Efficacy and Safety of Bufei Jiedu Granules in Treating Multidrug-Resistant Pulmonary Tuberculosis: A Multi-center, Double-Blinded and Randomized Controlled Trial

Objective: To assess the efficacy and safety of Bufei Jiedu(BFJD) Granules as adjuvant therapy for patients with multidrug-resistant pulmonary tuberculosis(MDR-PTB). Methods: A large-scale, multi-center, double-blinded, and randomized controlled trial was conducted in 18 sentinel hospitals in China from December 2012 to December 2016. A total of 312 MDR-PTB patients were randomly assigned to BFJD Granules or placebo groups(1:1) using a stratified randomization method, which both received the long-course chemotherapy regimen for 18 months(6 Am-Lfx-P-Z-Pto, 12 Lfx-P-Z-Pto). Meanwhile, patients in both groups also received BFJD Granules or placebo twice a day for a total of 18 months, respectively. The primary outcome was cure rate. The secondary outcomes included time to sputum-culture conversion, changes in lung cavities and quality of life(QoL) of patients. Adverse reactions were monitored during and after the trial. Results: A total of 216 cases completed the trial, 111 in the BFJD Granules group and 105 in the placebo group. BFJD Granules, as adjuvant treatment, increased the cure rate by 13.6% at the end of treatment, compared with the placebo(58.4% vs. 44.8%, P=0.02), and accelerated the median time to sputum-culture conversion(5 months vs. 11 months). The cavity closure rate of the BFJD Granules group(50.6%, 43/85) was higher than that of the placebo group(32.1%, 26/81; P=0.02) in patients who completed the treatment. At the end of the intensive treatment, according to the 36-item Short Form, the BFJD Granules significantly improved physical functioning, general health, and vitality of patients relative to the placebo group(all P<0.01). Overall, the death rates in the two groups were not significantly different; 5.1%(8/156) in the BFJD Granules group and 2.6%(4/156) in the placebo group. Conclusions: Supplementing BFJD Granules with the long-course chemotherapy regimen significantly increased the cure rate and cavity closure rate, and rapidly improved QoL of patients with MDR-PTB(Registration No. ChiCTR-TRC-

Benefit-Risk Assessment of Chinese Medicine Injections for Primary Liver Cancer Based on Multi-criteria Decision Analysis

ObjectiveTo evaluate the benefit-risk of 3 commonly used Chinese medicine injections, Aidi Injection (ADI), Cinobufagin Injection (CINI) and Compound Kushen Injection (CKI), in the treatment of primary liver cancer (PLC), so as to provide a reference for clinical decision-making.MethodsRandomized controlled trials (RCTs) of ADI, CINI and CKI in the treatment of PLC published in the databases of China National Knowledge Infrastructure, Wanfang, China Science and Technology Journal Database, SinoMed, PubMed, Cochrane Library, and Web of Science were retrieved from January 2020 to October 2022. The data of benefit and risk indicators were combined to obtain the effect value. The multi-criteria decision analysis (MCDA) model was applied to build the decision tree. The benefit value, risk value and benefit risk value of the 3 injections in PLC treatment were calculated. Monte Carlo simulation was carried out to calculate the 95% confidence interval and probability of differences among the 3 injections, so as to optimize the evaluation results.ResultsA total of 71 RCTs were included. The benefit values of ADI, CINI and CKI combined with transcatheter arterial chemoembolization (TACE) were 42, 38 and 36, respectively. The risk values were 42, 25 and 37, respectively. The benefit risk values were 42, 31 and 37, respectively. The benefit risk differences of ADI vs. CINI, ADI vs. CKI, and CKI vs. CINI were 11 (−0.86, 17.75), 5 (−5.01, 11.09), and 6 (−1.87, 12.63), respectively. The probability that ADI superior to CINI, ADI superior to CKI, and CKI superior to CINI was 96.26%, 77.27%, and 92.62%, respectively.ConclusionBased on the results of MCDA model, CINI combined with TACE has the greatest risk in the treatment of the PLC. Considering the efficacy and safety, the possible priority of the 3 Chinese medicine injections combined with TACE in the treatment of PLC is ADI, CKI and CINI.

