PURPOSE: Patients with AD commonly exhibit behavioral and psychological symptoms of dementia (BPSD). This study is aimed to compare the efficacy of yokukansan (YKS) and risperidone (RIS) on BPSD in patients with severe Alzheimer’s disease (AD). METHODS: Thirty eight inpatients with AD were investigated. Patients were randomly as-signed to the YKS group (N = 18) or the RIS group (N = 20) and treated for 4 weeks. The primary outcomes were changes in the scores on the Neuropsychiatric Inventory (NPI), the Mini-Mental State Examination (MMSE), the Bar-thel Index, and the Cohen-Mansfield Agitation Inventory (CMAI). The frequency of extrapyramidal symptoms (EPS) and other adverse events were recorded at every visit. RESULTS: All participants in both groups completed the trial. The Barthel Index did not significantly change either in the RIS group or the YKS group. The MMSE scores did not change either in the RIS group or the YKS group. Significant improvements in mean total NPI and CMAI scores showed in both groups. Between the YKS and the RIS groups, there were no significant differences in the NPI or the CMAI scores. EPS and other serious adverse effects were not observed in either group. CONCLUSIONS: In this 4-week trial, YKS treatment significantly improved BPSD in the patients with severe AD. The present study suggests that YKS is as effective as RIS on BPSD with severe AD.

Helium and Argon Isotopic Studies of Fossil Material and the Theoretical Evolution of He and Ar in Earth’s Atmosphere through Time

We analyzed the elemental concentrations and the isotopic compositions of helium and argon in Cambrian to Jurrassic aged Gastropod, Ammonite and Trilobite fossils in order to examine variation in these gases through time. Fossil samples yielded He and Ar isotopic ratios close to the present day atmospheric values, but also indicated some addition of a radiogenic component. We compared the results to theoretical values calculated from a mathematical model of Earth’s atmosphere assuming mantle degassing. Results from our mathmatical models showed that the 40Ar/36Ar ratio of Earth’s atmosphere increased rapidly after the formation of the Earth, but has been almost identical to the present day value for the last 1 Ga. For atmospheric helium, model results were consistent with present day atmospheric values, assuming complete helium degassing from the continental crust into the atmosphere. The model suggests that the atmospheric 3He/4He ratio has remained relatively constant for the last 0.1 Ga. Given the similarity between present day and ancient He and Ar isotopic ratios, we conclude that the corresponding ratios measured in ancient fossil material may partially reflect composition of the ancient atmosphere and are not necessarily due to contamination by the present day atmosphere.

Under the JI-EPA （Japan-Indonesia Economic Partnership Agreement）, approximately one thousand nurse and certified care worker candidates have been migrated to Japan since 2008. Migration has been known as a risk factor of mental well-being. Little is known about how the in pre-migration stage may affect the migrant＇s mental health. The study examines the mental health predictor in pre-migration stage using cross sectional design. Gender, Age, EPA course, educational degree, working background, language proficiency, motivation for migration and knowledge about destination country selected as mental health predictor. The results showed that the variable knowledge about living environment is the only independent variable which is correlate with GHQ in the model, β = -0.20, P 〈 0.05. The finding may assume that the knowledge as part of cognitive preparation plays important role in explaining the mental health of the EPA candidates in pre-migration stage.

Pain in Non-Athlete Patients Undergoing Hip Arthroscopy for Femoroacetabular Impingement and Hip Labral Tears: A Longitudinal Study

Background: Pain in patients with orthopedic problems is an important aspect of surgical evaluation. However, the effect on the quality of life depends not only on the degree of pain but also on its type. Hip arthroscopy has been performed for hip joint symptoms caused by femoroacetabular impingement (FAI) and hip labrum tears. However, the degree and type of pain after hip arthroscopy remains unknown. Objective: This study aimed to clarify the degree and type of pain during 6 months after hip arthroscopy. Methods: Non-athlete patients aged 20 - 65 years who underwent hip arthroscopy between December 2018 and October 2019 participated in a questionnaire survey before hospitalization and at 1, 3, and 6 months after surgery. The questionnaire comprised the Japanese Orthopaedic Association Hip-Disease Evaluation Questionnaire (JHEQ) for quality of life, International Physical Activity Questionnaire (IPAQ) for activities, and the Short-Form McGill Pain Questionnaire (SF-MPQ-2) for pain. Results: The analysis included 10 patients (men: 4, women: 6, mean age: 38 ± 8.6 years, FAI: 6 cases, hip labrum tears: 4 cases). The average time from symptom onset to surgery was 26.9 ± 19.0 months. The analysis from before surgery to 6 months after surgery showed improvement over time in all scale scores (JHEQ, IPAQ, and SF-MPQ-2). The degree of pain was significantly improved based on the JHEQ visual analog scale evaluation (P = 0.019) 3 months after surgery. Significant improvement in intermittent pain (P = 0.011) based on SF-MPQ-2 was noted 3 months after surgery;however, no significant improvement in continuous pain was noted. Conclusions: Patients who underwent hip arthroscopy showed significant improvements in the degree of pain and type of intermittent pain from before surgery to 3 months after surgery, however, no change was observed in continuous pain.

