Uterus-reserved or hysterectomized total pelvic floor reconstruction for female pelvic dysfunction:a clinical analysis of 74 cases

Objective To evaluate the effectiveness and safety of the entire pelvic floor reconstruction(Prolift) with uterus reserved in the treatment of pelvic floor dysfunction *** From March 2008 to January 2009,74 female cases from our hospital who had different defects in pelvic organ prolapse were treated with the entire reconstructive pelvic surgery,in which 38 had uterus retained(observation group) and 36 cases had hysterectomy(control group).The two groups were *** patients with combined stress urinary incontinence had transobturator tension-free vaginal tape(TVT-O technique) at the same *** results of operations were objectively evaluatedaccording to Pelvic Organ Prolapse Quantification sub-degree method(POP-Q) developed by the International Continence *** For observation group,the average operative time was 50 min,and the average amount of bleeding was 100 *** control group,the average operative time was 110 min,and the average amount of bleeding was 200 *** the postoperative follow-up(8 ~18 months),in the observation group the structures of patients' pelvic floor were normal;stress urinary incontinence was all cured;related symptoms disappeared or were markedly improved;and no postoperative infection *** control group,two cases had postoperative infection;the patients ' pelvic structures were normal during the postoperative follow-up;the related symptoms disappeared or were significantly *** 3 months,POP-Q score was significantly elevated in the two *** Full reconstructive pelvic surgery with uterus retained can complete the whole pelvic floor structure and function of all or part of the reconstruction with fast recovery and clear short-term ***,the long-term efficacy remains to be revealed.

Background:Fecal incontinence(FI)has been shown to be a common symptom in Western countries;however,there is few researches focusing on its epidemic condition in Chinese *** conducted this national population-based epidemiology study to estimate the prevalence and risk factors of FI among adult Chinese women living in urban ***:This is a subgroup analysis of a national population-based epidemiology study of *** 28,196 adult women from urban regions of six provinces and municipalities participated in this research from 2014 to *** finished the questionnaire under the direction of trained *** was defined as accidental leakage of flatus and/or liquid or solid stool at least once in the *** FI prevalence trend and risk factors were identified by the Cochran-Armitage test,Chi-square test,and multivariable logistic ***:The prevalence of FI in adult females in urban China was 0.43%(95%confidence interval:0.35%-0.51%).Among women with FI,42.96%,82.96%,and 42.22%reported having leakage of solid,liquid stool,and gas,*** overall FI prevalence and the incidence rate of solid stool/liquid stool/gas leakage increased with *** mean Wexner score was 4.0%and 12.0%FI patients reported Wexner score≥*** mass index≥24 kg/m2,pelvic organ prolapses,chronic constipation,chronic cough,alcohol consumption,physical diseases including chronic bronchitis and cancer,gynecological diseases like gynecological inflammation are risk factors for *** delivery was the risk factor for FI in females with labor ***:FI was not a common symptom in adult Chinese women living in urban areas and there were some potential modifiable risk factors.

Asperuloside Promotes Apoptosis of Cervical Cancer Cells through Endoplasmic Reticulum Stress-Mitochondrial Pathway

Objective: To investigate the effects of asperuloside on cervical cancer based on endoplasmic reticulum(ER) stress and mitochondrial pathway. Methods: Different doses(12.5–800 μg/mL) of asperuloside were used to treat cervical cancer cell lines Hela and CaSki to calculate the half maximal inhibitory concentration(IC_(50)) of asperuloside. The cell proliferation was analyzed by clone formation assay. Cell apoptosis, intracellular reactive oxygen species(ROS) and mitochondrial membrane potential were determined by flow cytometry. The protein expressions of cleaved-caspase-3, Bcl-2, Bax, Cyt-c, cleaved-caspase-4 and glucose-regulated protein 78(GRP78) were analyzed by Western blot. And the inhibitor of ER stress, 4-phenyl butyric acid(4-PBA) was used to treat cervical cancer cells to further verify the role of ER stress in the apoptosis of cervical cancer cells induced by asperuloside. Results: Asperuloside of 325, 650, and 1300 μg/mL significantly inhibited the proliferation and promoted apoptosis of Hela and CaSki cells(P<0.01). All doses of asperuloside significantly increased intracellular ROS levels, reduced mitochondrial membrane potential, significantly reduced Bcl-2 protein expression level, and increased Bax, Cyt-c, GRP78 and cleaved-caspase-4 expressions(P<0.01). In addition, 10 mmol/L 4-PBA treatment significantly promoted cell proliferation and reduced apoptosis(P<0.05), and 650 μg/mL asperuloside could reverse 4-PBA-induced increased cell proliferation, decreased apoptosis and cleaved-caspase-3,-4 and GRP78 protein expressions(P<0.05). Conclusion: Our study revealed the role of asperuloside in cervical cancer, suggesting that asperuloside promotes apoptosis of cervical cancer cells through ER stress-mitochondrial pathway.

