Ehlers-Danlos syndrome:prevalence and outcomes in gender affirming surgery-a single institution experience

Aim:Patients with Ehlers-Danlos Syndrome(EDS)are considered to have an increased risk for wound healing complications.Surgeons may therefore be hesitant to offer elective surgeries,including gender-affirming surgeries(GAS),to EDS patients.At our center,we frequently encountered patients presenting for GAS evaluation with the co-existing diagnosis of EDS.This study aims to establish the prevalence of EDS diagnosis in our GAS patients and compare their post-operative complications to patients without EDS diagnosis.Methods:This is a single-institution retrospective case-control study on all patients who underwent GAS from 2016-2020.Data include EDS diagnosis,demographics,operation,and complications(including minor wound healing issues).Results:Of 1363 patients presenting for GAS,36(2.6%)had EDS diagnoses and were matched with 108 control patients.Major complications requiring surgical intervention in the OR occurred in 6 patients(4.2%),(2.8% EDS vs.5.4%controls;P=0.63),while 8.3% of EDS and 14% of controls required minor interventions(P=0.38).The rate of wound healing issues of any severity was 28% in EDS vs.47% in control groups(P=0.04).Conclusion:The prevalence of EDS diagnosis in our patient population is 132 times the highest reported prevalence in the general population.Wound healing issues and the need for additional post-operative interventions in the group with EDS diagnosis were not significantly different from the control group.Our findings suggest that patients with a diagnosis of EDS undergoing GAS have comparable outcomes to patients without EDS.Concerns for post-operative complications should not be a barrier to offering GAS to patients presenting with an EDS diagnosis.

The 28th Conference of Parties(COP 28)of the United Nations Framework Convention on Climate Change(UNFCCC)is being held in Dubai,United Arab Emirates,from November 30 to December 12,2023,with the participation of over 160 world leaders(Figure 1).The tone of the COP 28 is set by the observation that,clearly,the nationally determined contributions related to CO_(2) emission reduction are not on track to curb the temperature rise as per the Paris Agreement.In order to hold the increase in global mean temperature to well below 2℃ above the pre-industrial level and to pursue efforts to limit the warming to 1.5℃,there is a need for ratcheting up ambition for near-term climate action.

Britanin–a beacon of hope against gastrointestinal tumors?

Britanin is a bioactive sesquiterpene lactone known for its potent anti-inflammatory and anti-oxidant properties.It also exhibits significant anti-tumor activity,suppressing tumor growth in vitro and in vivo.The current body of research on Britanin includes thirty papers predominantly related to neoplasms,the majority of which are gastrointestinal tumors that have not been summarized before.To drive academic debate,the present paper reviews the available research on Britanin in gastrointestinal tumors.It also outlines novel research directions using data not directly concerned with the digestive system,but which could be adopted in future gastrointestinal research.Britanin was found to counteract liver,colorectal,pancreatic,and gastric tumors,by regulating proliferation,apoptosis,autophagy,immune response,migration,and angiogenesis.As confirmed in pancreatic,gastric,and liver cancer,its most commonly noted molecular effects include nuclear factor kappa B and B-cell lymphoma 2 downregulation,as well as Bcl-2-associated X protein upregulation.Moreover,it has been found to induce the Akt kinase and Forkhead box O1 axis,activate the AMP-activated protein kinase pathway,elevate interleukin-2 and peroxisome proliferator-activated receptor-γlevels,reduce interleukin-10,as well as downregulate matrix metalloproteinase-9,Twist family bHLH transcription factor 1,and cyclooxygenase-2.It also inhibits Myc–HIF1αinteraction and programmed death ligand 1 transcription by interrupting the Ras/RAF/MEK/ERK pathway and mTOR/P70S6K/4EBP1 signaling.Future research should aim to unravel the link between Britanin and acetylcholinesterase,mast cells,osteolysis,and ischemia,as compelling data have been provided by studies outside the gastrointestinal context.Since the cytotoxicity of Britanin on noncancerous cells is significantly lower than that on tumor cells,while still being effective against the latter,further in-depth studies with the use of animal models are merited.The compound exhibits pleiotropic biological activity and offers

