病案首页信息囊括了患者住院期间整个诊疗过程，是实施DRGs付费的关键部分。新医改以来，医保DRGs支付已广泛应用于我国大部分的大型综合性医院和各种医疗机构，并逐渐在全国进行推广，在此背景下，如何提高住院病案首页内涵质量是当前医院管理部门所面临的首要问题。因此，本文就病案首页常见缺陷问题、对DRGs分组造成的影响、病案首页控制方法研究进展作一综述，期待医疗机构结合医院实际情况，制定一套行之有效的质控方式，保证中医住院病案首页的完整性和准确性，达到在减轻临床医生负担的同时提高中医病案首页质量，为医疗改革和医院管理奠定坚实的数据基础。

目的分析自拟建中补肾消癥汤联合达格列净对糖尿病肾病患者的影响。方法吉安市万安县中医院共计收治98例糖尿病肾病患者,时间为2020年1月—2022年1月,周期为2年。将98例患者随机等分为对照组与研究组,各49例。对照组采用达格列净治疗,研究组在对照组的基础上采取自拟建中补肾消癥汤治疗,对比2组患者临床疗效,治疗前后中医证候评分及血糖、肾功能指标水平。结果研究组总有效率98.0%(48/49)高于对照组的81.6%(40/49)(P<0.05)。研究组中医证候评分显著低于对照组(P<0.05)。治疗后,研究组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于对照组(P<0.05)。肾功能指标方面,治疗后,研究组血肌酐、尿蛋白排泄率、24 h尿蛋白定量均低于对照组(P<0.05)。结论自拟建中补肾消癥汤联合达格列净治疗糖尿病肾病患者,与西医治疗相比效果更具有独特性,值得应用。

蛋白质糖基化是最常见的蛋白质翻译后修饰(post-translational modification,PTM)之一,其广泛存在于生命体中。真核细胞中,糖基化修饰对蛋白质的折叠、构象、分布、稳定性和活性具有重要的影响。研究表明,糖蛋白的糖链对于维持多细胞生物所有分化细胞间相互作用的秩序至关重要。蛋白质糖基化的功能障碍可能导致疾病的发生和感染性疾病的发展。迄今已有许多蛋白质的N/O-聚糖改变被确定为肿瘤和某些传染性疾病发展的生物标志物。因此,本文针对B细胞受体(B-cell receptor,BCR)、T细胞受体(T-cell receptor,TCR)、细胞因子(cytokines,CK)、补体(complement)和免疫球蛋白(immunoglobulin,Ig)等主要的免疫分子的糖基化修饰,以及相关糖基化修饰与传染性疾病之间的关系进行综述,旨在阐释免疫分子的糖基化与传染性疾病之间的关联,为治疗感染性疾病提供新思路和策略。

BACKGROUND Enhanced magnetic resonance imaging(MRI)is widely used in the diagnosis,treatment and prognosis of hepatocellular carcinoma(HCC),but it can not effectively reflect the heterogeneity within the tumor and evaluate the effect after *** imaging analysis of voxel changes can effectively reflect the internal heterogeneity of the tumor and evaluate the progression-free survival(PFS).AIM To predict the PFS of patients with HCC before operation by building a model with enhanced MRI *** Delineate the regions of interest(ROI)in arterial phase,portal venous phase and delayed phase of enhanced *** extracting the combinatorial features of ROI,the features are fused to obtain deep learning radiomics(DLR)_***'s test was used to evaluate the diagnostic performance of different typological features.K-M analysis was applied to assess PFS in different risk groups,and the discriminative ability of the model was evaluated using the *** Tumor diameter and diolame were independent factors influencing the prognosis of ***'s test revealed multi-phase combined radiomic features had significantly greater area under the curve values than did those of the individual phases(P<0.05).In deep transfer learning(DTL)and DLR,significant differences were observed between the multi-phase and individual phases feature sets(P<0.05).K-M survival analysis revealed a median survival time of high risk group and low risk group was 12.8 and 14.2 months,respectively,and the predicted probabilities of 6 months,1 year and 2 years were 92%,60%,40%and 98%,90%,73%,*** C-index was 0.764,indicating relatively good consistency between the predicted and observed *** and DLR have higher predictive value for 2-year PFS in *** Based on the multi-temporal characteristics of enhanced MRI and the constructed Nomograph,it provides a new strategy for predicting the PFS of transarterial chemoembolization treatment of HCC.

Physical and numerical investigation on fluid flow and inclusion removal behavior in a single-strand tundish

Aiming at the problem that the existence of inclusions in the tundish continuous casting process can easily lead to quality defects of the slab,the stainless steel continuous casting tundish was taken as the research *** effects of flow control device,inclusion density and inclusion size on the mixing characteristics of molten steel and inclusion behavior in tundish were *** results showed that compared with the tundish without flow control device,the average residence time of molten steel was prolonged by about 49 s,the dead zone volume fraction was reduced by 8.93%,and the piston fluid integral rate was increased by 12.68%.In the turbulence inhibitor(TI)tundish with weir-dam combination,the removal rate of inclusions with a density of 2700 kg m^(-3) and a particle size of 5 lm is 63.32%,while the removal rate of large inclusions with a density of 150μm could reach 89.04%.When the inclusion particle size was 10-50μm and the density was 2700-4500 kg m^(-3),the effect of inclusion density on inclusion removal rate was *** the same time,when weir-dam combination TI tundish was set,the inclusions were mainly limited to the slag-metal interface of the first and second chambers of the *** removal rate of inclusions in the first chamber was generally improved,with 10μm inclusions accounting for 47.67% and 150μm inclusions accounting for 60.69%.Furthermore,it has the best effect on the removal of small-size inclusions,especially those less than 70μm.

