Efficacy and Safety of Bufei Jiedu Granules in Treating Multidrug-Resistant Pulmonary Tuberculosis:A Multi-center,Double-Blinded and Randomized Controlled Trial

Objective: To assess the efficacy and safety of Bufei Jiedu(BFJD) Granules as adjuvant therapy for patients with multidrug-resistant pulmonary tuberculosis(MDR-PTB). Methods: A large-scale, multi-center, double-blinded, and randomized controlled trial was conducted in 18 sentinel hospitals in China from December 2012 to December 2016. A total of 312 MDR-PTB patients were randomly assigned to BFJD Granules or placebo groups(1:1) using a stratified randomization method, which both received the long-course chemotherapy regimen for 18 months(6 Am-Lfx-P-Z-Pto, 12 Lfx-P-Z-Pto). Meanwhile, patients in both groups also received BFJD Granules or placebo twice a day for a total of 18 months, respectively. The primary outcome was cure rate. The secondary outcomes included time to sputum-culture conversion, changes in lung cavities and quality of life(QoL) of patients. Adverse reactions were monitored during and after the trial. Results: A total of 216 cases completed the trial, 111 in the BFJD Granules group and 105 in the placebo group. BFJD Granules, as adjuvant treatment, increased the cure rate by 13.6% at the end of treatment, compared with the placebo(58.4% vs. 44.8%, P=0.02), and accelerated the median time to sputum-culture conversion(5 months vs. 11 months). The cavity closure rate of the BFJD Granules group(50.6%, 43/85) was higher than that of the placebo group(32.1%, 26/81;P=0.02) in patients who completed the treatment. At the end of the intensive treatment, according to the 36-item Short Form, the BFJD Granules significantly improved physical functioning, general health, and vitality of patients relative to the placebo group(all P<0.01). Overall, the death rates in the two groups were not significantly different;5.1%(8/156) in the BFJD Granules group and 2.6%(4/156) in the placebo group. 目的分析耐多药肺结核并发肺毁损的临床特征,为结核性毁损肺的诊治及防治提供依据。方法采用回顾性病例系列研究方法,选取从2013年1月—2017年12月来自15个省18家医院确诊的耐多药肺结核患者662例。其中,并发肺毁损患者73例(肺毁损组),未并发肺毁损患者589例(非肺毁损组),分析两组患者的人口学资料、实验室检查、耐药情况、结核病病史、中医证候积分、影像学表现及治疗结局等临床资料。结果肺毁损组患者的平均年龄高于非肺毁损组患者(P0.05)。肺毁损组患者的肺结核初次诊断到耐多药肺结核初次诊断时间、耐多药肺结核初次诊断到耐多药肺结核治疗的时间均长于非肺毁损组患者(P<0.05)。肺毁损组患者咳嗽、咳痰、胸痛、气短及潮热等症状严重于非肺毁损组患者(P<0.05)。肺毁损组患者的病灶平均肺叶受累数量多于非肺毁损组患者(P<0.05)。肺毁损组患者的治愈率低于非肺毁损组患者(P<0.05)。结论关注伴有毁损肺高危因素(农民、中年、诊断治疗延迟、肺叶病变广泛等)的耐多药肺结核患者,加强抗结核全程督导治疗,减少肺毁损的发生,提高治愈率。

An analytical model of hot rolling force for a thick plate by combining globally optimal approximation yield criterion and egg-circular velocity field

For the sake of solving the problem that it is difficult to be integrated for the Mises specific plastic power due to its nonlinearity,a new linear criterion,named the globally optimal approximation criterion,is constructed by the polygonal approximation to the Mises *** new criterion is proved to be the linear function of the principal stress componentsσ1,σ2 andσ3 and the trajectory of it on theπ-plane is a non-equiangular but equilateral dodecagon intersecting the Mises *** theoretical results of the current criterion described by the Lode stress parameters are in excellent accordance with the experimental ***,according to the trend that the metallic flow velocity between rollers aggrandizes gradually from the inlet to the outlet during the hot rolling of a thick plate,a biomimetic velocity field is proposed in which the horizontal velocity component fits the egg-circular curve *** velocity field and its simulated results agree quite ***,using the determined linear criterion,energy analysis of the constructed velocity field is utilized to obtain the interior deformation power,while the vector decomposition approach is utilized to obtain the frictional power and shear *** this basis,the overall power is obtained and the analytical solutions are generated for the rolling torque,rolling force and the coefficient of the stress state under different egg curves by minimizing the neutral ***,the parameter optimization of the characteristic parameterρwhich affects the slope of the egg-circular curve is carried out and the best egg-circular curve which can minimize the energy consumption is *** best agreement between the theoretical and observed values of rolling force and rolling torque is under this curve,and the mean relative errors of the rolling torque and rolling force are no more than 2.93%,while the maximum error is no more than 8.35%.