A Training Effect by Three-Month Watering on the Frontal Pole Activation of Healthy Older Adults

The effectiveness of training prefrontal lobe function for suppressing cognitive decline or improving cognitive function has been reported. Some epidemiological studies suggested that gardening activities can contribute to suppression of cognitive decline. In our previous studies using functional Near-Infrared Spectroscopy (fNIRS), we reported the possibility of frontal pole (FP) activation of healthy older adults during gardening activities in the quasi-laboratory setting. The objective of this study was to examine the impacts of a three-month continual practice of the watering task in the actual gardening settings on the FP activation of healthy older adults. The participants were 18 right-handed healthy older adults without a diagnosis of mild cognitive impairment or dementia (M = 71.7 years ± SD 5.0, range: 63 - 84 years) who lived independently at home. We asked the participants to continue watering at home for three months in the same way as the research task in the fNIRS measurement. After a three-month intervention, the participants’ task performance improved compared with that of pre-intervention. In addition, the Oxy-Hb values in the right lateral FP during the third trial in the post-intervention measurement were significantly greater compared with the pre-intervention measurement. This study suggested that even a gardening activity with a small number of processes such as watering, if the activity is continued for a certain period of time, can suppress the decline of the FP activation of healthy older adults. Combination of various gardening activities has the potential to be a further cognitive training for activating the FP.

The Success of Healthcare Providers Who Fought the Epidemic of the Meiji Era in Mizunami City, Gifu Prefecture, Japan

Introduction: Knowing an epidemic existed in Mizunami city and knowing the healthcare provided to overcome the disease may give hints on how the epidemic was handled. Aim: To examine the literature on the epidemic that occurred in the Meiji era and the contributions of healthcare providers who were active in the history of Mizunami city. Method: This is a historical study, mainly using literature. In collaboration with the Mizunami City Board of Education, we searched for materials stored by Mizunami city. In addition, with the cooperation of the local residents of the healthcare providers who were active in Mizunami city, we collected materials in reference to our purpose. Result: The outbreak of the epidemic near Mizunami city was seen as a fight against smallpox. In addition, cholera, dysentery, Spanish flu, and typhoid fever also occurred. Vaccinations were carried out in the Tono region. Healthcare providers who were active in the Meiji era include six doctors and two midwives. Discussion: In the future, it will be necessary to provide more in-depth evidence through surveys, such as interviews with the cooperation of relatives of the active healthcare providers. Conclusion: In the Meiji era, in the vicinity of Mizunami city, doctors and midwives actively fought against epidemics, such as smallpox and other infectious diseases.

Lengthening periods of hospitalization, increasing numbers of patients with age-related complications, and a shortage of nursing staff have been of great concern in medical psychiatry in Japan. Under these circumstances, countries such as Japan that face a super-aging society and a decline in the working-age population, have been recommended for use of advanced information and communications technology (ICT) to improve the efficiency of medical treatment and care. This study aims to develop the “Monitoring Application for Inappropriate Expressions in Nursing Records” for using PsyNACS©, which will enable psychiatric nursing plans to harness the advantages of ICT. The functions considered necessary are as follows: 1) identification of users who enter information;2) a necessary database for lists of inappropriate expressions;3) development of a matching function to recommend proper writing input and a warning function;and 4) a management function for an inappropriate expression list database. A demonstration experiment for developing the application in this study was conducted at a specialized psychiatric hospital. To introduce them to the application, nurses and nurse managers were informed about the system developed in this study, and a survey regarding their opinions on the functions of the application was conducted with ten nurse managers. The results were evaluated in terms of usefulness for nursing care, documentation, and the education of nurses from an ethical perspective. This study suggests that matching their input with an inappropriate expression database will allow nurses to record more appropriate expressions. From an ethical perspective, the ability to use appropriate expressions makes records more likely to withstand disclosure requests from patients and their families.

Challenges in Work-Family Balance and Support Needs of Japanese Parents with Nursery School-Aged Children

We aimed to identify the challenges parents face raising preschool-aged children when trying to maintain a good work-family balance. We also highlight how public health nurses can support the community in this regard. Seven Japanese parents volunteered to participate, and individual in-depth interviews were conducted from May to October 2021. We analyzed the recorded interview data using qualitative descriptive methods. Three main themes concerning work-family balance challenges were extracted: 1) conflicts between parents’ childcare and work roles, 2) issues related to institutions and social systems, and 3) a lack of informal support. Factors contributing to the difficulties were a lack of time to spend with their children;an imbalance in the division of housework and childcare roles between married couples;the employment environment in Japan that makes it difficult to pursue a career while raising children;a lack of childcare services, including childcare for sick children;inadequate consultation systems;and an aging grandparents’ generation owing to late marriages. To solve this problem, we suggest that in addition to establishing a reliable system for providing childcare, local public health nurses should work with childcare workers to create an environment in which parents feel comfortable discussing work-family balance, considering the time constraints of parents during the childrearing period, and use nursery schools as a place for parent-child support.
