Background and objectives Right ventricular apical(RVA)pacing has been reported impairing left ventricular(LV)*** pacing sites in right ventricle(RV)has been explored to obtain better cardiac *** study was designed to compare the hemodynamic effects of right ventricular septal(RVS)pacing with RVA *** Ten elderly patients with chronic atrial fibrillation(AF)and long RR interval or slow ventricular response(VR)received VVI *** hemodynamic difference between RVS and RVA pacing were examined by transthoracic echocardiography(TTE).Results Pacing leads were implanted successfully at the RVA and then RVS in all patients without *** left ventricular(LV)parameters,measured during RVA pacing including left ventricular ejection fraction(LVEF),FS,stroke volume(SV)and peak E wave velocity(EV)were decreased significantly compared to baseline data,while during RVS pacing,they were significantly better than those during RVA ***,after 3-6 weeks there was no statistical significant difference between pre-and post-RVS *** The LV hemodynamic parameters during RVA pacing were significantly worse than baseline *** short term LV hemodynamic parameters of RVS pacing were significantly better than those of RVA pacing;RVS pacing could improve the hemodynamic effect through maintaining normal ventricular activation sequence and biventricular contraction synchrony in patients with chronic AF and slow ventricular response.

目的了解离退休党支部功能发挥的现状,探索完善医疗卫生机构离退休党支部党建工作机制的策略建议。方法2023年6月至9月,对5家医疗卫生机构(中国医学科学院肿瘤医院、中国医学科学院阜外医院、中国中医科学院广安门医院、中国医学科学院基础医学研究所和中国医学科学院医药生物技术研究所)121名离退休党员进行问卷调查,对中国医学科学院肿瘤医院12名离退休党支部书记、党员进行深度访谈。结果参与本次调查的离退休党员共121名,年龄多>60~70岁,多为女性,文化程度集中在大学本科/专科,高级职称占比较大,离退休前多在研究和行政管理岗位工作,有职务的党员占55.37%,目前居住状态多为“不与子女共同居住”,退休后最主要生活内容是“发展兴趣爱好”,住所距离单位较近,39.67%的离退休党员每月回单位>1次,目的多为参加党内组织生活和个人医疗需要。97.52%的党员对离退休党支部工作认可,认为“有党组织觉得有依靠”及“希望能经常听到党组织的声音”,多数党员认为政治功能发挥有力、组织活动规范和凝聚力强;建议按照业务相近的方式设置离退休党支部,认为选任离退休党支部书记的重要条件是具有威望、人品正直公正,其次是具有很强的组织功能和管理能力,“做好为老同志的服务工作”在离退休党支部的基本任务中占比最高。结论探索离退休党支部功能发挥,尤其是政治功能及组织功能的实现机制,需结合新特点、适应新形势,充分发挥离退休党支部在党员中的组织、引导、教育和管理作用,开展灵活多样、方便高效的组织生活,与老干部处等行政部门紧密结合,促进离退休工作高质量开展。

目的探讨结肠息肉切除术在服用抗血栓药物患者中的安全性及有效性。方法回顾性分析2022年8月至2023年8月在中国医学科学院阜外医院消化内科行结肠息肉切除术不间断服用抗血栓药物治疗的患者,对抗血栓药物的种类、切除息肉的部位、长径、术后迟发性出血、穿孔、切除的完整性,术后缺血性事件发生等进行统计分析。结果研究共纳入息肉103例,根据息肉直径是否>10 mm,分成两组(小息肉组87例vs大息肉组16例),两组中息肉部位、形态、服用抗血栓药物的种类、治愈性切除率、术后出血事件发生率、术后缺血事件发生率中组间均无显著统计学差异;大息肉组更多的选择EMR治疗(小息肉组vs大息肉组,4.6%vs 45.5%,P=0.004);术后的病理大息肉组更多腺瘤性息肉(77.0%vs 93.8%,P=0.027)。结论结肠息肉切除术可安全、有效应用于不间断服用抗血栓药物的患者。

心肌梗死后室间隔穿孔(post-infarct ventricular septal rupture,PIVSR)是心肌梗死的严重并发症,常表现为心源性休克,即使提供及时干预,患者院内病死率仍极高。目前指南建议对所有患者立即进行手术,但理想的干预时机仍存在争议。近年来,包括IABP、体外膜肺氧合(extracorporeal membrane oxygenation,ECMO)、Impella和TandemHeart等在内的现代机械循环支持(mechanical circulatory support,MCS)技术发展迅速,为患者提供稳定血流动力学支持的同时使延迟手术治疗成为可能,其可能成为PIVSR患者成功救治的新希望。然而,相关临床证据的缺乏一定程度上限制了MCS的进一步推广,不同MCS策略的比较和选择也成为PIVSR患者救治过程中丞待解决的关键性问题,基于这些疑问本文就PIVSR患者中MCS的辅助治疗作一综述。

Background The traditional chinese medicine injury, but the mechanism of its action is not we protective role of Tongxinluo. Tongxinluo can protect myocardium against documented. We examined the involvement schaemia/reperfusion of nitric oxide in the Methods Miniswine were randomized to four groups of seven： sham, control, Tongxinluo and Tongxinluo coadministration with a nitric oxide synthase inhibitor N^ωnitro-L-arginine （L-NNA, 10 mg/kg i.v.）. Three hours after administration of Tongxinluo, the animals were anaesthetised and the left anterior descending coronary artery ligated and maintained in situ for 90 minutes followed by 3 hours of reperfusion before death. Area of no reflow and necrosis and risk region were determined pathologically by planimetry. The degree of neutrophil accumulation in myocardium was obtained by measuring myeloperoxidase activity and histological analysis. Myocardial endothelial nitric oxide synthase activity and vascular endothelial cadherin content were measured by colorimetric method and immunoblotting analysis respectively. Results Tongxinluo significantly increased the local blood flow and limited the infarct and size of no reflow. Tongxinluo also attenuated myeloperoxidase activity and neutrophil accumulation in histological sections and maintained the level of vascular endothelial cadherin and endothelial nitric oxide synthase activity in the reflow region when compared with control group. The protection of Tongxinluo was counteracted by coadministration with L-NNA. Conclusions Tongxinluo may limit myocardial ischaemia and protect the heart against reperfusion injury. Tongxinluo regulates synthesis of nitric oxide by altering activity of endothelial nitric oxide synthase.
