Dear Editor, Burned-out tumors of the testis are extremely rare form oftesticular malignancies, and they could disappear or regress spontaneously and completely. These tumors could be presented by metastatic form in retroperitoneum, lungs, retroperitoneal lymph nodes, and liver. In recent studies, differential diagnosis, clinic features, treatment modalities and follow-up outcome of burned-out tumor from primary retroperitoneal germ cell tumor are discussed. If patient with retroperitoneal lymph node involvement and histology of ＂testicular tumor＂ burned-out tumor of the testis was suspected.

Dear Editor, We would like to present a rare case ofpolyorchidism. Polyorchidism is an unusual abnormality of the genital tract and until date, there are approximately 178 case reports of polyorchidism in the literature？ However, the most of patients are asymptomatic and usually diagnosed incidentally. Some may present as inguinal hernia （30%）, maldescended testis （15% to 30%）, testicular torsion （13%） and hydrocele （9%）, varicocele （1%）, hypospadias （〈1%）, anomalous urogenital union （〈1%）, and malignancy （〈1%）.2 Management of polyorchidism is still controversial. Important factors of polyorchidism management are testis location, size and anatomical organization of the testicular drainage system and the age of the patient.

We compare the efficacy of intratesticular ozone therapy with intraperitoneal ozone therapy in an experimental rat model. For this purpose, 24 rats were divided into four groups including sham-operated, torsion/detorsion, torsion/detorsion plus intraperitoneal ozone （O-IP）, and torsion/detorsion plus intratesticular ozone （O-IT）. The O-IP ozone group received a 4 mg kg-1 intraperitoneal injection of ozone, and the O-IT group received the same injection epididymally. At 4 h after detorsion, the rats were sacrificed and orchiectomy materials were assessed histopathologically. Spermatogenesis in the seminiferous tubules and damage to the Sertoli cells were histopathologically evaluated in the testes using the Johnsen scoring system, i-NOS and e-NOS activities in the testis tissue were also evaluated. Torsion-detorsion caused a decreased Johnsen score and increased apoptosis of spermatogonial and Sertoli cells. Ozone injection prevented increases in Johnsen score and i-NOS level, e-NOS level of the O-IP group was significantly lower than that of the O-IP group, and i-NOS level of the O-IT group was significantly lower than that of the O-IP group. Local ozone therapy is more effective than systemic ozone therapy at improving IRI-related testicular torsion. Our study is the first to show that the efficacy of intratesticular implementation of ozone therapy is higher than that of intraperitoneal ozone therapy.

Caroli's disease is a rare congenital condition chara- cterized by non-obstructive saccular or fusiform dilatation of larger intrahepatic bile ducts. Cholangitis, liver cirrhosis, and cholangiocarcinoma are its potential complications. The diagnosis of Caroli's disease depends on demonstrating that the cystic lesions are in continuity with the biliary tree which can be showed by ultrasonography, computerized tomography, endoscopic retrograde cholangiopancreatography, percutaneous transhepatic cholangiography or magnetic resonance cholangiopancreatography. Treatment of Caroli's disease relies on the location of the biliary abnormalities. While localized forms confined to one lobe can be treated with surgery, liver transplantation is the only effective modality for diffuse forms. Although a rare disorder; Caroli's disease should always be considered in the differential diagnosis of chronic cholestasis of unknown cause.

Congenital hepatic fibrosis (CHF) is an autosomal recessive disorder that belongs to the family of fibropolycystic liver diseases. This family includes a spectrum of disorders which are usually found in combination with each other and are usually inherited. Clinically fibropolycystic diseases have three effects being present in different proportions, those of a space occupying lesion, of portal hypertension and of cholangitis. In most patients, the first manifestations of CHF are signs and symptoms related to portal hypertension such as splenomegaly and varices. Portal hypertension in these patients has been attributed to the hypoplasia or compression of the portal vein radicles in the fibrous bands. Cavernous transformation of the portal vein (CTPV) is a relatively rare condition resulting from extrahepatic portal vein obstruction with recanalization or collateral vein formation to bypass the obstruction. It has been found that patients with CHF having an accompanying CTPV have relatively large splenomegaly and suffers more frequent episodes of bleeding from esophageal *** believe that CTPV is a congenital component of CHF and also one of the important causative factors of portal hypertension in these patients.

