Analysis of factors impacting postoperative pain and quality of life in patients with mixed hemorrhoids:A retrospective study

BACKGROUND Hemorrhoids are among the most common and frequently encountered chronic anorectal diseases in anorectal surgery.They are venous clusters formed by con-gestion,expansion,and flexion of the venous plexus in the lower part of the rec-tum.Mixed hemorrhoids bleed easily and recurrently,and this can result in severe anemia.Hence,they may have a negative effect on the health of the patient and surgical treatment is required.Milligan-Morgan hemorrhoidectomy has been widely used since 1937 for the treatment of grade III and IV hemorrhoids.How-ever,most patients experience different degrees of postoperative pain that may cause anxiety.with mixed hemorrhoids post-surgery.METHODS The clinical data of patients with mixed hemorrhoids who underwent Milligan-Morgan hemorrhoidectomy were collected retrospectively.The basic character-istics of the enrolled patients with mixed hemorrhoids were recorded,and based on the Goligher clinical grading system,the hemorrhoids were classified as grades III or IV.The endpoint of this study was the disappearance of pain in all patients.Quantitative data were presented as mean±SD,such as age,pain score,and QoL score.Student’s t-test was used to compare the groups.RESULTS A total of 164 patients were enrolled.The distribution of the visual analog scale pain scores of all patients at 3,7,14 and 28 d after surgery showed that post-surgery pain was significantly reduced with the passage of time.Fourteen days after the operation,the pain had completely disappeared in some patients.Twenty-eight days after the surgery,none of the patients experienced any pain.Comparing the World Health Or-ganization Quality of Life-BREF self-reporting questionnaire scores of patients between 14 and 28 d after surgery,we observed that the quality-of-life scores of the patients post-surgery had significantly improved.There were six items that were compared at 14-and 28-d post-surgery.The mean QoL score 28 d after surgery(4.79±0.46)was higher than that at 14 d post-surgery(3.79±0.57).The mean health condition

Surgically treating a rare and asymptomatic intraductal papillary neoplasm of the bile duct:A case report

BACKGROUND Intraductal papillary neoplasms of the bile duct(IPNBs)are rare and characterized by papillary growth within the bile duct lumen.IPNB is similar to obstructive biliary pathology.In this report,we present an unexpected case of asymptomatic IPNB and consolidate our findings with the relevant literature to augment our understanding of this condition.Integrating relevant literature contributes to a more comprehensive understanding of the disease.CASE SUMMARY A 66-year-old Chinese male patient was admitted to our hospital for surgical intervention after gallstones were discovered during a routine physical examination.Preoperative imaging revealed a lesion on the left side of the liver,which raised the suspicion of IPNB.A laparoscopic left hemihepatectomy was performed,and subsequent histopathological examination confirmed the diagnosis of IPNB.At the 3-mo postoperative follow-up,the patient reported good recovery and no metastasis.IPNB can manifest both latently and asymptomatically.Radical surgical resection is the most effective treatment for IPNB.CONCLUSION Hepatic and biliary masses,should be considered to diagnose IPNB.Prompt surgery and vigilant follow-up are crucial in determining prognosis.

Root cause of corrosion failure of tin coating on wire clamp in marine atmosphere

The failure cause of a tinned copper wire clamp in marine atmosphere was studied systematically by X-ray photoelectron spectroscopy,scanning electron microscopy with energy dispersive spectroscopy and electrochemical measurements.The main components of the green rust on the surface of the damaged wire clamp are SnO_(2),CuO,Cu_(2)Cl(OH)_(3) and CuCO(OH)_(2).Much of green rust distributes at the platform edge along the axial direction on the crimp connection,and severe corrosion and corrosion pits occur at the platform edge zone along the axial direction.The enriching Cl-at the marine atmosphere and the existing O_(2) in air collectively enhance the corrosion process of the tin coating and the copper matrix.Finite element model results show that the residual stress and strain of the tin coating are the largest at the platform edge along the axial direction on the crimp connection,and the corresponding electrode potential of the tin coating at this zone drops significantly.The above results indicate that the residual strain increases the driving force of the corrosion electrochemical reactions and accelerates the corrosion rate and the pit corrosion of the tin coating at this zone.

