Self-management of coronary heart disease in older patients after elective percutaneous transluminal coronary angioplasty

objective To explore how older patients self-manage their coronary heart disease （ChD） aider undergoing elective percutaneous transluminal coronary angioplasty （PTCA）. Methods This mixed methods study used a sequential, explanatory design and recruited a convenience sample of patients （n = 93） approximately three months after elective PTCA. The study was conducted in two phases. Quantitative data collected in Phase 1 by means of a self-administered survey were subject to univariate and bivariate analysis. Phase 1 findings in- formed the purposive samplhag for Phase 2 where ten participants were selected from the original sample for an in-depth interview. Qualita- tive data were analysed using thematic analysis. This paper will primarily report the findings from a sub-group of older participants （n = 47） classified as 65 years of age or older. Results 78.7% （n = 37） of participants indicated that they would manage recurring angina symptoms by taking glyceryl trinitrate and 34% （n = 16） thought that resting would help. Regardless of the duration or severity of the symptoms 40.5% （n = 19） would call their general practitioner or an emergency ambulance for assistance during any recurrence of angina symptoms. older participants weighed less （P = 0.02） and smoked less （P = 0.01） than their younger counterparts in the study. Age did not seem to affect PTCA patients＇ likelih.od of altering dietary factors such as fruit, vegetable and saturated fat consumption （P = 0.237）. Conclusions The findings suggest that older people in the study were less likely to know how to correctly manage any recurring angina symptoms than their younger counterparts but they had fewer risk factors for ChD. Age was not a factor that influenced participants＇ likelih.od to alter lifestyle factors.

Data on the extent of the burden due to schistosomiasis is sparse in most Sub-Saharan African ***,this data is crucial for triggering medical attention.A review of extent of morbidity and determinants associated with schistosomiasis in Malawi was therefore conducted to quantify the infection in order to concretise the need for medical intervention.A systematic and traditional search strategy was used to find literature for the review,whilst exclusion and inclusion criteria were used to identify appropriate *** regression curves of epidemiological model Y=(a+bxc)/(1+bxc)and the recommendation that schistosomiasis prevalence can be used to estimate morbidity were employed to quantify morbidity at various infection *** was quantified as a direct proportion of the population and the respective national schistosomiasis *** showed that both *** and *** are present in Malawi with the latter highly prevalent(50%).Furthermore,out of the estimated population of 16,829 million,approximately 8.4 million have schistosomiasis,with about 4.4 million of these aged 18 years and *** most frequent manifestation is Katayama syndrome,while ascites is the lowest,impacting about 3.0 million and 960 individuals,*** studies on association of schistosomiasis infection to risk factors such as occupation,age and gender found odds ratio(oR)ranging from 1.29 to *** due to schistosomiasis is high in *** is therefore recommended that a more detailed study on the determinants of high schistosomiasis and re-evaluation of the current control measures be conducted if the current morbidity statistics are to be remarkably reduced.

Self healing Asphalt Concrete-state of the Art

Similar to engineering materials used in civil engineering,aerospace engineering and other applications,an asphalt concrete also suffers from fatigue cracking due to a cyclic loading conditions on the road,which results in a decrease of the stiffness and the strength. The self healing capability has been investigated widely due to its unique contribution to the recovery of the stiffness and the strength from cracking and then to extend the service life of asphalt pavements. In this paper,general concepts of self healing and self healing asphalt concrete were introduced. Then,the parameters that influence the self healing capability of the asphalt binder and asphalt concrete were discussed. Inspired from other self healing materials,the possibilities of applying microcapsule self healing system in asphalt concrete were investigated in detail.

