BACKGROUND Massive pulmonary embolism(PE)results in extremely high mortality ***-arterial extracorporeal membrane oxygenation(VA-ECMO)can provide circulatory and oxygenation support and rescue patients with massive ***,there are relatively few studies of extracorporeal cardiopulmonary resuscitation(ECPR)in patients with cardiac arrest(CA)secondary to *** aim of the present study is to investigate the clinical use of ECPR in conjunction with heparin anticoagulation in patients with CA secondary to *** SUMMARY We report the cases of six patients with CA secondary to PE treated with ECPR in the intensive care unit of our hospital between June 2020 and June *** six patients experienced witnessed CA whilst in *** had acute onset of severe respiratory distress,hypoxia,and shock rapidly followed by CA and were immediately given cardiopulmonary resuscitation and adjunctive VA-ECMO *** hospitalization,pulmonary artery computed tomography angiography was performed to confirm the diagnosis of *** anticoagulation management,mechanical ventilation,fluid management,and antibiotic treatment,five patients were successfully weaned from ECMO(83.33%),four patients survived for 30 d after discharge(66.67%),and two patients had good neurological outcomes(33.33%).CONCLUSION For patients with CA secondary to massive PE,ECPR in conjunction with heparin anticoagulation may improve outcomes.

西部山区工程建设揭露了众多大型弯曲倾倒变形体,多具有软硬互层结构,水平深度可达300 m。为进一步探明软硬互层反倾边坡的倾倒变形机制,融合离心模型试验与UDEC模拟,研究了此类边坡的破坏模式与影响因素,并通过点对分析,讨论了变形的力学机制。数值模拟时,在岩层内预置随机裂隙,获得了破裂面的演化规律。结果表明:数值模拟与试验的位移曲线及破裂面形态吻合较好,边坡变形可分为起始蠕变阶段、稳态变形阶段和失稳破坏阶段;坡体前部为压剪复合变形,后部则以拉张为主;边坡主破裂面呈弧形,由坡脚快速贯通至坡顶,整体为拉–剪性破裂面;坡体内发育3条破裂面,可作为分界线将变形体分为极强倾倒区、强倾倒区和弱倾倒区;坡脚岩体变形后期压致拉裂,逐渐折断脱离母岩,最终导致变形岩体沿不同的破裂面形成渐进后退式破坏;边坡在倾角与坡角之和大于等于120°时才较易破坏,坡角主要影响破坏难易,倾角则控制变形规模。

目的设计一条原辅材料易得、低毒,操作条件简单可控的熊果酸A环开环结构修饰的工艺路线,从而得到更多具有广泛生物活性的熊果酸衍生物。方法对类似的化合物进行国内外文献资料的调查与研究工作,根据活性拼接原理提出设计路线。结果提出自行设计的熊果酸A环开环结构修饰路线成本较低,试剂低毒。结论熊果酸A环结构修饰路线设计合理,为熊果酸结构修饰物的深入研究提供基础。

Purpose: To evaluate the application of a high-frequency electrosurgical scalpel and methylene blue staining in the endonasal ***: This retrospective study included 37 patients(43eyes).undergoing endonasal dacryocystorhinostomy in our hospital between 2011 and 2013 using methylene blue staining of the lacrimal sac and a high-frequency electrosurgical scalpel for cutting nasal mucosa, intraoperative stanch, and fixation of lacrimal sac and nasal mucosal flaps. Surgical efficacy, intraoperative challenges, and corresponding handling methods were evaluated and ***: Among 43 eyes, 42 were successfully cured(97.7%)and the symptoms in 1 eye were improved(2.3%). Total efficacy rate was 100%. All surgeries were successfully performed. No severe intraoperative complications were ***: A high-frequency electrosurgical scalpel, combined with methylene blue staining of the lacrimal sac, is efficacious for nasal mucosal cutting, intraoperative stanch, and fixation of mucosal flap by cauterization, which significantly alleviates intraoperative complications and enhances surgical success rate. It deserves widespread application in clinical practice.

Multifunctional drug delivery systems(DDSs)have shown great prospects in overcoming the heterogeneous barrier of delivery drugs to the complex tumor microenvironment(TME).In this study,multifunctional AS/Ge-pNAB microgels with dual-active targeting,triple environment responsiveness,and fluorescence imaging capability were prepared through a straightforward *** was aimed to improve the antitumor therapeutic application of gambogic acid(GA)based on the biological characteristics of *** microgels have a uniform double-layer structure with aptamer in the outer layer which helps in recognizing receptors on the tumor *** GA loaded nano-herb exhibited environment-responsive drug release profiles under acidic pH,reductant and high *** nano-herb significantly improved the accumulation of GA in tumor sites through the synergistic combination of the enhanced permeability and retention effect and dual-ligand mediated ***,it accelerated intracellular drug release and killed tumor ***,the nano-herb had specific therapeutic effects on the tumor in vitro and in vivo as they remarkably inhibited tumor growth while depicting optimal biosafety and lower levels of off-target ***,these findings demonstrate the great potential of the multifunctional AS/Ge-pNAB microgels for precisely targeted GA delivery and open a new avenue for the facile preparation of multifunctional DDSs.

