Comparison of fondaparinux sodium and low molecular weight heparin in the treatment of hypercoagulability secondary to traumatic infection

Purpose: To compare the effects and side-effects of fondaparinux sodium and low molecular weight heparin in patients with hypercoagulability accompanied with traumatic infection. Methods: Thirty-six patients with post-traumatic infections in our hospital intensive care center were diagnosed with hypercoagulability from February 2012 to February 2013. These patients were randomly divided into 2 groups. In group F (18 patients), the patients were treated with fondaparinux sodium, 2.5 mg, 1/d for 11 d. In group L (18 patients), the patients were treated with low molecular weight heparin, 4100 U, 1/12 h for 11 d. The incidence of deep vein thrombosis, bleeding events and multiple organ dysfunction syndrome (MODS) and mortality of two groups after anticoagulation therapy were analyzed. Fibrinogen, D-dimer level and activity of antithrombin III were measured by the coagulation analyzer. Results: The incidence of deep vein thrombosis, MODS incidence and mortality were not significantly different between the two groups. The rate of bleeding evens in group F was lower than group L ( p 0.05). D-dimer was significantly decreased after anticoagulant therapy for 5 d ( p 0.05). Conclusion: Fondaparinux sodium and low molecular weight heparin can effectively improve coagulopathy in patients with traumatic infection. Compared with low molecular weight heparin, fondaparinux sodium may reduce the risk of bleeding events in patients with hypercoagulability accompanied by traumatic infection.

Current assessment and management of measurable residual disease in patients with acute lymphoblastic leukemia in the setting of CAR-T-cell therapy

Chimeric antigen receptor(CAR)-modified T-cell therapy has achieved remarkable success in the treatment of acute lymphoblastic leukemia(ALL).Measurable/minimal residual disease(MRD)monitoring plays a significant role in the prognostication and management of patients undergoing CAR-T-cell therapy.Common MRD detection methods include flow cytometry(FCM),polymerase chain reaction(PCR),and next-generation sequencing(NGS),and each method has advantages and limitations.It has been well documented that MRD positivity predicts a poor prognosis and even disease relapse.Thus,how to perform prognostic evaluations,stratify risk based on MRD status,and apply MRD monitoring to guide individual therapeutic decisions have important implications in clinical practice.This review assesses the common and novel MRD assessment methods.In addition,we emphasize the critical role of MRD as a prognostic biomarker and summarize the latest studies regarding MRD-directed combination therapy with CAR-T-cell therapy and allogeneic hematopoietic stem cell transplantation(allo-HSCT),as well as other therapeutic strategies to improve treatment effect.Furthermore,this review discusses current challenges and strategies for MRD detection in the setting of disease relapse after targeted therapy.

Cross-spectral recognition method of bridge deck aerodynamic admittance function

This study proposes a new identification algorithm about the admittance function, which can estimate the full set of six aerodynamic admittance functions considering cross power spectral density functions about the forces and the turbulence components. The method was first numerically validated through Monte Carlo simulations, and then adopted to estimate the aerodynamic admittance of a streamlined bridge deck. The identification method was further validated through a comparison between the numerical calculation and wind tunnel tests on a moving bridge section. ? 2015, Institute of Engineering Mechanics, China Earthquake Administration and Springer-Verlag Berlin Heidelberg.

Estimating the number of Chinese cancer patients eligible for and benefit from immune checkpoint inhibitors

The total number of cancer patients who are eligible for and will benefit from immune checkpoint inhibitors(ICIs)in China has not been quantified.This cross-sectional study was conducted to estimate the number of Chinese cancer patients with eligibility and response to ICIs based on the 2015 Chinese cancer statistics and the immune checkpoint inhibitor clinical practice guideline of the Chinese Society of Clinical Oncology.A total of 11 ICIs were recommended for 17 cancer types.The estimated number of eligible patients annually was 1290156(55.18%),which included 888738 males(60.05%)and 400468 females(46.67%).The estimated number of responders annually was 448972(19.20%),which included 309023 males(20.88%)and 139764 females(16.29%).Gastric cancer(n=291000,12.45%),non-small-cell lung cancer(n=289629,12.39%),and hepatocellular carcinoma(n=277100,11.85%)were the top three cancer types with the highest number of eligible patients.Non-small-cell lung cancer(n=180022,7.70%),hepatocellular carcinoma(n=75648,3.24%),and small-cell lung cancer(n=64362,2.75%)were the top three cancer types with the highest number of responders.In conclusion,ICIs provide considerable benefit in Chinese cancer patients under optimal estimation.

A combination weighting model based on iMOEA/D-DE

This paper proposes a combination weighting(CW)model based on iMOEA/D-DE(i.e.,improved multiobjective evolutionary algorithm based on decomposition with differential evolution)with the aim to accurately compute the weight of evaluation methods.Multi-expert weight considers only subjective weights,leading to poor objectivity.To overcome this shortcoming,a multiobjective optimization model of CW based on improved game theory is proposed while considering the uncertainty of combination coefficients.An improved mutation operator is introduced to improve the convergence speed,and thus better optimization results are obtained.Meanwhile,an adaptive mutation constant and crossover probability constant with self-learning ability are proposed to improve the robustness of MOEA/D-DE.Since the existing weight evaluation approaches cannot evaluate weights separately,a new weight evaluation approach based on relative entropy is presented.Taking the evaluation method of integrated navigation systems as an example,certain experiments are carried out.It is proved that the proposed algorithm is effective and has excellent performance.

