Long-term survival after resection of pancreatic cancer:A single-center retrospective analysis

Impact of laparoscopic liver resection on bleeding complications in patients receiving antithrombotics

To assess the impact of laparoscopic liver resection (LLR) on surgical blood loss (SBL), especially in patients with antithrombotics for thromboembolic risks. METHODSConsecutive 258 patients receiving liver resection at our institution between 2010 and 2016 were retrospectively reviewed. Preoperative antithrombotic therapy (ATT; antiplatelets and/or anticoagulation) was regularly used in 100 patients (ATT group, 38.8%) whereas not used in 158 (non-ATT group, 61.2%). Our perioperative management of high thromboembolic risk patients included maintenance of preoperative aspirin monotherapy for patients with antiplatelet therapy and bridging heparin for patients with anticoagulation. In both ATT and non-ATT groups, outcome variables of patients undergoing LLR were compared with those of patients receiving open liver resection (OLR), and the independent risk factors for increased SBL were determined by multivariate analysis. RESULTSThis series included 77 LLR and 181 OLR. There were 3 thromboembolic events (1.2%) in a whole cohort, whereas increased SBL (≥ 500 mL) and postoperative bleeding complications (BCs) occurred in 66 patients (25.6%) and 8 (3.1%), respectively. Both in the ATT and non-ATT groups, LLR was significantly related to reduced SBL and low incidence of BCs, although LLR was less performed as anatomical resection. Multivariate analysis showed that anatomical liver resection was the most significant risk factor for increased SBL [risk ratio (RR) = 6.54, P < 0.001] in the whole cohort, and LLR also had the significant negative impact (RR = 1/10.0, P < 0.001). The same effects of anatomical resection (RR = 15.77, P < 0.001) and LLR (RR = 1/5.88, P = 0.019) were observed when analyzing the patients in the ATT group. CONCLUSIONLLR using the two-surgeon technique is feasible and safely performed even in the ATT-burdened patients with thromboembolic risks. Essentials of Proteolytic Machineries in Chloroplasts

Plastids are unique organelles that can alter their structure and function in response to environmental and developmental stimuli. Chloroplasts are one type of plastid and are the sites for various metabolic pro- cesses, including photosynthesis. For optimal photosynthetic activity, the chloroplast proteome must be properly shaped and maintained through regulated proteolysis and protein quality control mechanisms. Enzymatic functions and activities are conferred by protein maturation processes involving consecutive proteolytic reactions. Protein abundances are optimized by the balanced protein synthesis and degrada- tion, which is depending on the metabolic status. Malfunctioning proteins are promptly degraded. Twenty chloroplast proteolytic machineries have been characterized to date. Specifically, processing peptidases and energy-driven processive proteases are the major players in chloroplast proteome biogenesis, remod- eling, and maintenance. Recently identified putative proteases are potential regulators of photosynthetic functions. Here we provide an updated, comprehensive overview of chloroplast protein degradation ma- chineries and discuss their importance for photosynthesis. Wherever possible, we also provide structural insights into chloroplast proteases that implement regulated proteolysis of substrate proteins/peptides.

Phenomenological aspects of possible vacua of a neutrino flavor model

We discuss a supersymmetric model with discrete flavor symmetry A4×Z3. The additional scalar fields which contribute masses of leptons in the Yukawa terms are introduced in this model. We analyze their scalar potential and find that they have various vacuum structures. We show the relations among 24 different vacua and classify them into two types. We derive expressions of the lepton mixing angles, Dirac CP violating phase and Majorana phases for the two types. The model parameters which are allowed by the experimental data of the lepton mixing angles are different for each type. We also study the constraints on the model parameters which are related to Majorana phases. The different allowed regions of the model parameters for the two types are shown numerically for a given region of two combinations of the CP violating phases.

Two cases of pancreatic ductal adenocarcinoma with intrapancreatic metastasis

There are no standardized diagnostic criteria for intrapancreatic metastasis of pancreatic ductal adenocarcinoma(PDAC). Here, we report two cases of patients with PDAC who were pathologically diagnosed as harboring intrapancreatic metastasis. In both cases, the main lesions were located in the pancreatic body, and no other lesion was detected preoperatively. The patients were diagnosed with pancreatic body cancers and distal pancreatectomy was performed. Pathological findings revealed microscopic cancer nests, which had connections to neither the main lesion nor the premalignant lesion in the pancreatic tail parenchyma. In both cases, the histological type of the daughter lesion was quite similar to that of the main lesion. Hence, we diagnosed the daughter lesions as metastatic foci in the pancreas. Although intrapancreatic metastasis of PDAC has been regarded as a poor prognostic factor, few reports of intrapancreatic metastasis are available. This article reports two such cases and provides a review of the literature.

