Clinical Characteristics and Current Interventions in Shock Patients in Chinese Emergency Departments： A Multicenter Prospective Cohort Study

Background： Shock is a life-threatening condition in emergency departments （EDs） and is associated with a high mortality： however, its clinical characteristics and current interventions in China are seldom reported. This study investigated the clinical characteristics and current interventions of shock patients in Chinese EDs. Methods： This multicenter prospective cohort study was conducted in the EDs of 33 academic hospitals in 16 Chinese provinces. Adult shock patients were enrolled from December 2013 to April 2014. Age, sex, comorbidities, shock subtype, and vital signs were recorded on ED an＇ival; details of subsequent interventions and treatments were added. We compared those data between survivors and nonstu＇vivors. All patients were lbllowed tip for 3 days. The primary outcome was 3-day mortality. Binary logistic regression analysis identified the independent predictors of that mortality. Results： We enrolled 1095 shock patients. The 3-day mortality was 27.5%, 36.3%, and 29.0%, respectively, in the whole cohort and for cardiogenic and septic shock. Within the first 24 11, 1039 patients （94.9%） were admitted to the Intensive Care Unit. Use of bicarbonate, epinephrine, and dopamine is an independent predictor lbr mortality. Hemorrhage and trauma （39.1%）, along with sepsis （40.4%） were the most commonly observed causes of shock in the ED. In nondiabetic patients with cardiogenic shock, 3-day mortality was 69.2% for patients needing glucose control -- much higher than in those not needing glucose control （30.0%, P- 0.01 ）. Hydroxyethyl starch （HES） was applied in 29.6% of septic shock patients, and the mortality of septic patients who received HES was much higher than those who did not （38.2% vs. 25. 1%, P = 0.006）. Conclusions： In China, short-term mortality of shock patients in EDs is still high, especially among those with cardiogenic and septic shock. HES application needs to be restricted -- particularly in septic shock patients.

Background： The quick Sequential Organ Failure Assessment （qSOFA） score emerged recently. We investigated its contribution to risk stratification in acute pulmonary embolism （PE） by combining with electrocardiography （ECG）. Methods： Acute PE patients diagnosed in Beijing Chao-Yang Hospital, Capital Medical University, from 2008 to 2018 were retrospectively studied and divided into high- and low-risk groups by imaging and biomarkers. The ECG scores consisted oftachycardia, McGinn-White sign （S1Q3T3）, right bundle branch block, and T-wave inversion of leads V1-V3. A new combination of qSOFA scores and ECG scores by logistic regression for predicting high-risk stratification patients with acute PE was evaluated by a receiver operating characteristic curve. Results： Totally 1318 patients were enrolled, including 271 in the high-risk group and 1047 in the low-risk group. A combination predictive scoring system named qSOFA-ECG = qSOFA score ＋ ECG score was created. The optimal cutoffvalue for qSOFA-ECG was 2, and the sensitivity, specificity, positive predictive value, and negative predictive value were 81.5%, 72.3%, 43.2%, and 93.8%, respectively. For predicting high-risk stratification and reperfusion therapy, the qSOFA-ECG is superior to PE Severity Index （PESI） and simplified PESI. Conclusions： The qSOFA score contributes to identify acute PE patients with potentially hemodynamic decompensation that need monitoring and possible reperfusion therapy at the emergency department arrival when used in combination with ECG score.

Cases of psoriasis complicated with venous thromboembolism are rarely reported. Here, we report two cases and review the current literature on the subject. Two patients with long-standing severe psoriasis presented with chest pain,shortness of breath and breathing difficulties. The patients were diagnosed using lung ventilation-perfusion scans or computed tomographic pulmonary angiography. Anticoagulation or thrombolytic therapy was initiated, and long-term continuous anticoagulation with warfarin prevented any recurrences.

采用平凹多通腔和固体薄片组的非线性脉冲光谱展宽与压缩方案开展了100 W皮秒激光非线性脉冲压缩的研究.以多片熔融石英薄片作为非线性介质,在平凹腔中皮秒激光通过自相位调制将光谱宽度由0.24 nm展宽4.8 nm,用光栅对进行色散补偿压缩,实现压缩后的脉冲宽度为483 fs,对应压缩比为22,最终输出飞秒激光的平均功率为44.2 W.相对于常规多通腔方案,该平凹腔结构紧凑,光路稳定性好,压缩比高,非常有利于非线性光谱展宽与压缩的实现.

To the Editor： With the growing importance of Sepsis-related Organ Failure Assessment （SOFA）,t^l clinicians are increasingly concerned about its reliability and accuracy. In Intensive Care Units, SOFA scoring was found to be insufficiently accurate among both physicians and nurses.I-＇31 Few studies have reported the pertbnnance of emergency department （ED） physicians in manually assigning the SOFA score. We therefore conducted a single-center prospective investigation to examine that issue.