Stabilization/solidification mechanisms of tin tailings and fuming slag-based geopolymers for different heavy metals

Tin mine tailings(TMT)and fuming slag(FS)contain many heavy metals(As,Cr,Cu,Zn and Mn)that cause severe pollution to the ***,geopolymers were prepared using TMT,FS and flue gas desulfurization gypsum(FGDG)to immobilize heavy metals,and their compressive strength and heavy metal leaching toxicity were *** was first determined that T4F5(TMT:FS=4:5)sample exhibited the highest compressive strength(7.83 MPa).T4F5 achieved 95%immobilization efficiency for As and Cr,and nearly 100%for Cu,Zn and Mn,showing good immobilization performance.A series of characterization analyses showed that heavy metal cations can balance the charge in the geopolymer and replace Al in the geopolymer structure to form covalent *** addition,about 2%–20%of heavy metal Fe was immobilized in hydration products,heavy metal hydroxides and non-bridging Si–O and Al–O coordination with silica-aluminate ***_(3)^(3−) was oxidized into AsO_(4)^(3−),which may form Ca–As or Fe–As ***_(2)O_(7)^(2−)was converted to CrO_(4)^(2−)under alkaline environment and then combined with OH−to form Cr(OH)3 ***^(2+)may react directly with dissolved silicate to form Mn_(2)SiO_(4) and also form Mn(OH)_(2) *** unstable Mn(OH)_(2) can be further oxidized to MnO_(2).The heavy metal cations were immobilized in the silicoaluminate lattice,while the anions tended to form insoluble *** results may benefit the industry and government for better handling of TMT,FS and solid wastes containing the abovementioned five heavy metals.

Communication Resource-Efficient Vehicle Platooning Control With Various Spacing Policies

Platooning represents one of the key features that connected automated vehicles may possess as it allows multiple automated vehicles to be maneuvered cooperatively with small headways on roads. However, a critical challenge in accomplishing automated vehicle platoons is to deal with the effects of intermittent and sporadic vehicle-to-vehicle data transmissions caused by limited wireless communication resources. This paper addresses the co-design problem of dynamic event-triggered communication scheduling and cooperative adaptive cruise control for a convoy of automated vehicles with diverse spacing policies. The central aim is to achieve automated vehicle platooning under various gap references with desired platoon stability and spacing performance requirements, while simultaneously improving communication efficiency. Toward this aim, a dynamic event-triggered scheduling mechanism is developed such that the intervehicle data transmissions are scheduled dynamically and efficiently over time. Then, a tractable co-design criterion on the existence of both the admissible event-driven cooperative adaptive cruise control law and the desired scheduling mechanism is derived. Finally, comparative simulation results are presented to substantiate the effectiveness and merits of the obtained results.

Application of Gaussian Beam Summation Migration in Reflected In-seam Wave Imaging

The geological conditions for coal mining in China are complex,with various structural issues such as faults and collapsed columns seriously compromising the safety of coal mine ***-seam wave exploration is an effective technique for acquiring detailed information on geological structures in coal seam working ***,the existing reflected in-seam wave imaging technique can no longer meet the exploration precision requirements,making it imperative to develop a new reflected in-seam wave imaging *** study applies the Gaussian beam summation(GBS)migration method to imaging coal seams'reflected in-seam wave ***,with regard to the characteristics of the reflected in-seam wave data,methods such as wavefield removal and enveloped superposition are employed for the corresponding wavefield separation,wave train compression and other processing of reflected in-seam ***,imaging is performed using the GBS migration *** feasibility and effectiveness of the proposed method for reflected in-seam wave imaging are validated by conducting GBS migration tests on 3D coal-seam fault models with different dip angles and *** applying the method to reflected in-seam wave data for an actual coal seam working face,accurate imaging of a fault structure is obtained,thereby validating its practicality.

Emerging as a prominent area of focus in energy conversion and storage technologies,the development of highly active metal-based single-atom catalysts(SACs)holds great signiﬁcance in searching alternatives to replace precious metals toward the efficient,stable,and low-cost hydrogen evolution reaction(HER),as well as the oxygen evolution reaction(OER)and the oxygen reduction reaction(ORR).Combining the tremendous tunability of ligand and coordination environment with rich metal-based electrocatalysts can create breakthrough opportunities for achieving both high stability and ***,we propose a novel and stable holey graphene-like carbon nitride monolayer g-C_(16)N5(N_(4)@g-C_(16)N_(3))stoichiometries interestingly behaving as a natural substrate for constructing SACs((TM-N_(4))@g-C_(16)N_(3)),whose evenly distributed holes map rich and uniform nitrogen coordination positions with electron-rich lone pairs for anchoring transition metal(TM)***,we employed density functional theory(DFT)calculations to systematically investigate the electrocatalytic activity of(TM-N_(4))@g-C_(16)N_(3) toward HER/OER/ORR,meanwhile considering the synergistic modulation of H-loading and O-coordination((TM-N_(x)O_(4-x))@g-C_(16)N_(3)^(-)H_(3),x=0–4).Together a“four-step procedure”screening mechanism with the first-principles high-throughput calculations,we find that(Rh-N_(4))and(Ir-N_(2)O_(2)-II)distributed on g-C_(16)N_(3)^(-)H_(3) can modulate the adsorption strength of the adsorbates,thus achieving the best HER/OER/ORR performance among 216 candidates,and the lowest overpotential of 0.098/0.3/0.46 V and 0.06/0.48/0.45 V,***,the d-band center,crystal orbital Hamilton population(COHP),and molecular orbitals are used to reveal the OER/ORR activity ***,the Rh/Ir-d orbital is dramatically hybridized with the O-p orbital of the oxygenated adsorbates,so that the lone-electrons incipiently locate at the antibonding orbital pair up and popula
