Spondyloarthropathies （SPA） are commonly observed extra-intestinal manifestations of both Crohn＇s disease （CD） and ulcerative colitis （UC）, the two major forms of inflammatory bowel diseases （[BD）. However, the immunological link between these two clinical entities is still poorly understood. Several lines of evidence indicate that SpA may originate from the relocation to the joints of the immune process primarily induced in the gut. The transfer of the intestinal inflammatory process into the joints implicates that immune cells activated in the gut-draining lymph nodes can localize, at a certain point of the intestinal disease, either into the gut or into the joints. This is indicated by the overlapping expression of adhesion molecules observed on the surface of intestinal and synovial endothelial cells during inflammation. Moreover bacterial antigens and HLA-B27 expression may be implicated in the reactivation of T cells at the articular level. Finally, accumulating evidence indicates that a T helper 17 cell-mediated immune response may contribute to IBD and IBD-related SpA with a crucial role played by tumor necrosis factor-α in CD and to a lesser extent in UC.

Snowfall and the subsequent evolution of the snowpack have a large effect on the surface energy balance and water cycle of the Tibetan Plateau(TP).The effects of snow cover can be represented by the WRF coupled with a land surface *** widely used Noah scheme is computationally efficient,but its poor representation of albedo needs considerable *** this study,an improved albedo scheme is developed using a satellite-retrieved albedo that takes snow depth and age into *** experiments were then conducted to simulate a severe snow event in March *** performance of the coupled WRF/Noah model,which implemented the improved albedo scheme,is compared against the model’s performance using the default Noah albedo scheme and against the coupled WRF/CLM that applied CLM albedo *** the improved albedo scheme is implemented,the albedo overestimation in the southeastern TP is reduced,reducing the RMSE of the air temperature by 0.7°*** improved albedo scheme also attains the highest correlation between the satellite-derived and the model-estimated albedo,which provides for a realistic representation of both the snow water equivalent(SWE)spatial distribution in the heavy snowbelt(SWE>6 mm)and the maximum SWE in the eastern *** underestimated albedo in the coupled WRF/CLM leads to underestimating the regional maximum SWE and a consequent failure to estimate SWE in the heavy snowbelt *** study demonstrates the feasibility of improving the Noah albedo scheme and provides a theoretical reference for researchers aiming to improve albedo schemes further.

To overcome large deformation of deep phosphate rock roadways and pillar damage,a new type of constant-resistance large-deformation negative Poisson’s ratio(NPR)bolt that can withstand a high prestress of at least 130 KN was *** the conducted tests,the amount of deformation was 200-2000 mm,the breaking force reached 350 KN,and a high constant-resistance pre-stress was maintained during the deformation process.A stress compensation theory of phosphate rock excavation based on NPR bolts is proposed together with a balance system for bolt compensation of the time-space effect and high NPR *** split-set rock bolts are unable to maintain the stability of roadway roofs and *** verify the support effect of the proposed bolt,field tests were conducted using both the proposed NPR bolts and split-set rock bolts as support systems on the same mining *** addition,the stress compensation mechanism of roadway mining was simulated using the particle flow code in three dimensions(PFC^(3D))-fast Lagrangian analysis of continua(FLAC^(3D))particle-flow coupling numerical ***-site monitoring and numerical simulations showed that the NPR excavation compensation support scheme effectively improves the stress state of the bolts and reduces the deformation of the surrounding *** to the original support scheme,the final deformation of the surrounding rock was reduced by approximately 70%.These results significantly contribute to domestic and foreign research on phosphate-rock NPR compensation support technology,theoretical systems,and engineering practices,and further promote technological innovation in the phosphate rock mining industry.

The purpose of the present review is to summarise the current knowledge on the association between single nucleotide polymorphisms (SNPs) in the interferon L3 (IFNL3) gene and hepatitis C virus (HCV) infection in children. Many studies in adults have demonstrated that genetic variation in IFNL3 is a strong predictor of the virological response in treatment-naive patients with HCV genotype 1 who were treated with Pegylated-IFN-α and ribavirin. Genetic variation in IFNL3 is also associated with the spontaneous clearance of HCV. Thus far, few paediatric studies have explored the association between variations in the IFNL3 gene and either spontaneous or treatment-induced clearance of HCV. The CC genotype of the rs12979860 SNP is associated with the spontaneous clearance of HCV in children independently of HCV genotype. Four paediatric studies have shown that both the CC genotype of the rs12979860 SNP and the TT genotype of the rs8099917 SNP are associated with the treatment-induced (IFN monotherapy and Pegylated-IFN-α and ribavirin association) clearance of HCV, while the rs12980275 SNP did not affect the virological response. The possible role of IFNL3 gene variation as a pre-treatment and on-treatment predictor of virological response in children is highly attractive but still undetermined. Further paediatric studies are needed to evaluate if testing for SNPs in IFNL3, either alone or together with other predictors of response to treatment, could be used to direct treatment strategies, including an avoidance of unnecessary protease inhibitor therapy and the duration of treatment.

The importance of the vascular supply for bone is well-known to orthopaedists but is still rather overlooked within the wider field of skeletal research. Blood supplies oxygen, nutrients and regulatory factors to tissues, as well as removing metabolic waste products such as carbon dioxide and acid. Bone receives up to about 10% of cardiac output, and this blood supply permits a much higher degree of cellularity, remodelling and repair than is possible in cartilage, which is avascular. The blood supply to bone is delivered to the endosteal cavity by nutrient arteries, then flows through marrow sinusoids before exiting via numerous small vessels that ramify through the cortex. The marrow cavity affords a range of vascular niches that are thought to regulate the growth and differentiation of hematopoietic and stromal cells, in part via gradients of oxygen tension. The quality of vascular supply to bone tends to decline with age and may be compromised in common pathological settings, including diabetes, anaemias, chronic airway diseases and immobility, as well as by tumours. Reductions in vascular supply are associated with bone loss. This may be due in part to the direct effects of hypoxia, which blocks osteoblast function and bone formation but causes reciprocal increases in osteoclastogenesis and bone resorption. Common regulatory factors such as parathyroid hormone or nitrates, both of which are potent vasodilators, might exert their osteogenic effects on bone via the vasculature. These observations suggest that the bone vasculature will be a fruitful area for future research.

Pegylated interferon (IFN) α-2a or 2b in combination with ribavirin for children aged 3 years and older is the standard treatment for paediatric chronic hepatitis C. This treatment regimen was developed firstly in adults. In recent years, a number of direct-acting antiviral agents (DAAs) are under development for treatment of chronic hepatitis C virus (HCV) infection. These agents block viral replication inhibiting directly one of the several steps of HCV lifecycle. DAAs are classified into several categories based on their molecular target: HCV NS3/4A protease inhibitors, HCV NS5B polymerase inhibitors and HCV NS5A inhibitors. Other promising compounds are cyclophilin A inhibitors, mi-RNA122 and IFN-λ. Several new drugs associations will be developed in the near future starting from the actual standard of care. IFN-based and IFN-free regimens are being studied in adults. In this constantly evolving scenario new drug regimens targeted and suitable for children would be possible in the next future. Especially for children, it is crucial to identify the right combination of drugs with the highest potency, barrier to resistance and the best safety profile.
