Antibody and complement levels in patients with hypersplenism associated with cirrhotic portal hypertension and therapeutic principles

BACKGROUND Hypersplenism associated with cirrhotic portal hypertension is a common condition often resulting from hepatitis B-related cirrhosis.However,the levels of immunoglobulin(Ig)and complement in patients with hypersplenism associated with cirrhotic portal hypertension remain unclear.This study was undertaken to determine the levels of Ig and complement in these patients,the relationship between these levels and Child-Pugh class and their clinical significance.AIM To investigate the antibody(Ig)and complement levels in patients with hypersplenism associated with cirrhotic portal hypertension and their clinical significance.METHODS Clinical data of 119 patients with hypersplenism associated with cirrhotic portal hypertension were statistically analyzed and compared with those of 128 control patients.RESULTS IgA and IgG levels in patients with hypersplenism were significantly higher than controls(P<0.001).There was no significant difference in IgM between the two groups(P=0.109).C3 and C4 levels in patients with hypersplenism were significantly lower than controls(P<0.001).As liver function decreased,IgA and IgG levels increased(P<0.001),and C3 and C4 levels decreased(P<0.001).CONCLUSION Patients with hypersplenism associated with cirrhotic portal hypertension have significantly higher antibody(IgA and IgG)levels and significantly lower complement(C3 and C4)levels,which are both related to liver damage.Clinically,the administration of anti-hepatitis virus agents and protection of liver function should be strengthened.

Segmental ureteroileal conduit resection for the treatment of distal upper urinary tract recurrence of bladder cancer fol lowing cystectomy

Segmental ureterectomy is less invasive than radical nephroureterectomy and results in nephron preservation and satisfactory tumor control.This study was to determine the feasibility of segmental ureteroileal conduit resection(SUICR) for patients with distal upper urinary tract recurrence of bladder cancer following radical cystectomy.Four patients with high-grade distal upper urinary tract recurrence underwent SUICR 15-108 months after radical cystectomy.The surgical technique details of SUICR,operative results,and follow-up outcomes are reported.The median operation time was 280 min,and estimated blood loss was less than 100 ml One patient suffered from ileus 5 days after surgery and was managed conservatively.Histopathologic evaluation showed high-grade stages pTa-pT1 diseases for these patients,and ureteral margins were all negative.No patient suffered from tumor recurrence,with a median follow-up of 39 months.SUICR preserved the ipsilateral renal unit and conformed to oncological principles during surgery.The oncological outcome was satisfactory for these properly selected patients.This technique provides a valid alternative to nephroureterectomy for patients with imperative indications and high-grade upper urinary tract recurrence of bladder cancer following radical cystectomy.

Dear Editor,Primary tumors of the seminal vesicle are extremely rare in the genitourinary system. There have only been about 70 cases reported in the literature since the first case of primary seminal vesicle carcinoma was reported in 1925 by Lyons/Seminal vesicle tumors are often difficult to diagnose at an early stage because of a lack of typical symptoms. Moreover, differentiation of the seminal vesicle mass from secondary tumor invasion into adjacent organs has also been a difficult problem for urologists. Nevertheless, accurate and prompt diagnosis of this tumor is important, since timely treatment is associated with improved long-term survival.

Transplantation of fetal liver epithelial progenitor cells ameliorates experimental liver fibrosis in mice

AIM: To investigate the effect of transplanted fetal liver epithelial progenitor (FLEP) cells on liver fi brosis in mice. METHODS: FLEP cells were isolated from embryonal day (ED) 14 BALB/c mice and transplanted into female syngenic BALB/c mice (n = 60). After partial hepatectomy (PH), diethylnitrosamine (DEN) was administered to induce liver fibrosis. Controls received FLEP cells and non-supplemented drinking water, the model group received DEN-spiked water, and the experimental group received FLEP cells and DEN. Mice were killed after 1, 2, and 3 mo, and alanine aminotransferase (ALT), aspartate aminotransferase (AST), hyaluronic acid (HA), and laminin (LN) in serum, and hydroxyproline (Hyp) content in liver were assessed. Alpha-smooth muscle actin (α-SMA) of liver was tested by immunohistochemistry. Transplanted male mice FLEP cells were identifi ed by immunocytochemistry for sry (sex determination region for Y chromosome) protein. RESULTS: Serum ALT, AST, HA, and LN were markedly reduced by transplanted FLEP cells. Liver Hyp content and α-SMA staining in mice receiving FLEP cells were lower than that of the model group, which was consistent with altered liver pathology. Transplanted cells proliferated and differentiated into hepatocytes and bile duct epithelial cells with 30%-50% repopulation in the liver fi brosis induced by DEN after 3 mo. CONCLUSION: Transplanted FLEP cells proliferate and differentiate into hepatocytes and bile duct epithelialcells with high repopulation capacity in the fiberized liver induced by DEN, which restores liver function and reduces liver fi brosis.

