Purpose:Sepsis is a common acute life-threatening condition that emergency physicians routinely *** options within the Emergency Department(ED)are limited due to lack of infrastructure,consequently limiting the use of invasive hemodynamic monitoring or imaging *** mortality rate due to sepsis can be assessed via multiple scoring systems,for example,mortality in emergency department sepsis(MEDS)score and sepsis patient evaluation in the emergency department(SPEED)score,both of which quantify the variation of mortality rates according to clinical findings,laboratory data,or therapeutic *** study aims to improve the management processes of sepsis patients by comparing SPEED score and MEDS score for predicting the 28-day mortality in cases of emergency ***:The study is a cross-sectional,prospective study including 61 sepsis patients in ED in Suez Canal University Hospital,Egypt,from August 2017 to June *** were selected by two steps:(1)suspected septic patients presenting with at least one of the following abnormal clinical findings:(a)body temperature higher than 38℃ or lower than 36℃,(b)heart rate higher than 90 beats/min,(c)hyperventilation evidenced by respiratory rate higher than 20 breaths/min or PaCO2 lower than 32 mmHg,and(d)white blood cell count higher than 12,000/μL or lower than 4000/μL;(2)confirmed septic patients with at least a 2-point increase from the baseline total sequential organ failure assessment(SOFA)score following *** inclusion criteria included adult patients with an age≥18 years regardless of gender and those who had either systemic inflammatory response syndrome or suspected/confirmed *** were shortly follow-up for the 28-day *** patient was subject to SPEED score and MEDS score and then the results were compared to detect which of them was more effective in predicting *** receiver operating characteristic curves were also done for MEDS and SPE

In this paper,load frequency control is performed for a two-area power system incorporating a high penetration of renewable energy sources.A droop controller for a type 3 wind turbine is used to extract the stored kinetic energy from the rotating masses during sudden load *** auxiliary storage controller is applied to achieve effec-tive frequency *** coot optimization algorithm(COA)is applied to allocate the optimum parameters of the fractional-order proportional integral derivative(FOPID),droop and auxiliary storage *** fitness function is represented by the summation of integral square deviations in tie line power,and Areas 1 and 2 frequency *** robustness of the COA is proven by comparing the results with benchmarked optimizers including:atomic orbital search,honey badger algorithm,water cycle algorithm and particle swarm *** assessment is confirmed in the following four scenarios:(i)optimization while including PID controllers;(ii)optimization while including FOPID controllers;(iii)validation of COA results under various load disturbances;and(iv)validation of the proposed controllers under varying weather conditions.

Evaluating the influence of a structured nursing protocol on targeted outcomes in rheumatoid arthritis patients

Objective:Rheumatoid arthritis(RA)requires comprehensive *** nursing protocols may enhance outcomes,but evidence is *** study evaluated the effect of a structured nursing protocol on RA *** and Methods:In this one-group pre-post study,30 Egyptian RA patients completed assessments before and after a 12-week nursing protocol comprising education,psychosocial support,and self-management *** included clinical evaluation of joint counts,erythrocyte sedimentation rate(ESR),and C-reactive protein(CRP)and patient-reported Arthritis Self-Efficacy Scale(ASES),Health Assessment Questionnaire(HAQ),Visual Analog Scale(VAS)for pain,and Hospital Anxiety and Depression Scale(HADS).Results:The study demonstrated significant improvements in both clinical-and patient-reported *** count decreased from 18.4±4.2 to 14.2±3.8(P<0.001),ESR from 30.1±6.8 mm/h to 25.5±6.8 mm/h(P<0.01),and CRP levels from 15.2±3.6 mg/L to 11.8±2.9 mg/L(P<0.01)***-reported outcomes showed a marked increase in ASES score from 140±25 to 170±30(P<0.001)and reductions in HAQ from 1.6±0.4 to 1.3±0.3(P<0.01),VAS pain score from 7.8±1.7 to 6.2±1.2(P<0.001),and HADS anxiety and depression scores from 11±3 to 8±2(P<0.05)and 10±2 to 7±1(P<0.05),***:A structured nursing protocol significantly improved clinical disease activity,physical functioning,pain,self-efficacy,and emotional well-being in RA patients.A multifaceted nursing intervention appears beneficial for optimizing RA outcomes.