Eff ects of stand condition and root density on fi ne-root dynamics across root functional groups in a subtropical montane forest

Fine roots play key roles in belowground C cycling in terrestrial ecosystems.Based on their distinct functions,fi ne roots are either absorptive fi ne roots(AFRs)or transport fi ne roots(TFRs).However,the function-based fi ne root dynamics of trees and their responses to forest stand properties remain unclear.Here,we studied the dynamics of AFRs and TFRs and their responses to stand conditions and root density in a subtropical montane mixed forest based on a 2-a root window experiment.Mean(±SE)annual production,mortality,and turnover rate of AFRs were 7.87±0.17 m m^(−2)a^(−1),8.13±0.20 m m^(−2)a^(−1)and 2.96±0.24 a^(−1),respectively,compared with 7.09±0.17 m m^(−2)a^(−1),4.59±0.17 m m^(−2)a^(−1),and 2.01±0.22 a^(−1),respectively,for TFRs.The production and mortality of fi ne roots were signifi cantly higher in high root-density sites than in low-root density sites,whereas the turnover of fi ne roots was faster in the low root-density sites.Furthermore,root density had a larger positive eff ect than other environmental factors on TFR production but had no obvious impact on AFR production.Tree species diversity had an apparent positive eff ect on AFR production and was the crucial driver of AFR production,probably due to a complementary eff ect,but had no evident impact on TFR.Both tree density and tree species diversity were positively correlated with the mortality of AFRs and negatively related to the turnover of TFRs,suggesting that higher root density caused stronger competition for rooting space and that plants tend to reduce maintenance costs by decreasing TFR turnover.These fi ndings illustrated the importance of root functional groups in understanding root dynamics and their responses to changes in environmental conditions.

An Efficient Local Radial Basis Function Method for Image Segmentation Based on the Chan-Vese Model

In this paper,we consider the Chan–Vese(C-V)model for image segmentation and obtain its numerical solution accurately and efficiently.For this purpose,we present a local radial basis function method based on a Gaussian kernel(GA-LRBF)for spatial discretization.Compared to the standard radial basis functionmethod,this approach consumes less CPU time and maintains good stability because it uses only a small subset of points in the whole computational domain.Additionally,since the Gaussian function has the property of dimensional separation,the GA-LRBF method is suitable for dealing with isotropic images.Finally,a numerical scheme that couples GA-LRBF with the fourth-order Runge–Kutta method is applied to the C-V model,and a comparison of some numerical results demonstrates that this scheme achieves much more reliable image segmentation.

A step parameters prediction model based on transfer process neural network for exhaust gas temperature estimation after washing aero-engines

The prediction of Exhaust Gas Temperature Margin(EGTM)after washing aeroengines can provide a theoretical basis for airlines not only to evaluate the energy-saving effect and emission reduction,but also to formulate reasonable maintenance plans.However,the EGTM encounters step changes after washing aeroengines,while,in the traditional models,a persistence tendency exists between the prediction results and the previous data,resulting in low accuracy in prediction.In order to solve the problem,this paper develops a step parameters prediction model based on Transfer Process Neural Networks(TPNN).Especially,“step parameters”represent the parameters that can reflect EGTM step changes.They are analyzed in this study,and thus the model concentrates on the prediction of step changes rather than the extension of data trends.Transfer learning is used to handle the problem that few cleaning records result in few step changes for model learning.In comparison with Long Short-Term Memory(LSTM)and Kernel Extreme Learning Machine(KELM)models,the effectiveness of the proposed method is verified on CFM56-5B engine data.

Efficacy of acupuncture on repair of glial scars in rats with traumatic brain injury

OBJECTIVE:To explore the underlying mechanism of acupuncture on nerve repair by investigating its effect on the differentiation of glial cells and the repair of glial scars.METHODS:Sprague-Dawley rats were randomly allocated to three groups:normal group,model group,and acupuncture group.Acupuncture was applied at Renzhong(GV26),Baihui(GV20),Fengfu(GV16),Yamen(GV15)and Hegu(LI4)within 12 h after TBI modeling with a frequency of one session per day for 4 weeks.Neurobehavioral assessment,hematoxylin and eosin staining,immunofluorescence detection,and magnetic resonance imaging scanning were performed on days 3,7,14,and 28 after modeling of traumatic brain injury(TBI).RESULTS:Acupuncture promoted the proliferation of glial cells and glial scars at an early stage but inhibited the proliferation of glial cells and glial scars at a late stage.Morphological observations and immunofluorescence histochemistry showed that the morphology of the perilesional cortex in the acupuncture group was improved and the number of neurons was increased when compared with the model group.The lesion size of ipsilateral brain parenchyma in the acupuncture group was smaller than in the model group on days 7,14,and 28(Pling.CONCLUSIONS:Acupuncture might have a bidirectional regulatory effect on glial scar repair after TBI by promoting the proliferation of glial cells and glial scars to limit the injured area and relieve nerve injury during the early stages,and by inhibiting glial scar hyperplasia to benefit the regeneration and repair of neurons and axons and promote neurological function recovery during the later stages.