转谷氨酰胺酶4（TGM4）在前列腺癌组织和正常组织中差异表达，且与前列腺癌细胞的多种生物学特性相关，如前列腺癌细胞间质化，细胞侵袭性和细胞基质粘附等。本研究主要探索TGM4表达水平与前列腺癌病理特征及前列腺癌生化复发之间的关系。取160名接受前列腺癌根治术患者的石蜡标本构建组织芯片，进而利用组织芯片行TGM4免疫组织化学染色；TGM4的表达情况采用染色范围与染色强度相结合的方式来评价。通过查阅病历获得患者的临床及病理信息；随访信息通过查阅病历、调用我院前列腺癌随访数据库和电话随访等多种方式获得。进而，我们比较了TGM4在癌和癌旁组织中的表达差异情况，及TGM4不同表达水平与前列腺癌病理特征和生化复发之间的关系。同癌旁组织相比，TGM4在前列腺癌组织中明显高表达（P〈0．001），并且其表达水平与Gleason评分（P〈0．001）和PSA水平（P=0．005）呈明显正相关。单变量分析提示TGM4高表达是前列腺癌生化复发的高危因素（P=0．042），但多变量分析并不支持TGM4高表达是前列腺癌生化复发的高危因素（P=0．139）。TGM4在前列腺癌中高表达，其表达水平与Gleason评分和PSA水平呈正相关，TGM4高表达可能作为前列腺癌生化复发的潜在预测因子。

Modulation of d-orbital to realize enriched electronic cobalt sites in cobalt sulfide for enhanced hydrogen evolution in electrocatalytic water/seawater splitting

Herein,a novel single-atomic Pt doping and interface-rich CoS/Co(OH)_(2)(Pt-CoS/Co(OH)_(2)/C)electrocatalyst has been successfully prepared.Benefiting from precise regulation of d-orbital electronic structure modulation on Co site,Pt-CoS/Co(OH)_(2)/C exhibited remarkable HER activity and high stability for hydrogen evolution in splitting both water(73 mV@10 mA·cm^(−2)) and seawater(87 mV@10 mA·cm^(−2)).Notably,atomic Pt doping was introduced into CoS/Co(OH)_(2),which could produce local unbalanced Coulombic force and significantly increased the number of S vacancies,and then expose abundant Co sites.Meantime,Co(OH)_(2) in Pt-CoS/Co(OH)_(2)/C could act as the adsorption sites for H_(2)O in hydrogen evolution reaction process.Density functional theory results also proved that atomic Pt doping,S vacancies and Co(OH)_(2) coupling could result in the formation of enriched electronic Co sites and optimize d_(z2) orbital electronic structure,and then realize the depth upward shift of d-band center and enhance the adsorption of H*on Co sites.

Research on a Monte Carlo global variance reduction method based on an automatic importance sampling method

Global variance reduction is a bottleneck in Monte Carlo shielding calculations.The global variance reduction problem requires that the statistical error of the entire space is uniform.This study proposed a grid-AIS method for the global variance reduction problem based on the AIS method,which was implemented in the Monte Carlo program MCShield.The proposed method was validated using the VENUS-Ⅲ international benchmark problem and a self-shielding calculation example.The results from the VENUS-Ⅲ benchmark problem showed that the grid-AIS method achieved a significant reduction in the variance of the statistical errors of the MESH grids,decreasing from 1.08×10^(-2) to 3.84×10^(-3),representing a 64.00% reduction.This demonstrates that the grid-AIS method is effective in addressing global issues.The results of the selfshielding calculation demonstrate that the grid-AIS method produced accurate computational results.Moreover,the grid-AIS method exhibited a computational efficiency approximately one order of magnitude higher than that of the AIS method and approximately two orders of magnitude higher than that of the conventional Monte Carlo method.

