Evaluation of safety for hepatectomy in a novel mouse model with nonalcoholic-steatohepatitis

AIM To investigate whether the liver resection volume in a newly developed nonalcoholic steatohepatitis(NASH) model influences surgical *** For establishment of a NASH model, mice were fed a high-fat diet for 4 wk, administered CCl_4 for the last 2 wk, and administered T0901317 for the last 5 d. We divided these mice into two groups: A 30% partial hepatectomy(PH) of NASH liver group and a 70% PH of NASH liver group. In addition, a 70% PH of normal liver group served as the control. Each group was evaluated for survival rate, regeneration, apoptosis, necrosis and DNA expression after *** In the 70% PH of NASH group, the survival rate was significantly decreased compared with that in the control and 30% PH of NASH groups(P < 0.01). 10 of 32 mice in the NASH 70% PH group died within 48 h after PH. Serum aspartate aminotransferase(AST) levels and total bilirubin(T-Bil) in the NASH 70% PH group were significantly higher than the levels in the other two groups(AST: P < 0.05, T-Bil: P < 0.01). In both PH of NASH groups, signaling proteins involved in regeneration were expressed at lower levels than those in the control group(P < 0.01). The 70% PH of NASH group also exhibited a lower number of Ki-67-positive cells and higher rates of apoptosis and necrosis than the NASH 30% PH group(P < 0.01). In addition, DNA microarray assays showed differences in gene expression associated with cell cycle arrest and *** The function of the residual liver is impaired in fatty liver compared to normal liver. Predictive Factors for Smartphone Dependence: Relationship to Demographic Characteristics, Chronotype, and Depressive State of University Students

We investigated factors contributing to smartphone dependence. To 196 medical university students, we administered a set of self-reporting questionnaires designed to evaluate demographic characteristics, smartphone dependence, chronotype, and depressive state. Smartphone dependence was evaluated using the Wakayama Smartphone-Dependence Scale (WSDS) with 3 subscales: Subscale 1, immersion in Internet communication;Subscale 2, using a smartphone for extended periods of time and neglecting social obligations and other tasks;Subscale 3, using a smartphone while doing something else and neglect of etiquette. Multiple regression analyses revealed that living in a family, eveningness, and presence of depression were associated with Subscale 1, that living in a family and eveningness were also associated with Subscale 2, and that being a man was associated with Subscale 3. These findings suggest that smartphone dependence can be predicted by factors such as gender, mode of residence, chronotype, or depressive state.

Quantitative evaluation of long-term liver repopulation and the reconstitution of bile ductules after hepatocellular transplantation

AIM: The treatment of liver disease is severely limited by a shortage of donor livers. In trying to address this growing problem, hepatocellular transplantation (HTx) has received much attention as an alternative to whole organ ***, the expansion of transplanted cells is at low level,and the reconstitution of functional liver tissue is limited by this cellular property. We set up an animal model to better understand cell dose effect and the kinetics of liver repopulation following ***: Dipeptidyl peptidase Ⅳ (DPPIV)-deficient rats treated with retrorsine and subjected to partial hepatectomy were infused with DPPIV-positive hepatocytes. Rats were injected with varying numbers of donor hepatocytes down to 100 cells low, and liver repopulation was examined at different time points up to 20 mo long. Repopulation was assessed by computer-aided quantitative ***: Transplanted hepatocytes underwent multiple rounds of proliferation and stably repopulated the injured livers after 20 mo and at all cell doses. Transplanted cells divided 14 times within the 3-mo time period following infusion, and the liver repopulation reached a plateau between 3 and 20 mo. Approximately 90% replacement occurred. Donor-derived cells also reconstituted the bile ductules of the ***: The ability of transplanted hepatocytes to fully reconstitute injured livers strongly supports further investigation into the clinical potential of HTx. Additionally,the observation that transplanted hepatocytes also form components of the biliary system suggests that these cells may have bi-potential property of the stem cells.

