The development of pancreatic fluid collections(PFC) is one of the most common complications of acute severe pancreatitis. Most of the acute pancreatic fluid collections resolve and do not require endoscopic drainage. However, a substantial proportion of acute necrotic collections get walled off and may require drainage. Endoscopic drainage of PFC is now the preferred mode of drainage due to reduced morbidity and mortality as compared to surgical or percutaneous drainage. With the introduction of new metal stents, the efficiency of endoscopic drainage has improved and the task of direct endoscopic necrosectomy has be-come easier. The requirement of re-intervention is less with new metal stents as compared to plastic stents. However, endoscopic drainage is not free of adverse events. Severe complications including bleeding, perforation, sepsis and embolism have been described with endoscopic approach to PFC. Therefore, the endoscopic management of PFC is a multidisciplinary affair and involves interventional radiologists as well as GI surgeons to deal with unplanned adverse events and failures. In this review we discuss the recent advances and controversies in the endoscopic management of PFC.

Objectives:Growing trend of street-vended food in underdeveloped countries offers low-cost food to many sections of *** it provides job opportunities to many urban dwellers,several health hazards are associated with this *** present study investigates the burden of foodborne pathogens in Ready-To-Eat(RTE)beverages in relation to vending practices among street vendors of Rawalpindi City,Pakistan according to standardized methods and *** and Methods:Six densely populated locations of Rawalpindi city were *** consumed sugar cane juice(SCJ)and tamarind prune(dried plums)drink(TPD)(locally called as Imli Alu Bukhara sherbet)from five vendors from each location were chosen in summer season where the temperature reaches above 40℃.Mean and the standard deviation were obtained by univariate and bivariate *** between the study variables was assessed through cross-tabulations,chi-square,and correlation ***:All the samples were found unsatisfactory in comparison to guidelines of aerobic plate *** coliform was observed in 86.7 per cent of SCJ and 70.0 per cent of TPD *** samples of SCJ exceeded the limit of>1100 MPN/ml value,whereas samples of TPD exceeded this limit for Escherichia *** of SCJ and 93.3 per cent of TPD samples depicted the presence of Salmonella *** *** found significantly high in 73.3 per cent samples of SCJ and 23.3 per cent samples of ***:The incidence of high bioloads attributes towards a potential reservoir of foodborne pathogens due to unhygienic vending practices.

Sustainable Tourism in Gilgit-Baltistan

The current research aimed to explore how to achieve sustainable tourism in Gilgit-Baltistan region located in Pakistan,while at the same time not compromising the ability to attract tourists in *** specific objectives of the study included investigating issues which affect sustainable tourism in Gilgit-Baltistan and policies and strategies which can be implemented to promote sustainable tourism in the *** achieve the set goals,a qualitative research design was ***,the methodology of the research involved sampling thirteen participants in the Gilgit-Baltistan tourism sector and conducting interviews concerning different issues of sustainable *** analysis,a crucial finding obtained was that ecotourism model can be employed in Gilgit-Baltistan to ensure conservation for the environment and local culture while also marketing the region as an exemplary tourist destination.A crucial aspect of ecotourism was realized to involve channeling financial proceeds from tourism in boosting the region’s infrastructure as well as conserving the environment to ensure tourism spots remain well-maintain to attract more tourist even in ***,the obtained results showed that the Gilgit-Baltistan local government can create spaces for national and international events which can encourage more international tourists to visit the region for different sporting and entertainment ***,in such cases,the local government should work closely with the local community to ensure conservation of the environment including maintaining the ecosystem is considered a joint effort where different projects are initiated to benefit everyone in the *** addition,the research findings revealed that the Gilgit-Baltistan local government can create visa and custom free region to attract different companies in the area which can create jobs for the residents as well as boost revenue which can be employed to improve infrastructure and tourism spo

Ex-Situ Cultivation of Medicinal Plant Species in High Altitudes at Swat,Pakistan

An ex-situ experiment was conducted to evaluate the growth performance of six medicinal species (Bergenia ciliata,Valeriana jatamansi,Dioscorea deltoidea,Paeonia emodi,Polygonum amplexicaule and Viola serpense) from upper Swat,*** were conducted at four different locations in the upper Swat valley at altitudes ranging from 1200 to 1900 *** objectives were:1) to determine the suitability of ex-situ cultivation of different medicinal species,and;2) to assess the economic geasibility of growing medicinal plants in the area.A highest mean survival of 80.7% across all locations was observed for Viola serpense,followed by 58.7% for Valeriana *** remaining four species exhibited very poor survival rates,although Polygonum amplexicaule,did show encouraging signs of growth and flowered,before experiencing high mortality rates late in the *** generally seemed to enhance the degree of sprouting for all species except Viola ***,the productive yield of *** was certainly not reduced,but rather slightly enhanced in the higher altitude ***,cultivation of only two of the investigated species,Valeriana jatamansi and Viola serpense,appeared successful and potentially economically viable under farmland conditions at upper Swat.

