Adjuncts to colonic cleansing before colonoscopy

Pre-procedural cleansing of the bowel can maximize the effectiveness and efficiency of ***,efficacy of the current gold standard colonic preparation method-high-volume oral administration of purgative agents 12-24 h prior to the procedure-is limited by several factors,such as patient compliance(due to poor palatability and inconvenience of the dosing regimen)and risks of complications(due to drug interactions or intolerance).Attempts to resolve these limitations have included providing adjunctive agents and methods to promote the colonic cleansing ability of the principal purgative agent,with the aim of lessening unpleasant side effects(such as bloating)and reducing the large ingested volume *** promising adjunctive agents are bisacodyl,magnesium citrate,senna,simethicone,metoclopramide,and prokinetics,and each are being investigated for their *** review provides an up to date summary of the reported investigations into the potencies and weaknesses of the key adjuncts currently being applied in clinic as supplements to the traditional bowel preparation *** the comparative analysis ofthese adjuncts showed that no single agent or method has yet achieved the goal of completely overcoming the limitations of the current gold standard preparation method,they at least provide endoscopists with an array of alternatives to help improve the suboptimal efficacy of the main cleansing solutions when used *** aid in this clinical endeavor,a subjective grade was assigned to each adjunct to indicate its practical *** addition,the systematic review of the currently available agents and methods provides insight into the features of each that may be overcome or exploited to create novel drugs and strategies that may become adopted as effective bowel cleansing adjuncts or alternatives.

What is the best strategy for successful bowel preparation under special conditions?

Adequate bowel preparation is important for successful colonoscopic examination. Several effective colonic cleansing agents are available and routinely prescribed, but each carries its own limitations and benefits from particular dosing regimens. The most frequently prescribed colonic cleansing agent, the polyethylene glycol (PEG) cathartic solution, suffers from low patient compliance in general, due to its unpalatable taste and smell coupled with the large ingested volumes required. However, PEG is preferred over other cathartics for use in individuals of advanced age, sufferers of chronic kidney disease, heart failure and inflammatory bowel disease, and women who are pregnant or lactating. The laxative agents sodium phosphate (NaP) and sodium picosulfate plus magnesium citrate have been applied and have improved patient compliance and tolerance. NaP, however, should be avoided in individuals with impaired renal function or plasma clearance, such as those with chronic kidney disease, who are taking drugs that affect renal function, or who suffer from heart failure. Other special conditions that may affect an individual’s tolerance of the cathartic agent or ability to complete the administration routine include stroke, severe constipation, hematochezia, suspicious lower gastrointestinal bleeding, and mental disorders such as dementia. All ingestible bowel preparation solutions can be instilled into the stomach and duodenum through nasogastric tube or esophagogastroduodenoscope with the aid of a water irrigation pump for patients with difficulties swallowing or ingesting the large volumes of fluid required. In addition, dietary regimens based on clear liquids and low-residue foods for 1-4 d prior to the colonoscopy may be supplemental bowel preparation strategies. Achieving an effective and safe cleansing of the bowel is important for successful colonoscopy in all patients, so full knowledge of the individual’s condition and capabilities is necessary

Risk factor analysis for meta plastic gastritis in Koreans

AIM： To conduct a retrospective study to determine the risk factors for development of metaplastic gastritis in Korean population. METHODS： The database of 113449 subjects who underwent a gastroscopy for the purpose of a regular check-up at center for health promotion, Samsung medical center during 5 years was collected and retrospectively analyzed. Among them, 5847 subjects who had endoscopically diagnosed as a metaplastic gastritis or 10076 normal as well as answered to questionnaire were included for present study. The subjects were divided into 2 groups; Group Ⅰ, normal and Group Ⅱ, metaplastic gastritis. Age, gender, Helicobacter pylori （H pylori） seropositivity, body mass index （BMI）, family history of cancer, smoking, alcohol consumption, total daily calories, folate and salt intake and dietary habit （out-eating, overeating, irregular eating） were retrieved from questionnaire or electronic medical record and compared between group Ⅰ and group Ⅱ. RESULTS： The prevalence of group Ⅱwas 11% （13578/113449） increasing its prevalence with age （P=0.000）. But, there was no significant association between 2 groups in BMI, family history of cancer, alcohol consumption, total daily calories, folate and salt intake and dietary habit （out-eating, overeating, irregular eating）. Old age （P=0.000）, male gender （P=0.000）, H pylori seropositivity （P= 0.010） and current smoker （P=0.000） were significantly more common in group II at multiple logistic regression model. CONCLUSION： Our data suggested that old age, male gender, H pylori seropositivity and smoking were risk factors for metaplastic gastritis, precancerous lesion of gastric cancer.

Celecoxib attenuates 5-fluorouracil-induced apoptosis in HCT-15 and HT-29 human colon cancer cells

AIM： To investigate the combined chemotherapeutic effects of celecoxib when used with 5-FU in vitro. METHODS： Two human colon cancer cell lines （HCT-15 and HT-29） were treated with 5-FU and celecoxib, alone and in combination. The effects of each drug were evaluated using the MTT （3-（4, 5-dimethylthiazol-2-yl）-2, 5-diphenyltetrazolium bromide） assay, flow cytometry, and western blotting. RESULTS： 5-FU and celecoxib showed a dosedependent cytotoxic effect. When treated with 10-3 mol/L 5-FU （IC50） and celecoxib with its concentration ranging from 10.8 mol/L to 10.4 mol/L of celecoxib, cells showed reduced cytotoxic effect than 5-FU （10.3 mol/L） alone. Flow cytometry showed that celecoxib attenuated 5-FU induced accumulation of cells at subG1 phase. Western blot analyses for caspase-3 and poly （ADP-ribose） polymerase （PARP） cleavage showed that celecoxib attenuated 5-FU induced apoptosis. Western blot analyses for cell cycle molecules showed that G2/M arrest might be possible cause of 5-FU induced apoptosis and celecoxib attenuated 5-FU induced apoptosis via blocking of cell cycle progression to the G2/M phase, causing an accumulation of cells at the GI/S phase. CONCLUSION： We found that celecoxib attenuated cytotoxic effect of 5-FU. Celecoxib might act via inhibition of cell cycle progression, thus preventing apoptosis induced by 5-FU.

