＜正＞Background To investigate the prevalence of psoriasis in hainan province of China through a multi-center and large-population epidemiological survey. Methods A community-based survey was conducted in 18 cities from hainan province of *** random sampling,Cluster sampling,and cross-sectional study method were *** were required to fulfill the self-report questionnaires and received physical examination by *** 3.1 was utilized for statistical analysis. Results Totally 35314 persons participated in this study,and 30935 valid questionnaires were retrieved with a return rate of 87.60%.Of them,462 patients were found to have *** prevalence rate and standardized prevalence rate of psoriasis was 1.49%,and,26.85%in male patients and 73.15%in female patients.A significant elevation was observed in the prevalence rate of psoriasis in males than in females（P ＜0.01）.It was increased with age and peaked in patients aging from 30 to 39 *** city had the highest prevalence rate of psoriasis in all of the *** most common type was psoriasis vulgaris,while the rarest type was pustular *** main foods which can induce aggravation included seafood,cayenne pepper,alcohol,dog meet,beef,*** patient’s condition obviously aggravated after catching the *** prognosis period of disease was affected by such the disadvantages as insomnia, introversion,depression,anxiety,suspicion,nervous agitation or excitement,bad interpersonal *** life quality which was affected by psoriasis presented that the patients did not dare to wear summer dresses,swim,marry,give birth, apply for a job and attend school. Conclusions The standardized prevalence of psoriasis was 1.49%in hainan province of *** seemed to have a higher prevalence of psoriasis than females.

妊娠合并泌尿系结石是妊娠期妇女一种常见疾病,由于需要兼顾患者及胎儿的安全、且缺乏大规模临床研究,其诊断及治疗方法目前还存在较多争议。为进一步规范妊娠合并泌尿系结石的诊治,本文以中国泌尿外科疾病诊断治疗指南为基础,结合最新国内外相关文献,由中华医学会泌尿外科学分会结石学组、中国尿石症联盟牵头多次组织专家进行深入讨论,结合孕妇特点和我国基本国情,制定该共识,为临床医师对妊娠合并泌尿系结石的诊治提供参考意见。

Background:Clinical outcomes of undifferentiated arthritis(UA)are diverse,and only 40%of patients with UA develop rheumatoid arthritis(RA)after 3 *** predictive markers at disease onset for further intervention is ***,our objective was to analyze the clinical outcomes of UA and ascertain the predictors for RA ***:We performed a prospective,multi-center study from January 2013 to October 2016 among chinese patients diagnosed with UA in 22 tertiary-care *** and serological parameters were obtained at ***-up was undertaken in all patients every 12 weeks for 2 *** factors of disease progression were identified using multivariate Cox proportional hazards ***:A total of 234 patients were recruited in this study,and 17(7.3%)patients failed to follow up during the *** the 217 patients who completed the study,83(38.2%)patients went into *** patients who developed RA had a higher rheumatoid factor(RF)-positivity(42.9%vs.16.8%,χ^2=8.228,P=0.008),anti-cyclic citrullinated peptide(CCP)antibodypositivity(66.7%vs.10.7%,χ^2=43.897,P<0.001),and double-positivity rate of RF and anti-CCP antibody(38.1%vs.4.1%,χ^2=32.131,P<0.001)than those who did ***-CCP antibody but not RF was an independent predictor for RA development(hazard ratio 18.017,95%confidence interval:5.803–55.938;P<0.001).Conclusion:As an independent predictor of RA,anti-CCP antibody should be tested at disease onset in all patients with UA.

Purpose Malnutrition is severe among gastric cancer patients in China. While the Patient-Generated Subjective Global Assessment (PG-SGA) is a widely used nutritional assessment tool, the validation for chinese patients with gastric cancer has not been performed yet. The aim of this research is to validate the chinese version of the PGSGA in chinese gastric cancer patients by conducting analyses of nutrition screening and assessment data. Methods Two thousand consecutive adult patients with gastric cancer were included in this observational, cross-sectional study in several tertiary hospitals in China. A survey was made among those patients, which included several questionnaires and some anthropometric parameters. We calculated the parameters to validate the use of PG-SGA categorizing the nutritional status of gastric cancer patients. Then we performed statistical analysis to evaluate the internal consistency, external consistency, and construct validity. Results This study verified the ability of the PG-SGA to differentiate gastric cancer patients into different nutritional groups. The confirmatory factor analysis verified the original four-factor structure of the PG-SGA. The internal and external consistency was measured by a concordance analysis which showed an intraclass correlation coefficients (ICC) of 0.987. The correlations of the Box 4 score with the Karnofsky performance status score demonstrated alternative form validity with good external consistency (Pearson, r = 0.643, P chinese version of the PG-SGA is a valid tool to access the patients’ nutritional status for chinese gastric patients.

Validity of the chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Lung Cancer Patients

The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the chinese version of the PG-SGA in chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity. Granulomatous lobular mastitis(GLM) is a rare and chronic benign inflammatory disease of the breast. Difficulties exist in the management of GLM for many front-line surgeons and medical specialists who care for patients with inflammatory disorders of the breast. This consensus is summarized to establish evidence-based recommendations for the management of GLM. Literature was reviewed using PubMed from January 1, 1971 to July 31, 2020. Sixty-six international experienced multidisciplinary experts from 11 countries or regions were invited to review the *** of evidence were determined using the American College of Physicians grading system, and recommendations were discussed until consensus. Experts discussed and concluded 30 recommendations on historical definitions,etiology and predisposing factors, diagnosis criteria, treatment, clinical stages, relapse and recurrence of GLM. GLM was recommended as a widely accepted definition. In addition, this consensus introduced a new clinical stages and management algorithm for GLM to provide individual treatment strategies. In conclusion, diagnosis of GLM depends on a combination of history, clinical manifestations, imaging examinations, laboratory examinations and *** approach to treatment of GLM should be applied according to the different clinical stage of GLM. This evidencebased consensus would be valuable to assist front-line surgeons and medical specialists in the optimal management of GLM.

活血化瘀中药抗同种异体移植排斥潜在活性成分与机制的网络药理学研究

目的通过网络药理学方法挖掘活血化瘀中药作用于同种异体移植排斥的潜在关键活性成分及机制。方法选取《中药学》课本上活血化瘀类32种中药作为研究对象,通过BATMAN-TCM在线工具获取中药有结构的活性成分且预测其作用靶点,利用PALM-IST及Gene Cards在线数据库获取同种异体移植排斥的作用靶点,使用Cytoscape 3.3.0软件获取核心靶标集,利用DAVID及STRING数据库对所获得的关键靶标进行基因本体(GO)功能富集分析及基因组百科全书(KEGG)信号通路分析,以构建关键中药-活性成分-靶标-同种异体移植排斥网络。结果活血化瘀中药活性成分可通过作用于17个核心靶标,共获得52条生物学含义富集结果,其中包括生物学过程41项,分子功能8项,细胞成分3项,共获得16条信号通路,主要包括核因子-κB(NF-κB)信号通路、白细胞介素17(IL-17)信号通路、肿瘤坏死因子(TNF)信号通路以及过氧化物酶体增殖物激活受体(PPAR)信号通路。结论莪术二酮、香芹酮、丹参新酮、百里酚、姜黄酮等26种主要活血化瘀中药的潜在活性成分可通过NF-κB信号通路、IL-17信号通路、TNF信号通路以及PPAR信号通路作用于前列腺素氧化环化酶2、淋巴毒素-α、共济失调-毛细血管扩张突变、TNF、载脂蛋白A2、PPAR受体、IL-13、IL-1B靶蛋白从而减轻或抑制同种异体移植排斥反应。
