Background:Few studies have examined the associations between urban design attributes and older adults’physical function.Especially,it is not well known how built-environment attributes may influence physical function in Asian cities.The aim of this study was to examine associations between objectively measured environmental attributes of walkability and objectively assessed physical function in a sample of Japanese older adults.Methods:Cross-sectional data collected in 2013 from 314 older residents(aged 65-84 years)living in Japan were used.Physical function was estimated from objectively measured upper-and lower-body function,mobility,and balance by a trained research team member.A comprehensive list of built-environment attributes,including population density,availability of destinations,intersection density,and distance to the nearest public transport station,were objectively calculated.Walk Score as a composite measure of neighborhood walkability was also obtained.Results:Among men,higher population density,availability of destinations,and intersection density were significantly associated with better physical function performance(1-legged stance with eyes open).Higher Walk Score was also marginally associated with better physical function performance(1-legged stance with eyes open).None of the environmental attributes were associated with physical function in elderly women.Conclusion:Our findings indicate that environmental attributes of walkability are associated with the physical function of elderly men in the context of Asia.Walking-friendly neighborhoods can not only promote older adults’active behaviors but can also support their physical function.

In recent years, Taiwan has enhanced the manufacturing advantages in the competitive global market because of excellent manufacturing processes, product quality, and the flexible delivery date and speed. However, how to maintain the competitive advantages in the global market has become an issue. Due the advanced development of the Internet, the cloud computing technology has significant progress and become more matured. Therefore, if the cloud computing technology can be applied in the supply chain of the manufacturing industry, the information among manufacturing, assembly, logistics, sales, and customers can be shared and accessed easily. In this study, a cloud-based supply chain integration service platform of the manufacturing industry is developed, and this platform includes the supplier management, procurement and materials management, manufacturing management, facilities planning, customer service, transportation and physical distribution, and information flow to form a supply chain integration service cloud. Thus, the members of the whole supply chain integration service cloud are able to work together, and rapidly deploy and dynamically allocate resources, and the computer hardware and software resources of the upstream and downstream can be appropriately shared. By doing so, the inventory fluctuations caused by the bullwhip effect can also be reduced. Eventually, it will achieve the goals of shortening delivery time, raising production capacity utilization, and decreasing the material preparation time.

Gallstone ileus:Report of two cases and review of the literature

Gallstone ileus is a rare disease and accounts for 1%-4% of all cases of mechanical intestinal obstruction. It usually occurs in the elderly with a female predominance and may result in a high mortality rate. Its diagnosis is difficult and early diagnosis could reduce the mortality. Surgery remains the mainstay of treatment. We report two cases of gallstone ileus. The first was a 78-year old woman who had a 2-d history of vomiting and epigastralgia. Plain abdominal film suggested small bowel obstruction clinically attributed to adhesions. Later on, gallstone ileus was diagnosed by abdominal computed tomography (CT) based on the presence of pneumobilia, bowel obstruction, and an ectopic stone within the jejunum. She underwent emergent laparotomy with a one-stage procedure of enterolithotomy, cholecystectomy and fistula repair. The second case was a 76-year old man with a 1-wk history of epigastralgia. Plain abdominal film showed two round calcified stones in the right upper quadrant. Fistulography confirmed the presence of a cholecystoduodenal fistula and gallstone ileus was also diagnosed by abdominal CT. We attempted to remove the stones endoscopically, but failed leading to an emergent laparotomy and the same one-stage procedure as for the first case. The postoperative courses of the two cases were uneventful. Inspired by these 2 cases we reviewed the literature on the cause, diagnosis and treatment of gallstone ileus.

重组SARS冠状病毒棘突蛋白S1区基因片段的克隆、表达及免疫原性研究

目的构建SARS冠状病毒(SARS-CoV)棘突(spike)蛋白(S蛋白)S1区基因(40～2001bp)分段表达载体,并研究其免疫原性。方法利用PCR技术扩增S1区基因,将其定向插入pQE30质粒,在pQE30质粒上将S1切成3段(40～751、746～1344、746～2001bp),均插入pQE30质粒,构建质粒在大肠埃希菌M15中表达,表达产物经亲和层析纯化及WesternBlot和ELISA鉴定。结果构建的3种重组质粒(pQE30/S1a、pQE30/S1b、pQE30/S1c)均高效表达,重组蛋白相对分子质量分别为26700、22500和46000,表达量分别占菌体总蛋白质的35%、35%和30%。3种重组蛋白经Ni亲和层析树脂得到成功纯化。经Western-Blot及ELISA证实,重组蛋白片段S1c(746～2001bp)可以被SARS患者血清所识别。结论成功构建了SARS-CoVS蛋白S1区基因分段表达载体,重组蛋白S1c具有较强的免疫原性,它的获得为下一步疫苗的研制奠定了基础。

Acute kidney injury (AKI) is one of the popular topics of discussions due to increasing development of biomarkers recently. The disease progression and prognosis may be determined by these biomarkers detected in blood and urine specimens. Since acute kidney injury is associated with a broad spectrum of disease conditions, prevention and early detection of AKI becomes very important in those clinical settings. Early measurements of AKI biomarkers predict subsequent development of intrinsic AKI, dialysis requirement, duration of intensive care unit stay and finally affect mortality. We, here, discuss the acute kidney injury in different clinical situations and associated natures of biomarkers, which may help us guide to prevent and treat AKI more effectively.

目的制备和鉴定禽流感病毒（H5N1）血凝素（H5）特异性单克隆抗体（mAb）,建立H5抗原的双抗体夹心ELISA捕获法。方法以H5血凝素和携带H5全长基因的质粒免疫Balb/c小鼠制备mAb,利用血凝抑制（HI）实验筛选和鉴定,通过竞争抑制试验分析抗体识别表位,并采用抗体配对试验筛选捕获抗体和检测抗体,建立测定H5抗原的双抗体夹心ELISA捕获法。结果获得16株特异性针对H5的单克隆抗体,与A型流感病毒H1、H3、H7、H9和B型流感病毒的血凝素无HI交叉反应,对H5血凝素的血凝抑制效价为1∶100-1∶51200;通过配对实验,建立以单克隆抗体H5M9为捕获抗体,辣根过氧化物酶标记单克隆抗体H5M11为检测抗体的双抗体夹心ELISA;检测多株H5N1病毒和H5血凝素的最低检出值为1/32血凝单位,检测A型流感病毒H1N1、H3N2、B型流感病毒以及H7、H9血凝素均为阴性。结论建立了一种灵敏度高、特异性强的测定H5抗原的ELISA捕获法,可应用于禽流感病毒H5N1感染的实验室早期诊断。

目的通过实行行动研究方法,从健康促进学校6大范畴介入,以改善学生身体质量指数（BMI）、体重控制行为及运动时间。方法使用问卷调查,以本校1~6年级学生为对象,先进行前测,然后进行介入,期间配合学校的体能活动与每天20min课间活动,进行健康操、跳绳和跑步等,并给予饮食营养教育。倡导（含家长）、督促饮用白开水每天2次,每天3蔬2果,共开展活动3个月,每天进行体重检测,每周进行身高检测,施行自我及饮食内容评估。结果通过6大范畴介入,学生BMI正常占比由58.9%提升至66.1%。总体而言学生正常BMI占比、运动的天数与时间有显著提升。结论经由学校推动全方位健康体重活动各项策略及定期测量身高、体重,有助于学生对体重的控制。