The 2022 Extreme Heatwave in Shanghai,Lower Reaches of the Yangtze River Valley:Combined Influences of Multiscale Variabilities

In the summer of 2022,China(especially the Yangtze River Valley,YRV)suffered its strongest heatwave(HW)event since 1961.In this study,we examined the influences of multiscale variabilities on the 2022 extreme HW in the lower reaches of the YRV,focusing on the city of Shanghai.We found that about 1/3 of the 2022 HW days in Shanghai can be attributed to the long-term warming trend of global warming.During mid-summer of 2022,an enhanced western Pacific subtropical high(WPSH)and anomalous double blockings over the Ural Mountains and Sea of Okhotsk,respectively,were associated with the persistently anomalous high pressure over the YRV,leading to the extreme HW.The Pacific Decadal Oscillation played a major role in the anomalous blocking pattern associated with the HW at the decadal time scale.Also,the positive phase of the Atlantic Multidecadal Oscillation may have contributed to regulating the formation of the double-blocking pattern.Anomalous warming of both the warm pool of the western Pacific and tropical North Atlantic at the interannual time scale may also have favored the persistency of the double blocking and the anomalously strong WPSH.At the subseasonal time scale,the anomalously frequent phases 2-5 of the canonical northward propagating variability of boreal summer intraseasonal oscillation associated with the anomalous propagation of a weak Madden-Julian Oscillation suppressed the convection over the YRV and also contributed to the HW.Therefore,the 2022 extreme HW originated from multiscale forcing including both the climate warming trend and air-sea interaction at multiple time scales.

The anti-inflammatory effects of exercise on autoimmune diseases:A 20-year systematic review

Background:The anti-inflammatory effect of exercise may be an underlying factor in improving several autoimmune diseases.The aim of this systematic review was to examine the evidence on the role of exercise training in mitigating inflammation in adolescents and adults with autoimmune disease.Methods:PubMed,Web of Science,and Embase databases were systematically reviewed for related studies published between January 1,2003,and August 31,2023.All randomized and non-randomized controlled trials of exercise interventions with autoimmune disease study participants that evaluated inflammation-related biomarkers were included.The quality of evidence was assessed using the Tool for the assEssment of Study qualiTy and reporting in EXercise scale and Cochrane bias risk tool.Results:A total of 14,565 records were identified.After screening the titles,abstracts,and full texts,87 were eligible for the systematic review.These studies were conducted in 25 different countries and included a total of 2779 participants(patients with autoimmune disease,in exercise or control groups).Overall,the evidence suggests that inflammation-related markers such as C-reactive protein,interleukin 6,and tumor necrosis factor a were reduced by regular exercise interventions.Regular exercise interventions combined with multiple exercise modes were associated with greater benefits.Conclusion:Regular exercise training by patients with autoimmune disease exerts an anti-inflammatory influence.This systematic review provides support for the promotion and development of clinical exercise intervention programs for patients with autoimmune disease.Most patients with autoimmune disease can safely adopt moderate exercise training protocols,but changes in inflammation biomarkers will be modest at best.Acute exercise interventions are ineffective or even modestly but transiently pro-inflammatory.

