Efficacy and Safety of Bufei Jiedu Granules in Treating Multidrug-Resistant Pulmonary Tuberculosis:A Multi-center,Double-Blinded and Randomized Controlled Trial

Objective: To assess the efficacy and safety of Bufei Jiedu(BFJD) Granules as adjuvant therapy for patients with multidrug-resistant pulmonary tuberculosis(MDR-PTB). Methods: A large-scale, multi-center, double-blinded, and randomized controlled trial was conducted in 18 sentinel hospitals in China from December 2012 to December 2016. A total of 312 MDR-PTB patients were randomly assigned to BFJD Granules or placebo groups(1:1) using a stratified randomization method, which both received the long-course chemotherapy regimen for 18 months(6 Am-Lfx-P-Z-Pto, 12 Lfx-P-Z-Pto). Meanwhile, patients in both groups also received BFJD Granules or placebo twice a day for a total of 18 months, respectively. The primary outcome was cure rate. The secondary outcomes included time to sputum-culture conversion, changes in lung cavities and quality of life(QoL) of patients. Adverse reactions were monitored during and after the trial. Results: A total of 216 cases completed the trial, 111 in the BFJD Granules group and 105 in the placebo group. BFJD Granules, as adjuvant treatment, increased the cure rate by 13.6% at the end of treatment, compared with the placebo(58.4% vs. 44.8%, P=0.02), and accelerated the median time to sputum-culture conversion(5 months vs. 11 months). The cavity closure rate of the BFJD Granules group(50.6%, 43/85) was higher than that of the placebo group(32.1%, 26/81;P=0.02) in patients who completed the treatment. At the end of the intensive treatment, according to the 36-item Short Form, the BFJD Granules significantly improved physical functioning, general health, and vitality of patients relative to the placebo group(all P<0.01). Overall, the death rates in the two groups were not significantly different;5.1%(8/156) in the BFJD Granules group and 2.6%(4/156) in the placebo group. Siliceous diatoms are one of the most prominent actors in the oceans,and they account for approximately 40%of the primary production and particulate organic carbon export *** is believed that changes in carbon flux caused by variations in diatom distribution can lead to significant climate *** the fundamental pathways of diatom-driven carbon sequestration have long been established,there are no reports of CaCO_(3) precipitation induced by marine diatom *** manuscript introduces novel details regarding the enhancement of aragonite precipitation during photosynthesis in Skeletonema costatum in both artificial and natural *** direct measurements of cell surfaces via a pH microelectrode and zeta potential analyzer,it was determined that the diatom-mediated promotion of CaCO_(3) precipitation is achieved through the creation of specific microenvironments with concentrated[CO_(3)^(2-)]and[Ca^(2+)]and/or the dehydrating effect of adsorbed Ca^(2+).Based on this mechanism,it is highly plausible that diatom-mediated calcification could occur in the oceans,an assertion that was supported by the significant deviation of total alkalinity(TA)from the conservative TA-salinity mixing line during a Skeletonema costatum bloom in the East China Sea and other similar *** newly discovered calcification pathway establishes a link between particulate inorganic and organic carbon flux and thus helps in the reassessment of marine carbon export fluxes and CO_(2) sequestration *** discovery may have important ramifications for assessing marine carbon cycling and predicting the potential effects of future ocean acidification.

The appropriate terminal attitude of missile is important to improve the damage effectiveness of fragmentation warhead against aircraft *** this paper,three missile terminal attitude selection methods are proposed to solve the problem of terminal attitude selection in different situations according to their respective evaluation *** MVE-based method uses the damage probability of each detonation point around the aircraft at a given attitude of missile to calculate the Mean Volume of Effectiveness(MVE)at the corresponding attitude,and then uses the MVE for different attitudes to select the terminal strike *** detonation position-based method addresses the case where the missile detonation position can be assessed in *** the effects of missile guidance errors and fuze activation position errors,Monte Carlo simulations are used to calculate the damage probability of aircraft with different strike attitudes,from which the terminal strike attitude is *** BP-ANN model-based method uses the constructed Back Propagation Artificial Neural Network(BP-ANN)model instead of simulation to calculate the evaluation indexes in the corresponding cases of MVE-based method and Detonation position-based method,which can improve the efficiency of attitude *** are conducted for different scenarios to verify the feasibility and effectiveness of the proposed method.

Numerical insight into heat transfer enhancement by granules inner-migration in a moving bed:A performance comparison between slanted-stick and plow-shaped agitator

Waste heat recovery of high-temperature granules is one of the most promising sustainable energy supply and carbon reduction ways for industry.A moving bed indirect heat exchanger(MBIHE)with inner-migration was proposed for granular heat *** migration and the enhanced heat transfer induced by two types of agitators(i.e.,agitator with slanted stick A_(s)and with plow-shaped surface A_(p))in the MBIHE are analyzed based on DEM coupled with *** to the effective agitation,the average heat transfer coefficient in the granule side is enhanced to∼3 times compared to that without *** heat recovery efficiency in the moving bed reached more than 70%with the agitations of either A_(s)or A_(p).The heat efficiency of A_(s)is∼7%higher than that of A_(p),but with at least 60%greater rotational *** ensure reliable agitation,the A_(p)is suggested to be adopted in the MBIHE to induce granule migration.

The "International Standard of Traditional Chinese Medicine Techniques: The Operating Specifications for Chinese Medicine Enema is drawn up by the Beijing Hospital of Traditional Chinese Medicine affiliated to Capital Medical University and Beijing University of Chinese Medicine Xiamen Hospital in collaboration with domestic TCM universities and hospitals. The specification includes definition, operating process, points for attention and contraindications. It is targeted to provide reference for TCM providers at home and abroad with TCM background in clinical decision-making.

Long-term Evolution of Solar Activity and Prediction of the Following Solar Cycles

Solar activities have a great impact on modern high-tech systems,such as human aerospace activities,satellite communication and navigation,deep space exploration,and related scientific ***,studying the long-term evolution trend of solar activity and accurately predicting the future solar cycles are highly *** on the wavelet transform and empirical function fitting of the longest recorded data of the annual average relative sunspot number(ASN)series of 323 yr to date,this work decisively verifies the existence of the solar century cycles and confirms that its length is about 104.0 yr,and the magnitude has a slightly increasing trend on the timescale of several hundred *** on this long-term evolutionary trend,we predict solar cycles 25 and26 by using phase similar prediction *** for solar cycle 25,its maximum ASN will be about146.7±33.40,obviously stronger than solar cycle *** peak year will occur approximately in 2024,and the period will be about 11±1 *** for solar cycle 26,it will start around 2030,and reach its maximum between2035 and 2036,with maximum ASN of about 133.0±3.200,and period of about 10 yr.
