Intensive insulin therapy has been extensively used to control blood glucose levels because of its ability to reduce the risk of chronic complications of *** to current guidelines,intensive glycemic control requires individu-alized glucose goals rather than as low as *** intensive therapy,rapid blood glucose reduction can aggravate microvascular and macrovascular complications,and prolonged overuse of insulin can lead to treatment-induced neuropathy and retinopathy,hypoglycemia,obesity,lipodystrophy,and insulin antibody ***,we need to develop individualized hypoglycemic plans for patients with diabetes,including the time required for blood glucose normalization and the duration of intensive insulin therapy,which deserves further study.

Permafrost degradation caused by climate warming is posing a serious threat to the stability of cast-in-place pile foundations in warm permafrost *** cold energy can be effectively utilized by two-phase closed thermosyphons(TPCTs)to cool the ***,we installed TPCTs in a cast-in-place pile foundation to create a unique structure called a thermal pile,which effectively utilizes the TPCTs to regulate ground *** we conducted a case study and numerical simulation to exhibit the cooling performance,and optimize the structure of the thermal *** purpose of this study is to promote the application of thermal piles in unstable permafrost *** on the findings,the thermal pile operated for approximately 53%of the entire year and effectively reduced the deep ground temperature at a rate of at least-0.1℃per ***,it successfully raised the permafrost table that is 0.35 m shallower than the natural ground *** characteristics prove highly beneficial in mitigating the adverse effects of permafrost degradation and enhancing infrastructure *** the length of the condenser section and the diameter of the TPCT in a suitable manner can effectively enhance the cooling capability of the thermal pile and ensure the long-term mechanical stability of the pile foundation even under climate warming.

Background:A high prevalence of diabetes mellitus(DM)coexisting with autoimmune pancreatitis(aiP)is ***,evidence on the circumstances under which corticosteroid therapy(CST)for aiP improves or worsens DM is *** study aimed to demonstrate and identify predictors of DM control under the influence of ***:Patients diagnosed with type 1 aiP were enrolled from a prospectively maintained cohort and were classified into three groups according to the chronology in which aiP and DM were diagnosed:pre-existing DM(pDM),concurrent DM(cDM),and non-DM(nDM).The responses of DM to CST were assessed when corticosteroid was ceased or tapered to a maintenance dose and classified as‘improvement’and‘non-improvement’(including‘no change’and‘exacerbation’).Results:Among 101 patients with type 1 aiP,52(51.5%)patients were complicated with DM at the time of aiP diagnosis,with 36 patients in the cDM group and 16 patients in the pDM *** incidences of diffuse pancreatic swelling(72.2%)and pancreatic body/tail involvement(91.7%)were significantly higher in the cDM group than in both the pDM and nDM *** the 52 patients with DM,CST was administered in 48 *** logistic analysis identified that elevated serum gamma-glutamyl transferase(GGT)level at aiP diagnosis[odds ratio(OR)=0.032,95%confidence interval(CI):0.003-0.412,P=0.008]and pancreatic atrophy after CST(OR=0.027,95%CI:0.003-0.295,P=0.003)were negatively associated with DM control ***:Patients with diffuse pancreatic swelling and pancreatic body/tail involvement in pancreatitis tended to be complicated with cDM at aiP *** exerted a beneficial effect on the clinical course of DM in nearly half of the aiP patients complicated with DM at diagnosis,particularly in those without elevated serum GGT levels at diagnosis and who did not experience pancreatic atrophy after CST.

A scheme of optical four-level pulse amplitude modulation(PAM-4) is proposed based on dual-Raman process in Rydberg atoms. A probe field counter-propagates with a dual-Raman field which drives the ground and the excited states transition, respectively, and the Rydberg transition is driven by a microwave(MW) field. A gain peak appears in the probe transmission and is sensitive to the MW field strength. Optical PAM-4 can be achieved by encoding an MW signal and decoding the magnitude of a probe signal. Simulation results show that the differential nonlinearity and the integral nonlinearity of the proposed scheme can be reduced by 5 times and 6 times, respectively, compared with the counterparts of previous scheme, and the ratio of level separation mismatch is close to the ideal value 1. Moreover, the scheme is extended to optical PAM-8 signal, which may further improve the spectral efficiency.

Relative performance of two equations for estimation of glomerular filtration rate in a Chinese population having chronic kidney disease

Background The new Chronic Kidney Disease Epidemiology Collaboration （CKD-EPI） equation was developed to address the systematic underestimation of glomerular filtration rate （GFR） by the Modification of Diet in Renal Disease （MDRD） Study equation in patients with relatively well-preserved kidney *** of the new equation in the Chinese population is *** goal of the present study was to compare performance of these two equations in Chinese patients with chronic kidney disease （CKD）.Methods We enrolled 450 Chinese patients （239 women and 211 men） with CKD in the present *** renal dynamic imaging method was used to measure the referenced standard GFR （rGFR） for comparison with estimations using the two *** overall performance was assessed with the Bland-Altman method and receiver-operating characteristics （ROC） *** of the two equations in lower and higher estimated GFR （eGFR） subgroups was further *** Both eGFRs correlated well with rGFR （r=0.88,0.81,P〈0.05）.In overall performance,the CKD-EPI equation showed less bias,higher precision and improved accuracy,and was better for detecting *** the higher-eGFR subgroup,the CKD-EPI equation corrected the underestimation of GFR by the abbreviated MDRD *** The CKD-EPI equation outperformed the abbreviated MDRD equation not only in overall performance but also in the subgroups *** the present,the CKD-EPI equation appears to be the first-choice prediction equation for estimating GFR.

Zinc extraction residue,a solid waste generated from the treatment of zinc-containing dust in rotary kilns,is commonly stockpiled in steel companies for extended *** poses significant disposal challenges and environmental pollution *** far,research on the treatment of zinc extraction residues has been slow,inadequate,and *** this gap,a novel approach was proposed to effectively treat the zinc extraction residue via the iron ore sintering *** was feasible to add 1 wt.%of zinc extraction residues to the sintering raw *** more adequate mineralization reaction resulted in higher yield and tumbler indexes,despite a slight decrease in sintering *** this may result in a slight decrease in sintering speed,the more complete mineralization reaction leads to improved sintering yield and tumbler ***,the addition of zinc extraction residues reduced the CO and NO_(x) concentrations in the sintering flue ***,the iron ore sintering process provided a viable solution for resource utilization and environmentally friendly treatment of zinc extraction residues.