The effect of LaNi on thermal storage properties of MgH2 prepared by ball milling under hydrogen atmosphere was *** thermal storage properties,cyclic property and thermal storage mechanism were studied by pres sure-composition-temperature(PC T),X-ray diffraction(XRD)and transmission electron microscopy(TEM).The Van't Hoff curve indicates that the formation enthalpy of Mg-16 wt%LaNi is 74.62 kJ·mol^(-1),which approaches to the theoretical values of *** isothermal measurement indicates that Mg-16 wt%LaNi can absorb 6.263 wt%H_(2)within 30 min at 390℃for the first absorption,the absorption reaction fraction within2 min is over 90.00%,and the desorption reaction fraction within 2 min is 72.63%,increasing by 55.36%compared with that of ***-16 wt%LaNi has better cyclic stability than that of Mg,only decreasing by 0.609 wt%after *** enhancement in thermal storage performances of Mg by adding LaNi is mainly ascribed to the formed Mg_(2)NiH_(4),H_(0.3)Mg_(2)Ni and La_(4)H_(12.19)during the cyclic process which act as catalysts and inhibit the growth of *** above results prove that Mg-16 wt%LaNi is suitable for use as a heat storage material.

Plant hormone abscisic acid(ABA)plays an indispensable role in the control of leaf senescence,during which ABA signaling depends on its ***,the role of ABA transport in leaf senescence remains ***,we identified two novel RING-box protein-encoding genes UBIQUITIN LIGASE of SENESCENCE 1 and2(ULS1 and ULS2)involved in leaf *** of ULS1 and ULS2 accelerates leaf senescence,which is specifically promoted by ABA ***,the expression of senescence-related genes is significantly affected in mature leaves of uls1/uls2 double mutant(versus wild type(WT))in an ABA-dependent manner,and the ABA content is substantially ***1 and ULS2 are mainly expressed in the guard cells and aging leaves,and the expression is induced by *** RNA-seq and quantitative proteomics of ubiquitination reveal that ABA transporter ABCG40 is highly expressed in uls1/uls2 mutant versus WT,though it is not the direct target of ULS1/***,we show that the acceleration of leaf senescence,the increase of leaf ABA content,and the promotion of stomatal closure in uls1/usl2 mutant are suppressed by abcg40 loss-of-function *** results indicate that ULS1 and ULS2 function in feedback inhibition of ABCG40-dependent ABA transport during ABA-induced leaf senescence and stomatal closure.

The near future has been envisioned as a collaboration of humans with mobile robots to help in the day-to-day *** this paper,we present a viable approach for a real-time computer vision based object detection and recognition for efficient indoor navigation of a mobile *** mobile robotic systems are utilized mainly for home assistance,emergency services and surveillance,in which critical action needs to be taken within a fraction of second or *** object detection and recognition is enhanced with utilization of the proposed algorithm based on the modification of You Look Only Once(YOLO)algorithm,with lesser computational requirements and relatively smaller weight size of the network *** proposed computer-vision based algorithm has been compared with the other conventional object detection/recognition algorithms,in terms of mean Average Precision(mAP)score,mean inference time,weight size and false positive *** presented framework also makes use of the result of efficient object detection/recognition,to aid the mobile robot navigate in an indoor environment with the utilization of the results produced by the proposed *** presented framework can be further utilized for a wide variety of applications involving indoor navigation robots for different services.

目的了解2020—2022年青岛市手足口病病原学特征和柯萨奇病毒A6型(coxsackievirus A6, CVA6)的VP1基因进化特征, 为手足口病的防控工作提供依据。方法利用荧光RT-PCR对青岛地区2020—2022年的手足口病患者咽拭子样本进行总肠道病毒(EV)、肠道病毒71型(EV71)、CVA16、CVA6和CVA10检测, 并对CVA6阳性分离株进行VP1基因全长序列扩增及基因序列测定, 以MEGA-X软件进行系统发育及分子特征分析。结果 1 645份手足口病患者样本中肠道病毒总阳性率为80.73%(1 328/1 645)。EV71、CVA16、CVA6、CVA10及其他肠道病毒构成比分别为0.08%(1/1 328)、19.65%(261/1 328)、68.15%(905/1 328)、0.98%(13/1 328)、11.14%(148/1 328), 5岁以下儿童占全部病例的75.68%(1 245/1 645), 占重症病例的100%(2/2)。获得的45株CVA6 VP1区基因测序绘制了基因树, 属于D3a亚型, 基因进化分析其VP1区蛋白质发生了10处氨基酸的非同义突变。结论 2020—2022年青岛地区手足口病以CVA6 D3a亚型为主要流行株, 且在5岁以下高发。
