Objective:The mainstay treatment of esophageal squamous cell carcinoma(ESCC)involves chemotherapy and ***,alternative therapies are required for patients who are refractory or intolerant to existing ***:In this single-arm,multicenter,open-label phase Ib study,30 patients received an intravenous infusion of SCT200,an antiepidermal growth factor receptor(EGFR)monoclonal antibody,6.0 mg/kg once a week for 6 weeks,followed by 8.0 mg/kg once every 2 weeks until disease progression or intolerable *** primary endpoint was the objective response rate(ORR).The secondary endpoints were progression-free survival(PFS),overall survival(OS),and ***:Thirty patients were enrolled between July 2018 and May *** ORR was 16.7%(95%CI:5.6%–34.7%).The median PFS and OS were 3.1 months(95%CI:1.5–4.3)and 6.8 months(95%CI:4.7–10.1),respectively.A numerical difference without any statistical significance in ORR was observed in patients with different EGFR expressions(≥50%:25.0%vs.<50%:0%,P=0.140)or TP53 mutation abundance(<10%:23.8%vs.≥10%:0%,P=0.286).Improved median PFS(3.4 vs.1.4 months,P=0.006)and OS(8.0 vs.4.2 months,P=0.027)were associated with TP53 mutation abundance of<10%.The most common treatment-related adverse events of grade 3 or 4(occurring in≥2 patients)were hypomagnesemia[7(23.3%)]and rash[2(6.7%)].No treatmentrelated death ***:SCT200 monotherapy as the second-or further-line treatment for advanced ESCC showed favorable efficacy,with an acceptable safety ***53 mutation abundance might serve as a potential predictive biomarker.

Objective:In this post-hoc analysis,we evaluated anlotinib treatment-induced hypertension as a potential predictive factor of efficacy in esophageal squamous cell carcinoma(ESCC)***:A total of 109 patients enrolled in the anlotinib group in a phase 2 trial were *** tumor response was assessed by computed tomography at week 3,week 6,and then every 6 weeks until progressive disease was *** primary endpoint of the study was progression free survival(PFS).The secondary endpoints included overall survival(OS)and objective response rate(ORR).Results:In all patients,the median PFS was 3.02 months[95%confidence interval(CI):2.63–3.65 months]and the OS was 6.11 months(95%CI:4.40–7.79 months).The ORR was 7.34%(95%CI:3.22%–13.95%).A total of 59(54%)patients were diagnosed with treatment-induced hypertension(Group A),and the remaining patients(n=50,46%)were in Group *** prognostic factors were similar between the 2 *** in Group A had a longer PFS and OS and higher *** stratifying patients using a previously known history of hypertension,treatment-induced hypertension was a predictor only for patients without previous hypertension,who had longer PFS[hazard ratio(HR):0.40,95%CI:0.24–0.68]and OS(HR:0.37,95%CI:0.21–0.67).Conclusions:We showed,for the first time,a correlation between treatment-induced hypertension and better prognoses in recurrent or metastatic ESCC patients treated with anlotinib,without a previously known history of ***-induced hypertension may be a simple and low cost predictor for anlotinib antitumor efficacy in these patients,which may also reflect the intended target inhibition.

Arsenic trioxide enhances the therapeutic efficacy of adjuvant post-operative chemotherapy of gastric carcinoma while protecting bone marrow

Objective： The aim of the study was to investigate the prospective study if treatment with arsenic trioxide （AS2O3） could enhance disease-free survival as adjuvant post-operative chemotherapy for gastric cancer patients and protect bone marrow from the negative effects of chemotherapy. Methods： 84 adults were randomized into two groups. Patients in treament group were treated with As2O3 and FOLFOX regimen, the other were administered with FOLFOX regimen only. Results： Four patients were withdrawn in treatment group after 3-4 cycles and the reasons were headache and fidgety （n = 2）, rhythmia （n = 1） and AST/ALT elevation （n = 1）, while 1 patient in control group after 4 cycles for neutropenia. In the treatment group, the median DFS was 28.34 months （95% CI, 25-33 months）. While in control group, the median DFS was 24.50 months （95% CI, 20-30 months）. This difference was not statistically significant （chi-square： 2.8885; P value： 0.0892）. Pa- tients in the same subgroup of node-positive was 29 in the treatment group and 32 in control group, respectively. The median DFS was 27.87 months （95% CI, 25-31 months） in the treatment group and 24.18 months （95% CI, 19-31 months） in the control group with promising statistical significance （HR 1.89; chi-square： 4.78; P value： 0.0287）. The most common grades 3-4 toxicity was leucopenia （n = 11） in control group and the difference was significant （chi-square： 3.9768, P value： 0.046） compared with that in treatment group （n = 4）. Conclusion： The combination of arsenic trioxide and FOLFOX regimen has a potential advantage of enhancing disease-free survival in patients with gastric cancer in nodal-positive status as post-operative chemotherapy, and protect bone marrow from the negative effects of chemotherapy.

