Ultrasonically activated scalpel versus monopolar electrocautery shovel in laparoscopic total mesorectal excision for rectal cancer

AIM： To investigate the feasibility and safety of monopolar electrocautery shovel （ES） in laparoscopic total mesorectal excision （TME） with anal sphincter preservation for rectal cancer in order to reduce the cost of the laparoscopic operation, and to compare ES with the ultrasonically activated scalpel （US）. METHODS： Forty patients with rectal cancer, who underwent laparoscopic TME with anal sphincter preservation from june 2005 to june 2007, were randomly divided into ultrasonic scalpel group and monopolar ES group, prospectively. White blood cells （WBC） were measured before and after operation, operative time, blood loss, pelvic volume of drainage, time of anal exhaust, visual analogue scales （VAS） and surgery-related complications were recorded. RESULTS： All the operations were successful; no one was converted to open procedure. No significant differences were observed in terms of preoperative and postoperative d I and d 3 WBC counts （P = 0.493, P = 0.375, P = 0.559）, operation time （P = 0.235）, blood loss （P = 0.296）, anal exhaust time （P = 0.431）, pelvic drainage volume and VAS in postoperative d 1 （P = 0.431, P = 0.426） and d 3 （P = 0.844, P = 0.617） between ES group and US group. The occurrence of surgery-related complications such as anastomotic leakage and wound infection was the same in the two groups.CONCLUSION： ES is a safe and feasible tool as same as US used in laparoscopic TME with anal sphincter preservation for rectal cancer on the basis of the skillful laparoscopic technique and the complete understanding of laparoscopic pelvic anatomy. Application of ES can not only reduce the operation costs but also benefit the popularization of laparoscopic operation for rectal cancer patients.

Comparative Study on Different Methods for Prediction of Thermal Insulation Performance of Thermal Barrier Coating Used on Turbine Blades

As turbine inlet temperature gets higher and higher,thermal barrier coating(TBC) is more and more widely used in turbine *** turbine blades with TBC,it is of great significance to evaluate the temperature distribution of its substrate metal quickly and accurately,especially during the design *** different degrees of simplification such as whether to consider the change of the geometric size of the fluid domain by TBC and whether to consider the planar heat conduction in TBC,three different methods used in conjugate heat transfer(CHT) simulation to model the TBC of the turbine blades have been developed and widely used by ***,little research has been conducted to investigate the influence of the three methods on the temperature distribution of turbine *** fill this gap,three geometric models were *** are a solid conduction model with a substrate metal layer and a TBC layer,a transonic turbine vane with internal cooling and TBC,and a plate cylindrical film hole cooling model with *** methods were used in these geometric models and their differences were carefully analyzed and *** result shows that for the conduction model used in this paper,with the same TBC surface temperature distribution,the difference between the three methods is very small and can be *** a transonic turbine vane with internal cooling,regarding the local maximum temperature of the substrate-TBC interface,the largest difference between the method in which TBC is considered as a thermal resistance or a virtual layer of cells and the method in which three-dimensional heat conduction equation of TBC is solved occurs at the trailing *** difference near the leading edge is below *** employed to the film cooling model,the difference of the laterally averaged temperature of the substrate-TBC interface can be 8 K which is mainly due to the change of the length to diameter ratio of the film cooling hole by *** the substrate

Three-dimensional porous bimetallic metal–organic framework/gelatin aerogels: A readily recyclable peroxymonosulfate activator for efficient and continuous organic dye removal

As promising catalysts for the degradation of organic pollutants,metal–organic frameworks(MOFs)often face limitations due to the particle agglomeration and challenging recovery in liquid-catalysis application,stemming from their powdery *** macroscopic structures from pulverous MOF is thus of great importance for broadening their practical *** this study,three-dimensional porous MOF aerogel catalysts were successfully fabricated for degrading organic dyes by activating peroxymonosulfate(PMS).MOF/gelatin aerogel(MOF/GA)catalysts were prepared by directly integrating bimetallic FeCo-BDC with gelatin solutions,followed by freeze-drying and low-temperature *** FeCo-BDC-0.15/GA/PMS system exhibited remarkable performance in degrading various organic dyes,eliminating 99.2%of rhodamine B within a mere 5 *** to the GA/PMS system,there was over a 300-fold increase in the reaction rate ***,high removal efficiency was maintained across varying conditions,including different solution pH,co-existing inorganic anions,and natural water *** trapping experiments and electron paramagnetic resonance analysis revealed that the degradation involved radical(SO_(4)^(-)·)and non-radical routes(^(1)O_(2)),of which ^(1)O_(2) was ***,even after a continuous 400-min reaction in a fixed-bed reactor at a liquid hourly space velocity of 27 h^(-1),the FeCo-BDC/GA composite sustained a degradation efficiency exceeding 98.7%.This work presents highly active MOF-gelatin aerogels for dye degradation and expands the potential for their large-scale,continuous treatment application in organic dye wastewater management.

