Risk factors for fatal recurrence of hepatocellular carcinoma and their role in selecting candidates for liver transplantation

BACKGROUND: With the maturation and popularization of skills in liver transplantation (LT), patients with hepatocellular carcinoma (HCC) have an alternative choice. LT as a curative treatment for HCC provides good liver function and systemic condition to recipients. Preoperative tumor characteristics are critical in selecting optimal candidates for LT to optimize the use of donor livers and to achieve a long-term survival. The present study aimed to elucidate the risk factors of HCC involved in fatal recurrence in the first year after LT and to investigate their utility in selecting suitable candidates for LT. METHODS: From April 2002 to October 2005, 303 patients who had received orthotopic LT for HCC were reviewed. Of these patients, those with diffuse intrahepatic or multiple systemic recurrent lesions who died within I year after surgery were investigated as fatal recurrence group (48 patients) and the remaining patients including those who were disease-free without recurrence, those who were alive with recurrence in the first year, or those who died in the first year of other causes, served as control group (255). The two groups were compared by demographics, tumor, and histopathological characteristics for their prognostic significance by logistic regression analysis. RESULTS: Multivariate analysis between the fatal recurrence group and the control group showed that the presence of vascular invasion, a tumor size greater than 6.5 cm, and a preoperative serum alpha-fetoprotein (AFP) level greater than 1000 mu g/L were risk factors for fatal recurrence. Increased risk factors reduced the suitability of candidates for LT and diminished survival in the first year. 85.71% of the patients with all three risk factors, 37.84% of those with two factors, 13.64% of those with one factor, and 6.71% of those without risk factors died from tumor recurrence within I year after transplantation. High-resolution remote thermometry based on Bi^(3+)-doped 0D zinc halide hybrids

Developing highly sensitive optical thermometers is of great significance due to their capability to enable remote and non-contact temperature measurements, rendering them highly applicable in diverse and harsh environments. Herein we report a temperature-dependent phosphor of 0D metal halide hybrid by incorporating Bi^(3+)into the(MePPh_(3))_(2)ZnCl_(4) matrix. Through Bi^(3+)doping,the initially non-luminescent(MePPh_(3))_(2) ZnCl_(4) matrix exhibits a deep-blue emission centered at 453 nm, with a photoluminescence quantum yield(PLQY) of 5.71% and a Stokes shift of 75 nm at room temperature. Experimental characterization demonstrates that exciton-like luminescence of Bi^(3+)is mainly responsible for the blue emission. Single crystals of Bi^(3+)-doped(MePPh_(3))_(2)ZnCl_(4) show an unusual correlation between photoluminescence(PL) lifetime and temperature. Particularly, the dependence of luminescence lifetime on temperature is most remarkable in the temperature range of 80 to 100 K with an exceptional sensitivity up to 0.09 K^(-1), representing one of the best levels for thermometry based on PL decay lifetime. Our work not only provides a viable strategy for designing a novel, environmentally friendly, and stable blue emitter, but also paves the way for precise thermometric application at low temperature.

An adaptive physics-informed deep learning method for pore pressure prediction using seismic data

Accurate prediction of formation pore pressure is essential to predict fluid flow and manage hydrocarbon production in petroleum *** deep learning technique has been receiving more interest due to the great potential to deal with pore pressure ***,most of the traditional deep learning models are less efficient to address generalization *** fill this technical gap,in this work,we developed a new adaptive physics-informed deep learning model with high generalization capability to predict pore pressure values directly from seismic ***,the new model,named CGP-NN,consists of a novel parametric features extraction approach(1DCPP),a stacked multilayer gated recurrent model(multilayer GRU),and an adaptive physics-informed loss *** machine training,the developed model can automatically select the optimal physical model to constrain the results for each pore pressure *** CGP-NN model has the best generalization when the physicsrelated metricλ=0.5.A hybrid approach combining Eaton and Bowers methods is also proposed to build machine-learnable labels for solving the problem of few *** validate the developed model and methodology,a case study on a complex reservoir in Tarim Basin was further performed to demonstrate the high accuracy on the pore pressure prediction of new wells along with the strong generalization *** adaptive physics-informed deep learning approach presented here has potential application in the prediction of pore pressures coupled with multiple genesis mechanisms using seismic data.

