Anatomical Variants of Celiac Trunk in Relation to Its Branching: A Preliminary Sub-Saharan Study

Background: Surgical treatment of upper mesocolic organs is improved by preoperative diagnosis of anatomical variants of celiac trunk. According to the literature, these anatomical variants are little known in sub-Saharan Africa. Purpose: To evaluate the prevalence of anatomical variants of celiac trunk in relation to its branching. Materials and methods: This was a cross-sectional study of descriptive type. It retrospectively evaluated 160 abdominal contrast enhanced CT-scan, from patients attending Yalgado OUEDRAOGO teaching hospital, from 1 January 2015 to 30 September 2016. Patients with a history of heavy abdominal surgery were excluded. Images obtained by 64-row CT-scan were analyzed for anatomical variants of the celiac trunk. Results: One hundred and twenty-eight patients (80%) had a classic anatomical configuration of celiac trunk, while thirty-two (20%) had at least one anatomical variant. Two anatomical variants were found in fifteen patients (9.4%) while five other patients (3.1%) had more than two variants. The most frequent anatomical variant was the hepato-splenic bifurcation, found in fourteen patients (8, 8%). It was followed by common celiac and mesenteric trunk, and then collateral arteries, in particular left hepatic artery and right lower diaphragmatic artery, each with three patients (1.9%). Conclusion: Anatomical variants related to celiac trunk branching, are as frequent in our study as in the literature. However, the two most common anatomical variants were hepato-splenic bifurcation and common celiac and mesenteric trunk.

Introduction: The purpose of this study was to determine the role of medical imaging in the management of ballistic trauma in the experience of the Burkina Faso Army Health Service. Methods: This was a retrospective, prospective, cross-sectional study of 54 cases of ballistic trauma that occurred in Burkinabe military personnel on Burkinabe territory between January 2016 and October 2019 and were managed by the Burkina Faso Army Health Service. Results: The mean age of the ballistic trauma victims was 28.9 years. The patients were all male. Multiple injuries were present in 19% of cases. The limbs were predominantly affected, followed by the trunk. Medical imaging was used both for the initial assessment and for the search for complications and follow-up after treatment. Standard radiography was the most commonly used modality followed by CT scan, particularly for the search for visceral lesions or for a more precise bone assessment. Ultrasound (E. FAST) was sometimes used. Magnetic resonance imaging was rarely used because of the ferromagnetic projectiles often present in ballistic trauma patients. Conclusion: Medical imaging has an important place in the management of military ballistic trauma.

Objectives: To carry out the assessment of CT scans in children at the Charles de Gaulle University Hospital Center (CGUPH). Patients and Methods: This was a retrospective collection descriptive cross-sectional study conducted from July 1 to November 29, 2021 in the Radiology Department of the CGUPH, having included computed tomography (CT) reports of patients under 18 years of age. The variables explored were the data reported on CT reports (type of scan, reason for examination, dose delivered, and abnormalities described). Results: Two hundred and ninety explorations were collected. The average age was 2.93 years with extremes of 0 and 17 years. The sex ratio was 1.35. Computed tomography scans mainly explored the cranioencephalic, spinal and abdominal stages. The most common reasons for exploration were fetal suffering and head injuries. The most frequently described lesions were anoxo-ischemic sequelar lesions. Mean irradiation doses were 1096 ± 670 mGy·cm for the cranial-encephalic stage, 603 ± 332 mGy·cm for the spine and 311 ± 395 mGy·cm for the thoracic stage. Conclusion: CT in children mainly explored the brain and concerned children under 5 years. Anatomical irradiation doses were higher than diagnostic reference levels in several countries. Optimization of scanning protocols and availability of MRI would reduce ionizing radiation in children.

Aim of the Study: The aim is to analyse the epidemiological, clinical and CT aspects of hydrocephalus in children. Method: This was a cross-sectional descriptive study with retrospective collection from 1 June 2021 to 31 December 2022 within the radiology department of Charles De Gaulle Pediatric Teaching Hospital in Ouagadougou, Burkina Faso. The study included the records of patients with hydrocephalus who underwent cerebral computed tomography within this radiology department. Results: Hydrocephalus is due to a disorder of the hydrodynamics of the cerebrospinal fluid causing an increase in the volume allocated to this fluid in the brain and being accompanied by an increase in the pressure of this fluid. Its diagnosis is established by Doppler ultrasound or MRI prenatally, transfontanellar ultrasound in newborns and young children whose fontanel is permeable and by CT or MRI in older children, who can also determine the etiology. MRI remains an imaging technique that is less available and less accessible than CT in developing countries like ours. In Burkina Faso, MRI is only available in two private health facilities. The objective of this study was to describe the epidemiological, clinical and computed tomography aspects of hydrocephalus in children at the Charles de Gaulle pediatric teaching hospital in order to contribute to the diagnostic assessment and better management of this pathology. Over 19 months, 105 cases of hydrocephalus were recorded out of a total population of 115 children, or an average of 6 cases per month. The mean age was 17.52 months, with extremes of 03 days and 13 years, and the sex ratio was 1.38. Macrocrania, convulsions and psychomotor retardation were the main clinical signs, with rates of 49%, 34% and 30% respectively. On cerebral CT scan, hydrocephalus was triventricular (41.90%), tetraventricular (40%), and predominantly non-communicating (60%), with a mean Evans score of 0.53. A regional study connecting geolelectrical surveys with geology and hydrogeology was carried out in the western part of the Iullemmeden basin, precisely in the Dosso region in Niger. One hundred and four (104) vertical electrical sounds have been realized, among them nineteen representative were thus be used as parametric surveys. The local resistivity values of the geological formations of Quaternary range from 100 Ω⋅m to 1000 Ω⋅m (sands and lateritic sandstones). The Oligo-Miocene formation of the Continental terminal (Ct) shows resistivity values ranging from 1 to 5 Ω⋅m (brackish groundwaters) to 1500 Ω⋅m (clay sandstones) while the Upper Cretaceous formation of the Continental “hamadien” (Ch) indicates values ranging from 20 Ω⋅m (sandy clay) to 5000 Ω⋅m (clayey sandstones). The geological formations of Paleocene have values from 2 Ω⋅m (marls) to 60 Ω⋅m (calcareous marl), while the Precambrian basement exhibits values of granite around 300 Ω⋅m to 60,000 Ω⋅m. The update of the structural settings reveals many faults in the study area which explain both the shape of the basin and the geometry of the aquifers. Tectonics is also consistent with the hydraulic characteristics of aquifers. In addition, brackish groundwaters were identified as perched aquifer groundwaters in different depths in Dosso region. They probably come from the marine brines during the regression of the Paleocene Sea.