Prevalence of intestinal parasitic infections and its relationship with socio-demographics and hygienic habits among male primary schoolchildren in Al-Ahsa,Saudi Arabia

Objective:To investigate the prevalence of different intestinal parasitic infections among male primary schoolchildren in Al-Ahsa,Saudi Arabia,and to explore the possible relative socioeconomic, environmental and behavioural sanitary ***:A cross-sectional descriptive study was performed.A total of 1 289 male students aged from 7 to 12 years were selected randomly from 20 urban and 12 rural primary schools by multistage sampling method. Data collection was carried out by self administered questionnaire form to the parents/guardians of students and included inquires about socio-demographics,environmental conditions,and behavioural sanitary *** analysis was carried out to examine the presence of intestinal parasitic ***:Overall prevalence of parasitic infections was 27.2%,more among rural *** encountered infections included Entamoeba histolytica/dispar(8.2%), Giardia lamblia(6.5%),Entamoeba coli(4.0%) and Enterobius vermicularis(1.6%).Logistic regression revealed that lower maternal educational level and occupational status,low family income,big family size,poor personal hygienic practices and positive history of previous intestinal infections among family members increased the likelihood of ***: Although of low magnitude,intestinal parasitic infections still represent a public health concern among male schoolchildren in Al-Ahsa ***-demographic and poor personal hygienic habits are the main predictors for these infections.

Impact of auditory stimulation nursing protocol on auditory response in cerebrovascular stroke patients

Objectives:The objective of the study was to explore the impact of auditory stimulation protocol on auditory response in cerebrovascular stroke(CVS)*** and Methods:A pre-posttest nonequivalent control group quasi-experimental study was conducted at Al Mainal Hospital at Al Kars Einy Hospital,Cairo University,among 60 CVS patients,and the sample was randomly assigned through a coin to allocate subjects to study or control group(30 for each group).Auditory stimulation techniques were applied three times per week for the study group for two consecutive *** in the control group received only routine hospital care during the study *** Glasgow Coma Scale(GCS)score for assessing the neurological function and level of arousal and the auditory subscale score of Western Neuro Sensory Stimulation Profile(WNSSP)for assessing auditory response were recorded and compared between the two groups before and after the ***:There were statistically significant differences between the study group and the control group regarding third reading of GCS score(11.5±0.3 vs.10.6±0.9,t=4.676,P=0.000).The scores of all auditory subscales of WNSSP in the study groups increased after implementing the auditory stimulation protocol(P<0.05).Conclusion:Utilizing the auditory stimulation protocol by nurse proves to be an effective method for enhancing hearing responses in individuals with CVS.

Instrumented assisted soft tissue mobilization vs extracorporeal shock wave therapy in treatment of myofascial pain syndrome

BACKGROUND Active myofascial trigger points(TrPs)often occur in the upper region of the upper trapezius(UT)*** TrPs can be a significant source of neck,shoulder,and upper back pain and *** TrPs and their related pain and disability can adversely affect an individual’s everyday routine functioning,work-related productivity,and general quality of *** To investigate the effects of instrument assisted soft tissue mobilization(IASTM)vs extracorporeal shock wave therapy(ESWT)on the TrPs of the UT *** A randomized,single-blind,comparative clinical study was conducted at the Medical Center of the Egyptian Railway Station in *** patients(28 females and 12 males),aged between 20-years-old and 40-years-old,with active myofascial TrPs in the UT muscle were randomly assigned to two equal groups(A and B).Group A received IASTM,while group B received *** group was treated twice weekly for 2 *** groups received muscle energy technique for the UT *** were evaluated twice(pre-and posttreatment)for pain intensity using the visual analogue scale and for pain pressure threshold(PPT)using a pressure *** Comparing the pre-and post-treatment mean values for all variables for group A,there were significant differences in pain intensity for TrP1 and TrP2(P=0.0001)and PPT for TrP1(P=0.0002)and TrP2(P=0.0001).Also,for group B,there were significant differences between the pre-and post-treatment pain intensity for TrP1 and TrP2 and PPT for TrP1 and TrP2(P=0.0001).There were no significant differences between the two groups in the post-treatment mean values of pain intensity for TrP1(P=0.9)and TrP2(P=0.76)and PPT for TrP1(P=0.09)and for TrP2(P=0.91).CONCLUSION IASTM and ESWT are effective methods for improving pain and PPT in patients with UT muscle *** is no significant difference between either treatment method.

