Objective: to determine the incidence of abnormal laboratory test results among isotretinoin users. Design: Retrospective cohort. Setting: Comprehensive managed care health plan in Northern California. Participants: the study popul.t.on comprised 13 772 patients aged 13 to 50 yearswith acne,undergoing oral isotretinoin therapy between March 1995 and September 2002. Main Outcome Measures: laboratory values for serum triglyceride, total cholesterol, and liver transaminase levels; white blood cell count, hemoglobin level, and pl.t.l.t.count; and frequency of abnormal laboratory results by severity grade as defined by the National Cancer Institute Common terminology Criteria for Adverse Events v3.0. Results: Substantial increases in the cumul.t.ve incidence of abnormal.t.es were seen in serum lipid and transaminase levels, but not in hematologic parameters, during isotretinoin treatment compared with the baseline period. the cumul.t.ve incidence of new abnormal.t.es in patients with normal values at baseline was 44%for triglyceride level, 31%for total cholesterol level, and 11%for transaminase level. Moderate to severe abnormal.t.es in lipid and transaminase levels were generally transient and reversible. New abnormal.t.es in hematological.t.st results were uncommon. Conclusions: the incidence of abnormally high serum lipid levels during isotretinoin treatment may be greater than previously estimated. Elevations in transaminase level are generally mild. Normal baseline values of serum lipid and transaminase levels do not preclude the development of new abnormal.t.es during isotretinoin treatment. Routine monitoring of white blood cell count, hemoglobin level, and pl.t.l.t.count during isotretinoin therapy may be of l.t.le util.t. without clinical suspicion of an abnormal.t.. the clinical significance of laboratory abnormal.t.es during isotretinoin therapy remains to be determined.

We describe a newborn infant who was born with a purpuric rash and subcutaneous nodules. Skin biopsies demonstrated acute myeloid leukemia. Cytogenetic studies revealed an 11q23 rearrangement. Initial bone marrow and cerebrospinal fluid examination did not demonstrate medullary or meningeal disease. Chemotherapy was initiated on the basis of the abnormal cytogenetic findings in the skin biopsy. Intensive chemotherapy was, given but the infant s leukemia progressed. the patient died of refractory leukemia and secondary fungal disease. this case report supports the observation that leukemia cutis with an 11q23 rearrangement should be treated aggressively.

Since the discovery of the hydroformyl.t.on(oxo-synthesis or Roelen reaction)and the Reppe-reaction,the transition metal-catalyzed carbonyl.t.on reactions,providing versatile,facile,and even atom-economic methods for the selective incorporation of C=O functional.t. to various skel.t.ns,have gained tremendous importance in synthetic organic chemistry from laboratories to industrial *** carbonyl.t.on of carbon-carbon multiple bonds,aromatic halides,trifl.t.s,etc.,in the presence of various nucleophiles has led the way to produce aldehydes,carboxylic acids,esters,amides,etc.,in the fine chemical ***,these protocols usually proceed in conventional,fossil-based,and usually toxic reaction ***,several attempts have been directed to developing efficient carbonyl.t.on methods in alternative,less harmful,non-fossil-based and even in renewable reaction *** this review,we overview the recent applications of alternative solvents such as water,biomass-based alcohols,γ-valerolactone(GVl),2-methyltetrahydrofuran(2-MetHF),ionic liquids(Ils),deep eutectic solvents(DES),alkyl levulinates,limonene,α-pinene,and dimethyl carbonate as well as fluorous media to improve efficiency,safety and environmentally benign nature of carbonyl.t.on protocols.

