The left has dominated the political landscape of Latin America since the beginning of the 21st *** the election of Hugo Chávez as President of Venezuela in late 1998,the cycle of“Pink Tide”began and lasted until around *** then,there has been a situation featuring the retreat of the left and the advance of the right in Latin America,which lasted only for about three *** in 2018,and with Fernández,the candidate of the left-wing coalition Front for All elected President of Argentine in 2019,there was a clear swing back to the left in the Latin American political landscape.

Objective： To validate the necessity of increasing the examined lymph node （ELN） count for enhancing the accuracy of prognostic evaluation of gastric cancer （GC） patients after curative gastrectomy in multiple medical centers of ***： The clinicopathological data of 7,620 patients who underwent the curative resection for GC between 2001 and 2011 were included to demonstrate whether the ELN count is indispensable for enhancing the accuracy of prognostic evaluation of GC patients after surgery. After a meticulous stratification by using the cut-point survival analysis, all included 7,620 patients were allocated into three groups as： less than 16 （〈16）, between 16 and 30 （16-30）, and more than 30 （〉30） ELNs. Survival differences among various subgroups of GC patients were analyzed to assess the impact of the ELN count on the stage migration in accordance with the overall survival （OS） of GC ***： Survival analyses revealed that the ELN count was positively correlated with the OS （P：0.001） and was an independent prognostic predictor （P〈0.01） of 7,620 GC patients. Stratum analysis showed that the accuracy of prognostic evaluation could be enhanced when the ELN count was no less than 16 （≥16） for node-negative patients and 〉30 for node-positive patients. Stage migrations were mainly detected in the various subgroups of patients with specific pN stages as follows： pN0 with 16-30 ELNs （pN016-30） and pN0 with 〉30 ELNs （pN0〉30）, pN0 with 〈16 ELNs （pN0〈16） and pNl〉30, pNl〈l6 and pN216_30, pNl：6_30 and pN2〉30, pN3a〈l6 and pN3b16-30, and pN3a〈16 and pN3 b〉30. These findings indicate that increasing the ELN count is a prerequisite to guarantee precisely prognostic evaluation of GC ***： The ELN count should be proposed to be 〉30 for acquiring the accurate prognostic evaluadort for GC patients, especially for node-positive patients.

Before his multi-stop visit to Mexico, Argentina, Peru, Colombia and Jamaica in Latin America and the Caribbean （LAC） early February 2018, then U.S. Secretary of State Rex Tillerson released many signals for Latin America. Especially in his remarks at the University of Texas at Austin, he portrayed the presence of China and Russia in LAC as a ＂new imperial power＂ and called on nations of Latin America and the Caribbean to review and reassess China-LAC relations. By identifying China and Russia as critical reference variables for future U.S.- Latin America relations, he obviously went beyond the bilateral level of U.S.- Latin relations and implied the important changes in U.S. policy toward LAC that pressured by growing influence of extra-regional actors in LAC, the United States might strengthen its diplomatic efforts in LAC to maintain and consolidate its dominance in the region.

Background:The prognosis of gastric cancer patients with a limited number of pathologically examined lymph nodes(eLN,<16)is dismal compared to those with adequately eLN(≥16),yet they are still classified within the same subgroups using the American Joint Committee on Cancer(AJCC)staging *** aimed at formulating an easy-to-adopt and clinically reliable stratification approach to homogenize the classification for these two categories of ***:Patients staged according to the 8th AJCC pathological nodal(N)and tumor-node-metastasis(TNM)clas-sification were stratified into a Limited and Adequate eLN cohort based on their number of pathologically examined *** statistical differences between the 5-year overall survival(OS)rates of both cohorts were determined and based on which,patients from the Limited eLN cohort were re-classified to a proposed modified nodal(N′)and TNM(TN′M)classification,by matching their survival rates with those of the Adequate eLN *** prognostic perfor-mance of the N′and TN′M classification was then compared to a formulated lymph-node-ratio-based nodal classifica-tion,in addition to the 8th AJCC N and TNM ***:Significant heterogeneous differences in 5-year OS between patients from the Limited and Adequate eLN cohort of the same nodal subgroups were identified(all P<0.001).However,no significant differences in 5-year OS were observed between the subgroups N0,N1,N2,and N3a of the Limited eLN cohort when compared with N1,N2,N3a,and N3b from the Adequate eLN cohort,respectively(P=0.853,0.476,0.114,and 0.230,respectively).A novel approach was formulated in which only patients from the Limited eLN cohort were re-classified to one higher nodal subgroup,denoted as the N′*** re-classification demonstrated superior stratifying and prognostic ability as compared to the 8th AJCC N and lymph-node-ratio classification(Akaike information criterion values[AIC]:12,276 vs.12,358 vs.12,283,respectively).Th

Objective: Conversion surgery is a surgery with a purpose of R0 resection in primary advanced gastric cancer(GC) that responded well to systemic chemotherapy. This study aimed to explore the efficacy of conversion surgery for advanced ***: A total of 618 advanced GC patients receiving systemic chemotherapy were stratified into low-,moderate-and high-risk groups based on a nomogram-predicted probability of overall survival. The survival of conversion surgery and chemotherapy alone groups was compared using the log-rank test and Cox regression analysis after propensity score matching(PSM).Results: A nomogram with good discrimination(concordance index: 0.65) and accurate calibration was constructed. After PSM, the median survival time(MST) of conversion surgery was 26.80 months, compared with16.60 months of chemotherapy alone(P<0.001). Conversion surgery was associated with a longer MST for patients in the low-risk group(30.40 months vs. 20.90 months, P=0.013), whereas it was not associated with prolonged survival in the moderate-and high-risk groups(P=0.221 and P=0.131, respectively).Conclusions: Conversion surgery was associated with longer survival, especially for low-risk population.
