Objective Frequent premature ventricular contractions(FPVCs) in a structurally normal heart generally are considered as a benign ***,whether FPVCs have an effect on the cardiac structure and function in those "seemingly-normal" patients is not *** study is was to evaluate whether radiofrequency ablation(RFA) hads any effect on cardiac structure and function in patients with idiopathic FPVCs originating from the right-ventricular outflow tract(RVOT) and with a structurally normal heart. Methods Sixty-eight patients with idiopathic RVOT FPVCs and normal heart structure and function,were enrolled in this *** patients were divided into three groups:RFA group, anti-arrhythmia medicine(AAM) group and control group(without any treatment). Results The basic conditions including age,gender and PVCs history,were comparable among the three groups,except in the control group ’which had lower FPVC *** were closely followed-up for six *** PVC rate was significantly reduced in RFA and AAM groups,but had no significant change in the control *** with the increase in the FPVC rate was significant cardiac structure *** sizes of right atrium,right ventricle,and left ventricle were reduced significantly in the RFA and the AAM groups,but no changes were found in the control *** comparisons between the RFA and the AAM groups showed a greater reduction in the cardiac cavities in the RFA *** was also a tendency of improvement in the cardiac functions in both the RFA and the AAM ***,the differences was not statistical significance. Conclusions RFA reduces the FPVC rate and cardiac cavities in patients with a structurally normal heart,which might indicate that RFA can reverse the cardiac remodeling even in me structurally normal heart.

目的探讨高血压伴或不伴2型糖尿病患者血压晨峰的患病特点及其与冠脉病变严重程度的相关性。方法选择2014年1月至2015年8月我科住院的高血压患者244例,其中男性118例,女性126例,年龄(64.7±11.5)岁,根据是否合并糖尿病及其动态血压监测结果将其分为4组:糖尿病晨峰组(D-S,36例)、糖尿病非晨峰组(D-n S,56例)、非糖尿病晨峰组(n D-S,72例)、非糖尿病非晨峰组(n D-n S,80例),入选的244例患者全部进行冠状动脉造影检查,分别比较4组患者冠状动脉病变程度。结果糖尿病晨峰组、糖尿病非晨峰组和非糖尿病晨峰组24 h平均收缩压、白天平均收缩压均高于非糖尿病非晨峰组(P<0.05);糖尿病晨峰组24 h平均舒张压、白天平均舒张压高于糖尿病非晨峰组、非糖尿病晨峰组和非糖尿病非晨峰组(P<0.05);晨峰组三支病变率、C型病变率及晨峰组Gensini总积分显著高于非晨峰组(P<0.01);Pearson相关分析显示,冠状动脉病变严重程度与年龄(r=0.786,P<0.01)、BMI(r=0.284,P<0.05)、空腹血糖(r=0.712,P<0.05)、LDL-C(r=0.765,P<0.05)、晨峰程度(r=0.852,P<0.01)及24 h MSBP(r=0.804,P<0.01)呈正相关;多元线性回归分析显示,年龄、空腹血糖、24 h平均收缩压及血压晨峰为冠状动脉病变严重程度独立危险因素。结论原发性高血压合并糖尿病患者血压晨峰是预测冠状动脉病变严重程度的独立危险因素,有效控制该类患者的晨峰血压及24 h长效平稳的降压可减少对靶器官的损害,降低心血管事件发生。

目的探索盐阈值与外周动脉粥样硬化的相关性,为预防外周动脉粥样硬化发生、发展提供科学依据。方法采用横断面研究设计,选取2017年12月至2019年1月陆军军医大学大坪医院心血管内科住院的2079例患者为研究对象,应用梯度浓度的NaCl溶液测定患者的盐阈值(测试实施者遵循盲法),而后进行双侧颈动脉和双侧股动脉超声检查,根据是否存在外周动脉粥样斑块分为外周动脉粥样斑块组和无外周动脉粥样斑块组,比较两组盐阈值并分析盐阈值与外周动脉粥样硬化的关系。结果外周动脉粥样斑块组盐阈值明显高于无外周动脉粥样斑块组[(0.07±0.04)vs.(0.05±0.04),P<0.05]。多因素logistic回归模型在控制外周动脉粥样硬化的传统危险因素后,发现盐阈值不独立增加外周动脉粥样斑块发生的风险[(OR=0.919,95%CI:0.477~1.774),P=0.802]。结论外周动脉粥样斑块患者的盐阈值较高,但盐阈值升高不是发生外周动脉粥样硬化的独立危险因素。