In accordance with the World Health Organization data,cancer remains at the forefront of fatal diseases.An upward trend in cancer incidence and mortality has been observed globally,emphasizing that efforts in developing detection and treatment methods should continue.The diagnostic path typically begins with learning the medical history of a patient;this is followed by basic blood tests and imaging tests to indicate where cancer may be located to schedule a needle biopsy.Prompt initiation of diagnosis is crucial since delayed cancer detection entails higher costs of treatment and hospitalization.Thus,there is a need for novel cancer detection methods such as liquid biopsy,elastography,synthetic biosensors,fluorescence imaging,and reflectance confocal microscopy.Conventional therapeutic methods,although still common in clinical practice,pose many limitations and are unsatisfactory.Nowadays,there is a dynamic advancement of clinical research and the development of more precise and effective methods such as oncolytic virotherapy,exosome-based therapy,nanotechnology,dendritic cells,chimeric antigen receptors,immune checkpoint inhibitors,natural product-based therapy,tumor-treating fields,and photodynamic therapy.The present paper compares available data on conventional and modern methods of cancer detection and therapy to facilitate an understanding of this rapidly advancing field and its future directions.As evidenced,modern methods are not without drawbacks;there is still a need to develop new detection strategies and therapeutic approaches to improve sensitivity,specificity,safety,and efficacy.Nevertheless,an appropriate route has been taken,as confirmed by the approval of some modern methods by the Food and Drug Administration.

Twenty years of coal mining-induced subsidence in the Upper Silesia in Poland identified using InSAR

The paper presents the results of terrain subsidence monitoring in Poland's Upper Silesian Coal Basin(USCB)mining area using Differential Interferometry Synthetic Aperture Radar(DInSAR)and Persistent Scatterer Interferometry(PSI).The study area accounts for almost three million inhabitants where mining which started in the 19th century,has produced severe damage to buildings and urban infrastructures in past years.The analysis aimed to combine eight different datasets,processed in two techniques,coming from various sensors and covering different periods.As a result,a map of areas that have been exposed to subsidence within 3045 square kilometers was obtained.The map covers a period of twenty years of intensive mining activities,i.e.1992-2012.A total of 81 interferograms were used in the study.The interferograms allowed not only to determine subsidence troughs(basins)formed from 1992 to 2012 but also to observe subsidence development over time.The work also included five sets of PSI processing,covering different temporal and spatial ranges,which were used to determine zones of residual subsidence.Based on InSAR datasets,an area of 521 square kilometers under the influence of mining activities were determined.Within the subsiding zones,an area of 312.5 square kilometers of the rapid increase in subsidence was identified on the interferograms.The study of combined different InSAR datasets provided large-area and long-term information on the impact of mining activities in the Upper Silesia Coal Basin.

Experimental analysis of ballistic trauma in a human body protected with 30 layer packages made of biaxial and triaxial Kevlar^(■)29 fabrics

Behind armour blunt trauma(BABT)is a body injury resulting from the deformation of the back surface of armour as a result of a bullet impact.In the case of textile body armour,the severity of the injury may depend on the material of the fibres,but also on the geometric structure of the fabric.The article focuses on experimental research into injuries of the human body protected by ballistic packets made of biaxial and triaxial fabrics,during a non-penetrating impact from a Parabellum 9 mm×19 mm Full Metal Jacket(FMJ)bullet,at a speed of 406±5 m/s.In experimental research,the fabrics had a comparable surface weight and were made of the same Kevlar^(■)29 yarn.The ballistic packages were made of 30 layers.As part of the work,a physical model of the human body was developed.The human body model consisted of a model of the heart,lungs,and skeletal and muscular systems.During the bullet impact,the pressure forces were recorded using sensors located at selected points of the human body.The bullets hit five selected places on the body that were considered critical,from the point of view of maintaining a human’s vital functions.It was found that,during firing,pressure increases both at the site of impact and in the internal organs,which can lead to multi-organ damage.As a result of the experimental analysis,it has been shown that the pressures exerted on specific organs are always lower in the case of body protection with a ballistic packet made of triaxial fabrics,compared to a packet made of biaxial fabrics.