A case of scirrhous gastric cancer with peritonitis carcinomatosa controlled by TS-1~ + paclitaxel for 36 mo after diagnosis

A 34-year-old female complaining of abdominal fullness was diagnosed as scirrhous gastric cancer （type 4＇） with peritonitis carcinomatosa in July 2002. A combined chemotherapy regimen was selected to control massive ascites; TS-1 80 mg/m^2 was given orally on d 1-14, 22-35, and paclitaxel 50 mg/m^2 was administered intravenously on d 1, 8, 22 and 29. After 2 courses of this regimen, the primary tumor was markedly reduced, and ascites completely vanished. Alopecia （grade 1, since d 30）, leukocytopenia （grade 2, on d 34） and anemia （grade 2, on d 34） were the only adverse events throughout the following courses. The chemotherapy was effective for 28 mo, and then it was discontinued upon the patient＇s own request, and she survived for 36 mo after diagnosis.

JAK3 inhibitor Ⅵ is a mutant specific inhibitor for epidermal growth factor receptor with the gatekeeper mutation T790M

AIM:To identify non-quinazoline kinase inhibitors effective against drug resistant mutants of epidermal growth factor receptor(EGFR).METHODS:A kinase inhibitor library was subjected to screening for specific inhibition pertaining to the in vitro kinase activation of EGFR with the gatekeeper mutation T790 M,which is resistant to small molecular weight tyrosine kinase inhibitors(TKIs) for EGFR in nonsmall cell lung cancers(NSCLCs). This inhibitory effect was confirmed by measuring autophosphorylation of EGFR T790M/L858 R in NCI-H1975 cells,an NSCLC cell line harboring the gatekeeper mutation. The effects of a candidate compound,Janus kinase 3(JAK3) inhibitor Ⅵ,on cell proliferation were evaluated using the MTT assay and were compared between T790M-positive and-negative lung cancer cell lines. JAK3 inhibitor Ⅵ was modeled into the ATP-binding pocket of EGFR T790M/L858 R. Potential physical interactions between the compound and kinase domains of wild-type(WT) or mutant EGFRs or JAK3 were estimated by calculating binding energy. The gatekeeper residues of EGFRs and JAKs were aligned to discuss the similarities among EGFR T790 M and JAKs. RESULTS:We found that JAK3 inhibitor Ⅵ,a known inhibitor for JAK3 tyrosine kinase,selectively inhibits EGFR T790M/L858 R,but has weaker inhibitory effects on the WT EGFR in vitro. JAK3 inhibitor Ⅵ also specifically reduced autophosphorylation of EGFR T790M/L858 R in NCI-H1975 cells upon EGF stimulation,but did not show the inhibitory effect on WT EGFR in A431 cells. Furthermore,JAK3 inhibitor Ⅵ suppressed the proliferationof NCI-H1975 cells,but showed limited inhibitory effects on the WT EGFR-expressing cell lines A431 and A549.A docking simulation between JAK3 inhibitor Ⅵ and the ATP-binding pocket of EGFR T790M/L858 R predicted a potential binding status with hydrogen bonds. High-Speed Time-Domain En Face Optical Coherence Tomography System Using KTN Optical Beam Deflector

We developed high-speed time-domain (TD) en face optical coherence tomography (OCT) system using KTN optical beam deflector. The KTN optical beam deflector operates at a high repetition rate of 200 kHz with a fairly large beam deflection angle. We proposed a high-speed en face OCT system that used a KTN optical deflector as the sample beam scanning. In the experiment, we obtained en face OCT images of human fingerprint with a frame rate of 800 fps, which is the fastest speed obtained by a TD-OCT imaging. Furthermore, a 3D-OCT image was also obtained at 0.2 s (=5 volumes/s) by our imaging system.

Abstract： This paper presents a review of a beamed energy propulsion rocket, the Microwave Rocket, WlllCn proauces propmslve thrust from millimeter-wave beams transferred from the ground. The thrust is generated through millimeter-wave discharge driven in a cylindrical thruster. As a high-power millimeter-wave generator, a Gyrotron is promising as the beam source. The salient benefit of Microwave Rockets is the resultant drastic cost reduction of mass transportation into space. We have already conducted launch experiments and have achieved continuous thrust generation under multi-pulse operation. Recently, a long-distance beam transfer system has been developed. Ignition tests have been conducted. The physics of the millimeter-wave discharge remain unclear. Additional studies using exoerimentation and calculations must be conducted to optimize the thrust generation.
