Objective: To assess the efficacy and safety of Bufei Jiedu(BFJD) Granules as adjuvant therapy for patients with multidrug-resistant pulmonary tuberculosis(MDR-PTB). Methods: A large-scale, multi-center, double-blinded, and randomized controlled trial was conducted in 18 sentinel hospitals in China from December 2012 to December 2016. A total of 312 MDR-PTB patients were randomly assigned to BFJD Granules or placebo groups(1:1) using a stratified randomization method, which both received the long-course chemotherapy regimen for 18 months(6 Am-Lfx-P-Z-Pto, 12 Lfx-P-Z-Pto). Meanwhile, patients in both groups also received BFJD Granules or placebo twice a day for a total of 18 months, respectively. The primary outcome was cure rate. The secondary outcomes included time to sputum-culture conversion, changes in lung cavities and quality of life(QoL) of patients. Adverse reactions were monitored during and after the trial. Results: A total of 216 cases completed the trial, 111 in the BFJD Granules group and 105 in the placebo group. BFJD Granules, as adjuvant treatment, increased the cure rate by 13.6% at the end of treatment, compared with the placebo(58.4% vs. 44.8%, P=0.02), and accelerated the median time to sputum-culture conversion(5 months vs. 11 months). The cavity closure rate of the BFJD Granules group(50.6%, 43/85) was higher than that of the placebo group(32.1%, 26/81;P=0.02) in patients who completed the treatment. At the end of the intensive treatment, according to the 36-item Short Form, the BFJD Granules significantly improved physical functioning, general health, and vitality of patients relative to the placebo group(all P<0.01). Overall, the death rates in the two groups were not significantly different;5.1%(8/156) in the BFJD Granules group and 2.6%(4/156) in the placebo group. Objective:To investigate whether the combination of chemotherapy with staged chinese herbal medicine(CHM) therapy could enhance health-related quality of life(QoL) in non-small-cell lung cancer(NSCLC)patients and prolong the time before deterioration of lung cancer symptoms,in comparison to chemotherapy ***:A prospective,double-blind,randomized,controlled trial was conducted from December 14,2017 to August 28,2020.A total of 180 patients with stage ⅠB-ⅢA NSCLC from 5 hospitals in Shanghai were randomly divided into chemotherapy combined with CHM(chemo+CHM) group(120 cases) or chemotherapy combined with placebo(chemo+placebo) group(60 cases) using stratified blocking *** European Organization for Research and Treatment of Cancer(EORTC) Quality-of-Life-Core 30 Scale(QLQ-C30) was used to evaluate the patient-reported outcomes(PROs) during postoperative adjuvant chemotherapy in patients with early-stage *** events(AEs) were assessed in the safety ***:Out of the total 180 patients,173 patients(116in the chemo+CHM group and 57 in the chemo+placebo group) were included in the PRO *** initial mean QLQ-C30 Global Health Status(GHS)/QoL scores at baseline were 57.16±1.64 and 57.67±2.25 for the two respective groups(P>0.05).Compared with baseline,the chemo+CHM group had an improvement in QLQ-C30GHS/QoL score at week 18 [least squares mean(LSM) change 17.83,95% confidence interval(CI) 14.29 to 21.38].Conversely,the chemo+placebo group had a decrease in the score(LSM change-13.67,95% CI-22.70 to-4.63).A significant between-group difference in the LSM GHS/QoL score was observed,amounting to 31.63 points(95% CI25.61 to 37.64,P<0.001).The similar trends were observed in physical functioning,fatigue and appetite *** week 18,patients in the chemo+CHM group had a higher proportion of improvement or stabilization in GHS/QoL functional and symptom scores compared to chemo+placebo group(P<0.001).The median time to deterioration

Black rice， as a dietary supplement， has received increasing attention because of its beneficial health properties. Although black rice and its major ingredients have been considered a potential therapy for ulcerative colitis (UC)， its effect and underlying mechanism of action remain obscure. In this study， black rice was demonstrated to improve Dextran Sulfate Sodium Salt (DSS)-induced UC and restore intestinal barrier dysfunction. Cyanidin 3-O-glucoside (C3G) is the most abundant ingredient in black rice. Similarly， in vivo and in vitro studies have demonstrated that C3G alleviates colitis and intestinal barrier dysfunction. Proteomic analysis showed that C3G significantly reduced abnormally elevated swiprosin-1 levels in mice with colitis. Furthermore， the effect of C3G on mitigating colitis was inhibited after swiprosin-1 was overexpressed in intestinal tissue in vivo and Caco-2 cells in vitro. Acyl-CoA synthetase long-chain family member 1 (ACSL1) was identified as a potential protein that interacts with swiprosin-1 by co-immunoprecipitation and liquid chromatography–tandem mass spectrometry. By integrating metabolomics experiments and bioinformatics analysis of single-cell sequencing， ACSL1 was found to selectively bind to swiprosin-1 and regulate downstream linoleic acid metabolism in colitis. Moreover， C3G was observed to prevent the localization of ACSL1 within mitochondria via swiprosin-1， thereby inhibiting the oxidation of linoleic acid. This study demonstrated that C3G targeted swiprosin-1 to ameliorate UC via ASCL1-mediated linoleic acid metabolism， thereby providing a novel potential insight into the mechanism of C3G protection against colitis and establishing the groundwork for its clinical application.