Caroli's syndrome is characterized by multiple segmental cystic or saccular dilatations of intrahepatic bile ducts associated with congenital hepatic fibrosis. The clinical features of this syndrome reflect both the characteristics of congenital hepatic fibrosis such as portal hypertension and that of Caroli's disease named as recurrent cholangitis and cholelithiasis. The diagnosis depends on both histology and imaging methods which can show the communication between the sacculi and the bile ducts. Treatment consists of symptomatic treatment of cholangitis attacks by antibiotics, some endoscopic, radiological and surgical drainage procedures and surgery. Liver transplantation seems the ultimate treatment for this disease. Prognosis is fairly good unless recurrent cholangitis and renal failure develops.

Congenital hepatic fibrosis is part of many different malformation syndromes, of which oculo-encephalohepato-renal syndrome is the most common. These syndromes largely overlap, and so accurate classification of individual patients may be difficult. We present herein three syndromic siblings who were products of a consanguineous marriage. We investigated in detail at least six organ systems in these patients, namely the liver, brain, eye, kidneys, skeleton, and gonads. The common features observed in these three cases were congenital hepatic fibrosis, retinitis pigmentosa, truncal obesity, rotatory nystagmus, mental retardation, advanced myopia, and high-arched palate. The clinical dysmorphology in these patients was distinct and lacked the major features of the known syndromes associated with congenital hepatic fibrosis. Although some features of these presented cases are similar to those found in Bardet-Biedl syndrome(BBS), the absence of some major criteria of BBS(polydactyly, renal abnormality, and hypogonadism) suggests that this may be a new syndrome. All three patients remain under follow-up in the departments of Gastroenterology, Ophthalmology, and Neurology at Hacettepe University.

The plausibility of human exposure to particulate matter(PM)has witnessed an increase within the last several *** of different sizes has been discovered in the atmosphere given the role of dust transport in weather and climate *** a regulator,the lung epithelium orchestrates the innate response to local ***,we developed a lung epithelium-ona-chip platform consisting of easily moldable polydimethylsiloxane layers along with a thin,flexible,and transparent ionic liquid-based poly(hydroxyethyl)methacrylate gel *** epithelium was formed through the culture of human lung epithelial cells(Calu-3)on this *** mechanical stress at the air–liquid interface during inhalation/exhalation was recapitulated using an Arduino-based servo motor system,which applied a uniaxial tensile strength from the two sides of the chip with 10%strain and a frequency of 0.2 ***,the administration of silica nanoparticles(PM0.5)with an average size of 463 nm to the on-chip platform under static,dynamic,and dynamic+mechanical stress(DMS)conditions demonstrated the effect of environmental pollutants on lung *** viability and release of lactate dehydrogenase were determined along with proinflammatory response through the quantification of tumor necrosis factor-α,which indicated alterations in the epithelium.

It is a well-established fact that wireless sensor networks (WSNs) are very power constraint networks, but besides this, they are inherently more fault-prone than any other type of wireless network and their protocol design is very application specific. Major reasons for the faults are the unpredictable wireless communication channel, battery depletion, as well as fragility and mobility of the nodes. Furthermore, as traditional protocol design methods have proved inadequate, the cross-layer design (CLD) approach, which allows for interactions between different layers, providing more flexible and energy-efficient functionality, has emerged as a viable solution for WSNs. In this study we define a fault tolerance management module suitable to the requirements, limitations, and specifics of WSNs, encompassing methods for fault detection, fault prevention, fault management, and recovery. The suggested solution is in line with the CLD approach, which is an important factor in increasing the network performance. Through simulations the functionality of the network is evaluated, based on packet loss, delay, and energy consumption, and is compared with a similar solution not including fault management. The results achieved support the idea that the introduction of a unified approach to fault management improves the network performance as a whole.
