目的通过荟萃分析系统评价常规超声、超声造影、超声弹性成像及CT检查单独或者四者中两者联合检查在临床确诊甲状腺癌淋巴结转移中的实际价值。方法检索2006年1月至2023年8月的中英文数据库,筛选出关于超声、超声造影、弹性成像和CT在诊断甲状腺癌淋巴结转移的相关文献。两位学者独立评估文献质量,并使用QUADAS量表进行评分。然后,通过Meta-Disc软件分析单一及联合检查的诊断比值比,进行异质性检验和Meta分析,计算敏感性、特异性、阳性似然比、阴性似然比、诊断比值比、准确性以及ROC曲线下面积(AUC)。最终,通过两两比较计算OR值及P值,评估不同诊断技术及联合应用鉴别甲状腺癌淋巴结转移的诊断效能。结果最终纳入69个诊断性研究,共计病灶30666个。元分析研究表明,超声造影敏感性最高[0.83(0.77~0.87)],常规超声特异性最高[0.84(0.79~0.88)],AUC最高的为超声造影(0.90),超声造影和超声弹性成像准确性较高(分别为0.81、0.80)均高于常规超声(0.77)和CT(0.69),超声造影和超声弹性成像敏感性均高于CT,两者之间差异有统计学意义(均P0.05),但后者特异性最高[0.90(0.88~0.92)],两者之间差异有统计学意义(P<0.01),常规超声+超声造影和超声造影+超声弹性成像敏感性均高于常规超声+CT,两者之间差异有统计学意义(均P<0.05),常规超声+CT特异性最低[0.76(0.70~0.81)]。结论甲状腺癌淋巴结转移的诊断效能以超声造影最高;诊断甲状腺癌淋巴结转移超声新技术较CT诊断价值高,可作为常规超声的补充检查。两种技术结合使用时,超声造影+超声弹性成像准确性最高,超声造影+超声弹性成像与常规超声+超声造影均具有较高敏感性和特异性,两者诊断性能均高于常规超声+超声弹性成像和常规超声+CT,实际临床中当超声弹性成像联合超声造影检查受限,常规超声联合超声造影能有效作为替代手段,提供良好的诊断支持。

Prediction model for the initial seed clearing angles of a precision seed meter based on vector fields

Seed clearing is a critical stage during precision seed metering process to ensure high seed singulation. However, there is a lack of understanding of the dynamics in the seed clearing process. In this study, a model was developed to predict initial seed clearing angle, in the seed clearing process using vector fields. The model was applied to an existing high-speed metering device and soybean seeds, and the model was evaluated with bench testing results. Results showed that dynamic changes in forces and constraints of seeds during the seed clearing process could be abstracted as vectors, and the changes of vector directions could be described by their phase angles. The phase angles were functions of the rotational angle of the seed meter. The phase angle of the constraint boundary linearly increases with the increase of the rotational angle. The phase angle of the force fluctuates, as the rotational angle changes. Initial seed clearing angle obtained from the phase angles varies from 8 degrees to 59 degrees, depending on the seeder travel speed. When comparing the values of the initial seed clearing angles predicted by the model with those from the bench tests, the root mean square error (RMSE) were from 2.73 to 3.14, and the correlation (r) between predict and observer were all higher than 0.98, indicating that the model had reasonably good accuracy.

Possible mechanisms underlying treatment of Alzheimer’s disease with Traditional Chinese Medicine: active components, potential targets and synthetic pathways of Bulao Elixir

OBJECTIVE: To elucidate the mechanisms underlying the treatment of Alzheimer’s disease(AD) with Traditional Chinese Medicine(TCM), by examining the active components, potential targets and synthetic pathways of Bulao Elixir(BLE).METHODS: The Absorption, Distribution, Metabolism, Excretion(ADME)/Toxicology(T) calculation was used to screen the active components of Bulao Elixir. Based on the TCM Systems Pharmacology Analysis Platform(TCMSP database) and a text mining tool(GoPubMed database), we predicted and screened the active components of Bulao Elixir and its therapeutic targets for AD. Using the Database for Annotation, Visualization and Integrated Discovery(DAVID), we obtained the targets for AD. Cytoscape software was used to establish a network map of the active component-target and target-pathway of Bulao Elixir. Gene function, related biological processes and signaling pathways were analyzed using the DAVID ***: Twelve active components were selected from 196 components of Bulao Elixir. Among 2209 targets, 102 effective targets were selected, and 30 important targets were identified via matching with the disease targets. After further analysis, 14 core targets were identified. Enrichment analysis revealed that most of these important targets were involved in multiple biological processes, including apoptosis, inflammatory reactions, and cell regulation cycles. The synthetic pathways for AD treatment were identified after analyzing and confirming the relevant pathways, providing potentially useful information for diagnosis and treatment methods for ***: The current study elucidated the potential treatment mechanisms of Bulao Elixir in AD using network pharmacology, providing a foundation for further clarification of its treatment targets.

电力电子变压器的运行可靠性水平受历史运行工况、元器件健康水平等因素直接影响。基于功率器件损耗分析,该文提出模块化多电平级(modular multilevel converter,MMC)型电力电子变压器运行可靠性评估方法,将运行条件纳入分析范畴以评估短期运行时设备可靠性变化情况。首先详细分析电力电子变压器输入级、隔离级和输出级的绝缘栅双极型晶体管(insulated gate bipolar transistor,IGBT)模块损耗及其结温波动情况。然后,分析了不同时间尺度结温波动对元器件故障率的影响,建立了电力电子变压器可靠性状态空间,并利用可靠性灵敏度指标辨识运行阶段设备的薄弱环节。最后,以RBTS-BUS2系统算例进行验证,分析表明不同时间尺度的结温波动对功率器件可靠性影响存在差异,隔离级二次侧绝缘栅双极型晶体管对装置故障率贡献度最大,所提方法能够有效反应不同工况下装置及配电网可靠性水平的时变特性,验证了该文模型与评估方法的有效性。

鲁迅早年的留日生活不仅促使他走上文学道路,也使他的创作在一定程度上受到日本作家的影响。《伤逝》作为鲁迅反思“青年男女追求个性解放与爱情自由”主题的作品,与森鸥外的《舞姬》在情节等方面体现出一定程度的相似性,同时又深入探究了森鸥外所忽视的女性意识、妇女思想解放等问题,使得作品显示出对前人的超越以及与中国社会现实的结合。在比较视域的鉴照下,两部作品共同描绘了不同时代的青年男女在个性与爱情自由受到压迫时所作出的必然反抗,以及这种反抗在各种因素掣肘下所走向的必然投降,从而揭示了青年男女在追求个性解放与爱情自由道路上“由必然的反抗走向必然的投降”的规律。这一规律既解释了两场爱情何以走向悲剧,又给予了读者重新审视这两部作品及其所描绘的时代的崭新角度。

可再生能源规模化接入配电网已经成为共识。针对大量可再生电源以及电力电子器件所导致的配电网无功波动、网损增加以及电压波动等系列问题,提出了一种基于改进自适应粒子群算法的集成可再生能源的配电网无功优化方法。通过对风机以及光伏系统出力进行建模,获得其出力预测数据;建立含有电压越限罚函数的集成可再生能源的配电网无功优化模型;引入了电压稳定指标的概念,对配电网各个时段的电压状况进行评价;并通过改进的自适应粒子群优化算法进行模型求解;利用改进的IEEE-33节点系统进行了仿真验证。仿真结果表明文中提出的模型能够有效地降低网损,减少电压波动,保证电网的稳定运行,可以为后续的无功优化提供参考与借鉴。