Filiform polyposis in the sigmoid colon: A case series

Filiform polyposis is a rare condition of uncertain patho-genesis that is usually found in association with Crohn’s disease, ulcerative colitis, intestinal tuberculosis or histiocytosis X. We report seven interesting cases of polyposis with various pathologic components, mainly located in the left side of the colon with no associated inflammatory bowel disease, intestinal tuberculosis or histiocytosis X. Multiple finger-like polypoid lesions with the appearance of stalactites were noted on the left side of the colon, especially in the sigmoid area, at the time of colonoscopy. The polyps had a variety of sizes and shapes and were shown to have various histopathologic components among the different patients. Although filiform polyposis localized in the sigmoid colon appears not to have high oncogenic potential, periodic follow-up seems to be needed.

Low-molecular-weight fucoidan inhibits the proliferation of melanoma via Bcl-2 phosphorylation and PTEN/AKT pathway

Flexible thermocouple using a thermoelectric graphene fiber with a seamless junction

Temperature is an important physical variable that indicates the condition of the human body and artificial *** wearable applications require the development of temperature sensors with different form *** this study,a fiber-shaped thermoelectric temperature sensor is fabricated using a continuous graphene fiber whose two halves possess different reduction states.A seamless junction is formed by partially reducing a wet-spun graphene oxide fiber with hydroiodic acid(HI)solutions of different ***-half of the fiber is mildly reduced with 0.97 wt%HI,while the other half is highly reduced with 30.6 wt%*** different reduction states of the graphene composite fiber result in different Seebeck coefficients,allowing for the fabrication of a fiber-shaped graphene thermocou-ple without any laborious *** flexible graphene thermocouple exhibits high sensitivity with a thermopower of 12.5μV K^(-1)in the temperature range of room temperature to∼70℃.Furthermore,it exhibits high linearity with a correlation coefficient exceeding 0.995 and fast response with a time constant of 0.24 *** to its mechanical robustness and flexibility,the stand-alone graphene ther-mocouple can be knitted into a cotton fabric glove,which presents a fast response to environmental changes without any external power *** work offers a unique fabrication method for producing a high-performance,flexible thermocouple that features a seamless and clear junction without the use of additional *** alternative method eliminates the complicated assembly processes typically required for conventional thermocouples.

Single-cell and spatial omics:exploring hypothalamic heterogeneity

Elucidating the complex dynamic cellular organization in the hypothalamus is critical for understanding its role in coordinating fundamental body functions. Over the past decade, single-cell and spatial omics technologies have significantly evolved, overcoming initial technical challenges in capturing and analyzing individual cells. These high-throughput omics technologies now offer a remarkable opportunity to comprehend the complex spatiotemporal patterns of transcriptional diversity and cell-type characteristics across the entire hypothalamus. Current single-cell and single-nucleus RNA sequencing methods comprehensively quantify gene expression by exploring distinct phenotypes across various subregions of the hypothalamus. However, single-cell/single-nucleus RNA sequencing requires isolating the cell/nuclei from the tissue, potentially resulting in the loss of spatial information concerning neuronal networks. Spatial transcriptomics methods, by bypassing the cell dissociation, can elucidate the intricate spatial organization of neural networks through their imaging and sequencing technologies. In this review, we highlight the applicative value of single-cell and spatial transcriptomics in exploring the complex molecular-genetic diversity of hypothalamic cell types, driven by recent high-throughput achievements.

Validity of fatty liver prediction scores for diagnosis of fatty liver by Fibroscan

Background:The Korea National Health and Nutrition Examination Survey nonalcoholic fatty liver disease(K-NAFLD)score was recently developed with the intent to operationally define nonalcoholic fatty liver disease(NAFLD).However,there remained an external validation that confirmed its diagnostic performance,especially in patients with alcohol consumption or hepatitis virus ***:Diagnostic accuracy of the K-NAFLD score was evaluated in a hospital-based cohort consisting of 1388 participants who received Fibroscan®.Multivariate-adjusted logistic regression models and the contrast estimation of receiver operating characteristic curves were used for validation of the K-NAFLD score,fatty liver index(FLI),and hepatic steatosis index(HSI).Results:K-NAFLD-moderate[adjusted odds ratio(aOR)=2.53,95%confidence interval(CI):1.13-5.65]and K-NAFLD-high(aOR=4.14,95%CI:1.69-10.13)groups showed higher risks of fatty liver compared to the K-NAFLD-low group after adjustments for demographic and clinical characteristics,and FLI-moderate and FLI-high groups revealed aORs of 2.05(95%CI:1.22-3.43)and 1.51(95%CI:0.78-2.90),*** addition,the HSI was less predictive for Fibroscan®-defined fatty *** K-NAFLD and FLI also demonstrated high accuracy in the prediction of fatty liver in patients with alcohol consumption and chronic hepatitis virus infection,and the adjusted area under curve values were comparable between K-NAFLD and ***:Externally validation of the K-NAFLD and FLI showed that these scores may be a useful,noninvasive,and non-imaging modality for the identification of fatty *** addition,these scores also predicted fatty liver in patients with alcohol consumption and chronic hepatitis virus infection.
