AIM: To assess the utility and safety of single-operator cholangiopancreatoscopy(SOCPS) using the Spy Glass system in widespread clinical application for biliary and pancreatic ***: This study was a prospective case series conducted in 20 referral centers in Japan. There were 148 patients who underwent SOCPS; 124 for biliary diseases and 24 for pancreatic diseases. The attempted interventions were SOCPS examination, SOCPS-directed tissue sampling, and therapy for stone removal, among others. The main outcomes were related to the procedure success rate in terms of visualizing the target lesions, SOCPS-directed adequate tissue sampling, and complete stone removal. RESULTS: A total of 148 patients were enrolled for the diagnosis of indeterminate biliary and pancreatic lesions or treatment of biliary and pancreatic disease. The overall procedure success rate of visualizing the target lesions was 91.2%(135/148). The overall procedural success rates of visualizing the target lesions of diagnostic SOCPS in the bile duct and pancreatic duct were 95.5%(84/89) and 88.2%(15/17), respectively. Diagnosis: the overall adequate tissue for histologic examination was secured in 81.4% of the 86 patients who underwent biopsy under SOCPS(bile duct, 60/75, 80.0%; pancreatic duct, 10/11, 90.9%). The accuracy of histologic diagnosis using SOCPS-directed biopsies in indeterminate bile duct lesions was 70.7%(53/75). In the pancreatic duct, the accuracy of SOCPS visual impression of intraductal papillary mucinous neoplasm was 87.5%(14/16). Stone therapy: complete biliary and pancreatic stone clearance combined with SOCPS-directed stone therapy using electrohydraulic lithotripsy or laser lithotripsy was achieved in 74.2%(23/31) and 42.9%(3/7) of the patients, respectively. 1939年,费孝通出版了中国首部现代民族志《江村经济——中国农民的生活》,他在书中对绵羊的投机性饲养、买卖及交易事业极为关注。费孝通眼中的"羊"叫作"湖羊",分布于浙江省至江苏省一带的太湖南部及上海周边地区。这些地区的自然环境并不适合饲养绵羊,所以湖羊的饲养只能采用圈养的方式。同时,这些地区的市场经济高度发展,饲养湖羊的主要目的在于小羊的贩卖,是迎合市场经济获取利益的重要手段;而公羊的快速更替这种繁殖形态也与市场经济相呼应,具有降低绵羊养殖成本的功能。此外,湖羊具有多胎性、四季繁殖性、早熟性的品种特性,这些生物学特征同样是由其所在的江南农村的社会、经济状况决定的。总之,"湖羊"与一般绵羊不同,是人为极端改良的品种,是适应江南农村自然、经济、社会环境的产物,它的身体里印刻着江南地区农业发展的历史。

Distinctive magnetic resonance imaging findings of hepatocellular carcinoma after hepatitis C virus eradication with direct-acting antivirals

Aim:The aim of the present study was to evaluate the characteristics of the magnetic resonance imaging features of hepatocellular carcinoma(HCC)that developed early after the eradication of hepatitis C virus(HCV)by direct-acting antiviral(DAA)***:This study included 26 patients who achieved sustained viral response with DAA and developed HCC thereafter within one year(DAA-SVR HCC).The radiologic characteristics of these patients were evaluated by contrast-enhanced magnetic resonance imaging,including diffusion-weighted imaging(DWI)and T2-weighted imaging(T2WI).For comparison,80 HCC patients with positive HCV RNA(HCV-positive HCC)were *** 42 patients where tumor biopsy was available,histological grade and radiologic findings were ***:The rates of high intensity on DWI and T2WI were significantly higher in DAA-SVR HCC compared to HCV-positive HCC(DWI:100%vs.67.5%,P<0.001T2WI:92.6%vs.67.5%,P=0.01).HCC with high intensity on DWI or T2WI was more likely to have moderately or poorly differentiated HCC compared to well-differentiated HCC(DWI:69.7%vs.30.3%,P=0.02;T2WI:66.7%vs.27.3%,P=0.03).Conclusion:High intensity on DWI and hyperintensity on T2WI were distinctive features of HCC that developed within one year after the end of DAA treatment.

Background: The ability to climb stairs (ascending and descending stair without using a handrail) and rise from a chair (rising from chairs without using an elbow rest) are among the most important measures of physical function for ADL evaluation for the independent living, and assessed by the questioners on many epidemiological studies in elderly. But little is known about the relationship between the self-reported performance level of the tasks and lower leg strength and power in very elderly people. The purpose of this study was to ascertain the relationship between the self-reported performance level of two tasks and the lower leg function in community-dwelling 80-year-old population. Methods: Out of 994 persons who were 80 years old living in Morioka City, Iwate Prefecture, Japan, 607 individuals (236 men and 371 women) underwent a physical fitness test that included measurements of leg extensor power and knee extensor strength. The ability to climb up stair and to rise from a chair was assessed by self- reported questionnaire which was ranked in three levels. The area under the receiver-operating characteristic (ROC) curve (AUC) was used to evaluate the measurements. Results: The cut-off points for the leg extensor power of subjects who could completely perform the stair-climbing and chair-rising functions were determined to be approximately 8.6 watt/kg body mass for men and 5.6 watt/kg body mass for women. In addition, the cut-off points for the knee extensor strength of subjects who could completely perform the stair-climbing and chair-rising functions were determined to be approximately 0.97 kg/kg body mass for men and 0.84 kg/kg body mass for women. Conclusions: From a practical viewpoint, the present study suggested that the cut-off points of leg extensor power and knee extensor strength can be used as targets in simple self-reported questionnaires to help in screening for mobility in 80-year-old

