This paper aims to design and implement an automatic heart disease diagnosis system using?MATLAB. The Cleveland data set for heart diseases was used as the main database for training and testing the developed system. In order to train and test the Cleveland data set, two systems were developed. The first system is based on the Multilayer Perceptron (MLP) structure on the Artificial Neural Network (ANN), whereas the second system is based on the Adaptive Neuro-Fuzzy Inference Systems (ANFIS) approach. Each system has two main modules, namely, training and testing,?where 80% and 20% of the Cleveland data set were randomly selected for training and testing?purposes respectively. Each system also has an additional module known as case-based module,?where the user has to input values for 13 required attributes as specified by the Cleveland data set,?in order to test the status of the patient whether heart disease is present or absent from that particular patient. In addition, the effects of different values for important parameters were investigated in the ANN-based and Neuro-Fuzzy-based systems in order to select the best parameters that obtain the highest performance. Based on the experimental work, it is clear that the Neuro-Fuzzy system outperforms the ANN system using the training data set, where the accuracy for each system was 100% and 90.74%, respectively. However, using the testing data set, it is clear that the ANN system outperforms the Neuro-Fuzzy system, where the best accuracy for each system was 87.04% and 75.93%, respectively.

The rise of new viruses, like SARS-CoV-2 causing the COVID-19 outbreak, along with the return of antibiotic resistance in harmful bacteria, demands a swift and efficient reaction to safeguard the health and welfare of the global population. It is crucial to have effective measures for prevention, intervention, and monitoring in place to address these evolving and recurring risks, ensuring public health and international security. In countries with limited resources, utilizing recombinant mutation plasmid technology in conjunction with PCR-HRM could help differentiate the existence of novel variants. cDNA synthesis was carried out on 8 nasopharyngeal samples following viral RNA extraction. The P1 segment of the SARS-CoV-2 Spike S protein was amplified via conventional PCR. Subsequently, PCR products were ligated with the pGEM-T Easy vector to generate eight recombinant SARS-CoV-2 plasmids. Clones containing mutations were sequenced using Sanger sequencing and analyzed through PCR-HRM. The P1 segment of the S gene from SARS-CoV-2 was successfully amplified, resulting in 8 recombinant plasmids generated from the 231 bp fragment. PCR-HRM analysis of these recombinant plasmids differentiated three variations within the SARS-CoV-2 plasmid population, each displaying distinct melting temperatures. Sanger sequencing identified mutations A112C, G113T, A114G, G214T, and G216C on the P1 segment, validating the PCR-HRM findings of the variations. These mutations led to the detection of L452R or L452M and F486V protein mutations within the protein sequence of the Omicron variant of SARS-CoV-2. In summary, PCR-HRM is a vital and affordable tool for distinguishing SARS-CoV-2 variants utilizing recombinant plasmids as controls.

The Zn0.5CuxMg0.5-xFe2O4 (where x = 0.0, 0.1, 0.2, 0.3 and 0.4) was prepared by sol-gel route and characterized in detail in terms of their structural, morphological, elemental and optical properties as a function of Cu concentration. X-ray diffractometer (XRD) results confirmed the formation of cubic spinel-type structure with average crystallized size in the range of 30.56 to 40.58 nm. Lattice parameter was found to decrease with Cu concentration due to the smaller ionic radius of Cu2+ ion. The HR-SEM images show morphology of the samples as prismatic shaped particles in agglomeration. The elemental dispersive X-ray Spectroscopy (EDX) confirmed the elemental composition of the as-prepared spinel ferrite material with respect to the initial concentration of the synthetic composition used for the material. The Fourier transform infrared (FTIR) spectroscopy confirmed the formation of spinel ferrite and showed the characteristics absorption bands around 463, 618, 876, 1116, 1442, 1622 and 2911 cm-1. The energy band gap was calculated for the samples were found to be in the range of 4.87 to 5.30 eV.

Co-precipitation is an important issue in chemical analysis, where it is often undesirable, but in some cases, it can be exploited. The Zn0.5Mn0.5−xLi2xFe2O4 nanomaterials (x = 0.0, 0.1, 0.2, 0.3 and 0.4) was afforded by utilizing co-precipitation method. The structural and optical characteristics were analyzed for the samples employing X-ray diffraction (XRD), Fourier transforms infrared spectroscopy (FTIR) and Ultraviolet-visible spectrophotometer (UV-Vis). XRD revealed that the structure of certain nanoparticles is a cubic spinel with space group (Fd-3m) and crystallite size in the scale 124 - 150 nm. Lattice parameter was determined to increments with Li+1 and that may occur due to the larger ionic radius of the Li1+ ion. FTIR spectroscopy confirmed the form of spinel ferrite and explicated the properties of absorption bands approximately 593, 1111, 1385, 1640, 2922 and 3430. The energy band gap was estimated for all samples with diverse ratios and was observed in the range of 2.58 - 2.52 eV.