To investigate whether masticatory fatigue affects the fracture resistance and pattern of lower premolars restored with quartz-fiber post-core and full crown, 44 single rooted lower premolars recently extracted from orthodontic patients were divided into two groups of 22 each. The crowns of all teeth were removed and endodontically treated and then restored with quartz-fiber post-core and full crown. Twenty-two teeth in one group were selected randomly and circularly loaded at 45° to the long axis of the teeth of 127.4 N at a 6 Hz frequency, and the other group was not delivered to cyclic loading and considered as control. Subsequently, all teeth in two groups were continually loaded to fail at 45° to the long axis of the teeth at a crosshead speed of 1 ***-1. The mean destructive force values were （733.88±254.99） and （869.14±280.26） N for the experimental and the control group, respectively, and no statistically significant differences were found between two groups （P〉O.05）. Bevel fracture and horizontal fracture in the neck of root were the major fracture mode of the specimens. Under the circumstances of this study, it seems that cyclic loading does not affect the fracture strength and pattern of the quartz-fiber post-core-crown complex.

Two-stage Planning Method for Energy Router Considering Its Optimal Operation in Distribution Networks

An energy router can effectively optimize the network loss and power quality of distribution networks. The optimal configuration of the energy router is relatively complex because issues dynamically influence each other, such as the location of the energy router, optimal number and capacity of its ports, and building new distribution lines for the ports. Presently, there has been very few research studies on the practical problems for the energy router. In this paper, a planning model of an energy router combined with the distribution network is established, which fully exploits the active and reactive power control abilities of the energy router to optimize the operation of the grid. The configuration problem is decoupled into two stages. The upper layer determines the location of the energy router and the parameters of the candidate new distribution lines for the ports. The lower layer calculates the optimal configuration of the energy router by minimizing the total annual cost. Because of the same rated bus voltage in the distribution network, the existing structure of the energy router is changed to be more applicable for the distribution network, and then the power flow model of the energy router is also modified. Due to the nonlinearity of the model, the planning model is converted into a mixed-integer second-order cone model to solve efficiently. In addition, some core factors influencing the optimal scheme of the energy router are also analyzed in this paper. Simulation results show that the optimal scheme of the energy router can significantly improve the economic deployment of the energy router, and optimize the network loss and power quality of the distribution network.

As a new concept,mass-entransy is one of the twins in the core of entransy *** can describe mass-transfer ability for masstransfer processes(MTPes),just as thermal-entransy for describing heat-transfer ***,mass-entransy dissipation can be utilized to evaluate the loss of mass-transfer *** mass-entransy dissipation(MMED) is utilized to optimize one-way isothermal diffusive MTPes with mass-leakage and mass-transfer law(g ∝ Δ(c),where c means concentration).For a given net amount of mass-transferred key components at the low-concentration side,optimality-condition for the MMED of isothermal diffusive MTPes is obtained by using the *** of the amount of mass-transferred and mass-leakage on optimal results are analyzed,and the obtained optimization profiles are compared with those for MTP profiles of constant-concentration-difference(c_1-c_2=const) and constant-concentration-ratio(c_1/c_2=const).The product of square of key-component-concentration(KCC) difference between high-and low-concentration sides and inert component concentration at high-concentration side for the MMED of the MTP with no mass-leakage is a constant,and the optimal relationship of the KCCs between high-and low-concentration sides with mass-leakage is significantly different from the *** mass-leakage is relatively small,the MTP with c_1-c_2=const strategy is superior to that with c_1/c_2=const strategy,and the latter is superior to the former with an increase in mass-leakage.A combination of mass-entransy concept,finite-time thermodynamics,and averaged-optimization-method is a meaningful tool for optimizing MTPes.

A high proportion of renewable energy affects the power quality of distribution networks,and surplus energy will be sold to the upstream grid at a low *** this paper,considering peer-to-peer energy transactions,the energy router-based multiple distribution networks are analyzed to solve the above problems and realize collaborative consumption of renewable ***,the investing cost of an energy router is high,and research on the economic operation of energy routers in distribution networks is ***,this paper establishes a planning model for energy routers considering peer-to-peer energy transactions among distribution networks,and explores the benefits of peer-to-peer energy transactions through energy router based multiple distribution networks.A structure of an energy router suitable for peer-to-peer energy transactions is selected,and a power flow calculation model based on a multilayer structure is *** energy router’s scheduling model is established,and unique functions of the energy router and revenue of each distribution network are considered.A power flow calculation model based on peer-to-peer interconnection of multiple distribution networks through energy routers is also ***,simulation results verify the effectiveness of the proposed planning *** show that peer-topeer energy transaction among distribution networks through energy routers can effectively reduce the comprehensive cost of distribution networks,significantly improve the power quality of the distribution networks,and reduce the impact of power fluctuation on the upstream grid incurred by the distribution network.
