Efficacy and safety of Cimicifuga foetida extract on menopausal syndrome in Chinese women

Background It is now recognized that Cimicifuga foetida extract is effective in alleviating menopausal symptoms. But the durations reported were usually short. This paper compares the clinical effects of different regimens of three-month course on climacteric symptoms in Chinese women, so as to evaluate the efficacy and safety of Cimicifuga foefida extract. Methods This was a prospective, randomized trial. Ninety-six early menopausal women were recruited and randomly assigned into 3 groups to take different kinds of medicine for 3 months; participants were given Cimicifuga foefida extract daily in group A （n=32）, given estradiol valerate and progesterone capsule cycle sequentially in group B （n=32）, and given estradiol valerate and medroxyprogesterone acetate cycle sequentially in group C （n=32）. The questionnaires of Kupperman menopause index, Menopause-Specific Quality of Life, and Hospital Anxiety and Depression Scale were finished before and after the treatment. The status of vaginal bleeding and breast tenderness was recorded every day. Results Eighty-nine participants （89/96, 92.7%） completed the treatment. Kupperman menopause index decreased after taking the medicine for 3 months in each group （with all P〈0.001）, but the score after the treatment was higher in group A than in the other 2 groups. Except for the score of sexual domain in group A （P=0.103）, the scores of all domains of the Menopause-Specific Quality of Life decreased significantly after the treatment in all groups （with all P〈-0.01）. Score of anxity decreased significantly in group A （P=0.0l 5） and B （P=0.003）. Incidence of breast tenderness was 12.9% （4/31） in groupA, 36.7% （11/30） in group B, and 14.3% （4/28） in group C. Incidence of vaginal bleeding was 6.5% （2/31）in group A, 26.7% （8/30） in group B, and 82.1% （23/28）in group C. How to determine the dosage of oral progesterone among patients with menstrual disorders？

Background Few studies have given suggestions on appropriate individual progesterone dosage in patients with progesterone deficiency. This study was designed to provide a reference for the clinical use of oral progesterone by exploring the relationship among Body Mass Index （BMI）, dosage of progesterone, and serum progesterone concentration. Many gynecology and obstetrics doctors are unfamiliar with progesterone treatment. Our study is intended to help determine the dosage of oral progesterone. Methods This was a block randomized, open-label, prospective clinical trial. Eighty women undergoing cessation of menses were recruited, given oral progesterone therapy for 10 consecutive days. They were randomly assigned to four groups （four different doses of progesterone, n=20）： group A 100 rag/d, group B 200 rag/d, group C 300 mg/d, and group D 400 mg/d. Results Seventy-four patients （92.5%, 74/80） completed the study. It was observed that administration of progesterone significantly increased serum progesterone concentration in the four groups （all P 〈0.001）. And there is a positive correlation between the increase and dosage （rp=0.613, P 〈0.001）. A further linear regression analysis found the major regression equation： when 18.5 kg/m^2 -≤BMI 〈24 kg/m^2, Y=8.4820×10^0.003x （R^2=0.425, P 〈0.001）; Y was the increase of serum progesterone concentration in nmol/L, and X was the dosage of oral progesterone in mg/d. Conclusions Serum progesterone levels went up linearly as the dosage increased. The higher the patient＇s BMI, the higher dosage would be needed to achieve the same serum progesterone for different patients can be roughly calculated progesterone concentration. The appropriate dosage of ora in light of the results of this study.