Multicenter Analysis of mFOLFOX6 with Oxaliplatin Stop-and-Go Strategy Using Oral Uracil-Tegafur with Leucovorin for Unresectable Colorectal Cancer in Elderly Patients

Background: This study evaluated the tolerability and efficacy of intermittent oxaliplatin treatment based on mFOLFOX6 using oral uracil-tegafur(UFT) and leucovorin(LV) maintenance therapy in the treatment of elderly patients with advanced colorectal cancer. Methods: Ten non-elderly patients (70 years) with advanced/recurrent colorectal cancer were enrolled in this prospective, multicenter cooperative group clinical trial. The mFOLFOX6 regimen was administered for eight cycles with maintenance therapy with oral UFT/LV treatment until progression. In cases with disease progression, mFOLFOX6 was reintroduced. Results: Grade 2 peripheral neuropathy was noted in 30.0% and 25.0% of the elderly and non-elderly patients, respectively. The observed time to treatment failure (TTF) was 6.3 months in the elderly patients and 6.4 months in the non-elderly patients. The disease control rate was 83.3% in each group. Conclusion: Our new stop-and-go strategy using oral UFT/LV is well-tolerated and effective even in elderly patients.

Impact of Chylous Ascites on Colon Cancer in Laparoscopic Surgery

Objectives: To investigate the epidemiology and risk factors of chylous ascites. Methods: We identified the cases of 913 consecutive patients who underwenturgery for colorectal cancerat our University Hospital between January 2005and December2016. We divided the patients into thosewith and those without chylous ascites and compared the two groups by age, gender, body mass index, tumor location, T, N factor, stage, operation time, intra-operative bleeding, and duration of postoperative hospital stay. Results: Chylous ascites developed in 8 of the 913 patients (0.8%). Neither age, sex, tumor location, and body mass indexnor number of lymph nodeswereassociated with postoperative chylous ascites. Ascites occurred significantly less frequently in the early stage of colorectal cancer than in the late stage (p = 0.04). There was no significant difference between the groups in operative factors, including operation time and blood loss. Postoperative hospital stays were longer in patients with chylous ascites (20.5 days) than in those without (11 days) (p = 0.02). Conclusions: Late stage was one of the risk factors for chylous ascites in the present study. Other risks, such as tumor location and blood loss,will vary from institution to institution.

Short term results of endoscopic submucosal dissection in superficial esophageal squamous cell neoplasms

AIM: To evaluate the efficacy of endoscopic submucosal dissection for superficial esophageal squamous cell neoplasms. METHODS: Between July 2007 and March 2009, 27 consecutive superficial esophageal squamous cell neoplasms in 25 enrolled patients were treated by endoscopic submucosal dissection. The therapeutic efficacy, complications, and follow-up results were assessed. RESULTS: The mean size of the lesions was 21 ± 13 mm (range 2-55 mm); the mean size of the resection specimens was 32 ± 12 mm (range 10-70 mm). The enblock resection rate was 100% (27/27), and en block resection with tumor-free lateral/basal margins was 88.9% (24/27). Perforation occurred in 1 patient who was managed by conservative medical treatments. None of the patients developed local recurrence or distant metastasis in the follow-up period. CONCLUSION: Endoscopic submucosal dissection is applicable to superficial esophageal squamous cell neoplasms with promising results.

Establishing a Calculation Method for Estimating the Normal Circumference of the Proximal Interphalangeal Finger Joints

Objective: To estimate the normal circumference of the interphalangeal (IP) thumb joint and the proximal IP (PIP) joints from the index finger to the little finger using individual physical parameters such as body height and body weight. Methods: The maximum size on the ring gauge was recorded which caused resistance when the IP joint of the thumb or PIP joints of the index to small fingers were passed through the gauge. RG was defined as the ring gauge number with the same diameter as the circumference of the IP thumb joint and PIP finger joints from the index finger to the little finger. There were 300 healthy subjects and 600 hands. Univariate and multivariate analyses were used to analyze the relationships between RG and the parameters of age, sex, height, weight, and dominant hand, and regression equations were derived. Results: Age, sex and body weight were all statistically significant predictive factors of RG in all fingers. R2, the coefficient of determination in the regression equation, was almost 0.7 for all fingers, indicating that a moderate or strong correlation was observed between RG and the regression equation. Conclusions: The circumference of the IP thumb joint and PIP finger joints from the index finger to the little finger can be estimated in healthy individuals using a calculation formula that takes age, sex, body height and body weight into account.