Symmetrization of quasi-regular patterns with periodic tilting of regular polygons

Computer-generated aesthetic patterns arewidely used as design materials in various ﬁelds. Themost common methods use fractals or dynamicalsystems as basic tools to create various patterns. Toenhance aesthetics and controllability, some researchershave introduced symmetric layouts along with thesetools. One popular strategy employs dynamical systemscompatible with symmetries that construct functionswith the desired symmetries. However, these aretypically conﬁned to simple planar symmetries. Theother generates symmetrical patterns under theconstraints of tilings. Although it is slightly moreﬂexible, it is restricted to small ranges of tilingsand lacks textural variations. Thus, we proposed anew approach for generating aesthetic patterns bysymmetrizing quasi-regular patterns using general kuniformtilings. We adopted a uniﬁed strategy toconstruct invariant mappings for k-uniform tilings thatcan eliminate texture seams across the tiling ***, we constructed three types of symmetriesassociated with the patterns: dihedral, rotational, andreﬂection symmetries. The proposed method can beeasily implemented using GPU shaders and is highlyefficient and suitable for complicated tiling with regularpolygons. Experiments demonstrated the advantages of our method over state-of-the-art methods in terms ofﬂexibility in controlling the generation of patterns withvarious parameters as well as the diversity of texturesand styles.

Pediatric liver transplantation in 31 consecutive children

Background Although liver transplantation has become a standard therapy for end-stage liver diseases, the experience of pediatric liver transplantation is limited in China. In this article we report our experience in pediatric liver transplantation, and summarize its characters in their indications, surgical techniques, and postoperative managements. Methods Thirty-one children （≤18 years old） underwent liver transplantation in our centers. The mean age at transplantation was 12.4 years old （ranged from 5 months to 18 years） with 7 children being less than 4 years of age at transplantation. The most common diagnosis of patients who underwent liver transplantation were biliary atresia, Wilson＇s disease, primary biliary cirrhosis, glycogen storage disease, hepatoblastoma, urea cycle defects, fulminant hepatic failure, etc. The surgical procedures included 12 standard （without venovenous bypass）, 6 pigyback, 6 reduced-size, 3 split, 3 living donor liver transplantation, and 1 Domino liver transplantation. The triple-drug （FK506, steroid, and mycophenolate mofetil） immunosuppressive regimen was used in most of patients. Patients were followed up for a mean of 21.8 months. Results Five of the 31 patients died during perioperative time; mortality rate was 16.1%. The reasons of death were infections, primary non-function, heart failure, and hypovolemic shock. Postoperative complications in 10 patients included biliary leakage, acute rejection, abdominal infection, hepatitis B virus （HBV） or hepatitis C virus （HCV） infection, and pulmonary infection. Overall patient cumulative survival rate at 1-, 3-, and 5-year was 78.1%, 62.6%, 62.6%, respectively. Conclusions The most common indications of pediatric liver transplantation were congenital end-stage liver diseases. According to patients＇ age and body weight, standard, piggyback, reduced-size, split, or living donor liver transplantation should be performed. Recently,the chemiluminescence(CL)induced by carbon nanodots(CDs)has intrigued researchers’extensive interests in various applications due to its special light emission ***,the difficulty of synthesizing chemiluminescent CDs with full-spectrum emission severely hinders the further regulation of the CL emission ***,the multi-color-emissive chemiluminescent CDs are rational designed and further synthesized by regulating the sp2-hybrid core and sp3-hybrid surface from the citrate-ammonia molecular in a single solvothermal *** experimental characterizations and density functional theory calculations reveal that the higher temperature can promote the crosslinking polymerization/carbonization of carbon core and the higher protonation of solvent can determine the core size of final CDs,resulting in the variant CL emission from molecular-,crosslinking-and ***,the CL emission of the CDs can be further synthesized by tuning the luminescence chromophores in the formation process via regulating the temperature and solvent,enabling the applications of the CL CDs in illumination and information *** study paves a new technology to understand the luminescence of CDs and affords an industry translational potential over traditional chemiluminescent molecular.