Objective To investigate the correlation between subaxial cervical spine instability and cervical spondylotic sympathetic symptoms as well as the difference of cervical spondylotic subaxial instability between male and female patients. Methods We analyzed retrospectively 318 surgical cases of cervical spondylosis treated at Department of Orthopedic Surgery of Peking Union Medical College Hospital between July 2003 and December 2007. All cases were divided into group A without sympathetic symptoms (n=284) and group B with sympathetic symptoms (n=34). Angular and horizontal translation values between two adjacent vertebral bodies from C2 to C7 were measured separately on hyperflexion and hyperextension lateral cervical spine radiographs. Fisher's exact test was used to evaluate the correlation between subaxial cervical instability and sympathetic symptoms. Intragroup correlation between patient gender and subaxial cervical instability was also evaluated. Results Subaxial instability incidences in groups A and B were 21.8% (62/284) and 55.9% (19/34), respectively. Statistical analysis indicated a definite correlation between subaxial cervical instability and sympathetic symptoms (P=0.000). Among patients without sympathetic symptoms, subaxial instability incidences were 21.4% (37/173) in males and 22.5% (25/111) in females, respectively (P=0.883). While among patients with sympathetic symptoms, subaxial instability incidences were 27.3% (3/11) in males and 69.6% (16/23) in females, respectively, indicating significant difference (P=0.030). Subaxial instability was most commonly seen at C4-C5 intervertebral space in sympathetic cervical spondylosis patients. Conclusions High correlation exists between subaxial cervical spine instability and cervical spondylotic sympathetic symptoms, especially in female patients. Hyperextension and hyperflexion radiographs of cervical spine are important to assess sympathetic cervical spondylotic subaxial instability.

BACKGROUND The treatment of benign biliary strictures(BBS)is a challenging clinical *** present,there is a lack of ideal models for the study of BBS *** To develop a novel animal model of BBS to simulate studies on the processes and mechanisms in the human *** A rabbit model of benign bile duct stricture was established by surgical injury of the bile *** removal of the gallbladder,a drainage tube was placed th-rough the cystic duct at the stump,and a BBS model was induced by surgical injury at the lower end of the common bile *** Compared with the control group,the model rabbits showed gross jaundice,increased serum bilirubin,and decreased liver *** showed segmental bile duct stenosis in the model *** staining showed inflammatory cell infiltration and fibrosis in the biliary tract of rabbits in the model *** was consistent with the clinical manifestations of *** model provided serology,imaging,pathology,and other aspects of *** We have successfully established an animal model of benign stricture of the lower bile duct with repeatable administration,which is consistent with the clinical manifestations of BBS.

Extraction, Purification, and Characterization of Insoluble Dietary Fiber from Oat Bran

The dietary fiber in oats is mainly concentrated in the bran;however,the oat bran is mostly used for livestock and poultry feed and thus has a low utilization rate and low added *** this study,insoluble dietary fiber(IDF)was extracted by a combination ofα-amylase and neutral *** optimal extraction conditions of the IDF were obtained by response surface *** material-to-water ratio was 1:12.1,the concentration ofα-amylase was 1.85%,and the hydrolysis time was 39.14 *** purifying the crude fiber with 4%sodium oxide(Na OH),70°C hot water,and anhydrous ethanol,the purity of dietary fiber exceeded 95%.X-ray diffraction analysis indicated that the IDF was predominantly *** electron microscopy showed that the IDF surface exhibited a loose porous network ***-transform infrared spectroscopy of the crude IDF showed characteristic absorption peaks at 3626,2929,1667,1538,1455,1242,and1048 cm^(-1),while the infrared spectrum of the purified IDF showed characteristic absorption peaks at 3401,2924,1744,1643,1418,and 1040 cm^(-1),which is consistent with the structure of cellulose *** scanning calorimetry analysis showed that there were three exothermic peaks at 270–310°C,320–350°C,and 440–460°C,which may represent the pyrolysis peaks of hemicellulose,cellulose,and lignin,respectively,indicating that the oat bran IDF had good thermal *** results indicate that the oat bran IDF can be used for the production of dietary fiber products and health-care products.