Investigations on influence of rifle automatics system action on values of energetic efficiency coefficient of muzzle brakes

The objective of the paper was to compare values of the muzzle brake efficiency coefficient for a rifle with active or inactive automatics systems.Special laboratory stand designed for investigating the recoil process was used.The motion of the rifle was detected by the use of the laser interferometer and the optical camera.The recoil velocity time courses were determined by smoothing and differentiation of experimental position records.The results of the experiments indicated that in the case of an active automatics system two values of the recoil velocity can be used for calculation of the energetic efficiency coefficient:the maximum recoil velocity and the final recoil velocity at the end of the automatics action cycle.The values of the coefficient,calculated using these two values of the recoil velocity,distinctly differ.However,it was shown that their values indicate the same relation between the efficiency of various muzzle brakes.The value of the efficiency coefficient,determined on the basis of the final recoil velocity value,is practically the same as that determined on the basis of the final recoil velocity value for the rifle with an inactive automatics system.

Recent advances and trends in roll bonding process and bonding model:A review

This review presents a thorough survey of the roll bonding process with a focus on the bimetallic bars/tubes as well as the bonding models and criteria.The review aims to provide insight into cold,hot and cryogenic bonding mechanisms at the micro and atomic scale and act as a guide for researchers working on roll bonding,other joining processes and bonding simulation.Mean-while,the shortcomings of roll bonding processes are presented from the aspect of formable shapes,while bonding models are shown from the aspect of calculation time,convergence,interface behav-ior of dissimilar materials as well as hot bonding status prediction.Two well-accepted numerical methodologies of bonding models,namely the contact algorithm and cohesive zone model(CZM)of bonding models and in simulations of the bonding process are highlighted.Particularly,recent advances and trends in the application of the combination of mechanical interlocking and metallurgical bonding,special energy fields,gradient structure,novel materials,green technology and soft computing method in the roll bonding process are also discussed.The challenges for advancing and prospects of the roll bonding process and bonding model are presented in an attempt to shed some light on the future research direction.

Thermal Activation of the Combustion Chamber of a Reciprocating Internal Combustion Engine

The dominant feature of the work of many specialists on the design of the internal combustion engine is to achieve the possible lowest fuel consumption, which is the most convenient measure of the efficiency of the thermodynamic cycle. From the very beginning of this heat machine, there are many of solutions that are better or worse fit to the global development trend. In this paper, the authors suggest the possibility closer to the optimal solution by introducing structural active zones, where the task is instantaneous(during one cycle) heat transfer to the skin layer on elements of the combustion chamber and to transfer part of the recovered heat to the next cycle, reaching a growth effect of the useful efficiency by 10% to 20%. The solution is different from the ones used by the fact that it is only partially insulating barrier, and the main goal is focused on supporting the next thermal cycle by which it is possible to affect as mentioned above. This paper describes the idea of the functioning of the active combustion chamber and pointed out the technical capabilities of its creation.

New complexes of heteroaromatic N-oxides with europium,uranyl and zinc ions

New solid complexes of europium, uranyl and zinc ions with N-oxides of 4-chloro-2,6-dimethylpyridine, quinoline and 4-meth- oxyquinaldinic acid in presence different anions were obtained and characterized by elemental and TG analyses, IR and luminescence spectra. The compounds are crystalline, hydrated or anhydrous salts with colours typical of metal ions. Thermal studies showed that in hydrated salts lattice or coordination water molecules are present. A role of different anions in the formation of various types of the complexes is presented.