Paraneoplastic syndromes associated with lung cancer

Paraneoplastic syndromes are signs or symptoms that occur as a result of organ or tissue damage at locations remote from the site of the primary tumor or metastases. Paraneoplastic syndromes associated with lung cancer can impair various organ functions and include neurologic, endocrine, dermatologic, rheumatologic, hematologic, and ophthalmological syndromes, as well as glomerulopathy and coagulopathy(Trousseau's syndrome). The histological type of lung cancer is generally dependent on the associated syndrome, the two most common of which are humoral hypercalcemia of malignancy in squamous cell carcinoma and the syndrome of inappropriate antidiuretic hormone secretion in small cell lung cancer. The symptoms often precede the diagnosis of the associated lung cancer, especially when the symptoms are neurologic or dermatologic. The proposed mechanisms of paraneoplastic processes include the aberrant release of humoral mediators, such as hormones and hormone-like peptides, cyto-kines, and antibodies. Treating the underlying cancer is generally the most effective therapy for paraneoplastic syndromes, and treatment soon after symptom onset appears to offer the best potential for symptom improvement. In this article, we review the diagnosis, potential mechanisms, and treatments of a wide variety of paraneoplastic syndromes associated with lung cancer.

Multiple primary malignant neoplasms （MPMNs） are rarely reported and it is important to give early diagnosis and proper therapy for these patients. Here reported a case of 62-year-old man with concomitant three early stage cancer lesions in upper gastrointestinal tract, all of which were detected by endoscopy. The first one was an llc-type lesion at angular part of stomach under endoscopy, which was histologically confirmed to be a mucosal well-differentiated *** patient underwent a standard radical gastrectomy for the lesion after the failure of endoscopic treatment. The other two neoplasms were observed during follow-up and were indicated as early stage lesions by synthesizing information from endoscopy, endoscopic ultrasonography, computed tomography and biopsy. One displayed as a hyperemic patch （3cm×4 cm in size） located at the part of esophagus 27 cm away from the incisor teeth and was proved to be moderately differentiated squamous cancer by histopathological examination. The other was an llc-type lesion （3.0 cm×3.5 cm in size） located at the part of esophagus 36 cm away from the incisor teeth, and the biopsy result showed a poorly differentiated squamous carcinoma. Both the two lesions were treated with radical radiation because the patient refused surgery management. No recurrence of former lesions or occurrence of novel lesions were observed during post-treatment follow-up, suggesting radical radiation might be effective for this patient.

Image quality and diagnostic performance of free-breathing diffusion-weighted imaging for hepatocellular carcinoma

AIM To retrospectively evaluate the diagnostic performance of free-breathing diffusion-weighted imaging(FB-DWI) with modified imaging parameter settings for detecting hepatocellular carcinomas(HCCs).METHODS Fifty-one patients at risk for HCC were scanned with both FB-DWI and respiratory-triggered DWI with the navigator echo respiratory-triggering technique(RTDWI).Qualitatively,the sharpness of the liver contour,the image noise and the chemical shift artifacts on each DWI with b-values of 1000 s/mm2 were independently evaluated by three radiologists using 4-point *** the image quality scores of each observer between the two DWI methods,using the Wilcoxon signed-rank ***,we compared the signal-to-noise ratios(SNRs) of the liver parenchyma and lesion-to-nonlesion contrast-to-noise ratios(CNRs) after measuring the signal intensity on each DWI with a b-factor of 1000 s/*** average SNRs and CNRs between the two DWI methods were compared by the paired *** detectability of HCC on each DWI was also analyzed by three *** detectability provided by the two DWI methods was compared using Mc Nemar's *** For all observers,the averaged image quality scores of FB-DWI were:Sharpness of the liver contour [observer(Obs)-1,3.08 ± 0.81;Obs-2,2.98 ± 0.73;Obs-3,3.54 ± 0.75],those of the distortion(Obs-1,2.94 ± 0.50;Obs-2,2.71 ± 0.70;Obs-3,3.27 ± 0.53),and the chemical shift artifacts(Obs-1,3.38 ± 0.60;Obs-2,3.15 ± 1.07;Obs-3,3.21 ± 0.85).The averaged image quality scores of RTDWI were:Sharpness of the liver contour(Obs-1,2.33 ± 0.65;Obs-2,2.37 ± 0.74;Obs-3,2.75 ± 0.81),distortion(Obs-1,2.81 ± 0.56;Obs-2,2.25 ± 0.74;Obs-3,2.96 ± 0.71),and the chemical shift artifacts(Obs-1,2.92 ± 0.59;Obs-2,2.21 ± 0.85;Obs-3,2.77 ± 1.08).All image quality scores of FB-DWI were significantly higher than those of RT-DWI(P < 0.05).The average SNR of the normal liver parenchyma by FB-DWI(11.0 ± 4.8) was not significantly different from that shown b