The Impact of a Structured Clinical Pathway on the Application of Management Standards in Patients with Diabetic Ketoacidosis and Its Acceptability by Medical Residents

Diabetic Ketoacidosis (DKA) is a serious and potentially a fatal complication of diabetes mellitus. Tools to guarantee proper, evidence-based, guideline implementation are of paramount importance and an essential element for quality patient care. Clinical pathways represent one such tool that clearly promotes the implementation of guidelines and research evidence into clinical practice. The aims of this study were to measure quantitatively and qualitatively the impact of a specially structured Resident-friendly, DKA clinical pathway on the application of evidence-based management standards and its acceptability by the treating resident physicians. A retrospective chart review of patients who were admitted prior to and after the launching of the clinical pathway and a questionnaire assessment of resident’s acceptance of the pathway format were undertaken. Eighty one episodes of DKA in a total of 58 patients fulfilled the criteria for inclusion in the study. Thirty seven admissions were on the pathway (45.7%) and 44 were not (54.3%). Documentation of severity indices of patients who were admitted under the pathway were significantly improved with a trend for a shorter hospital stay. The duration of intravenous insulin therapy, intensive care unit consultation and diabetes educator involvement in patient care were not different between the two groups. Residents found the pathway user-friendly, educationally very valuable, reduced their workload and had a positive effect on their DKA management skills.?Conclusions: Use of specially structured, resident-friendly pathway led to significant improvement in documentation of DKA severity indices and empowered our residents with evidence-based knowledge and skills to deal with this serious diabetic complication.

Investigating the Association of Smoking with Thyroid Volume and Function

This study was conducted to explore possible correlation between smoking habit and thyroid volume and function in Jeddah. A total of 226 volunteers i.e. 128 male and 98 female were screened at Radiology Department King Abdul-Aziz University Hospital. They were categorized as smokers and non- smokers;the number of smokers was 99 cases i.e. 48 Cigarette smokers and 51 Shisha smokers, and the non-smokers were 127 cases. The information was gathered via a questionnaire distributed at the radiology department. Ultrasonography of thyroid and thyroid function test were performed for 166 participants;and data were analyzed Using SPSS version 22 and Microsoft excel. The study was carried out based on random selection and findings revealed that most of the cases were in the age group between 20 to 30 years old, high body mass index (BMI) was 33.1 kg/m2 in persons above 60 years old and the highest percentage smokers in the age group from 20 to 30 years (40.02%). Ultrasonography of thyroid showed 96 (58%) normal cases, 46 (28%) abnormal “solid” cases and 24 (14%) abnormal “cyst” cases. A lower proportion of cigarette and shisha smokers (15.4% and 5.6% respectively) had an enlarged thyroid gland compared to no cigarettes or shisha smokers (47.9% and 47.3% respectively). The difference between these frequencies was statistically significant (Chi-square = 9.446 and 11.424, p = 0.002 and p = 0.001 for cigarette and shisha smoking respectively). Consequently, it can be concluded from this research that there are no direct significant values correlating smoking habit to thyroid volume or function. However, it is always recommended not smoke due to other well-known threats.

AIM: To investigate the merits of texture analysis on parametric maps derived from pharmacokinetic modeling with dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI) as imaging biomarkers for the prediction of treatment response in patients with head and neck squamous cell carcinoma(HNSCC). METHODS: In this retrospective study,19 HNSCC patients underwent pre- and intra-treatment DCEMRI scans at a 1.5T MRI scanner. All patients had chemo-radiation treatment. Pharmacokinetic modeling was performed on the acquired DCE-MRI images,generating maps of volume transfer rate(Ktrans) and volume fraction of the extravascular extracellular space(ve). Image texture analysis was then employed on maps of Ktrans and ve,generating two texture measures: Energy(E) and ***: No significant changes were found for the mean and standard deviation for Ktrans and ve between pre- and intra-treatment(P > 0.09). Texture analysis revealed that the imaging biomarker E of ve was significantly higher in intra-treatment scans,relative to pretreatment scans(P < 0.04). CONCLUSION: Chemo-radiation treatment in HNSCC significantly reduces the heterogeneity of tumors.
