Objective To evaluate the clinical characteristics and outcomes of patients with Gleason score 10 prostate cancer treated by external radiotherapy and hormone therapy. Methods From January 2003 to March 2014, 1832 patients with prostate cancer were treated, among which 9 patients(represented 0.49%) were identified as Gleason score 10 disease on prostate core biopsy without distant metastases when first diagnosed. All 9 patients were treated by whole pelvic external radiotherapy(The whole pelvic dose was 50.0 Gy and the boost dose ranged from 76.2 to 78.0 Gy) and long-term hormone therapy. We assessed the clinical characteristics, treatment outcomes and treatment toxicities. Survival curves were calculated using the Kaplan-Meier method. Results The median follow-up was 4.8 years. Six patients' pre-treatment prostate-specific antigen(PSA) levels were lower than 20.0 μg/L and three patients' pre-treatment PSA levels were higher than 70.0 μg/L. The median percentage of positive biopsy cores was 91%. Three, four and two cases were classified as T2 c, T3 a and T3 b stage, respectively. Three cases were assessed as N1 stage. The 5-year biochemical failure-free survival, distant metastasis-free survival, cancer specific survival and overall survival rates were 28.6%, 57.1%, 66.7% and 57.1%, respectively. Five patients experienced grade 1-2 acute gastrointestinal toxicities and six patients complained of grade 1-2 acute genitourinary toxicities. No bone fracture or cardiovascular disease was detected. Conclusions Gleason score 10 prostate cancer on core biopsy is usually combined with other high risk factors. The pre-treatment PSA levels lie in two extremes. Timely and active treatments are urgent needed because unfavourable oncological outcomes are often presented.

Hydrogels with precisely designed structures represent promising materials with a broad application spectrum, such as for sensor, tissue engineering and biomimetic technology. However, with highly reactive compounds, the preparation of hydrogels still needs an efficient approach for desired distribution of each component within hydrogels. In addition, a method for in situ preparation of gradient hydrogels is still lacking. Herein, we report the formation ofhydrogels with either uniform or gradient internal structures via a novel, simple but very efficient method by aerating ammonia gas （NH3 gas） into the solution of dialdehyde cellulose （DAC） and a diamine. As-prepared hydrogels exhibited uniform microscopic and chemical structure or gradient distribution Of functional groups. Due to lots of aldehyde groups on DAC chains, functional hydrogels can be prepared by using diverse diamines. For instance, hydrogels prepared by using 1,6-hexanediamine as a cross-linker were responsive to pH values. Moreover, this controllable process of aerating NH3 gas allows the in situ formation of gradient hydrogels; for instance, by using cyanamide as a reaction counterpart, gradient hydrogels with gradient distributions of cyanide groups were prepared.

San-Huang chicken is a high-quality breed in China with yellow feather, claw and break. However, the abnormal phenomenon of the yellow shank turning into green shank of San-Huang chicken has been a concern, as it seriously reduces the carcass quality and economic benefit of yellow-feathered broilers. In this study, the cause of this abnormal green skin in shank was systematically investigated. Physiological anatomy revealed that the abnormal skin in shank was primarily due to the deposition of melanin under the dermis. After analyzing multiple potential causes such as heredity(pedigree and genetic markers), environment(water quality monitoring) and feed composition(mycotoxin detection), excessive aflatoxin B1(AFB1) in feed was screened, accompanied with a higher L-dihydroxy-phenylalanine(L-DOPA)(Pmain cause of abnormal green skin in shank. Subsequently, the further results showed that a high concentration of AFB1(>170 μg kg–1)indeed induced the abnormal green skin in shank compared to the normal AFB1 content(<10 μg kg–1), and the mRNA levels of TYR, TYRP1, MITE, MC1R and EDN3 genes related to melanin deposition would significantly up-regulate(P<0.01) and the content and activity of tyrosinase(TyR) significantly increased(P<0.05). At the same time, the content of L-DOPA and melanin deposition also increased significantly(P<0.01), which also confirmed the effect of excessive AFB1 on melanin deposition in skin of shank. Results of additional experiments revealed that the AFB1's negative effect on melanin deposition in skin of shank could last for a longer time. Taken together, the results of this study explained the occurrence and possible mechanisms of the abnormal AFB1-related green skin in shank of chickens. Fragility curves are commonly used in civil engineering to assess the vulnerability of structures to earthquakes. The probability of failure associated with a prescribed criterion （e.g., the maximal inter-storey drift of a building exceeding a certain threshold） is represented as a function of the intensity of the earthquake ground motion （e.g., peak ground acceleration or spectral acceleration）. The classical approach relies on assuming a lognormal shape of the fragility curves; it is thus parametric. In this paper, we introduce two non-parametric approaches to establish the fragility curves without employing the above assumption, namely binned Monte Carlo simulation and kernel density estimation. As an illustration, we compute the fragility curves for a three-storey steel frame using a large number of synthetic ground motions. The curves obtained with the non-parametric approaches are compared with respective curves based on the lognormal assumption. A similar comparison is presented for a case when a limited number of recorded ground motions is available. It is found that the accuracy of the lognormal curves depends on the ground motion intensity measure, the failure criterion and most importantly, on the employed method for estimating the parameters of the lognormal shape.

Patients carrying mutations in polymerase epsilon/polymerase delta have shown positive responses to immune checkpoint ***,prospective trials exploring the efficacy in those with polymerase epsilon/polymerase delta mutations are still lacking.A phase II clinical trial was initiated to evaluate the efficacy of toripalimab,a humanized IgG4K monoclonal antibody to human PD-1,in patients with advanced solid tumors with unselected polymerase epsilon/polymerase delta mutations but without microsatellite instability-high.A total of 15 patients were enrolled,14 of whom were assessed for treatment *** was a 21.4%overall response rate,with a disease control rate of 57.1%.The median overall survival and median progression-free survival were 17.9(95%CI 13.5-not reach)months and 2.5(95%CI 1.4-not reach)months,*** patients with exonuclease domain mutations,the objective response rate was 66.7%(2/3),with a disease control rate of 66.7%(2/3).For those with non-exonuclease domain mutations,the rates were 9.1%(1/11)and 54.5%(6/11),***,patients with PBRM1 gene mutations exhibited a high response rate to toripalimab at 75.0%(3/4).This study showed that neither the exonuclease domain mutations nor non-exonuclease domain mutations could fully predict the efficacy of immunotherapy,urging the need for more investigations to clarify potential immune sensitization differences within polymerase epsilon/polymerase delta mutation variants.
