Sarcopenia adversely impacts clinical outcomes in patients undergoing pancreaticoduodenectomy: A systematic review and meta-analysis

BACKGROUND Sarcopenia is a syndrome marked by a gradual and widespread reduction in skeletal muscle mass and strength,as well as a decline in functional ability,which is associated with malnutrition,hormonal changes,chronic inflammation,distur-bance of intestinal flora,and exercise *** is a com-monly employed clinical intervention for conditions such as pancreatic head cancer,ampulla of Vater cancer,and cholangiocarcinoma,among others,with a notably high rate of postoperative *** is frequent in patients undergoing ***,data regarding the effects of sarcopenia in patients undergoing pancreaticoduodenectomy(PD)are both limited and *** PubMed,Cochrane Library,Web of Science,and Embase databases were screened for studies published from the time of database inception to June 2023 that described the effects of sarcopenia on the outcomes and complications of *** researchers independently assessed the quality of the data extracted from the studies that met the inclusion ***-analysis using RevMan 5.3.5 and Stata 14.0 software was *** and funnel plots were used,respectively,to demonstrate the outcomes of the sarcopenia group vs the non-sarcopenia group after PD and to evaluate potential publication *** Sixteen studies encompassing 2381 patients were included in the *** patients in the sarcopenia group(n=833)had higher overall postoperative complication rates[odds ratio(OR)=3.42,95%confidence interval(CI):1.95-5.99,P<0.0001],higher Clavien-Dindo class≥Ⅲ major complication rates(OR=1.41,95%CI:1.04-1.90,P=0.03),higher bacteremia rates(OR=4.46,95%CI:1.42-13.98,P=0.01),higher pneumonia rates(OR=2.10,95%CI:1.34-3.27,P=0.001),higher pancreatic fistula rates(OR=1.42,95%CI:1.12-1.79,P=0.003),longer hospital stays(OR=2.86,95%CI:0.44-5.28,P=0.02),higher mortality rates(OR=3.17,95%CI:1.55-6.50,P=0.002),and worse overall survival(hazard ratio=2.81,95%CI:1.4

AIM： To investigate the potential role of perioperative nutrition in reducing complications and mortality in malnourished gastrointestinal cancer patients. METHODS： Four hundred and sixty-eight elective moderately or severely malnourished surgical patients with gastric or colorectal cancers defined by the subjective global assessment （SGA） were randomly assigned to 7 d preoperative and 7 d postoperative parenteral or enteral nutrition vs a simple control group. The nutrition regimen included 24.6±5.2 kcal /kg per d non-protein and 0.23±0.04 g nitrogen /kg per d. Control patients did not receive preoperative nutrition but received 600±100 kcal non-protein plus or not plus 62 ± 16 g crystalline amino acids postoperatively. RESULTS： Complications occurred in 18.3% of the patients receiving nutrition and in 33.5% of the control patients （P= 0.012）. Fourteen patients died in the control group and 5 in those receiving nutrition. There were significant differences in the mortality between the two groups （2.1% vs 6.0%, P=0.003）. The total length of hospitalization and postoperative stay of control patients were significantly longer （29 vs 22 d, P=0.014） than those of the studied patients （23 vs 12 d, P= 0.000）. CONCLUSION： Perioperative nutrition support is beneficial for moderately or severely malnourished gastrointestinal cancer patients and can reduce surgical complications and mortality.

To improve the plastic deformation performance of a 08AL carbon steel ultra-thin strip,a pulsed electric field was integrated into the plastic processing of the ultra-thin strip,and the effects of high-energy current on its deformation ability were ***-assisted tensile tests were employed,and the results clarified that the pulsed current could reduce the activation energy of faults and promoted dislocation slip within grains and at grain boundaries,leading to a decrease in the deformation resistance of the metal and an increase in its plastic *** the current density of 2.0 A/mm2,the yield strength,tensile strength,and elongation of the rolled sample reached 425 MPa,467 MPa,and 12.5%,*** the rolling process,it was found that the pulsed current promoted the dynamic recrystallization of the ultra-thin strip,reduced its dislocation density and deformation resistance,and promoted the coordinated deformation of the metal.