objective:To evaluate in vitro immunomodulating properties and potential cytotoxicity of six tropical medicinal herbs and food plants namely Antidesma madagascariense(Euphorbiaceae) (AM),Erylh.oxylum macrocarpum(Eryth.oxylaceae)(EM),Faujasiopsis flexuosa(Asteraceae) (FF),Pittosporum senacia(Pittosporaceae)(PS),Momordica charantia(Cucurbitaceae)(MC) and orimum tenuiflorum(Lamiaceae)(of).Methods:Initially,the crude water and methanol extracts were probed for their capacity to trigger immune cells’ NADPh.oxidase and MPo-dependent activities as measured by lucigenin- and luminol-amplified chemiluminescence,respectively; as compared to receptor-dependent(serum opsonised zymosan- oPZ) or receptor-independent phorbol myristerate acetate(PMA).Results:Preliminary screening on whole human blood oxidative buret activity showed significant and concentration-dependent immunomodulating properties of three plants AM,FF and *** investigations of the fractions on isolated human polymorphonuclear cells(PMNs) and mice monocytes using two different pathways for activation of phagocytic oxidative burst showed that ethyl acetate fraction was the most potent extract. None of the active samples had cell-death effects on human PMNs,under the assay conditions as determined by the trypan-blue exclusion *** PMA and oPZ NADPh.oxidase complex is activated via different transduction pathways,these results suggest that AM,FF and oT does not affect a specific transductional pathway,but rather directly inhibit a final common biochemical target such as the NADPh.oxidase enzyme and/or scavenges ***:our findings suggest that some of these plants extracts/fractions were able to modulate significantly immune response of phagocytes and monocytes at different steps,emphasizing their potential as a source of new natural alternative immunomodulatory agents.

Environmental embrittlement of intermetallic compounds in Fe-Al alloys

First,it is proposed that hydrogen atoms occupy the interstitial sites in Fe3Al and *** the environmental embrittlement of intermetallic compounds in Fe-Al alloys is studied in the light of calculated valence electron structures and bond energy of Fe3Al and FeAl containing hydrogen *** the analyses it is found that the states of metal atoms will change,in which more lattice electrons will become covalent electrons to bond with hydrogen atoms when the atomic hydrogen diffuses into the intermetallic compounds in Fe-Al alloys,which will result in the decrease of local metallicity in Fe3Al and ***,it is found that the crystal will easily cleave since solute hydrogen bonds with metal atoms and severely anisotropic bonds *** a conclusion,these factors result in the environmental embrittlement of Fe3Al and FeAl.

Interfacial charge transfer plays an essential role in establishing the relative alignment of the metal Fermi level and the energy bands of organic *** the details remain elusive in many systems,this charge transfer has been inferred in a number of photoemission *** present electronic transport measurements in very short channel(Lors made from poly(3-hexylth.ophene)(P3hT).As channel length is reduced,the evolution of the contact resistance and the zero gate voltage conductance are consistent with such charge *** channel conduction in devices with Pt contacts is greatly enhanced compared to analogous devices with Au contacts,consistent with charge transfer *** current scanning tunneling microscopy(ACSTM)provides further evidence that holes are transferred from Pt into P3hT,while much less charge transfer takes place at the Au/P3hT interface.

Synthesis, Crystal and Molecular Structure and Biological Activity of Dimethyl Trans-3-(2-bromophenyl)-2-methylisoxazolidine-4,5-dicarboxylate

The compound of dimethyl trans-3-（2-bromophenyl）-2-methylisoxazolidine-4,5-dicarboxylate has been synthesized and characterized by IR, 1h-NMR, 13C-NMR, 2D-NMR （CoSY, NoESY, hMQC, hMBC） and UV-vis. spectroscopy techniques and single-crystal X-ray diffraction （XRD）. The biological activities of the title compound have been investigated in detail. The new compound crystallizes in monoclinic, space group C2/c with a = 26.9263（10）, b = 7.0970（2）, c = 19.8554（7）？, and β = 126.630（2）. In addition to the single crystal structure, the molecular geometry, vibrational frequencies, chemical shifts, molecular electrostatic potential and frontier molecular orbital analysis of the title compound in the ground state have been calculated by Density Functional Th.ory （DFT） method.

Perihepatic adhesions: An unusual complication of hemolysis, elevated liver enzymes and low platelet syndrome

We present a case of symptomatic perihepatic adhesions,which developed after a pregnancy complicated by hemolysis,elevated liver enzymes and low platelet(hELLP)syndrome,in which a subcapsular liver hematoma *** patient presented with complaints of persistent,severe right-sided upper abdominal *** complaints developed gradually,one year after a pregnancy that had been complicated by hELLP syndrome with a subcapsular liver *** hematoma had resolved *** upperabdominal magnetic resonance imaging revealed a density between liver and diaphragm at the site of the former subcapsular hematoma,suspect of perihepatic *** presence of perihepatic adhesions was confirmed during a laparoscopy and treated by adhesiolysis in the same *** adhesions may have developed in response to the degradation process of the subcapsular liver hematoma during conservative *** case of perihepatic adhesions may therefor be the first presentation of a long term sequel of subcapsular liver hematoma in hELLP syndrome.
