Paraaortic lymph node metastasis in patients with intra-abdominal malignancies:CT vs PET

AIM： To compare the diagnostic accuracy of computed tomography （CT） and positron emission tomography （PET） for the preoperative detection of paraaortic lymph node （PAN） metastasis in patients with intra- abdominal malignancies. METHODS： Sixty-six patients with intra-abdominal malignancies who underwent both CT and PET before lymphadenectomy were included in this study. Histopathologicaliy, 13 patients had metastatic PAN, while 53 had non-metastatic PAN. The CT criteria for metastasis were： short diameter ：）f 〉 8 mm, Iobular or irregular shape, and/or combined ancillary findings, including necrosis, conglomeration, vessel encasement, and infiltration. The PET criterion was positive fluorodeoxyglucose （FDG） uptake. The sensitivity, specificity, positive predictive value （PPV）, negative predictive value （NPV）, and accuracy of both modalities were compared with the pathologic findings, and the false positive and false negative cases with both CT and PET were ***： The sensitivity, specificity, PPV, NPV, and accuracy of CT were 61.5%, 84.9%, 50%, 90% and 80.3%, respectively. For PET, the percentages were 46.2%, 100%, 100%, 88.3%, and 89.4%. Additionally, there were 8 false positive CT cases （8/53, 15.1%） and zero false positive PET cases. Of the 13 metastatic PANs, there were 5 false negative CT scans （38.5%） and 7 （53.9%） false negative PET ***： For detecting PAN metastasis, CT is more sensitive than PET, while PET is more specific.

羊场超大型磷矿床是新近勘查发现的大埋深全隐伏浅海相碳酸盐岩型层状磷块岩矿床,赋矿地层为下寒武统梅树村组(?1ms),目前对其沉积环境和富集成矿机制认识薄弱。本文在代表性钻孔岩芯系统观测的基础上,利用沉积(微)相分析、矿物学及矿物化学示踪等手段,重点解析梅树村组各岩性段的岩石矿物(组合)特征、沉积(微)相类型及其时空分布规律,查明沉积环境演化,探究磷质来源、迁移及富集机制,建立其成矿模式。对比分析表明,赋矿地层垂向上表现出海侵-海退序列的近岸潮坪相沉积环境,识别出潮上坪、潮间坪和潮下坪3个亚相6种微相,潮上坪分为潮上滩、潮上灰坪,潮间坪分为潮间灰坪、潮间滩、风暴流,潮下坪发育低能潮下坪;潮间坪中的高能潮间滩是最有利的磷块岩形成环境,长期保持的浅水环境下多期次潮汐、波浪反复作用,利于富磷粒屑的簸选富集与再造,从而形成矿区北西侧矿层富厚、南东侧渐趋薄贫的磷块岩矿床。由此,羊场磷矿床的成矿作用可分为3个阶段:初始堆积沉淀成矿阶段(上升洋流携带的磷质来源)、生物化学凝聚沉淀与加积异化成矿阶段(微生物分解、转化及复合作用驱动的迁移与富集成矿)、簸选富集与再富集成矿阶段(多期次波浪、潮汐作用驱动的沉淀再次富集成矿)。这为矿区及外围地区找矿勘查提供了重要依据。

Delayed gait recovery in a stroke patient

We report on a stroke patient who showed delayed gait recovery between 8 and 11 months after the onset of intracerebral hemorrhage. This 32-year-old female patient underwent craniotomy and drainage for right intracerebral hemorrhage due to rupture of an arteriovenous malformation. Brain MR images revealed a large leukomalactic lesion in the right fronto-parietal cortex. Diffusion tensor tractography at 8 months after onset revealed that the right corticospinal tract was severely injured. At this time, the patient could not stand or walk despite undergoing rehabilitation from 2 months after onset. It was believed that severe spasticity of the left leg and right ankle was largely responsible, and thus, antispastic drugs, antispastic procedures （alcohol neurolysis of the motor branch of the tibial nerve and an intramuscular alcohol wash of both tibialis posterior muscles） and physical therapy were tried to control the spasticity. These measures relieved the severe spasticity, with the result that the patient was able to stand at 3 months. In addition, the improvements in sensorimotor function, visuospatial function, and cognition also seemed to contribute to gait recovery. As a result, she gained the ability to walk independently on even floor with a left ankle foot orthosis at 11 months after onset. This case illustrates that clinicians should attempt to find the cause of gait inability and to initiate intensive rehabilitation in stroke patients who cannot walk at 3-6 months after onset.