Objective To investigate the role and molecular mechanism of exosomal miR-224-5p in colorectal cancer(CRC).Methods The miR-224-5p expression in CRC patient tissues and cell-derived exosomes was measured by laser capture microdissection and qRT-PCR,***-luciferase reporter gene assay was used to determine the target gene of *** protein expressions of p53 and unc-51 like kinase 2(ULK2)in CRC cells were detected by western *** cytometry was used to detect cell cycle and *** proliferation was measured by CCK8 and EdU *** The miR-224-5p expression was upregulated in CRC tissues and increased progressively with the rise of CRC *** cells secreted extracellular miR-224-5p mainly in an exosome-dependent manner,and then miR-224-5p could be transferred to surrounding tumor cells to regulate cell proliferation in the form of autocrine or ***,ULK2 was characterized as a direct target of miR-224-5p and was downregulated in CRC ***,ULK2 inhibited CRC cell proliferation in a p53-dependent ***,exosome-derived miR-224-5p partially reversed the proliferation regulation of ULK2 on CRC *** Our findings demonstrate that exosome-transmitted miR-224-5p promotes p53-dependent cell proliferation by targeting ULK2 in CRC,which may offer promising targets for CRC prevention and therapy.

Mo element was added to cobalt-based alloy L605,and cold forging deformation was *** effects of the addition and cold forging deformation on the microstructure and mechanical properties of the alloy were studied by thermodynamic calculation,electron backscatter diffraction,transmission electron microscopy,and X-ray *** stacking fault energy(SFE)of the alloy decreased after the addition,and the formation of stacking faults and intersections were promoted to improve the strength and *** tensile strength of the alloy with Mo increased from 1190 to 1702 MPa after 24%cold deformation,producing significant work *** strengthening mechanism is strain-induced martensitic transformation(SIMT)and deformation *** alloy,combined with Mo and after 24%deformation,had both high strength and ductility in comparison with the original cobalt-based alloy *** is attributed to the lower SFE which caused the increase in stacking fault *** the tensile process,theε-hcp phase was easily generated at the stacking fault to reduce the stress concentration and increase the *** SIMT by adjusting the density of stacking faults can improve the mechanical properties of cobalt-based ***ε-hcp phase,the interaction between deformation twins and dislocations,and the interaction between e-hcp phases during cold forging deformation caused local stress concentration,lowering ductility and toughness.

This paper addresses the challenge of identifying abnormal states in Lithium-ion Battery(LiB)time series *** the energy sector increasingly focuses on integrating distributed energy resources,Virtual Power Plants(VPP)have become a vital new framework for energy *** are key in this context,owing to their high-efficiency energy storage capabilities essential for VPP ***,LiBs are prone to various abnormal states like overcharging,over-discharging,and internal short circuits,which impede power transmission *** methods for detecting such abnormalities in LiB are too broad and lack precision for the dynamic and irregular nature of LiB *** response,we introduce an innovative method:a Long Short-Term Memory(LSTM)autoencoder based on Dynamic Frequency Memory and Correlation Attention(DFMCA-LSTM-AE).This unsupervised,end-to-end approach is specifically designed for dynamically monitoring abnormal states in LiB *** method starts with a Dynamic Frequency Fourier Transform module,which dynamically captures the frequency characteristics of time series data across three scales,incorporating a memory mechanism to reduce overgeneralization of abnormal *** is followed by integrating LSTM into both the encoder and decoder,enabling the model to effectively encode and decode the temporal relationships in the time *** tests on a real-world LiB dataset demonstrate that DFMCA-LSTM-AE outperforms existing models,achieving an average Area Under the Curve(AUC)of 90.73%and an F1 score of 83.83%.These results mark significant improvements over existing models,ranging from 2.4%–45.3%for AUC and 1.6%–28.9%for F1 score,showcasing the model’s enhanced accuracy and reliability in detecting abnormal states in LiB data.