BACKGROUND The efficacy of Vonoprazan-amoxicillin dual therapy(VAT)in the treatment of Helicobacter pylori(***)is *** To evaluate the efficacy of VAT in the chinese *** This prospective,multicenter,randomized,open-label,and two-stage study was conducted at 23 centers in Fujian,China(May 2021-April 2022).***-infected patients were randomized to bismuth quadruple therapy(BQT),BQT-Vonoprazan(BQT-V),seven-day VAT(VAT-7),ten-day VAT(VAT-10),and fourteen-day VAT(VAT-14)*** primary endpoint was the *** eradication *** secondary endpoint was the frequency of adverse *** study was registered with the chinese Clinical Trial Registry,*** In the first stage,VAT-7 and BQT-V groups were selected for early termination because less than 23 among 28 cases were *** the second stage,the eradication rates for BQT,VAT-10,and VA-14 were 80.2%[95%confidence interval(95%CI):71.4%-86.8%],93.2%(86.6%-96.7%),92.2%(85.3%-96.0%)in the intention-to-treat(ITT)analysis,and 80.9%(95%CI:71.7%-87.5%),94.0%(87.5%-97.2%),and 93.9%(87.4%-97.2%)in the per-protocol *** ITT analysis showed a higher eradication rate in the VAT-10 and VAT-14 groups than in the BQT group(P=0.022 and P=0.046,respectively).The incidence of adverse events in the VAT-10 and VAT-14 groups was lower than in the BQT group(25.27%and 13.73%vs 37.62%,respectively;P<0.001).CONCLUSION VAT with a duration of 10 or 14 days achieves a higher eradication rate than the BQT,with a more tolerable safety profile in ***-infected patients in *** XP et *** for *** eradication.

目的探讨急性脑缺血/再灌注大鼠神经元细胞骨架蛋白时空动态变化。方法线栓法阻断大鼠大脑中动脉90 min后实现再灌注,在不同再灌注时间点观察及取材。尼氏染色观察神经细胞损伤,采用神经功能缺损评分和前肢放置实验评估神经功能;免疫组化染色、免疫印迹法观察细胞骨架成分微管相关蛋白2(microtubule associated protein 2,MAP2)、神经丝重链(neurofilament heavy chain,NF-H)的变化;透射电镜观察轴突、树突和神经丝亚显微结构。结果随着再灌注时间的延长,脑损伤和神经行为功能损害逐渐加重。纹状体损伤比皮层出现的更早、更严重。缺血区域的MAP2相关免疫反应强度降低,NF-H相关免疫反应强度升高。超微结构观察显示细胞骨架排列受损,密度降低。结论不同脑区对缺血/再灌注损伤的耐受性不同。神经元细胞骨架的主要成分对缺血和再灌注表现出动态反应,这可能进一步促进脑损伤和神经功能缺损。

运用证据图系统检索和梳理中成药治疗失眠的临床研究证据,了解该领域证据分布特点,发现中成药治疗失眠相关研究存在的问题。检索3个国家药物目录中明确提及治疗失眠的中成药,检索建库至2023年8月的中、英文文献。采用图、表展示结果。最终纳入中成药23种,相关文献299篇,包括随机对照试验(RCTs)236篇,非随机对照试验(non-RCTs)35篇,回顾性研究7篇,系统评价/Meta分析17篇,指南和专家建议或共识4篇。文献占比较大的是百乐眠胶囊、乌灵胶囊、养血清脑颗粒;结局指标包括睡眠评定量表、临床有效率、安全性指标、焦虑抑郁评分等。结果表明,中成药治疗失眠研究整体呈增长趋势,但研究证据相对较少,研究以单中心、小样本和短周期为主;临床定位宽泛,中医优势不足;结局指标对生活质量、随访及复发率方面关注度不够;RCT整体偏倚风险偏高,系统评价/Meta分析整体质量较低,回顾性研究总体评分不高,non-RCT均未提及随访时间、失访率及样本量估算,结果可信度降低。建议以中医药临床研究规范来设计治疗失眠的中成药研究方案,将中医证候积分评价作为重要结局指标,多关注患者生活质量、随访及复发情况,提高有效治疗失眠中成药的可及性和经济性,加强医保政策与中成药政策的衔接,合理提升具有明确疗效和安全证据支持的中成药在医保甲类目录中的占比。

国产1%林丹霜治疗疥疮1086例

笔者用1％林丹霜(武汉第三制药厂试制)外涂法治疗疥疮患者1086例,一次治愈率为83.5％,二次治愈率为96.3％,一次有效率为97.8％。无一例发生中枢神经中毒症状。该药治疗疥疮方便、高效、安全、值得推广应用。