The emerging biomass-based epoxy vitrimers hold great potential to fulfill the requirements for sustainable development of society.Since the existence of dynamic chemical bonds in vitrimers often reduces both the thermal and mechanical properties of epoxy resins, it is challenging to produce recyclable epoxy vitrimers with both excellent mechanical properties and good thermal stability. Herein, a monomer 4-(((5-(hydroxymethyl)furan-2-yl)methylene)amino)phenol(FCN) containing furan ring with potential to form high density of hydrogen bonding among repeating units is designed and copolymerized with glycerol triglycidyl ether to yield epoxy resin(FCN-GTE), which intrinsically has dual hydrogen bond networks, dynamic imine structure and resultant high performance. Importantly, as compared to the BPA-GTE, the FCN-GTE exhibits significantly improved mechanical properties owing to the increased density of hydrogen bond network and physical crosslinking interaction. Furthermore, density functional theory(DFT) calculation and in situ FTIR analysis is conducted to decipher the formation mechanism of hydrogen bond network. In addition, the FCN-GTE possesses superior UV shielding, chemical degradation, and recyclability because of the existence of abundant imine bonds. Notably, the FCN-GTE-based carbon fiber composites could be completely recycled in an amine solution.This study provides a facile strategy for synthesizing recyclable biomass-based high-performance epoxy vitrimers and carbon fiber composites.

Introduction:A retrospective study based on sentinel surveillance was conducted in 10 provinciallevel administrative divisions(PLADs)in China to enhance the understanding of the epidemiological characteristics of human parainfluenza viruses(HPIVs).Methods:From January 2019 to June 2023,respiratory specimens were collected from individuals with acute respiratory infections(ARIs)and screened for four HPIVs serotypes and other common respiratory viruses using multiplex real-time polymerase chain reaction(PCR).This study analyzed the association of HPIVs infections with seasonal patterns,geographical distribution,demographic profiles,clinical features,and co-infection status.Results:During the study period,a total of 12,866 ARIs were included.The overall detection rate of HPIVs was 6.15%,varying from 5.04%in 2022 to 9.70%in 2020.The median age of HPIVs-infected patients was 3 years.HPIV2 was more prevalent among individuals aged 5–17 years(42.57%),while HPIV4 was more common in those over 65 years(12.24%).HPIV3(54.16%)and HPIV1(27.18%)were the predominant serotypes,and their prevalence exhibited significant seasonal fluctuations postcoronavirus disease 2019(COVID-19)pandemic.The peak of HPIV3 shifted three months later in 2020 compared to 2019 and returned to a summer peak thereafter.Two peaks of HPIV1 were observed in 2021 following the peak of HPIV3.Additionally,coinfections were frequent in HPIVs cases(overall rate:22.12%),with human rhinovirus being the most common co-infecting virus.Conclusions:The prevalence of HPIVs in China was predominantly due to HPIV3 and HPIV1,and their seasonal patterns were altered by pandemic restrictions.Hence,continuous surveillance of HPIVs is essential.

Dielectric Constant Calculation of Poly(vinylidene fluoride)Based on Finite Field and Density Functional Theory

In this study,we proposed a novel method that integrates density functional theory(DFT)with the finite field method to accurately estimate the polarizability and dielectric constant of polymers.Our approach effectively accounts for the influence of electronic and geometric conformation changed on the dielectric constant.We validated our method using polyethylene(PE)and polytetrafluoroethylene(PTFE)as benchmark materials,and found that it reliably predicted their dielectric constants.Furthermore,we explored the impact of conformation variations in poly(vinylidene fluoride)(PVDF)on its dielectric constant and polarizability.The resulting dielectric constants ofα-andγ-PVDF(3.0)showed excellent agreement with crystalline PVDF in experiments.Our findings illuminate the relationship between PVDF’s structural properties and its electrical behavior,offering valuable insights for material design and applications.