AIM:To explore the role of positron emission tomographycomputed tomography(PET-CT)examination in the diagnosis and treatment of ocular adnexal mucosa associated lymphoid tissue lymphoma(OAML).METHODS:The general clinical data,postoperative PET-CT results,treatment regimens,and the prognosis of 21 histopathologically confirmed OAML patients between October 2017 and September 2021 were collected.Among the 21 patients,five patients underwent surgical treatment alone,13 patients underwent surgical treatment combined with radiotherapy,and three patients underwent surgical treatment combined with chemotherapy.RESULTS:The follow-up period ranged from 8 to 79mo,with four cases of recurrence and no deaths.Through PETCT examination,two patients exhibited both local ocular metabolic elevation and systemic metastasis,and one of these patients had cervical lymph node metastasis,while the other had submandibular and parotid gland metastasis.Nine patients showed only local ocular metabolic elevation,while 10 patients had no abnormal metabolic activity locally.CONCLUSION:PET-CT examination plays a crucial role in detecting residual lesions and recurrence following tumor resection,aiding in precise disease staging,and facilitating the development of personalized treatment plans,ultimately improving patient prognosis.

Preoperative prediction of perineural invasion of rectal cancer based on a magnetic resonance imaging radiomics model:A dual-center study

BACKGROUND Perineural invasion(PNI)has been used as an important pathological indicator and independent prognostic factor for patients with rectal cancer(RC).Preoperative prediction of PNI status is helpful for individualized treatment of RC.Recently,several radiomics studies have been used to predict the PNI status in RC,demonstrating a good predictive effect,but the results lacked generalizability.The preoperative prediction of PNI status is still challenging and needs further study.AIM To establish and validate an optimal radiomics model for predicting PNI status preoperatively in RC patients.METHODS This retrospective study enrolled 244 postoperative patients with pathologically confirmed RC from two independent centers.The patients underwent preoperative high-resolution magnetic resonance imaging(MRI)between May 2019 and August 2022.Quantitative radiomics features were extracted and selected from oblique axial T2-weighted imaging(T2WI)and contrast-enhanced T1WI(T1CE)sequences.The radiomics signatures were constructed using logistic regression analysis and the predictive potential of various sequences was compared(T2WI,T1CE and T2WI+T1CE fusion sequences).A clinical-radiomics(CR)model was established by combining the radiomics features and clinical risk factors.The internal and external validation groups were used to validate the proposed models.The area under the receiver operating characteristic curve(AUC),DeLong test,net reclassification improvement(NRI),integrated discrimination improvement(IDI),calibration curve,and decision curve analysis(DCA)were used to evaluate the model performance.RESULTS Among the radiomics models,the T2WI+T1CE fusion sequences model showed the best predictive performance,in the training and internal validation groups,the AUCs of the fusion sequence model were 0.839[95%confidence interval(CI):0.757-0.921]and 0.787(95%CI:0.650-0.923),which were higher than those of the T2WI and T1CE sequence models.The CR model constructed by combining clinical risk factors had the best predictive

The urgent demand of extreme(ultra-high/low)thermal conductivity materials is triggered by the high-power device,where exploring the theories and mechanisms of regulating thermal transport properties plays a key role.Herein,we elaborately investigate the effect of vertical(out-of-plane)piezoelectric characteristics on thermal transport,which is historically undiscovered.The different stacking-order(AA and AB)bilayer boron nitride(Bi-BN)in two-dimensional(2D)materials are selected as study cases.By performing state-of-the-art first-principles calculations,it is found that the polarization charge along the out-of-plane orientation ascends significantly with the increasing piezoelectric response in AB stacked Bi-BN(Bi-BN-AB)followed by the enhanced interlayer B–N atomic interactions.Consequently,the amplitude of phonon anharmonicity in Bi-BN-AB increases larger than that in the AA stacked Bi-BN(Bi-BN-AA),resulting in the dramatic weakening of the thermal conductivity by 20.34%under 18%strain.Our research reveals the significant role of the vertical(out-of-plane)piezoelectric characteristic in regulating thermal transport and provides new insight into accurately exploring the thermal transport performance of 2D van der Waals materials.