Cytotoxic activity and phytochemical analysis of Arum palaestinum Boiss.

Objective: To evaluate the in vitro cytotoxic activity of the fractionated extract as well as isolated compounds of Arum palaestinum Boiss.(A. palaestinum)(black calla lily), and to identify the volatile components which may be responsible for the potential antitumor ***: A. palaestinum was collected from its natural habitats and subjected to phytochemical analysis for separation of pure compounds. In vitro cytotoxic activity was investigated against four human carcinoma cell lines Hep2, He La, Hep G2 and MCF7 for the fractionated extract and isolated compounds. While, the diethyl ether fraction was subjected to GC–MS analysis as it exhibited the most potent cytotoxic effect to evaluate the active constituents responsible for the cytotoxic ***: Four flavonoid compounds were isolated(luteolin, chrysoeriol, isoorientin,isovitexin) from the diethyl ether and ethyl acetate. The extracts and the pure isolated compounds showed a significant high antiproliferative activity against all investigated cell lines. The GC–MS analysis revealed the separation and identification of 15 compounds representing 95.01% of the extract and belonging to different groups of chemical ***: The present study is considered to be the first report on the cytotoxic activities carried out on different selected fractions and pure compounds of A. palaestinum to provide evidences for its strong antitumor activities. In addition,chrysoeriol and isovitexin compounds were isolated for the first time from the studied taxa.

Trabeculectomy with Ologen implant versus perfluoropropane gas bubble for open angle glaucoma in pseudophakic eyes

AIM:To evaluate the safety and efficacy of augmented trabeculotomy with Ologen versus perfluoropropane in management of pseudophakic ***:This is a comparative randomized study included 57 pseudophakic eyes of 57 patients with medically uncontrolled open angle glaucoma(OAG).Twentynine patients were allocated in group I(trabeculectomy with Ologen;trab-ologen group),while 28 patients were assigned in group II(trabeculectomy with perfluoropropane gas bubble;trab-C3 F8 gas bubble group).RESULTS:The intraocular pressure(IOP)was significantly reduced in both study groups at all postoperative follow up intervals(1 wk,3,6,12,18,24,30 and 36 mo,P<0.001).The differences between the mean IOP values of both groups remained statistically insignificance during the early 12 months of follow ***,the trab-ologen group achieved a statistically significant reduction over the trab-C3 F8 gas bubble group during the last 24 months of follow ***:Augmentation of trabeculectomy with either Ologen implant or perfluoropropane gas bubble are associated with strict long term IOP control and evident safety in medically-uncontrolled pseudophakic eyes with OAG.

High platelet distribution width can independently predict testicular survival in testicular torsion among patients with steady-state sickle cell anemia

Objective This study aimed to evaluate the predictive value of platelet volume indices(PVI),such as mean platelet volume(MPV),platelet distribution width(PDW),and plateletcrit(PCT),as prognostic indicators of testicular viability in torsion patients with steady-state sickle cell anemia(SCA)who underwent surgical *** Forty-eight patients with SCA with testicular torsion and 46 male control subjects were enrolled in the *** patients underwent scrotal color Doppler ultrasonography before surgery,and PVI(MPV,PDW,and PCT)values were measured in all *** duration and testicular volume were also *** The testicular salvage rate in patients with SCA was 73%after *** showed that MPV,PDW,and PCT values were significantly higher in torsed SCA as compared with controls(p<0.05).Orchiectomy in patients with SCA showed significantly higher MPV,PDW,and PCT values than the orchiopexy group(p<0.05).The MPV values of orchiectomy patients showed a higher significant cut-off of≥11.5 fL,which is higher than in torsed patients without SCA,as an indicator of testis *** also demonstrated a higher significant cut-off of≥12.7 fL for detorsion outcomes in patients with *** duration of less than 7 hours was also significantly correlated with orchiopexy(p≤0.001).Univariate analysis showed that higher MPV,increased PDW,and symptom duration were indicative of the outcome of testicular detorsion in *** analysis showed that increased PDW and symptom duration are prognostic parameters for testicular viability in *** Increased PDW and symptom duration can be used as parameters for predicting testicular detorsion outcomes in patients with steady-state SCA.