Spin-crossover(SCO)materials that reversibly switch between high-and low-spin states have potential for the storage of spin state-relative information,and have gained much attention incorporating secondary physical properties,such as fluorescence and magneto-optical *** this study,we synthesized three octanuclear metal-organic cages(MOCs)using tetraphenylethylene-based luminophores,aldehydes,and Fe^(Ⅱ)salts,by subcomponent self-assembly approach,namely[Fe1]-[Fe3].By controlling the ligand-field strength and guest encapsulation,we finely tuned their SCO *** them,MOC[Fe2]displayed nearly complete SCO behavior in the solid state,which is rare for high-nuclearity *** also demonstrated the coupling of SCO with fluorescence emission in these MOCs by using isostructural Zn^(Ⅱ)complexes([Zn1]-[Zn3])as control experiments,for the first *** calculations revealed the energy-transfer mechanism between fluorophores and SCOactive centers,which emphasizes the significant contribution of d-d transitions in the interplay between the occurrence of SCO and fluorescence emission.

Currentless Multiple Switch Open-Circuit Faults Diagnosis for Modular Multilevel Converters with Nearest Level Modulation in HVDC Systems

Due to the large number of submodules(SMs),and modular multilevel converters(MMCs)in high-voltage applications,they are usually regulated by the nearest level modulation(NLM).Moreover,the large number of SMs causes a challenge for the fault diagnosis strategy(FDS).This paper proposes a currentless FDS for MMC with *** FDS,the voltage sensor is relocated to measure the output voltage of the *** acquire the capacitor voltage and avoid increasing extra sensors,a capacitor voltage calculation method is *** on the measurement of output voltages,the faults can be detected and the number of different-type switch open-circuit faults can be confirmed from the numerous SMs in an arm,which narrows the scope of fault ***,the faulty SMs and faulty switches in these SMs are further located without arm current according to the sorting of capacitor voltages in the voltage balancing *** FDS is independent of the arm current,which can reduce the communication cost in the hierarchical control system of ***,the proposed FDS not only simplifies the identification of switch open-circuit faults by confirming the scope of faults,but also detects and locates multiple different-type faults in an *** effectiveness of the proposed strategy is verified by the simulation results.

Constructing controllable thermal conduction networks is the key to efficiently improve thermal conductivities of polymer *** this work,graphite oxide(GO)and functionalized carbon nanotubes(f-CNTs)are combined to prepare“Line-Plane”-like hetero-structured thermally conductive GO@f-CNTs fillers,which are then performed to construct controllable 3D GO@f-CNTs thermal conduction networks via selfsacrificing template method based on oxalic ***,thermally conductive GO@f-CNTs/polydimethylsiloxane(PDMS)composites are fabricated via casting *** the size of oxalic acid is 0.24 mm and the volume fraction of GO@f-CNTs is 60 vol%,GO@f-CNTs/PDMS composites present the optimal thermal conductivity coefficient(λ,4.00 W·m^(-1)·K^(-1)),about 20 times that of theλof neat PDMS(0.20 W·m^(-1)·K^(-1)),also much higher than theλ(2.44 W·m^(-1)·K^(-1))of GO/f-CNTs/PDMS composites with the same amount of randomly dispersed ***,the obtained GO@f-CNTs/PDMS composites have excellent thermal stability,whoseλdeviation is only about 3%after 500 thermal cycles(20-200℃).