Background:The benefit of systemic treatments in esophageal squamous cell carcinoma(ESCC)which has pro-gressed after chemotherapy is still uncertain and optimal regimens based on randomized trials have not yet been *** aimed to compare the efficacy of irinotecan plus S-1 with S-1 monotherapy in recurrent or metastatic ESCC patients who had resistance to platinum-or taxane-based ***:We conducted a prospective randomized,multicenter,open-label,phase 3 trial in 15 centers across *** patients were adults with histologically confirmed recurrent or metastatic ESCC,and were randomly assigned(ratio,1:1)to receive either irinotecan plus S-1(intravenous infusion of irinotecan[160 mg/m2]on day 1 and oral S-1[80-120 mg]on days 1-10,repeated every 14 days)or oral S-1 monotherapy(80-120 mg/day on days 1-14,repeated every 21 days)using a central computerized minimization *** primary endpoint was progression-free survival(PFS).Results:Between December 23,2014 and July 25,2016,we screened 148 patients and randomly assigned 123 patients to receive either irinotecan plus S-1 regimen(n=61)or S-1 monotherapy(n=62).After a median follow-up of 29.2 months(95%confidence interval[CI]17.5-40.9 months),the median PFS was significantly longer in the irinotecan plus S-1 group than in the S-1 monotherapy group(3.8 months[95%CI 2.9-4.3 months]vs.1.7 months[95%CI 1.4-2.7 months],hazard ratio=0.58,95%CI 0.38-0.86,P=0.006).The objective response rates were 24.6%in the irinotecan plus S-1 group and 9.7%in the S-1 monotherapy group(P=0.002).The patients in the irinotecan plus S-1 group presented with increased rates of grade 3-4 leukopenia(16.4%vs.0%),neutropenia(14.8%vs.1.6%),and nausea(4.9%vs.0%).No significant difference in grade 3-4 diarrhea and no treatment-related deaths were observed in both ***: The combination of irinotecan with S-1 was similarly tolerable but significantly prolonged PFS compared to S-1 monotherapy as a second- o

免疫检查点抑制剂在某些恶性肿瘤治疗中带来令人瞩目的成果,为癌症的研究和治疗方案的发展开辟了一个新的时代。随着越来越多地使用这类药物,广泛的免疫相关不良事件随之产生,甚至部分患者被迫终止治疗,影响预后,这引起了研究者们的密切关注,由于免疫相关不良事件的发生机制尚不明确,所以现阶段暂无公认有效的控制途径。本文综述了免疫检查点抑制剂治疗中常见的相关不良事件,对重点问题总结分析,以期尽早做出诊断及处理,提高患者生存率。

目的:探讨三氧化二砷(As2O3)对人大肠癌ls174细胞相关耐药蛋白基因表达的影响。方法:确定三氧化二砷(As2O3)对人大肠癌ls174细胞的非细胞毒剂量,分别采用生化方法及RT-PCR方法检测非细胞毒剂量下As2O3对人大肠癌ls174细胞MRP、GST-π、TOPO-Ⅱ表达水平的变化。结果:As2O3的非细胞毒性剂量(生长率>95%)为4.0μg/ml。以此为逆转耐药剂量可使人大肠癌ls174细胞,MRP、GST-π、TOPO-ⅡmRNA表达水平下调。结论:As2O3可部分逆转人大肠癌ls174细胞对5-氟尿嘧啶的耐药性,MRP、GST-π、TOPO-Ⅱ改变参与人大肠癌耐药机制。

原发性肝癌起病隐匿,恶性程度高,易发生进展及转移,往往发现疾病时已是肝癌中晚期。较长一段时间内,对于肝癌的主要治疗手段包括手术、放化疗、介入治疗及靶向治疗。但传统治疗方法疗效有限,近年来,随着对原发性肝癌的免疫系统研究逐步深入,免疫治疗已然成为一种新兴的治疗手段,其中以程序性死亡蛋白-1(PD-1)/程序性死亡蛋白配体-1(PD-L1)为靶点的免疫疗法在非小细胞肺癌及肾细胞癌的临床应用中取得了明显疗效。因此,许多研究者开始进一步探索PD-1/PD-L1抑制剂在肝癌中的治疗效果。本文将对PD-1/PD-L1抑制剂治疗肝癌的机制及现阶段国内外多项免疫治疗取得的成效作一综述。
