Outcomes of curative esophageal cancer surgery in elderly:A metaanalysis

Ustekinumab: “Real-world” outcomes and potential predictors of nonresponse in treatment-refractory Crohn’s disease

MicroRNA sequences modulating inflammation and lipid accumulation in macrophage “foam” cells: Implications for atherosclerosis

Accumulation of macrophage"foam"cells,laden with cholesterol and cholesteryl ester,within the intima of large arteries,is a hallmark of early"fatty streak"lesions which can progress to complex,multicellular atheromatous plaques,involving lipoproteins from the bloodstream and cells of the innate and adaptive immune *** accumulation triggers induction of genes encoding proteins mediating the atheroprotective cholesterol efflux *** the arterial intima,however,this mechanism is overwhelmed,leading to distinct changes in macrophage phenotype and inflammatory *** the last decade marked gains have been made in understanding of the epigenetic landscape which influence macrophage function,and in particular the importance of small non-coding micro-RNA(miRNA)sequences in this *** review identifies some of the miRNA sequences which play a key role in regulating"foam"cell formation and atherogenesis,highlighting sequences involved in cholesterol accumulation,those influencing inflammation in sterol-loaded cells,and novel sequences and pathways which may offer new strategies to influence macrophage function within atherosclerotic lesions.

Stenting as a bridge to surgery in obstructing colon cancer:Longterm recurrence pattern and competing risk of mortality

BACKGROUND Stenting as a bridge to curative surgery(SBTS)for obstructing colon cancer(OCC)has been associated with possibly worse oncological *** To evaluate the recurrence patterns,survival outcomes,and colorectal cancer(CRC)-specific death in patients undergoing SBTS for *** Data from 62 patients undergoing SBTS at a single tertiary centre over ten years between 2007 and 2016 were retrospectively *** outcomes were recurrence patterns,overall survival(OS),cancer-specific survival(CSS),and CRC-specific *** and CSS were estimated using the Kaplan-Meier *** risk analysis with cumulative incidence function(CIF)was used to estimate CRC-specific mortality with other cause-specific death as a competing ***-Gray regressions were performed to determine prognostic factors of CRC-specific *** and multivariate subdistribution hazard ratios and their corresponding Wald test P values were *** 28 patients(45.2%)developed metastases after a median period of 16 *** the 18 patients with single-site metastases:Four had lung-only metastases(14.3%),four had liver-only metastases(14.3%),and 10 had peritoneum-only metastases(35.7%),while 10 patients had two or more sites of metastatic disease(35.7%).The peritoneum was the most prevalent(60.7%)site of metastatic involvement(17/28).The median follow-up duration was 46 mo.26(41.9%)of the 62 patients died,of which 16(61.5%)were CRC-specific deaths and 10(38.5%)were deaths owing to other causes. The 1-, 3-, and 5-year OS probabilities were 88%, 74%, and 59%;1-, 3-, and5-year CSS probabilities were 97%, 83%, and 67%. The highest CIF for CRC-specific death at 60 mowas liver-only recurrence (0.69). Prevalence of Thumb Sucking Habits amongst Children at the Knust Basic School

Main Objective: To determine the prevalence of thumb sucking habits amongst children at the KNUST Basic School. Methodology: This study was quantitative and descriptive with the design being a cross-sectional type. The data was obtained from KNUST Basic School with a sample size of 200. Data was obtained through questionnaires and clinical examination. Results: Out of the 200 children studied, 19 (9.5%) were identified as thumb suckers, with a majority being females (13, 68.42%) and the rest being males (6, 31.58%). Most of the thumb suckers (47.37%) engaged in the habit due to hunger. The clinical manifestations observed among the thumb suckers included Class I malocclusion (94.7%), high arched palate (89.5%), anterior open bite (4), increased overjet (3, 15.8%), proclined maxillary incisors (4, 22.2%), linguoversion of mandibular incisors (1, 5.6%), unilateral posterior crossbite (2, 10.5%), speech changes (7, 41.2%), thinner thumb (7, 36.8%), elongated thumb (9, 47.4%), and callus formation on the thumb (2, 10.5%). Conclusion: Thumb sucking is a common oral habit which stimulates the growth of the orofacial complex from ages 0 to 3 years. The habit of thumb sucking is usually outgrown by age 3 but beyond this age, it turns out as a deleterious habit which has to be seen as such and measures put in place to stop thumb sucking.

Peritoneal carcinomatosis of colorectal origin:Incidence,prognosis and treatment options

Peritoneal carcinomatosis (PC) is one manifestation of metastatic colorectal cancer (CRC). Tumor growth on intestinal surfaces and associated fluid accumulation eventually result in bowel obstruction and incapacitating levels of ascites, which profoundly affect the quality of life for affected patients. PC appears resistant to traditional 5-fluorouracil-based chemotherapy, and surgery was formerly reserved for palliative purposes only. In the absence of effective treatment, the histori-cal prognosis for these patients was extremely poor, with an invariably fatal outcome. These poor outcomes likely explain why PC secondary to CRC has received little attention from oncologic researchers. Thus, data are lacking regarding incidence, clinical disease course, and accurate treatment evaluation for patients with PC. Recently, population-based studies have revealed that PC occurs relatively frequently among patients withCRC. Risk factors for developing PC have been identi-fied:right-sided tumor, advanced T-stage, advanced N-stage, poor differentiation grade, and younger age at diagnosis. During the past decade, both chemother-apeutical and surgical treatments have achieved prom-ising results in these patients. A chance for long-term survival or even cure may now be offered to selected patients by combining radical surgical resection with intraperitoneal instillation of heated chemotherapy. This combined procedure has become known as hy-perthermic intraperitoneal chemotherapy. This edito-rial outlines recent advancements in the medical and surgical treatment of PC and reviews the most recent information on incidence and prognosis of this disease. Given recent progress, treatment should now be considered in every patient presenting with PC.
