Influence of education and working background on physicians' knowledge of secondary prevention guidelines for coronary heart disease: results from a survey in China

Background and objective： In clinical practice, the standard of secondary prevention for coronary heart disease （CHD） is quite disappointing in China. The physicians＇ shortage of knowledge of secondary prevention guidelines is thought to be a key factor contributing to the inadequate and delayed translation of guidelines into clinical practice. The purpose of this study is to investigate the influence of physicians＇ characteristics, including their educa- tion and work experience, on their knowledge of secondary prevention in China. Methods： A representative ques- tionnaire survey was made of physicians from cardiology departments in 35 tertiary hospitals in China. The survey contained 19 questions on knowledge of guideline recommendations for the secondary prevention of CHD. We col- lected basic information about the physicians, including their educational degree, clinical practice duration/work experience and geographic region. Results： In total, 864 physicians participated in the survey. Eight hundred and thirty-seven completed questionnaires were analyzed. For 6 of the 19 questions, physicians with a postgraduate degree were more likely to answer correctly than those without such a degree. For 11 of the 19 questions, physicians with more than three years＇ clinical experience were more likely to answer correctly than those who had less than three years＇ experience. For 5 of the 19 questions, *** eastern areas were more likely to answer correctly than those from mid/western areas. The mean total score of correct answers to the questionnaire was 11.69 points. Edu- cational degree and clinical practice duration affected total scores significantly while practice location did not （β=0.500, P=0.004; ,β=0.979, P=0.000; and 8=0.228, P=0.162, respectively）. Even if a relatively low score of 12 is taken as a threshold level of acceptable knowledge （defined as a pass）, the pass rate of all physicians was only 53.9%. Antibacterial peptides mixture purified from Ovotransferrin by pepsin digest was used as the raw material. Peptide sections with good antibacterial activity were determined after bacteriostasis experiments, its molecular weight and amino acid composition were analyzed. The results of experiments indicate that with Sephadex G-50 and distilled water as mobile phase, detection wavelength 220 nm, flow rate 1.5 mL/min, sample density 0.2 g/mL, and volume 0.2 mL are the optimal conditions. Bacteriostasis experiments of the fraction of purified peaks were carried out and the result was： peak 1peak 3peak 2; the molecular weight of peak 1 was about 3015 by high performance liquid chromatography; active peptide possessed positive charges by amino acid analysis, its cationic characteristics are in accordance with the nature of antimicrobial peptides.

With the continuous breakthrough in information technology and its integration into practical applications, industrial digital twins are expected to accelerate their development in the near future. This paper studies various control strategies for digital twin systems from the viewpoint of practical *** make full use of advantages of digital twins for control systems, an architecture of digital twin control systems, adaptive model tracking scheme, performance prediction scheme, performance retention scheme, and fault tolerant control scheme are proposed. Those schemes are detailed to deal with different issues on model tracking, performance prediction, performance retention, and fault tolerant control of digital twin systems. Also, the stability of digital twin control systems is analysed. The proposed schemes for digital twin control systems are illustrated by examples.

Objective： To investigate the relationship between renal function and clinical outcomes among patients with acute ST-segment elevation myocardial infamtion （ASTEMI）, who were treated with emergency percutaneous coronary intervention （PCI）. Methods： 420 patients hospitalized in Peking University First Hospital, diagnosed with ASTEMI treated with emergency （PCI） from January 2001 to June 2011 were enrolled in this study. Estimated glomerular filtration rate （eGFR） was used as a measure of renal function. We compared the clinical parameters and outcomes between ASTEMI patients combined renal insufficiency and the patients with normal renal function. Results： There was a significant increase in the concentrations of fibrinogen and D-Dimer （P〈0.05） and a much higher morbidity of diabetes mellitus in the group of patients with chronic kidney disease （CKD; eGFR〈60 ml/（min-1.73 m^2））（P〈0.01）. CKD （eGFR〈60 ml/（min.1.73 m^2）） was an independent predictor of in-hospital mortality for patients hospitalized with ASTEMI receiving PCI therapy rapidly （P=0.032, odds ratio （OR） 4.159, 95% confidence interval （CI） 1.127-15.346）. Conclusions： Renal insufficiency is an independent predictor of in-hospital mortality for patients hospitalized with ASTEMI treated with primary PCI.

The similarities and differences in inherent mechanism and characteristic frequency between the onedimensional(1D) poroelastic model and the layered White model were investigated. This investigation was conducted under the assumption that the rock was homogenous and isotropic at the mesoscopic scale. For the inherent mechanism, both models resulted from quasi-static flow in a slow P-wave diffusion mode, and the differences between them originated from saturated fluids and boundary conditions. On the other hand, for the characteristic frequencies of the models, the characteristic frequency of the 1D poroelastic model was first modified because the elastic constant and formula for calculating it were misused and then compared to that of the layered White model. Both of them moved towards higher frequencies with increasing permeability and decreasing viscosity and diffusion length. The differences between them were due to the diffusion length. The diffusion length for the 1D poroelastic model was determined by the sample length, whereas that for the layered White model was determined by the length of the representative elementary volume(REV). Subsequently, a numerical example was presented to demonstrate the similarities and differences between the models. Finally, published experimental data were interpreted using the 1D poroelastic model combined with the Cole-Cole *** prediction of the combined model was in good agreement with the experimental data, thereby validating the effectiveness of the 1D poroelastic model. Furthermore, the modified characteristic frequency in our study was much closer to the experimental data than the previous prediction, validating the effectiveness of our modification of the characteristic frequency of the 1D poroelastic model.