Nonlinear-Optical Analogies in Nuclear-like Sol.t.n Reactions:Selection Rules,Nonlinear tunneling and Sub-Barrier Fusion–Fission

the main goal of our study is to reveal unexpected but intriguing analogies arising between optical sol.t.ns and nuclear physics,which still remain hidden from *** consider the main cornerstones of the concept of nonlinear optics of nuclear reactions and the well-dressed repulsive-core *** the base of this model,we reveal.t.e most intriguing properties of the nonlinear tunneling of nucleus-like sol.t.ns and the sol.t.n selfinduced sub-barrier transparency *** describe novel interesting and stimul.t.ng analogies between the interaction of nucleus-like sol.t.ns on the repulsive barrier and nuclear sub-barrier *** main finding of this study concerns the conservation of total number of nucleons(or the baryon number)in nuclear-like sol.t.n *** show that inelastic interactions among well-dressed repulsive-core sol.t.ns arise only when a“cloud”of“dressing”spectral side-bands appears in the frequency spectra of the *** property of nucleus-like sol.t.ns is directly rel.t.d to the nuclear density distribution described by the dimensionless small shape-squareness *** the Fourier spectra of nucleus-like sol.t.ns are similar to the nuclear form *** show that the nuclear-like reactions between well-dressed sol.t.ns are realized by“exchange”between“particle-like”side bands in their spectra.

Whole-person care and holistic care approach has been proposed for complementary and integrative health care for type 2 diabetes ***,some doubts still exist on the feasibil.t. of replicating processes followed in clinical.t.ials and observational studies in real-world *** narrative l.t.rature review summarized and assessed existing clinical evidence(clinical.t.ials,observational studies,and case reports)describing holistic and integrated care approach in adult and adolescent individuals with type 2 diabetes mell.t.s in clinical *** goal was to highlight existing evidence for implementation and outcomes of whole-medical systems and holistic integrated care approach for type 2 diabetes mell.t.s.A nonsystematic l.t.rature search was performed on Google Scholar,PubMed,Web of Science,ProQuest and ScienceDirect to identify clinical evidence from different parts of the world,evaluating the use of whole-medical systems and/or holistic care interventions in clinical practice for management of type 2 diabetes *** keywords were used in the *** were analyzed using content analysis and simple descriptive statistics(percentages).Most of the studies(64%)were mainly conducted in Eastern countries(India,China and Israel)while 36%of the studies were conducted in the Western countries(USA,Netherlands,Canada and Mexico).lifestyle medicine and integrated naturopathy were shown to be the commonly used whole-medical systems for type 2 diabetes mell.t.s *** improvements in type 2 diabetes parameters,medication use,other symptoms,and overall feeling of wellness were observed in all *** review study revealed limited utilization and/or documentation of whole-medical systems or holistic care treatments for type 2 diabetes mell.t.s in regions of the world other than eastern *** medicine,naturopathy,yoga,Ayurveda and traditional Chinese medicine were shown to be effective for type 2 diabetes mell.t.

In situ tEM investigation of electron irradiation and aging-induced high-density nanoprecipitates in an Mg-10Gd-3Y-1Zn-0.5Zr alloy

In-situ electron irradiation and aging are applied to introduce high-density precipitates in an Mg-10Gd-3Y-1Zn-0.5Zr(GWZ1031K,wt.%)alloy to improve the *** results show that the hardness of the Mg alloy after irradiation for 10 h and aging for 9 h at 250℃ is 1.64 GPa,which is approximately 64% higher than that of the samples before being *** is mainly attributed to γ'precipitates on the basal plane after irradiation and the high-density nanoscale β'precipitates on the prismatic plane after aging,which should be closely rel.t.d to the irradiation-induced homogenous *** l.t.er plays a key role in precipitation *** result paves a way to improve the mechanical properties of metallic materials by tailoring the precipitation through irradiation and aging.

Sl.t. have been widely used in laser-plasma interactions as plasma optical components for generating high-harmonic light and controlling laser-driven particle ***,we propose and demonstrate that periodic thin sl.t. can be regarded as a new breed of optical elements for efficient focusing and guiding of intense laser *** fundamental physics of intense laser interaction with thin sl.t. is studied,and it is revealed that rel.t.vistic effects can lead to enhanced laser focusing far beyond the pure diffractive focusing *** addition,the interaction of an intense laser pulse with periodic thin sl.t. makes it feasible to achieve multifold enhancement in both laser intensity and energy transfer efficiency compared with conventional *** results provide a novel method for manipul.t.ng ultra-intense laser pulses and should be of interest for many laser-based applications.
