In the introduction Lamb （1924a） wrote for his translation of Plato＇s dialogues, he, knowingly or unknowingly, moves from the discussion of Plato the author to Socrates the character in Plato＇s dramatic depictions, revealing the entangling relationship between the two in the representations of the master by his disciple. In such cases, it takes more effort than usual to figure out whom the pronoun ＂he＂ stands for in this introduction. This example is just a tip of the iceberg of this phenomenon which does have an influence on the study of Plato＇s works. The names of Plato and Socrates are interchangeable in most titles where the ethics of Plato is taken for that of Socrates or vice versa. This paper examines the difficulties that arise as a result and brings up four alternatives as possible perspectives from which one may look at the same issues.

Spermatogenesis is an androgen-regulated process that depends on the action of androgen receptor （AR）. Sperm production may be affected in men treated for testicular cancer （TC）, and it is important to identify the factors influencing the timing of spermatogenesis recovery following cancer treatment. It is known that the CAG and GGN repeat numbers affect the activity of the AR; therefore, the aim of this study is to investigate if the CAG and GGN polymorphisms in the AR gene predict recovery of sperm production after TC treatment. TC patients （n = 130） delivered ejaculates at the following time points： postorchiectomy and at 6, 12, 24, 36, and 60 months posttherapy （TO, T6, T12, T24, T36, and T60）. The CAG lengths were categorized into three groups, 〈22 CAG, 22-23 CAG, and 〉23 CAG, and the GGN tracts were also categorized into three groups, 〈23 GGN, 23 GGN, and 〉23 GGN. At T12, men with 22-23 CAG presented with a statistically significantly （P = 0.045） lower sperm concentration than those with other CAG numbers （8.4 × 10^6 ml^-1 vs 16 × 10^6 ml^-1; 95% CI： 1.01-2.65）. This association was robust to omitting adjustment for treatment type and sperm concentration at TO （P= 0.021; 3.7× 10^6 ml^-1 vs 10 × 10^6 ml^-1; 95% CI： 1.13-4.90）. The same trends were observed for total sperm number. The least active AR variant seems to be associated with a more rapid recovery of spermatogenesis. This finding adds to our understanding of the biology of postcancer therapy recovery of fertility in males and has clinical implications.

Venepuncture is the commonest procedure performed in health care settings. A review of literature and search on complications of venepuncture and blood collection. This procedure is without complications, which sometime can be fatal. Complications that can arise from venepuncture include haematoma formation, nerve damage, pain, haemaconcentration, extravasation, iatrogenic anaemia, arterial puncture, petechiae, allergies, fear and phobia, infection, syncope and fainting, excessive bleeding, edema and thrombus.

Pristinamycin,produced by Streptomyces pristinaespiralis,which is a streptogramin-like antibiotic consisting of two chemically unrelated components:pristinamycin I(PI)and pristinamycin II(PII),shows potent activity against many antibiotic-resistant ***,so far pristinamycin production titers are still quite low,particularly those of *** this study,we constructed a PI single component producing strain by deleting the PII biosynthetic genes(snaE1 and snaE2).Then,two metabolic engineering approaches,including deletion of the repressor gene papR3 and chromosomal integration of an extra copy of the PI biosynthetic gene cluster(BGC),were employed to improve PI *** final engineered strain DPIIDpapR3/PI produced a maximum PI level of 132 mg/L,with an approximately 2.4-fold higher than that of the parental strain *** *** that the PI biosynthetic genes are clustered in two main regions in the 210 kb“supercluster”containing the PI and PII biosynthetic genes as well as a cryptic polyketide BGC,these two regions were cloned separately and then were successfully assembled into the PI BGC by the transformation-associated recombination(TAR)***,the metabolic engineering approaches employed is very efficient for strain improvement in order to enhance PI titer.

BACKGROUND Patients with myasthenia gravis(MG)are at a higher risk of developing Takotsubo cardiomyopathy(TTC),particularly during a myasthenic *** crisis-associated TTC occurs predominantly in *** this case report,we present a man with metastasized prostate carcinoma who developed TTC after new-onset *** SUMMARY An 81-year-old man with non-insulin dependent diabetes mellitus and metastasized prostate carcinoma presented with *** primary assessment examination at the emergency department,there was evident blepharoptosis of his right *** electrocardiograms were suggestive of an acute anterior wall myocardial infarction,for which he underwent emergency coronary *** obstructive coronary artery disease was *** the coronary angiography,the patient developed respiratory failure and was admitted to the Intensive Care Unit for non-invasive respiratory *** following day,diagnostic neostigmine test revealed a myasthenic *** echocardiography revealed left ventricular apical ballooning with a typical appearance of *** the potentially reversible character of both MG and TTC,the patient and family requested an end of support in the Intensive Care Unit due to age and chronic malignancy with reduced quality of life in recent months after non-chemo-responding prostate *** patient died soon after treatment *** Elderly men should be carefully evaluated for TTC when new-onset MG is diagnosed.

Hereditary non-polyposis colorectal carcinoma （HNPCC） is an autosomal dominant disorder associated with colorectal and endometrial cancer and a range of other tumor types. Germline mutations in the DNA mismatch repair （MMR） genes, particularly MLH1, MEH2, and MEH5, underlie this disorder. The vast majority of these HNPCC-associated mutations have been proven, or assumed, given the family history of cancer, to be transmitted through several generations. To the best of our knowledge, only a single case of a de novo germline MMR gene mutation （in MEH2） has been reported till now. Here, we report a patient with a de novo mutation in MLH1. We identified a MLH1 Q701X truncating mutation in the blood lymphocytes of a male who had been diagnosed with rectal cancer at the age of 35. His family history of cancer was negative for the first- and second-degree relatives. The mutation could not be detected in the patient＇s parents and sibling and paternity was confirmed with a set of highly polymorphic markers. Non-penetrance and small family size is the common explanation of verified negative family histories of cancer in patients with a germline MMR gene mutation. However, in addition to some cases explained by non-paternity, de novo germline mutations should be considered as a possible explanation as well. As guidelines that stress not to restrict MMR gene mutation testing to patients with a positive family history are more widely introduced, more cases of de novo MMR gene germline mutations may be revealed.