AIM:To compare effectiveness,safety,and cost of photodynamic therapy(PDT)and radiofrequency ablation(RFA)in treatment of Barrett’s dysplasia(BD).METHODS:Consecutive case series of patients undergoing either PDT or RFA treatment at single center by a single investigator were ***-three patients with high-grade dysplasia(HGD)had treatment with porfimer sodium photosensitzer and 630 nm laser(130 J/cm),with maximum of 3 treatment ***-three patients with BD(47 with low-grade dysplasia-LGD,6 with HGD)had step-wise circumferential and focal ablation using the HALO system with maximum of 4 treatment *** groups received proton pump inhibitors twice *** biopsies were acquired at 2 and 12 mo after enrollment,with 4-quadrant biopsies every 1 cm of the original BE extent.A complete histological resolution response of BD(CR-D)was defined as all biopsies at the last endoscopy session negative for ***’s exact test was used to assess differences between the two study groups for primary *** all outcomes,a two-sided P value of less than 0.05 was considered to indicate statistical ***:Thirty(91%)PDT patients and 39(74%)RFA were men(P=0.05).The mean age was 70.7±12.2 and 65.4±12.7(P=0.10)year and mean length of BE was 5.4±3.2 cm and 5.7±3.2 cm(P=0.53)for PDT and RFA patients,*** CR-D was(18/33)54.5%with PDT vs(47/53)88.7%with RFA(P=0.001).One patient with PDT had an esophageal perforation and was managed with non-surgical measures and no perforation was seen with *** was five times more costly than RFA at our *** two groups were not randomized and had different BD grading are the limitations of the ***:In our experience,RFA had higher rate of CR-D without any serious adverse events and was less costly than PDT for endoscopic treatment of BD.

The kinetics and mechanism of lactic acid oxidation in the presence of Mn(II)and Ce(IV)ions by chromic acid were studied *** oxidation of lactic acid by Cr(VI)was found to proceed in two measurable steps,both of which gave pyruvic acid as the primary product in the absence of Mn(II).2Cr(VI)+2CH3CHOHCOOH→2CH3COCOOH+Cr(V)+Cr(III)Cr(V)+CH3CHOHCOOH→Cr(III)+CH3COCOOH The observed kinetics was explained due to the catalytic and inhibitory effects of Mn(II)and Ce(IV)on the lactic acid oxidation by Cr(VI).The reactivity of lactic acid depends upon the experimental *** acts as a two-or three-equivalent reducing agent in the absence or presence of Mn(II).It was examined that Cr(III)products resulting from the direct reduction of Cr(VI)by three-equivalent reducing *** oxidation of lactic acid follows the complex order kinetics with respect to [lactic acid].The activation parameters Ea,ΔH#,and ΔS# were calculated and discussed.

Expeditious synthesis,antileishmanial and antioxidant activities of novel 3-substituted-4-hydroxycoumarin derivatives

A series of novel 3-substituted-4-hydroxycoumarin derivatives 6（a-1） were synthesized in high yield using one-pot three component coupling reaction catalyzed by ceric ammonium nitrate. These compounds were evaluated for antileishmanial activity against Leishmania donovani promastigotes and antioxidant activity（DPPH-radical scavenging activity）. Two compounds, 6h（IC50= 9.90 μmol/L） and 6i（IC50= 6.90 μmol/L） displayed potent antileishmanial activity when compared with standard antileishmanial agents pentamidine（IC50= 16.15 μmol/L） and miltefosine（IC50= 12.50 μmol/L）. Three compounds, 6c（IC50= 10.79 μmol/L）, 6h（IC50= 10.60 μmol/L）, and 6i（IC50= 10.73 μmol/L） showed significant antioxidant activity favorably with the antioxidant standards butylated hydroxy toluene（IC50= 16.47 μmol/L） and ascorbic acid（IC50= 12.69 μmol/L）. A molecular docking study of compounds 6（a-1） suggested a possible mode of binding with the Adenine phosphoribosyltransferase enzyme of ***. ADME properties were predicted in silico and support the potential of 6（a-1） to show favorable drug-like properties.