Low-noise and low-power pixel sensor chip for gas pixel detectors

Topmetal-M2 is a large-area pixel sensor chip fabricated using the GSMC 130 nm CMOS process in 2021.The pixel array of Topmetal-M2 consists of pixels of 400 rows×512 columns with a pixel pitch of 45μm×45μm.The array is divided into 16 subarrays,with pixels of 400 rows×32 columns per subarray.Each pixel incorporates two charge sensors:a diode sensor and a Topmetal sensor.The in-pixel circuit primarily consists of a charge-sensitive amplifier for energy measurements,a discriminator with a peak-holding circuit,and a time-to-amplitude converter for time-of-arrival measurements.The pixel of Topmetal-M2 has a charge input range of~0-3 k e-,a voltage output range of~0-180 mV,and a charge-voltage conversion gain of~59.56μV∕e-.The average equivalent noise charge of Topmetal-M2,which includes the readout electronic system noise,is~43.45 e-.In the scanning mode,the time resolution of Topmetal-M2 is 1 LSB=1.25μs,and the precision is^()7.41μs.At an operating voltage of 1.5 V,Topmetal-M2 has a power consumption of~49 mW∕cm~2.In this article,we provide a comprehensive overview of the chip architecture,pixel working principles,and functional behavior of Topmetal-M2.Furthermore,we present the results of preliminary tests conducted on Topmetal-M2,namely,alpha-particle and soft X-ray tests.

尊严一开始是一个代表等级的概念,也不对应权利,后来才悖论性地被扩展适用于所有的人。以尊严的传统范式/现代范式为参照点,可以看到,支配着法律制度和价值体系的康德尊严理论解释力不足,有必要在现代范式下发展康德的尊严理论,以回应生命科技对尊严的内在挑战。尊严理论需要更进一步普遍化,需要将尊严的涵摄范围扩张到超越时间的“人的生命”。因此,对人性尊严的关注点应当从受到时间限制的“人”转向“人的生命”,界定“人的生命的尊严”的内涵,并以之为根据在法哲学上辩护人的各种生命形态所具有的规范性意义,从而为赋予其权利奠定理论基础。

单向冻结粉质黏土已冻区分凝冰分布规律试验研究

在单向冻结中,已冻区分凝冰的产生和发展是影响冻融土结构性特征的重要因素。选取降温速率(0.5~0.005℃/s)、冷端温度(–30~–70℃)、暖端补水(无补水、恒压补水)等3种单向冻结边界条件,探究其对已冻区分凝冰发育的影响规律。基于光学显微数字拍摄,自主研发了裂纹显微测量系统,为分凝冰裂纹的原位、无接触式测量提供了便利。试验研究发现:(1)不仅是温度梯度、暖端补水条件,单向冻结冷端降温速率对已冻区分凝冰分布也会产生影响,传统模型(如分凝模型)计算的水分迁移量难以准确反映已冻区冰裂纹分布特征;(2)冻结锋面移动速率对已冻区分凝冰宽度的影响大于温度梯度;(3)冻结过程中已冻区分凝冰宽度随0℃线移动速率呈指数形式变化。最后基于0℃线移动速率提出了已冻区分凝冰宽度预测的经验公式,为工程中评估变降温速率条件下黏土的单向冻胀破坏提供了参考。

Effect of impaction on gene-modified cells seeded on granular bone allografts in vitro and in vivo

Background While attempting to restore bone stock, impaction bone grafting employed during revision joint surgery may result in slow and limited allograft incorporation into host bone. A new approach including gene-modified bone marrow stromal cells （BMSCs） in combination with impaction bone grafting may effectively restore bone stock and improve allograft incorporation. This study aimed to investigate the effect of impaction on gene-modified BMSCs seeded on granular bone allografts in vitro and in vivo.Methods Deep-frozen, granular, cancellous bone allografts from canines were prepared to serve as cell delivery scaffolds and were seeded with green fluorescent protein （GFP） genetically-modified BMSCs to construct cell-allograft composites. The composites were impacted in a simulative, in vitro impaction model and cultured for further analysis under standard conditions. Four Beagle dogs, treated with bilateral, uncemented proximal tibial joint hemiarthroplasty with a prosthesis, were implanted with autologous GFP gene-modified cell-allograft composites to repair the bone cavity around each prosthesis.Results A significant reduction in cell viability was observed after impaction by fluorescence microscopy in vitro.However, there remained a proportion of GFP-positive cells that were viable and functionally active, as evidenced by the secretion of GFP protein in vitro and in vivo.Conclusions Gene-modified BMSCs seeded on granular allografts were able to withstand the impaction forces and to maintain their normal functions in vitro and in vivo, in spite of a partial loss in cell viability.