It's a challenge for detecting the therapeutic targets of a polypharmacological drug from variations in the responsed networks in thedfferentiated populations with complex diseases,as stable coronary heart ***,in an adaptive,31-center,randomized,double-blind trial invoving 920 patients with moderate symptomatic stable angina treated by 14-day Danhong injection(DHI),a kind of polyphamacological drug with high quality control,or placebo(0.9%saline),with 76-day following-up,we firstly confrmed that DHl couldincrease the proportion of patients with clinically significant changes on angina-frequency assessed by Seattle Angina Questionnaire(ASAQ-AF220)(12.78%at Day 30,95%confidence interval[C]5.86-19.71%,P=0.0003,13.82%at Day 6C0,95%CI 6.82-20.82%,P=0.0001and 8.95%at Day 90,95%CI 2.06-15.85%,P=0.01).We also found that there were no significant differences in new-onset major vascularevents(P=0.8502)and serious adverse events(P=0.9105)between DHl and *** performing the RNA sequencing in 62 selectedpatients,we developed a systemic modular approach tp identfy differentilly expressed modules(DEMs)of DHI with the Z_(summay)valueless than 0 compared with the control group,calculated by weighted gene co-expression network analysis(WGCNA),and sketched out thebasic framework on a modular map with 25 functional modules targeted by ***,the effective therapeutic module(ETM),defined as the highest corelation value with the phenotype alteration(SAQ-AF,the change in SAQ-AF at Day 30 from baseline)calculatedby WGCNA,was identifed in the population with the best effect(ASAQ-AF240),which is related to anticoagulation and regulation ofcholesterol *** assessed the modular flexbility of this ETM using the global topological D value based on Euclidean distance,which is corelated with phenotype alteration(r^(2):0.8204,P=0.019)by linear *** study identified the ant-angina therapeuticmodule in the effective population treated by the multi-target

Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains *** A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of *** Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two *** Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. Women with comorbidities were more likely to have severe disease, while men with comorbidities were more likely to have unfavorable treatment outcomes.

The East Asian Very Long Baseline Interferometry(VLBI) Network(EAVN) is a rapidly evolving international VLBI array that is currently promoted under joint efforts among China,Japan and *** aims at forming a joint VLBI Network by combining a large number of radio telescopes distributed over East Asian *** the combination of the Korean VLBI Network(KVN) and the VLBI Exploration of Radio Astrometry(VERA) into Ka VA,further expansion with the joint array in East Asia is actively *** we report the first imaging results(at 22 and 43 GHz) of bright radio sources obtained with Ka VA connected to Tianma 65-m and Nanshan 26-m Radio Telescopes in *** test the EAVN imaging performance for different sources,we observed four active galactic nuclei(AGN) having different brightness and *** a result,we confirmed that the Tianma 65-m Radio Telescope(TMRT) significantly enhances the overall array sensitivity,a factor of 4 improvement in baseline sensitivity and 2 in image dynamic range compared to the case of Ka VA *** addition of the Nanshan 26-m Radio Telescope(NSRT) further doubled the east-west angular *** the resulting high-dynamic-range,high-resolution images with EAVN(Ka VA+TMRT+NSRT),various fine-scale structures in our targets,such as the counter-jet in M87,a kink-like morphology of the 3 C 273 jet and the weak emission in other sources are successfully *** demonstrates the powerful capability of EAVN to study AGN jets and to achieve other science goals in *** expansion of EAVN will further enhance the angular resolution,detection sensitivity and frequency coverage of the network.

目的了解中国服用糖皮质激素风湿病患者骨质疏松的发病情况和诊疗现状,以期引起对骨质疏松防治的重视。方法 2012年11月1日至2013年1月31日,在全国46家风湿病专科医院对3 136例曾经或正在服用糖皮质激素的风湿病患者进行临床资料、用药情况及骨量调查,了解骨量减少和骨质疏松的发生率。结果 (1)所有调查患者中,主要病种为类风湿关节炎(1 069例)和系统性红斑狼疮(983例)。3 136例患者中90%存在骨量减少或骨质疏松,其中骨质疏松的发生率为41.4%。44.7%的患者在调查当时糖皮质激素用量(等效于泼尼松)>7.5 mg/d,调查当时糖皮质激素剂量≤2.5 mg/d者仍有58.5%存在骨质疏松。(2)随着年龄增加,骨质疏松及骨折的发病率明显增加;小于30岁的患者骨量减少和骨质疏松总发生率为74.4%。(3)女性风湿病患者骨密度异常比例高于男性,平均停经年龄低于正常人群(45.8岁)。(4)抗骨质疏松药物使用不规范,32.7%的患者从未预防性给予抗骨质疏松药物治疗,在T<-2.5的患者中,35.6%未给予预防性抗骨质疏松治疗。主要用药为钙和维生素D,20.9%的患者仅给予钙剂单独治疗,仅有4%使用膦酸盐类药物。结论我国应用糖皮质激素的风湿病患者骨量减少和骨质疏松发生率明显增高,且预防性用药不规范。