Melnick-Needles syndrome is an osteo-dysplasia caused by a mutation of a gene, FLNA, coding for filamine A. It is at the origin of a set of complex congenital malformations, mainly of interest to the members, but also to the cranio-facial region. Melnick-Needles syndrome was first described in 1966 by two Americans John Melnick, a radiologist, and Carl F Needles, a pediatrician. They described cases of families of several generations who had a characteristic severe congenital bone disorder. They thought that human-to-human transmission had occurred in one case. We report a case of Melnick-Needles syndrome which is a pathology with very high mortality especially in a male subject. This was an observational study of a case received in consultation with the pediatric service of the Souro Sanou University Hospital Center in Bobo Dioulasso in Burkina Faso. It was a 3-year-old boy born in a low-term pregnancy without any prenatal consultation, imaging test and prenatal biology performed. The exact pathogenesis of this condition is not established but is linked to a mutation of the Filamine A gene linked to sexual chromosomes X. Pathology is rare, around a hundred cases have been reported worldwide. Its incidence, according to the literature is around 1/100,000. The sex ratio is at 7. The malformations of this acquired embryo-fetopathy are multiple, polymorphic and asymmetrical. The diagnosis of this pathology is suspected by the clinic and established by molecular biology by sequencing the genes responsible for the mutation. However, imagery, in particular the scanner has a major role by its protocol with multi-planar reconstructions and its analysis in double windowing which allow a better description of the malformations of this syndrome. Its management is complex, and multidisciplinary, and the unfortunate prognosis in our context is because of the precarious technical platform. Context: In spite of the development of cross-sectional medical imaging and endoscopy means, the prescription of specialized radiographic examinations is still relevant in the pediatric environment in our country. Objective: Show the contribution of specialized radiographic examinations performed in pediatric settings. Methodology: This was a retrospective descriptive study implemented over a five year-period, from June 10th, 2014 to June 10th, 2019 within the radiology department of Charles De Gaulle Pediatric Teaching Hospital in Ouagadougou. This study included the results of 192 specialized radiographic examinations collected within the concerned department. Results: The average age of the patients was 4.21 years with extremes of 8 days and 15 years. Infants (29 days to 24 months) were the predominant age group with a rate of 53.13% (n = 102 patients). Boys represented 76.51% of the sample with a sex ratio estimated at 3.25. Specialized radiographic examinations represented 0.46% of the overall imaging examinations and 0.72% of radiographic examinations performed during the study period. 67.19% of pathological examinations were reported. Colonic enema and retrograde urethrocystography (RUC) were the most performed examinations respectively accounting for 33.34% and 27.6%. The radiographic findings were consistent with the diagnostic hypotheses in 41.6% of cases. Conclusion: Specialized radiographic examinations keep their place in the exploration of several pathologies in pediatric settings.

We report a case of ambiguous genitalia in a 15-year-old patient to demonstrate input of medical imaging in the diagnosis and care of this rare congenital deformation. Disorders of sexual differentiation (DSD) have been defined as congenital conditions in which development of chromosome, gonad, or anatomic sex is atypical and that raise an issue in the sex assignment. In lower socioeconomic countries where antenatal diagnosis is almost non-existent, due to lack of technical facilities, their medical and surgical management is hard. The case which we report was investigated by ultrasonography and genitography, which remains primary indications in our context.

Objective: To correlate the epidemiological and histological data of breast le-sions ACR 3, ACR 4 and ACR 5 biopsied on ultrasound in the city of Ouaga-dougou. Materials and methods: This was a retrospective study of data for 02 years (2015-2017) in the city of Ouagadougou. With the approval of the Ethics Committee for Health Research, the study focused on the histological results and radiological reports of biopsied lesions. Results: A total of 150 patients files had been collected. Breast lesions classified as ACR 4 were 50%, ACR 3 40% and ACR 5 10%. The risk factors were related to the ACR class. Ultra-sound was performed in all patients. The masses were the most common ele-mentary lesions found in breast imaging. On histology 98 lesions were benign, 16 intermediate lesions and 36 malignant lesions. Fibroids were the most common benign lesions of the ACR 3 and ACR 4 classes. Invasive ductal car-cinoma was the most common histologic type among malignant lesions in all ACR classes. Conclusion: Cancerous lesions are discovered late in our context. The ACR classification must always take into account discriminative ele-ments.
