Do changes in intracoronary pressure aid coronary spasm diagnosis using the spasm provocation test?

BACKGROUND Although the spasm provocation test(SPT)can diagnose coronary spasms,it would be helpful if it could also predict their *** To investigate whether coronary spasms can be predicted using changes in intracoronary artery pressure measured using a pressure wire during the *** Seventy patients underwent SPTs with pressure-wire measurement of intracoronary artery *** each SPT,the pressure wire was advanced into the distal portion of the right coronary artery(RCA)and left anterior descending coronary artery,and the ratio of intracoronary pressure to aortic pressure(Pd/Pa)was *** spasm was defined as an arterial narrowing of>90%in response to the administration of acetylcholine(ACh),with chest symptoms and/or ischemic electrocardiographic *** was administered to the RCA at low,moderate,or high doses of 20,50,or 80μg,respectively,and to the left coronary artery(LCA)at low,moderate,or high doses of 50,100,or 200μg,*** arteries with coronary spasms at low doses of ACh were defined as group L,and those with coronary spasms at moderate or high doses were defined as group *** who did not occur coronary spasms at any ACh dose were designated as group *** Among the 132 coronary arteries assessed using a pressure wire,there were 49 in group N,25 in group L,and 58 in group *** Pd/Pa was the lowest in group L(P=0.001).The decrease in the Pd/Pa between baseline to low doses of ACh was lower in group MH than in group N(P<0.001).A receiver-operating characteristics analysis showed that the cutoff baseline Pd/Pa value for predicting group L was 0.95,with a sensitivity of 0.600(15/25)and a specificity of 0.713(76/107)and that the cutoff value of Pd/Pa from baseline to low doses of ACh for predicting group MH was−0.04,with a sensitivity of 0.741(43/58)and a specificity of 0.694(34/49).CONCLUSION These findings suggest that indices of intracoronary pressure during SPT may be useful means for

A Case Report of Intestinal Myiasis in a Japanese Man

We recently experienced a case of intestinal myiasis caused by maggots of fly, Sarcophagidae. A 61-year-old Japanese man presented in August 2015 with a 2-day his-tory of diarrhea and white maggots in his stool. Two days before his presentation, he took Chinese noodles and lettuce. At his presentation, we observed a number of discharged insect bodies in his stool. The insect bodies were identified as the larvae of the 3rd instar flesh fly Sarcophagidae based on macroscopic and microscopic examinations. We finally diagnosed as intestinal myiasis due to the fact that the patient had the insect bodies mixed in his diet, lettuce. A few days after the treatment with antibiotics, his symptoms disappeared. Accurate diagnosis of intestinal myiasis in developed countries is necessary to avoid ineffective treatment.

The Development of the “Monitoring Application for Inappropriate Expressions in Nursing Records”for PsyNACS^©

Lengthening periods of hospitalization, increasing numbers of patients with age-related complications, and a shortage of nursing staff have been of great concern in medical psychiatry in Japan. Under these circumstances, countries such as Japan that face a super-aging society and a decline in the working-age population, have been recommended for use of advanced information and communications technology (ICT) to improve the efficiency of medical treatment and care. This study aims to develop the “Monitoring Application for Inappropriate Expressions in Nursing Records” for using PsyNACS©, which will enable psychiatric nursing plans to harness the advantages of ICT. The functions considered necessary are as follows: 1) identification of users who enter information;2) a necessary database for lists of inappropriate expressions;3) development of a matching function to recommend proper writing input and a warning function;and 4) a management function for an inappropriate expression list database. A demonstration experiment for developing the application in this study was conducted at a specialized psychiatric hospital. To introduce them to the application, nurses and nurse managers were informed about the system developed in this study, and a survey regarding their opinions on the functions of the application was conducted with ten nurse managers. The results were evaluated in terms of usefulness for nursing care, documentation, and the education of nurses from an ethical perspective. This study suggests that matching their input with an inappropriate expression database will allow nurses to record more appropriate expressions. From an ethical perspective, the ability to use appropriate expressions makes records more likely to withstand disclosure requests from patients and their families.