Nuclear charge radius predictions by kernel ridge regression with odd-even effects

The extended kernel ridge regression(EKRR)method with odd-even effects was adopted to improve the description of the nuclear charge radius using five commonly used nuclear models.These are:(i)the isospin-dependent A^(1∕3) formula,(ii)relativistic continuum Hartree-Bogoliubov(RCHB)theory,(iii)Hartree-Fock-Bogoliubov(HFB)model HFB25,(iv)the Weizsacker-Skyrme(WS)model WS*,and(v)HFB25*model.In the last two models,the charge radii were calculated using a five-parameter formula with the nuclear shell corrections and deformations obtained from the WS and HFB25 models,respectively.For each model,the resultant root-mean-square deviation for the 1014 nuclei with proton number Z≥8 can be significantly reduced to 0.009-0.013 fm after considering the modification with the EKRR method.The best among them was the RCHB model,with a root-mean-square deviation of 0.0092 fm.The extrapolation abilities of the KRR and EKRR methods for the neutron-rich region were examined,and it was found that after considering the odd-even effects,the extrapolation power was improved compared with that of the original KRR method.The strong odd-even staggering of nuclear charge radii of Ca and Cu isotopes and the abrupt kinks across the neutron N=126 and 82 shell closures were also calculated and could be reproduced quite well by calculations using the EKRR method.

Recent progress of Pt-based oxygen reduction reaction catalysts for proton exchange membrane fuel cells

With the increasing consumption of fossil fuels,proton exchange membrane fuel cells(PEMFCs) have attracted considerable attention as green and sustainable energy conversion devices.The slow kinetics of the cathodic oxygen reduction reaction(ORR) has a major impact on the performance of PEMFCs,and although platinum(Pt) can accelerate the reaction rate of the ORR,the scarcity and high cost of Pt resources still limit the development of PEMFCs.Therefore,the development of low-cost high-performance ORR catalysts is essential for the commercial application and development of PEMFCs.This paper reviews the research progress of researchers on Pt-based ORR catalysts in recent years,including Pt/C catalysts,Pt-based alloy catalysts,Pt-based intermetallic compounds,and Pt-based single-atom catalysts(SACs),with a focus on Pt-based alloy catalysts with different nanostructures.We described in detail the difficulties and solutions in the research process of various ORR catalysts and explained the principle of their activity enhancement with density functional theory(DFT).In addition,an outlook on the development of Pt-based catalysts is given,and reducing the amount of Pt used and improving the performance of catalysts are the directions to work on in the coming period.

Design Research of a Novel Aftercool-Humidifier Concept for Humid Air Turbine Cycle

Humid air turbine cycle(HAT)has potential of electrical efficiencies comparable to combined cycle,with lower investment cost and NO_(x) emission.The typical heat exchanger network of HAT consists of intercooler(if there is),aftercooler,recuperator,economizer and humidifier,which brings higher efficiency but makes the system more complex.To simplify HAT layout,a novel humidifier concept is proposed by integrating the aftercooler into traditional counter-current humidifier.Based on this concept,a one-dimensional model including pressure drop and exergy calculation is established to distinguish the thermodynamic and hydrodynamic characteristics,and then the structural parameters,such as the number of rows and columns,tube diameter,pitch and type for a micro HAT are identified.The results show that the aftercool-humidifier plays the same role as original aftercooler and humidifier,and can match the in-tube air,out-tube air and water stream well with lower volume.In the case of micro HAT cycle,the volume of heat and mass transfer area can be reduced by 47%compared with traditional design.The major thermal resistance occurred in the convection heat transfer process inside the tube;however,using enhanced tube cannot effectively improve the compactness of device.