Effects of the Rainfall-Triggered Lisse Effect on the Stability of Loess Slopes

This paper coupled a water-air two-phase hydrodynamic(WATPH)model with the Iverson’s method to analyze the influence of the Lisse effect on the fast groundwater pressure(P_(w))response and the slope ***,the sensitivities of the driving force and loess soil parameters were *** showed that the WATPH model simulated the height and rise of the depth to the water table reasonably *** depth to water table before rainfall(H0)had a significant impact on the Lisse effect and the slope *** the H_(0) was less than approximately 1 m,the rainfall triggered a significant Lisse effect and decreased the slope factor of safety(F_(s)).When the rainfall intensity(R_(i))was higher than the saturated hydraulic conductivity(K_(s)),the Lisse effect and the F_(s) slightly changed with the increase of the R_(i),and the slope tended to be unstable with continuous *** increasing K_(s),the Lisse effect noticeably increased,and the minimum F_(s) quickly *** analysis of the normalized sensitivity coefficient revealed that H_(0) had a dramatic impact on the Lisse effect and loess slope *** different R_(i) and K_(s) values had prominent influences on the Lisse effect and slight impacts on F_(s).

High-Order Two-Scale Asymptotic Paradigm for the Elastodynamic Homogenization of Periodic Composites

The classical two-scale asymptotic paradigm provides macroscopic and microscopic analyses for the elastodynamic homogenization of periodic composites based on the spatial or/and temporal variable,which offers an approximate framework for the asymptotic homogenization analysis of the motion ***,in this framework,the growing complexity of the homogenization formulation gradually becomes an obstacle as the asymptotic order *** such a context,a compact,fast,and accurate asymptotic paradigm is *** work reviews the high-order spatial two-scale asymptotic paradigm with the effective displacement field representation and optimizes the implementation by symmetrizing the tensor to be ***,the modified implementation gets rid of the excessive memory consumption required for computing the high-order tensor,which is demonstrated by representative one-and two-dimensional *** numerical results show that(1)the contrast of the material parameters between media in composites directly affects the convergence rate of the asymptotic results for the homogenization of periodic composites,(2)the convergence error of the asymptotic results mainly comes from the truncation error of the modified asymptotic homogenized motion equation,and(3)the excessive norm of the normalized wavenumber vector in the two-dimensional inclusion case may lead to a non-convergence of the asymptotic results.

DEM investigation of strength and critical state behaviours of granular materials under true triaxial loadings

This paper investigates the strength and critical state(CS)behaviours of granular materials via DEM simulations of true triaxial drained tests under three different loading modes including constant b(in-termediate stress ratio)tests with constantσ'1(major principal stress),constant p(mean pressure)and constantσ'3(minor principal stress)*** this end,a series of samples are generated with the same particle size distribution,and with the confining stresses ranging from 100 kPa to 900 *** CS is achieved for all *** the macroscopic behaviours and the microscopic behaviours are examined and compared considering different loading modes,confining stresses and intermediate stress ratios(b).The critical state lines(CSLs)are found to be unique and independent of the loading modes,but dependent on the b *** CSLs with b=0 and b=1 form the two boundaries of CSLs respectively beyond which CSLs under all other b tests cannot go *** different strength criteria are examined and compared in terms of both peak and CS *** Mohr-Coulomb strength criterion is found to be only suitable for axisymmetric loading *** Lade-Duncan criterion is only suitable for describing peak strengths,which is dependent on the loading modes and confining *** Satake criterion and Matsuoka-Nakai criterion are the more appropriate strength criteria for describing CS failures,indicating that the CS values of both the Satake parameter and the Matsuoka-Nakai parameter describe an inherent property that characterizes the CS failure for a given type of *** CS mechanical coordination number is fitted by a curved line for a given b value,which is unique regardless of the loading *** peak and CS values of both major and minor principal fabric tensors decrease with increasing b values,while they increase with increasing b values for the intermediate principal fabric tensor.
