Short-and long-term outcomes of endoscopically treated superficial non-ampullary duodenal epithelial tumors

BACKGROUND It is widely recognized that endoscopic resection(ER) of superficial nonampullary duodenal epithelial tumors(SNADETs) is technically challenging and may carry high risks of intraoperative and delayed bleeding and *** adverse events could be more critical than those occurring in other levels of the gastrointestinal tract. Because of the low prevalence of the disease and the high risks of severe adverse events, the curability including short-and long-term outcomes have not been standardized *** To investigate the curability including short-and long-term outcomes of ER for SNADETs in a large case *** This retrospective study included cases that underwent ER for SNADETs at our university hospital between March 2004 and July 2017. Short-term outcomes of ER were measured based on en bloc and R0 resection rates as well as adverse events. Long-term outcomes included local recurrence detected on endoscopic surveillance and disease-specific mortality in patients followed up for ≥ 12 mo after *** In the study, 131 patients with 147 SNADETs were analyzed. The 147 ERs consisted of 136 endoscopic mucosal resections(EMRs)(93%) and 11 endoscopic submucosal dissections(ESDs)(7%). The median tumor diameter was 10 *** pathology diagnosis was adenocarcinoma(56/147, 38%), high-grade intraepithelial neoplasia(44/147, 30%), or low-grade intraepithelial neoplasia(47/147, 32%). The R0 resection rate was 68%(93/136) in the EMR group and73%(8/11) in the ESD group, respectively. Cap-assisted EMR(known as EMR-C)showed a higher rate of R0 resection compared to the conventional method of EMR using a snare(78% vs 62%, P = 0.06). No adverse event was observed in the EMR group, whereas delayed bleeding, intraoperative perforation, and delayed perforation in 3, 3, and 5 patients occurred in the ESD group, respectively. Tentative New Terminology: Initial Comedonal Phase Preceding Acute Inflammatory Phase of Acne Vulgaris—Importance of Early Treatment

We report a case of a 12-year-old female with closed and open comedo without inflammatory lesions at the first visit. She had been treated with topical adapalene once daily, and closed and open comedo subsided without inflammatory lesions one month after the treatment without any side effects induced by adapalen. The recurrence of comedo had not been observed afterwards. In the Japanese Dermatological Association (JDA) guidelines for the treatment of acne vulgaris, comedonal phase has not been stated. Then we propose the tentative new terminology of a comedonal phase and emphasize its importance in the early initial treatment for comedo to prevent the development of the acute inflammatory phase such as red papules and pustules and formation of acne scars.

Antidiabetic Effects of Omega-3 Polyunsaturated Fatty Acids: From Mechanism to Therapeutic Possibilities

Diabetes mellitus (DM) is chronic disease characterized by hyperglycemia and insulin resistance caused by dysfunction of pancreatic β cells. Over the past few decades, epidemiological studies have suggested that dietary long-chain polyunsaturated fatty acids such as docosahexaenoic acid and eicosapentaenoic acid decrease the risk of metabolic diseases including DM. The mechanisms underlying the therapeutic efficacy of dietary long-chain polyunsaturated fatty acids in treating DM have been partly revealed. In this review, the authors describe the antidiabetic effects of long-chain polyunsaturated fatty acids and also discuss their possibilities as therapeutics for DM in the light of recent findings.

Surgically Important Relationships of Recurrent and Nonrecurrent Laryngeal Nerves to the Coexisting Variant Vessels

The retroesophageal right subclavian artery may be associated with multiple neurovascular variations, which may impact various cervicothoracic surgeries. During the dissection of 27 cadavers, the authors detected a retroesophageal right subclavian artery in old man arising from distal aortic arch, crossed ventrally by left recurrent laryngeal nerve and dorsally by thoracic duct. On the right side, the aberrant subclavian artery was associated with nonrecurrent laryngeal nerve crossing a large tortuous inferior thyroid artery at multiple intersection points. Importantly, the nonrecurrent laryngeal nerve gave rise to extra laryngeal branch passing over Zuckerkandl’s tubercle of thyroid gland before its termination into the larynx. The relationships of the aberrant subclavian artery in the present case to thoracic duct, recurrent and nonrecurrent laryngeal nerves may have clinical relevance to various operations such as thyroidectomy, esophagectomy and surgical correction of the aberrant vessel. Therefore, these relationships should be well known by surgeons for prevention of iatrogenic damage of essential neurovascular structures during various surgeries.

INTRODUCTION Surgical procedures are considered more challenging in cirrhotic patients with hepatocellular carcinoma(HCC)than in non-cirrhotic patients,because of the former’s high morbidity and mortality following surgery.[1]In open liver surgery,the extremely long incision required for mobilization and resection of the liver can result in significant intraoperative blood loss or postoperative intractable ascites,followed by reduced collateral circulation in the abdominal wall and ligaments around the *** complications may progress to postoperative hepatic failure in some patients.

Comparative study on pharmacokinetics and toxicity of intravitreal and sub-Tenon injection of triamcinolone acetonide in ocular tissues

AIM:To compare the differences in kinetics,distribution,and toxicity of triamcinolone acetonide(TA)between the injection methods,sub-Tenon and intravitreal injections in rabbit ocular ***:TA was injected into the vitreous or the sub-Tenon in *** pharmacokinetic study,rabbits were sacrificed periodically and then TA in blood and ocular tissues(retina/choroids,vitreous,and aqueous humor)were measured over 91 *** toxicological study,clinical signs,slit-lamp microscopic examination,ophthalmological test were *** eyeballs and surrounding tissues were collected and fixed with glutaraldehyde-formalin solution,and then paraffin embedded for histological ***:Higher levels of TA were distributed in the intraocular tissues when injected into the vitreous compared to the ***,TA level was remarkably lower in the rabbits which received intravitreal TA injections than those treated with sub-Tenon injection throughout the study period in *** discharge probably caused by systemic circulation of TA was observed by receiving sub-Tenon TA ***,technicassociated toxicological ocular symptoms and findings were more frequently observed in intravitreal injection than in sub-Tenon ***:There are significant differences in kinetics and distribution of TA in vitreous body,aqueous humor and plasma,between the two injection *** further study is needed to explain the species difference between human and rabbit,it is assumed that the difference in the frequency of intraocular pressure elevation and cataract formation by TA between the two injection methods are directly related to the TA concentrations in aqueous humor and vitreous body in each injection *** toxicity and technic-associated toxicity are also closely related to kinetics of TA in plasma and each injection method itself,respectively.

Ethanol-induced hepatic autophagy: Friend or foe?

Excessive alcohol intake may induce hepatic apoptosis, steatosis, fibrosis, cirrhosis and even cancer. Ethanolinduced activation of general or selective autophagy as mitophagy or lipophagy in hepatocytes is generally considered a prosurvival mechanism. On the other side of the coin, upregulation of autophagy in nonhepatocytes as stellate cells may stimulate fibrogenesis and subsequently induce detrimental effects on the liver. The autophagic response of other non-hepatocytes as macrophages and endothelial cells is unknown yet and needs to be investigated as these cells play important roles in ethanol-induced hepatic steatosis and damage. Selective pharmacological stimulation of autophagy in hepatocytes may be of therapeutic importance in alcoholic liver disease.