Energy Management of Price-maker Community Energy Storage by Stochastic Dynamic Programming

In this paper,we propose an analytical stochastic dynamic programming(SDP)algorithm to address the optimal management problem of price-maker community energy storage.As a price-maker,energy storage smooths price differences,thus decreasing energy arbitrage value.However,this price-smoothing effect can result in significant external welfare changes by reduc-ing consumer costs and producer revenues,which is not negligible for the community with energy storage systems.As such,we formulate community storage management as an SDP that aims to maximize both energy arbitrage and community welfare.To incorporate market interaction into the SDP format,we propose a framework that derives partial but sufficient market information to approximate impact of storage operations on market prices.Then we present an analytical SDP algorithm that does not require state discretization.Apart from computational efficiency,another advantage of the analytical algorithm is to guide energy storage to charge/discharge by directly comparing its current marginal value with expected future marginal value.Case studies indicate community-owned energy storage that maximizes both arbitrage and welfare value gains more benefits than storage that maximizes only arbitrage.The proposed algorithm ensures optimality and largely reduces the computational complexity of the standard SDP.Index Terms-Analytical stochastic dynamic programming,energy management,energy storage,price-maker,social welfare.

Influence of Nd^(3+) concentration on mid-infrared emission in PbF_(2) crystal co-doped with Ho^(3+) and Nd^(3+) ions

A promising series of Ho_(y)Nd_(x)Pb_((1-x-y))F_2(x = 0, 0.01, 0.02, 0.03, 0.04;y = 0.02) crystals was grown by the Bridgman method. The influence of the Nd^(3+)ions concentration on mid-infrared(~2.0, ~2.9 and ~3.9 μm)fluorescence emissions of Ho^(3+)ions in the PbF_(2) crystal excited by 808 nm laser diode was investigated in this work. The energy transfer mechanism between Nd^(3+)ions and Ho~(3+)ions under different concentrations of the Nd^(3+)ions was systematically analyzed. The results show that the Nd^(3+)ions have good sensitization and deactivation effect on the Ho^(3+)ions to stimulate the mid-infrared fluorescence emissions. The experimental analysis proves that the sensitization efficiency of the Nd^(3+)ions is relatively stable at around 93.45% with varying Nd^(3+)-doping concentrations. Concentration dependence studies indicate that the concentration of the Nd^(3+)ions has significant influence on mid-infrared emissions.When the doping concentration of the Nd^(3+)ions is up to 2.0 at%, the intensity of ~2.0, ~2.9 and ~3.9 μm emissions all reach the maximum. The output characteristics of a 3.9 μm laser are simulated, and it is found that with the increase of the Nd^(3+)-doping concentration, the peak power, pulse width, and peak energy all meet the trend of first increasing and then decreasing, and Ho_(0.02)Nd_(0.02)Pb_(0.96)F_(2) crystal displays the best performance. All the results show that the Nd^(3+)/Ho^(3+)co-doped PbF_(2) crystals might act as a useful optical medium for mid-infrared laser applications.

Multi-distortion suppression for neutron radiographic images based on generative adversarial network

Neutron radiography is a crucial nondestructive testing technology widely used in the aerospace,military,and nuclear industries.However,because of the physical limitations of neutron sources and collimators,the resulting neutron radiographic images inevitably exhibit multiple distortions,including noise,geometric unsharpness,and white spots.Furthermore,these distortions are particularly significant in compact neutron radiography systems with low neutron fluxes.Therefore,in this study,we devised a multi-distortion suppression network that employs a modified generative adversarial network to improve the quality of degraded neutron radiographic images.Real neutron radiographic image datasets with various types and levels of distortion were built for the first time as multi-distortion suppression datasets.Thereafter,the coordinate attention mechanism was incorporated into the backbone network to augment the capability of the proposed network to learn the abstract relationship between ideally clear and degraded images.Extensive experiments were performed;the results show that the proposed method can effectively suppress multiple distortions in real neutron radiographic images and achieve state-of-theart perceptual visual quality,thus demonstrating its application potential in neutron radiography.