Quality control measures for lowering the seroconversion rate of hemodialysis patients with hepatitis B or C virus

BACKGROUND: Hemodialysis (HD) patients are at high risk of infection by hepatitis B virus (HBV) or hepatitis C virus (HCV). The present study was designed to determine the impact of quality control measures on the prevention of transmission of blood-borne viruses. METHODS: A total of 6182 adult maintenance HD patients from all HD units in Zhejiang Province were recruited on january 1, 2007. The baseline demographic and clinical characteristics were recorded and all patients were followed up until death or survival at 4 years later. The Quality Control Standards of Hemodialysis were gradually implemented in HD units. The HBV or HCV seroconversion rates of the recruited patients were calculated and compared every year during the observation period. RESULTS: The prevalence of HBV was 8.3% at the beginning of the study, and 6.6% for HCV. With the implementation of the HD quality control measures, the HBV seroconversion rate tended to decrease year by year (χ 2 =6.620, P=0.085), and the HCV seroconversion rate decreased significantly (χ 2 =10.41, P=0.015). Compared with the data in 2007, the HBV seroconversion rate (χ 2 =4.204, P=0.040, relative risk ratio 0.393, 95% CI 0.156-0.991) and the HCV seroconversion rate (χ 2 =7.373, P=0.007, relative risk ratio 0.386, 95% CI 0.189-0.787) decreased significantly in 2010. CONCLUSION: Quality control measures for HD decreased the seroconversion rates of HBV or HCV in HD patients, showing that updated quality control measures reduce the risk for transmission of blood-borne viruses in the HD population.

Eutectic Solidification in Near-eutectic Al-Si Casting Alloys

Eutectic solidification in near-eutectic Al-13 wt pct Si casting alloys and the effect of trace addition of boron or strontium on it have been investigated using thermal analysis and microstructural characterization. In unmodified alloy, dual eutectic structure has been observed. The coarse eutectic （dendrite-like Al＋ coarse Si flakes） is formed above the equilibrium temperature of eutectic （Al＋Si） reaction （577℃）. The coarse eutectic （CE） grains nucleate from the primary silicon particles formed earlier due to local enrichment of silicon solute and grow in a divorced mode between the dendritic Al phase and large silicon flakes. The fine eutectic （FE） grains nucleate later on other potential sites activated by melt undercooling and grow in coupled-growing mode with the silicon crystals as fine flakes. The formation of the FE grains is favored in the alloys containing boron because of a great number of potential nucleation sites being added from boron-containing particles. Addition of strontium to the alloys restrains completely the formation of primary silicon particles and hence limits the nucleation of the CE. This is because the eutectic point has moved far enough to make the alloy, at this composition （Al-13 wt pct Si）, hypo-eutectic. Local cooling rate during solidification has an important influence on competition formation of these two eutectics.

Sequential good lattice point sets for computer experiments

Sequential Latin hypercube designs(SLHDs) have recently received great attention for computer experiments, with much of the research restricted to invariant spaces. The related systematic construction methods are inflexible, and algorithmic methods are ineffective for large designs. For designs in contracting spaces, systematic construction methods have not been investigated yet. This paper proposes a new method for constructing SLHDs via good lattice point sets in various experimental spaces. These designs are called sequential good lattice point(SGLP) sets. Moreover, we provide efficient approaches for identifying the(nearly)optimal SGLP sets under a given criterion. Combining the linear level permutation technique, we obtain a class of asymptotically optimal SLHDs in invariant spaces, where the L1-distance in each stage is either optimal or asymptotically optimal. Numerical results demonstrate that the SGLP set has a better space-filling property than the existing SLHDs in invariant spaces. It is also shown that SGLP sets have less computational complexity and more adaptability.

It is shown by the the law of mass action in combination with the coexistence theory of slag structure that KMn n =NMno /(NFeo [%Mn] does not change with basicities and maintains constant at constant temperature; the oxidizing capabilities of multicomponent slag systems containing CaO, MgO etc., can be expressed by NFe tO =NFeO + 6NFe2 O3 + 8NFe3O4; the desulfurizing capabilities of various basic oxides for multicomponent slag systems can be calculated by Ls=8(KCaSNCaO+KMgsNMgo +KFeSNFeO)∑n/[%O]; the dephosphorizing capabili- ties of CaO-MgO-FeO-FeO O3-P2 O5 as well as CaO-FeO-Fe2 O3-A12 O3-P2 O5 molten slags expressed respectively by Lpo=(%P2O5)/[P]2 = 141.94[%O]5∑nK00(1 + K5N2Cao + K6N3CaO + K7N4CaO + K8N2MgO + K9N3MgO + K10N3FeO + K11N4Feo); LPO=(%P2O5)/[P]2= 141.94[%O]5∑nK00(1 + K10N2CaO+ K11N3CaO+ K12N4CaO + K13N3FeO + K14N4FeO) have good agreement with practical values.
