患者（13／22）术后进食吞咽主观评价为好；41％（9／22）患者术后吞咽一般。3例（14％）患者术后出现咽瘘，其中1例咽瘘可以短期自愈，另外2例需要二期手术修补。结论带观察窗皮岛的游离股前外侧皮瓣修复下咽环形缺损稳定性高，技术相对简单，创伤小而且术后功能恢复好。

Patterns of Relapse and Management Options in Unknown Primary Cervical Metastatic Carcinoma

Objective: To analyze the patterns of relapse and management options in unknown primary cervical metastatic carcinoma (UPCMC) as well as to discuss its treatment strategies. Methods: Clinical data of 111 cases of eligible UPCMC were retrospectively studied. Neck control, primary relapse and long-term survival were compared among different groups. Results: Primary carcinomas ultimately appeared in 12 cases (10.8%, 12/111). Neck control rate was 36.9%. Overall 5-year survival rate was 41.4%. Neck control, N stage and primary control were prognostic factors for UPCMC. Neck control was affected by N stage and whether or not comprehensive radiotherapy was administered. Conclusion: UPCMC should primarily be treated by radiotherapy except for cases with N1 or N2 insensitive to radiation. Bilateral necks radiotherapy is the optimal choice in terms of neck radiotherapy. Selective total pharyngeal irradiation is recommended for potential primary treatment.

目的了解喉鳞状细胞癌(简称喉癌)声门上型微血管生成与其临床病理因素的关系,评估其微血管密度(microvessel density,MVD)对判断喉癌声门上型预后的价值。方法回顾性分析我院初治确诊并行原发灶切除的51例喉癌声门上型患者临床病理资料,选取6例声门上区非肿瘤病变作为对照组。取相应石蜡切片行CD105单克隆抗体免疫组化染色,并计数CD105染色阳性的MVD,统计分析MVD与临床病理因素及预后的关系。结果对照组的MVD中位数为2。实验组51例喉癌声门上型标本MVD中位数为9,较对照组明显增高。高MVD表达与淋巴结转移及临床晚期显著相关,P值分别为0.025和0.020。总体5年生存率为54.9%,肿瘤低分化、局部晚期、淋巴结转移及临床晚期均与5年生存率下降有关,MVD则对喉癌声门上型5年生存率无影响。结论喉癌声门上型微血管生成可能与肿瘤淋巴结转移及临床晚期相关,但尚不能说其导致患者生存率下降。