Enzyme-linked immunosorbent assay of 3 Screen Islet Cell Autoantibody in patients with autoimmune thyroid disease

Protective role of electrophile-reactive glutathione for DNA damage repair inhibitory effect of dibromoacetonitrile

Dibromoacetonitrile(DBAN) is a disinfection byproduct(DBP) and linked with cancer in rodents, but the mechanism of its carcinogenicity has not been fully elucidated. We recently reported that DBAN induced inhibition of nucleotide excision repair(NER). In this study, we investigated if glutathione(GSH) is involved in the DBAN-induced inhibition of NER. Human keratinocytes Ha Ca T were pretreated with L-buthionine-(S,R)-sulfoximine(BSO) to deplete intracellular GSH. BSO treatment markedly potentiated the DBAN-induced NER inhibition as well as intracellular oxidation. The recruitment of NER proteins(transcription factor IIH, and xeroderma pigmentosum complementation group G) to DNA damage sites was inhibited by DBAN, which was further exacerbated by BSO treatment. Our results suggest that intracellular GSH protects cells from DBAN-induced genotoxicity including inhibition of DNA damage repair.

Objective Shock heart syndrome(SHS)is associated with lethal arrhythmias(ventricular tachycardia/ventricular fibrillation,VT/VF).We investigated whether liposome-encapsulated human hemoglobin vesicles(HbVs)has comparable persistent efficacy to washed red blood cells(wRBCs)for improving arrhythmogenesis in the subacute to chronic phase of *** Optical mapping analysis(OMP),electrophysiological study(EPS),and pathological examinations were performed on blood samples from Sprague-Dawley rats following induction of hemorrhagic *** hemorrhagic shock,the rats were immediately resuscitated by transfusing 5%albumin(ALB),HbV,or *** rats survived for 1 *** and EPS were performed on Langendorff-perfused *** arrhythmias and heart rate variability(HRV)were evaluated using awake 24-h telemetry,cardiac function by echocardiography,and pathological examination of *** OMP showed significantly impaired action potential duration dispersion(APDd)in the left ventricle(LV)in the ALB group whereas APDd was substantially preserved in the HbV and wRBCs *** VT/VF was easily provoked by EPS in the ALB *** VT/VF was induced in the HbV and wRBCs ***,spontaneous arrhythmias,and cardiac function were preserved in the HbV and wRBCs *** showed myocardial cell damage and Connexin43 degradation in the ALB group,all of which were attenuated in the HbV and wRBCs *** LV remodeling after hemorrhagic shock caused VT/VF in the presence of impaired *** to wRBCs,HbV persistently prevented VT/VF by inhibiting persistent electrical remodeling,preserving myocardial structures,and ameliorating arrhythmogenic modifying factors in the subacute to chronic phase of hemorrhagic shock-induced SHS.

Does the intracoronary pressure differ according to two types(diffuse or focal)of coronary spasm?