Unified deep learning model for El Niño/Southern Oscillation forecasts by incorporating seasonality in climate data

Although deep learning has achieved a milestone in forecasting the El Niño-Southern Oscillation(ENSO),the current models are insufficient to simulate diverse characteristics of the ENSO,which depends on the calendar ***,a model was generated for specific seasons which indicates these models did not consider physical constraints between different target seasons and forecast lead times,thereby leading to arbitrary fluctuations in the predicted time *** overcome this problem and account for ENSO seasonality,we developed an all-season convolutional neural network(A_CNN)*** correlation skill of the ENSO index was particularly improved for forecasts of the boreal spring,which is the most challenging season to ***,activation map values indicated a clear time evolution with increasing forecast lead *** study findings reveal the comprehensive role of various climate precursors of ENSO events that act differently over time,thus indicating the potential of the A_CNN model as a diagnostic tool.

Translocator protein ligand, YL-IPA08, attenuates lipopolysaccharide-induced depression-like behavior by promoting neural regeneration

Translocator protein has received attention for its involvement in the pathogenesis of depression. This study assessed the effects of the new translocator protein ligand, YL-IPA08, on alleviating inflammation-induced depression-like behavior in mice and investigated its mechanism of action. mice were intracerebroventricularly injected with 1, 10, 100 or 1000 ng lipopolysaccharide. The tail-suspension test and the forced swimming test confirmed that 100 ng lipopolysaccharide induced depression-like behavior. A mouse model was then established by intraventricular injection of 100 ng lipopolysaccharide. On days 16-24 after model establishment, mice were intragastrically administered 3 mg/kg YL-IPA08 daily. Immunohistochemistry was used to determine BrdU and NeuN expression in the hippocampus. YL-IPA08 effectively reversed the depression-like behavior of lipopolysaccharide-treated mice, restored body mass, increased the number of BrdU-positive cells, and the number and proportion of BrdU and NeuN double-positive cells. These findings indicate that YL-IPA08 can attenuate lipopolysaccharide-induced depression-like behavior in mice by promoting the formation of hippocampal neurons.

Hepatocellular carcinoma(HCC) is one of major causes of cancer mortality worldwide. For decades, ^(18)F-fluorodeoxyglucose(FDG) positron emission tomography(PET) has been widely used for staging, predicting prognosis, and detecting cancer recurrence in various types of malignant diseases. Due to low sensitivity of FDG PET for detecting intrahepatic HCC lesions, the clinical value of FDG PET in HCC patients has been limited. However, recent studies with diverse analytic methods have shown that FDG PET has promising role in aiding management of HCC patients. In this review, we will discuss the clinical role of FDG PET for staging, predicting prognosis, and evaluating treatment response in HCC. Further, we will focus on recent clinical studies regarding implication of volumetric FDG PET parameters, the significance of FDG uptake in HCC for selecting treatment and predicting treatment response, and the use of radiomics of FDG PET in HCC.

A critical regulation of Th2 cell responses by RORa in allergic asthma

Allergic asthma is a chronic inflammatory disease of the lung and the airway,which is characterized by aberrant type 2 immune responses to otherwise unharmful *** the central role of Th2 cells and type 2 cytokines in the pathogenesis of allergic asthma is well documented,the regulation and plasticity of Th2 cells remain incompletely *** using an animal model of allergic asthma in IL-4-reporter mice,we found that Th2 cells in the lung expressed higher levels of Rora than those in the lymph nodes,and that treatment with an RORa agonist SRI 078 resulted in diminished Th2 cell responses in *** determine the T cell-intrinsic role of RORa in allergic asthma in vivo,we established T cell-specific RORa-deficient^(f/f)fyf(Cd4creRora^(f/f))*** intranasal allergen challenges,Cd4creRora^(f/f)mice exhibited a significantly increased Th2 cells in the lungs and the airway and showed an enhanced eosinophilic inflammation compared to littermate control *** with Foxp3^(YFP-cre)Rora^(f/f)mice and CD^(8+)T cell depletion showed that the increased Th2 cell responses in the Cd4creRora^(f/f)mice were independent of Treg cells and CD^(8+)T *** findings demonstrate a critical regulatory role of RORa in Th2 cells,which suggest that RORa agonists could be effective for the treatment of allergic diseases.