BACKGROUND Several reports show that two types of coronary vasospasm(diffuse and focal spasm)are associated with the severity or prognosis of coronary spasm in patients with vasospastic angina(VSA).It is unclear whether intracoronary pressure differs between the two spasm *** To investigate such relationships using a pressure wire during the spasm provocation test(SPT)in patients with *** Eighty-seven patients with VSA(average age:67 years;50 men,37 women)underwent *** the SPT,a pressure wire was advanced into the distal portion of the right coronary artery and left anterior descending coronary artery,and the ratio of the intracoronary pressure to the aortic pressure(Pd/Pa)was continuously *** SPT was performed using acetylcholine(ACh),and the presence of coronary spasm was defined as the presence of>90%arterial narrowing in response to an ACh infusion,with the usual chest symptoms and/or ischemic ECG *** spasm was defined as total or subtotal spasm within one segment of the AHA classification,while diffuse spasm was defined as>90%spasm with two or more *** Among 87 patients,the frequencies of metabolic syndrome and having coronary atherosclerosis were higher in the focal group(n=33)than in the diffuse spasm group(n=54,P<0.05).In the vessel analyses,in these 134 spastic segments,diffuse and focal spasms were detected in 100 and 34 vessels,*** Pd/Pa at baseline was similar in both groups(diffuse:0.96±0.05,focal:0.95±0.05,P=0.35);however,the Pd/Pa during coronary spasm was lower in focal spastic vessels(0.66±0.20)than in diffuse spastic vessels(0.76±0.11,P<0.01),and the reduction in Pd/Pa during an SPT was also lower in focal spastic vessels(-0.29±0.20)than in diffuse spastic vessels(-0.18±0.11,P<0.01).The presence of focal spasm was a significant factor responsible for reduction in Pd/Pa during *** These findings suggest that focal spasm may be more severe than diffuse spasm,judging from the

Vasospastic angina in women: Clinical backgrounds and prognoses of patients younger than and older than 60 years

BACKGROUND We frequently encounter cases of women with vasospastic angina(VSA).Additionally,some women with VSA are younger than 60 years ***,it is unknown whether the characteristics of VSA in women aged20%)detected using coronary angiography and focal spasms(coronary spasm within one segment of one coronary artery),and major cardiovascular adverse events(MACE)were assessed in both *** Smoking was more prevalent in Group Y than in Group O(P=0.04).FMD was similar in both groups(Group O:4.3%±3.2%,Group Y:4.5%±3.3%;P=0.75),whereas NID was higher in Group Y(20.5%±8.6%)than in Group O(13.6%±5.3%,P<0.01).Atherosclerosis was not detected in Group Y but was detected in Group O(61%,P<0.01).Focal spasms were less frequent in Group Y(12%)than in Group O(38%,P=0.04).The incidence of major adverse cardiac events did not differ between the two groups(P=0.40).CONCLUSIONWomen aged < 60 years with VSA have less atherosclerotic lesions and focal spasms. These characteristicsmay be affected by smoking habits and vascular smooth muscle dysfunction.

Acquisition of Knowledge and Skills about Delivery by Self-Evaluation of Midwifery Students Using the Labor and Delivery Assistance Evaluation Sheets

Aim: This study aims to clarify the current state of advancement of knowledge and skills in the educational process from self-assessments by midwifery student labor and delivery assistance evaluation sheets. Method: Metrology is an observational and retrospective study. Self-assessments in labor and delivery assistance evaluation sheets of midwifery students registered in an advanced midwifery course at Nursing College A from Apr. 2017 to Mar. 2018 were analyzed retrospectively. The end-points were every score of six classifications: health checkups and assessments of women who give birth, preparations for labor and delivery assistance, delivery assistance in the parturient second stage, delivery assistance in the parturient third and fourth stages, midwifery process, and the training manner. Moreover, they also included the scores of 64 evaluation items belonging to any of the six classifications and a comprehensive evaluation score in which students judged the acquisition of all knowledge and skills of delivery assistance. In addition, although not on the evaluation sheet, the average score of all 64 evaluation items was also included. The evaluation criteria were five levels, from one to five. The end-points were examined by comparing the experiences of delivery assistances in two periods of Results: 80 among 91 evaluation sheets from eight out of nine students were analyzed. The average score of all 64 evaluation items, every score of the six classifications, and the comprehensive evaluation score was significantly elevated in the experience periods of 6 ≤ times (P Conclusion: Training in adaptation and enforcement of amniotomy and assistance skills for lacerations sutures will be needed in future educational challenges. Self-assessment scores by midwifery students in labor and delivery assistance evaluation sheets were significantly elevated in the experience periods of 6 ≤ times.

Effects of alginate and laminaran on the microbiota and antioxidant properties of human faecal cultures
